The objective was to evaluate the effectiveness and safety of acupuncture for patients with rotator cuff diseases. There were 12 electronic databases and 3 trial registries searched up to November 30th, 2019. All randomized trials were eligible, regardless of language, date of publication, or settings. The primary outcomes were pain, shoulder function, and proportion of improved participants assessed within 12 weeks of randomization of the trial. The Cochrane risk of bias for the studies was assessed. Effects sizes were presented as a risk ratio, mean difference, or standardized mean difference with a 95% confidence intervals. Grading of Recommendations Assessment, Development and Evaluation approach was adopted to rate certainty of evidence. Of the 3,686 records screened, 28 randomized trials (2,216 participants) were included in this review. The types of acupuncture included manual acupuncture, dry needling, electroacupuncture, acupotomy, warm needle acupuncture, and fire needle acupuncture. All of the studies had an unclear or high risk of bias related to more than 1 domain. Significant benefits of acupuncture in terms of pain and shoulder function were observed in all comparisons, however, the proportion of improved participants was not described in 2 comparisons. There was substantial heterogeneity among meta-analyzed trials. No serious harm was observed. For primary outcomes, the overall certainty of evidence was very low. There was very low certainty of evidence for the benefits of acupuncture for patients with rotator cuff diseases. The safety of acupuncture remains unclear due to the incompleteness of reporting. Future well-designed randomized trials with transparent reporting are required.
Objectives : The purpose of this study is to make nine classical needles understood easily through interpreting Nine Classical Needles in Rhymes. Methods : The principle of classification and clinical application of Nine Classical Needle was sorted out referring to "The beginning of Nine Classical Needles in Rhymes" and "The prescription and treatment of Nine Classical Needles in Rhymes" which is the chapter of "Golden Mirror of Medicine". Results & Conclusions : The Nine Classical Needles, which were designed to get harmonious interaction between human beings and the nature, have been representative medical appliance in Korean Medicine. The needles consist of shear needle, round-pointed needle, spoon needle, lance needle, stiletto needle, round-sharp needle, filiform needle, long needle, and big needle. Nine Classical Needles in Rhymes was formed in a poem format to help us easily memorize the essential contents of Nine Classical Needles. Objectives : To understanding biological mechanism of acupuncture therapy, we have proposed a hypothesis. First of all, there exists electric property in meridian and meridian point. Second of all, energy flowing in meridian is related with electric property. Third of all, there is electronic interaction between the operator who performs acupuncture therapy and the receiver who is given acupuncture therapy. Forth of all, acupuncture effects may depend on the electric capacity which is transferred between the operator and the receiver via acupuncture needle. Methods : Under the hypothesis, we studied the effects of electric change in $ST_{37}(+)\;and\;ST_{39}(-)$ generated by Tanchim (彈鍼) manipulation which was stimulated at $ST_{36}$ point. And compared with data on the changes of electric change from the hand of the operator during acupuncture stimulation. Electric charge induced via acupuncture needle from the operator may be important factor that causes the changes of electric change in meridian and acupoint in the receiver. Therefore we investigated the changes of electric charge induced by the operator using Maclab 400 by the following methods. The one was in stable electric circle condition and the other was in unstable electric circle condition. Results : In this experiments, the changes of electric change from the stimulation type of Tanchim manipulation performed in our lab condition in acupuncture therapy was induced at least three factor, one was the difference of bio-potentials between the operator and the receiver of acupuncture therapy, another was the depth of acupuncture insertion from the skin of the receiver, the other was an electromyogram of the receiver. Conclusion : The data imply that the first factor should make a capacitance current when the operator touched the acupuncture needle which was inserted in the receiver. Therefore, the results suggest that capacitance currents stimulus in electronic view may be important factor in acupuncture therapy between the operator and the receiver of acupuncture therapy.
Objective : hypothesis. Methods : Under the hypothesis, I studied the effects of electric charge in ST37(+) and ST39(-) generated by Apchim (押鍼) manipulation which was stimulated at ST36 point. And compared with data on the changes of electric charge from the hand of the operator during acupuncture stimulation. Electric charge induced via acupuncture needle from the operator may be important factor that causes the changes of electric charge in meridian and acupoint in the receiver. Objectives : Warm needling is a combined treatment technique of acupuncture and moxibustion. In this study, we aimed to find out the components related with the thermal stimulation of the warm needling and to provide basic data for the guideline of the warm needling technique in the clinic. Methods : In this study, we measured thermal change of 3% agarose phantom embedding K-type thermocouples in depths of 0, 1, 2, 4, 8 and 16 mm. The warm needling was performed with acupuncture needles of various specifications (0.50×30, 0.50×40, 0.30×30, 0.30×40, 0.20×30 and 0.20×40 mm). A linear regression analysis was performed to find out the major component and quantify the effectiveness of the thermal stimulation during warm needling. Results : As a result of the measurement of temperature change, we could observe the thermal change pattern from the surface of the phantom to the 16mm deep part of the phantom. The thermal pattern was similar among the needles of different specifications. The regression analysis pointed the distance between the moxa cautery and the skin surface as the main component for the thermal stimulation of the warm needling. Conclusions : The authors suggest considering the distance between moxa cautery and the skin rather than the diameter of the acupuncture needle in accordance to the result of the study.
Objectives : Electroacupuncture is widely used because of its diverse curative influence. However, its electrical safety is not guaranteed when applied on the human body. Therefore, in this study, we did research on the corrosiveness of electroacupuncture when applied on the body fluid. Methods : We did research using acupuncture needles which were made of STS304 or STS316 and which were coated or not coated in silicone. They were 0.25 mm in diameter and 40 mm in length. We immersed them to the depth of about 1cm in Hank's Solution and gave a stimulus in 120 Hz for 60 minutes. Then, we measured the pH of Hank's solution and mass of needles and observed their shape. Results : Acupuncture needles which were made of STS304 corroded more easily than those which were made of STS316 when they were applied on the human body. Acupuncture needles which were coated in silicone corroded much more easily than those which were not coated in silicone when they were applied on the body fluid. Conclusions : In this study, we did research on the corrosiveness of electroacupuncture when it is applied on the body fluid using several acupuncture needles. We made a conclusion that when acupuncture needles were applied on the body fluid, the coated one made of STS304 corrodes more easily than the uncoated one made of STS316. We hope that additional further researches on the effect of the corrosiveness of an acupuncture needle will be carried out.
Sinpyeonjipsungmauibang is the classic on medical studies on horses written on the seventh year of Taejo of Chosun Dynasty, or the year of 1399. It was written with practical purpose, and it is the collection of medical studies on horses in our country from the last era of Koryo to the first era of Chosun. Acupuncture treatment in curing the disease of horses has much importance in this book. Most of the acupuncture was quoted from the classic Simuanjiji"司牧安驥集" from the Dang Dynasty, China. However, the contents were selected to only include what we need, and it was supplemented with our own experiences and explanations to be put into practical use. The idea of acupuncture point and meridian can be found in the book; it is, however, not as clear and complete as the current system. Also, the needle used in the book was usually the "needle with fire(火針)" and "method of burning(烙法)", and the "method of bloodletting(瀉血法)" was used often as well. The method of moxibustion was used as supplement for acupuncture.
Objectives : This study was performed to investigate the effect of invasive laser acupuncture therapy with green light (LAT-G) at HT7(Sinmun), SP3(Taebaek) on the blood pressure in hypertensive rat induced by two kidney one clip. Methods : The experiments were performed on Sprague Dawley rats. 2K1C hypertension model was prepared by constricting the left renal artery with a sliver clip. Animals were divided into four groups, which were simple acupuncture treatment group with straight needle insertion on the square(AT-SS), acupuncture treatment group with reducing manipulation in the opposite channel direction(AT-RD), laser acupuncture treatment group with green light 532 nm, 10mW power in the opposite channel direction(LAT-G10) and laser acupuncture treatment group with green light 532 nm, 20mW power in the opposite channel direction(LAT-G20). The treatments were performed once per two days for 10 days. Results : Body weight was increased significantly in LAT-G20 group compared with AT-RD group. The blood pressure was significantly decreased in LAT-G20 and LAT-G10 groups compared with AT-SS group. Conclusions : These results suggest that green laser acupuncture therapy at SP3 ${\cdot}$ HT7 is more effective than straight needle insertion on the square for controlling hypertension. It is possible that invasive green laser acupuncture therapy (532 nm) can be used as a reducing method of the traditional acupuncture techniques.

Objective : The aim of the study was to compare the therapeutic effect of the superficial and in-depth insertion of acupuncture needles in the treatment of patients with ankle sprain. Design : A prospective randomized single-blind study of superficial and deep acupuncture was conducted. Setting : The study was conducted in the Kang-nam Oriental Hosp. of Dong-Guk University. Patients: The study comprised 14 patients with ankle sprain who were divided into two groups (A and B). Intervention : In group A, the needle was introduced in the skin at a depth of 2 mm, whereas in group B the needle was placed deeply into muscular tissue or the articular capsule at a depth more than 1.5 Cm The treatment was planned for a duration of 1 week, 3 times. Outcome Measures : The intensity of pain was evaluated with the Ankle grade pain chart(AGPC) before and after treatment and at the 1-week follow-up examination. The AGPC includes Swelling, Tenderness, Exudation, Bruise, Medial/Lateral. stability test, Anterior drawer test, Squeeze test, Bearing weight, Pressure Algometer, Walking state and VAS. Results : Although at the end of the treatment there was no evidence of significant statistical differences between the two different groups, Pain reduction(VAS) was greater in the group treated with deep acupuncture. A statistical difference existed between the two groups at the 1 week follow up, with a better result in the deeply stimulated group in VAS. Conclusion : Clinical results show that deep stimulation has a better analgesic effect when compared with superficial stimulation in pain reduction(VAS, Visual analogic scale).
