A Survey of Actual State of Treatment with Acupuncture and Moxibustion in Korea (한국 침구 치료 현황 파악을 위한 설문조사)

  • Objectives : This survey was accomplished to find out how Korean medical doctors take acupuncture treatment in real clinics. Methods : The survey questions were developed by the consensus from the professors who major in acupuncture and moxibustion. The questionnaire was given the 2731 Korean medical doctors at the mending education site; In addition, it was given to 793 doctors working at the 105 Korean medical hospitals listed on the National Korean Medical Hospital Associations Address book by postal mail; and also it was given to the 142 public health care Korean medical doctors who attended the conference for the municipal and provincial representatives of the public health service Korean medical doctors. Total of 1277 questionnaires were retrieved out of the 3666 subjects. Results : 1. Nine hundred out of 1277 Korean medical doctors used method of differentiation syndromes(70.4%) and 1184 out of 1277 Korean medical doctors convalescence decide with subjective symptom improvement of patient(92.6%). 2. Nine hundred eight out of 1277 Korean medical doctors used both local and remote points(71.0%) and 916 out of 1277 Korean medical doctors treat with method of reinforcement-reduction(71.7%). 3. Eleven hundred fifth five out of 1277 Korean medical doctors used cupping a boil(90.3%) and 1023 out of 1277 Korean medical doctors used moxibustion(80.1%). Conclusion : When Korean medical doctors treat with acupuncture, most of them diagnose with differentiation methods and meridian theory, decide convalescence with subjective symptom improvement of patient, select out of both local and remote acupuncture points, treat with method of basic reinforcement-reduction, apply moxa and cupping a boil when they are necessary. The cupping a boil comes to be used when being muscle and joint disease. The moxa comes to be used when being chronic disease.

One Case of Systemic Lupus Erythematosus treated with traditional Korean Medicine (전신성 홍반성 낭창(SLE)환자 1예(例)에 대한 증례 보고)

  • After observing a patient diagnosed with Systemic Lupus Erythematosus from the September 21, 2000 to the January 13, 2001, the following results are obtained. Method and Result: We treated one case of Systemic Lupus Erythematosus with the Korean Bee Venom Therapy and the other Oriental Medical treatments. In acupuncture therapy, Korean Bee-venom Therapy is believed to be outstanding way to treat Systemic Lupus Erythematosus. Korean Bee Venom Therapy is treated on the following acupuncture points: ST36(B23(Shinsu:腎兪), BL26(Guanyuanshu:關元兪), ST36(Chok-Samni:足三里), LI4(Hapkok:合谷), LV3(Taechung), SP10(Hyolhae:血海) SP6 (Samumgyo;三陰交). In addition, CFC(Carthami Flos;紅花 and Cervi Pantotrichum Cornu;) Herbal-Acupuncture was applied on the acupucture points of GB20(Pungji: 風池), GB21(Kyonjong:), as well as BUM(It was made of Boviscalculus(牛黃), Moschus and Fel ursi Herbal-Acupuncture was given on CV17(Chonjung). In herbal medication, based on the Sasang Constitution, Taeyumin Chungsimyunja-Tang was given to the patient. As the results of these treatments, general conditions of this patient improved drastically. Conclusion: Based on the clinical results, traditional Korean Medical treatment is believed to be effective for treating Systemic Lupus Erythematosus, and futher studies should be carried out to provide more valuable information.

Review on the Point Location and Locating Method of GB18 (승령(GB18) 혈위와 취혈 방법에 대한 고찰)

  • Objectives : The GB18 belongs to one of the 92 controversial points in the development of WHO standard of acupuncture point location. The number of Chon measurement of GB18 is described differently in the A-B Classic of Acupuncture and Moxibustion (ChimGuGakEulKyung; AB Classic) and the Complete Compendium of Acupuncture and Moxibustion(ChimGuDaeSeong; Complete Compendium). The aim of the study is to review the point location of GB18 and compare the relation of the location of BL7. In addition, I would like to propose new locating method of GB18. Methods : In order to review I examined the expressions of the location of GB18 and the mapping location on the acupuncture chart in the landmark classic acupuncture literatures. Results : In the most literatures, the location of GB18 was described by the distance from the GB17. The distance was 1.5 chon with one exception of 1 chon. The intervals from GB15 to GB18 was 1 chon or 1.5 chon. Although the distance from the anterior hairline is the same as 4 chon, mapping points of GB18 and BL7 on the acupuncture chart were different. Conclusions : Consequently, in the AB Classic, meaning of the location is the actual distance of scalp surface. In the Complete Compendium, however, meaning of the location is the hypothetical distance in the imaginary scalp surface. Therefore, it is safe to find the GB18 on the same level with GV20 on the connecting line of bilateral auricular apexes. The relation of GB18 and BL7 should be reconsidered.

Mild stimulation of stomach 36 acupuncture point by organic nanoscale SP6 patch improves cellular physiologic functional status of different organs

  • Acupuncture works by stimulating peripheral sensory nerves and their endings causing an increase in cutaneous blood flow and microcirculation, as well as release of neurotransmitters, neuropeptides, and hormones. SP6 Patch is a nanoscale nontransdermal device that mildly stimulates Stomach 36 (zusanli) and other acupuncture points. As stimulation of these points has been indicated to have an effect on hypothalamic function, it is of great research interest to investigate the effect of SP6 Patch on the physiology of organs that are affected by hypothalamic regulation. Bioelectrical tissue impedance data indicative of cellular physiologic organ function, using an Electro Interstitial Scan (EIS) system, were acquired from hypothalamus, pancreas, liver, intestines, kidneys, thyroid and adrenal glands in 10 (1 male, 9 females) volunteers while wearing the SP6 Patch daily for 1 week. EIS testing was performed at baseline with no patch, 30 min after wearing the patch, and after wearing the patch 12 h/day for 1 week. Subjects were instructed to keep well hydrated during the study period. All subjects served as their own control. The hypothesis was: The SP6 Patch worn 12 h/day on the Stomach 36 acupuncture point for 1 week, may significantly improve cellular physiologic functional status of different organs measured by EIS. All tested organs achieved significant improvement in their functional physiologic status after wearing the SP6 Pach 12 h/day for 1 week compared to baseline with an overall average statistical power > 89%. Based upon these results the hypothesis was accepted as true.

A study of methods for Acupuncture and moxibustional treatment to Insomnia (불면(不眠)에 대한 침구치료(鍼灸治療)의 접근 방법론 연구 - 경락생리(經絡生理) 및 혈성(穴性)을 중심으로 -)

  • Objectives : This study was aimed to investigate the methods for acupuncture and moxibustional treatment to insomnia Methods : The insomnia is classified by seven cause at main subject. According to this, arrange the acupuncture and moxibustional treatment for classical books, and explain eight meridian related to insomnia. Results : 1. Insomnia means deficiency of sleeping and it is the word generally used when a short period of sleeping, difficulty for deep sleeping and difficulty for recovery of original vital energy comes out. In oriental medicine, they understand that uneasiness condition occurred by abnormal operation of the internal organs as heart, liver, gallbladder, spleen, stomach, kidney, etc. caused by external affection or internal injury becomes Insomnia. 2. Cause of insomnia can be classified majorly as fire-transformation of liver and gallbladder , inner shaking of phlegm-heat , insufficiency of both the heart and the spleen, disharmony between heat and kidney, the dysfunction of the stomach, timidity of heart and gallbladder, the deficiency of Qi of the lung. They make insomnia with bad influence upon body and spirit. 3. The meridian system such as Heart Meridian of Hand Soeum, Pericardium Meridian of Hand Gworeum, Stomach Meridian of Foot Yangmyeong, Spleen Meridian of Foot Taeeum, Bladder Meridian of Foot Taeyang, Kidney Meridian of Foot Soeum, Gallbladder Meridian of Foot Soyang, Liver Meridian of Foot Gworeum and Lung Meridian of Hand Taeeum are used to treat insomnia. Sinmun acupoint and Naegwan acupoint are often used in particular because the they are good for calmming the spirit, the heart and purging the heart of (pathogenic) fire. 4. Especialy, Back-Su points of Bladder Meridian of Foot Taeyang was used by each causes. The Back-su Points was mainly used for heal the insomnia because the Back-Su points has good competent to control the ability of internal organs by direct effect to it. It is because the vitality flows through around back-Su. Conclusions : It comes to a conclusion as follows with research for relevancy of the main cause of insomnia and meridian system includes meridian point.

The Distinctive Feature of Acupuncture Treatment in ZhangJinYoPian(藏珍要編) (19세기 조선 침구서 장진요편의 침법 연구)

  • Objective: ZhangJinYoPian(藏珍要編, ZJYP) is a book witten by Song-WooGe in the late 19th century. While Korea has lost this book to tell us Korea traditional acupuncture treatment in the late 19th century, This book appeared as a manuscript in Japan. There is no reaserch or study carried out for this book. Furthermore, a few people know existence of this book. So I analyzed acupuncture treatment of the ZJYP and sort out the distinctive feature of it. Method: For that, I studied bibliographic information of this book and classified contents of it into three parts - medical thought, needle manipulation and acupoint selection to consider it's caracteristics. Results and Conclusions: 1. The author of ZJYP thought that weakness of good power in the body and strongness of evil power out of the body cause a disease. so Doctor should examine quality of both to care patient's pain. 2. The author of ZJYP thought that needle manipulation is one of the most important thing. Doctors can use needle manipulation to control patient condition. Especially, he suggested cooling and heating manipulation except reinforcement and reduction. 3. He stressed viscera and bowels. he located viscera and bowels theory in front of the book to explain relationship of these. It shows us that ZJYP maintained academic characteristics of acupuncture in Chosun-Korea. 4. While He minimized the number of acupoints used in treatment, he multiplied its combination. He selected one or two acupoints from each meridian pathway. It include Eight Confluent points(八脈交會穴). But these points were used in different ways to control the viscera and bowels, not to care the eight extra meridians.

The network analysis of acupoints for primary dysmenorrhea (원발성 월경곤란 치료를 위한 경혈 및 경혈 조합의 네트워크 분석)

  • Objectives: Primary dysmenorrhea is a representative disease that causes intestinal pain, and it has long been known that acupuncture treatment is effective. In this study, a network analysis was conducted using acupuncture points reported in the existing systematic literature review, and the combination of acupuncture points used to treat primary dysmenorrhea was analyzed. Methods: Among of a total of 60 papers, excluding those that used ear acupuncture, the analysis results indicated that combinations of BL23, BL40, BL25, BL60, BL30, Ashi-points, BL26, GB34, and GV3 were used in combination. Results: Through this, it can be seen that previous studies to reveal the effectiveness of acupuncture on back pain mainly used the acupoints of the bladder meridian (BL), local acupoints group, and distal acupoints group. Conclusion: In order to reveal the correlation between acupoints and therapeutic effects through future studies and to provide guidelines on the principles of selecting and combining acupoints, it is necessary to employ a quantitative analysis method for therapeutic effects and to provide detailed reports on acupoints.

Study on the Treatment Mechanism of Back-Shu Points for Organ Dysfunction (배수혈의 내장기 치료 기전에 관한 연구)

  • Objectives : This study aims to overview the therapeutic mechanism of back-shu points in terms of sympathetic visceral motor nervous system. Methods : Studies about autonomic nervous system, and studies and ancient texts about back-shu points were reviewed. We interpreted possible mechanism of back-shu points considering similarities of anatomical and physiological characteristics of back-shu points and visceral motor nervous system. Results : Afferent signals for organ lesions that can develop the symptoms of autonomic neurological symptoms, pain, hyperalgesia through the skin segment. Through a physical examination of the myotome and dermatome, it is possible to diagnose segmental disorders. Treatment stimulation of the thick fibers of the disorder segment skin can reduce abnormal autonomic influence over the sympathetic reflex mechanism. In addition, if spinal muscles are relaxed, the pressure on the nerve roots could be reduced and consequently the hyperactivity of the sympathetic visceral motor signal would be suppressed. Conclusions : The back-shu points treatments work through the mechanism of the sympathetic nervous reflex. Moreover, segmental acupuncture can reduce tension of the spinal muscles, thereby improving pathological conditions of the sympathetic nervous system.

  • Myofascial pain syndrome is one of the major cause of chronic pain and trigger point injection, stretching, spray and electrical therapy are often used in clinical situation for treatment of myofascial pain syndrome. Myofascial pain syndrome is characterzied by the existence of a hypersensitive region, called the trigger point in a muscle or in the connective tissue, together with palpable noble, stiffness, limitation of motion and referred pain when trigger point is stimulated. Physiologically, they represent a self-sustaining vicious cycle of pain-spasm-pain. The purpose of this study is to illustrate mechanisms of pain by stimulation of acupuncture and trigger point, to introduce clinic application of orient and western stimulative point (acupuncture, trigger point)for treatment of MPS(myofascial pain syndrome), to make physiotherapist use both stimulative points for treatment of MPS.

  • In this paper, we describe a millimeter-wave radiation system developed for stimulating acupuncture points, and an analyzing system developed for monitoring the change of physiological signals after the stimulation such as ECG, skin temperature, skin potential and skin resistance. The systems are to be used to investigate the treatment efficacy and biological effects of the millimeter-wave, and eventually, can be used to study the acupuncture meridian system theory in the traditional Korean medicine.
