A Case Report of Hemiparesis and Hypesthesia in a Patient with an Intracerebral Hemorrhage at the Basal Ganglia Treated with Traditional Korean Medicine (기저핵 뇌출혈로 인한 반신부전마비, 감각장애에 대한 한방치험 1례)

  • Objectives: The aim of this study is to describe the effects of traditional Korean medicine treatment on a patient with an intracerebral hemorrhage (ICH) at the left basal ganglia. Methods: The patient was treated with herbal medicine (Gamicheongsim-tang, Hwangryunhaedok-tang-tablet), acupuncture, moxibustion, and rehabilitative therapy. The effects of these treatments were evaluated using the manual muscle test (MMT), sensory scale, Korean version of the modified Barthel index (K-MBI), and the National Institutes of Health Stroke Scale (NIHSS). Results: After treatment, the MMT grade improved from 3+/3+ to 4+/4+, and the sensory scale increased from 40%/40%/40% to 80%/80%/80%. In addition, the K-MBI score improved from 56 to 100, and the NIHSS score decreased from 7 to 1. Conclusion: The results suggest that traditional Korean medicine can be effective in the treatment of patients with ICH.

Comparative Study on Treatment of Acne Scar (여드름 흉터의 치료방법에 대한 비교연구)

  • Objective : This study performed to evaluate that duration of acne scars effects on Qualitative Global Acne Scarring Grading System(QGASC) grade changing and treatment duration. Also this study performed to evaluate the relationship between dysmenorrhea and acne scars, and the effects of each treatment on treatment duration and Korean Acne Grading System(KAGS) and QGASC grade.Methods : 95 patients with acne scars were understood by medical records and case photos, and we evaluated pre and post acne scar grade based on KAGS and QGASC. We analyzed data by SPSS 22 for windows program, and figured out the relation between duration of acne and QGASC grade and treatment duration by Spearman correlation analysis, and performed Mann-Whitney U-test to figure out the relation between dysmenorrhea and QGASC grade. We performed Wilcoxon test to evaluate that treatment of acne scars effects on KAGS grade and QGASC grade, and performed Kruskal-Wallis test to figure out the difference of each treatment which effects on KAGS grade and QGASC grade, and performed Post-hoc analysis to figure out the group comparison.Results & Conclusions : QGASC grade depending on duration of acne was statistically significance. Subcision and Microneedle Therapy System(MTS) and pharmacopuncture were significantly improved acne scars. Other treatments were significantly more effective than subcision and pharmacopuncture combined treatment. Treatment duration of Subcision and MTS combined treatment was significantly most shortest than other treatment.

A Narrative Review of Carpal Tunnel Syndrome in Traditional Chinese Medicine: Using China National Knowledge Infrastructure Database (수근관 증후군의 최근 6년간의 연구 동향 분석: China National Knowledge Ingrastructure (CNKI) 보고 연구를 중심으로)

  • Objective The purpose of this study was to investigate clinical studies between 2015 and 2020 on the traditional Chinese medicine treatment for carpal tunnel syndrome (CTS) using the China National Knowledge Infrastructure (CNKI) database. Methods We searched clinical studies on the Chinese medicine treatment published in traditional Chinese medical journals on CTS through the CNKI search. Fifteen published studies between 2015 and 2020 were chosen and examined for the year of publication, sample size, study design type, diagnosis and evaluation of diverse criteria, treatment period, purpose of the study, and classification based on the treatment technique. Results Out of 73 studies, 15 had practiced various Chinese medical treatments, such as acupuncture, moxibustion, manipulation, herbal medicine application therapy, and external herbal treatment. Conclusions Various studies have investigated the Chinese medicine treatment for CTS in China. To envelope the treatment of CTS in Korean medicine, steady and valuable therapies for high-level clinical research are required. In the future, this study could be helpful in the development of the treatment of CTS with Korean medicine.

Carthami Semen Pharmacopuncture Combined with Electroacupuncture on Carpal Tunnel Syndrome: A Retrospective Case Series Study

  • While carpal tunnel syndrome (CTS) is the most common entrapment neuropathy affecting the wrist, resulting in substantial physical, psychological, and economic effects, there is no gold standard therapy for CTS. In this case series study, we aimed to report CTS patients treated with Carthami Semen Pharmacopuncture (CSP) and electroacupuncture (EA) showing improvements in their symptoms, and the combinatorial effects of CSP and EA. We collected medical records of CTS outpatients who received CSP and EA at Chuku Acupuncture & Moxibustion Korean Medicine Clinic from August 2017 to September 2018. The outcome measures were the visual analog scale (VAS) for pain, paresthesia, the Korean version of the Boston carpal tunnel questionnaire (K-BCTQ) score, and changes in nocturnal pain, Tinel sign, and Phalen's test. We included patient satisfaction at the completion of all treatments. 17 patients were included for this case series study. After treatment, VAS for pain decreased significantly from 50.41 ± 16.19 to 9.59 ± 9.46, VAS for paresthesia also decreased significantly from 63.50 ± 11.49 to 14.75 ± 12.97, and K-BCTQ symptom severity scale decreased from 2.48 ± 0.68 to 1.89 ± 0.70 (all p < 0.001). Nocturnal pain, Tinel signs, and Phalen's test showed improvements after all the treatments. All the patients reported favorable overall satisfaction with the treatments, and 69.23% wanted future pharmacopuncture treatments if CTS recurred. No complications were detected. The combination of CSP and EA could be an effective and safe option in treating CTS.

A Case Report of a Subarachnoid Hemorrhage Patient whose Paresthesia Improved by Ssanghap-tang (쌍합탕 투약 후 수족부 저림증이 호전된 지주막하출혈 환자 증례보고)

  • ■ Objectives This case is to report the effect of Ssanghap-tang on paresthesia of a subarachnoid hemorrhage patient. ■ Methods A male Korean patient was treated with Ssanghap-tang and acupuncture, moxibustion for total 30 days. We observed limbs numbness, circumferences, other symptoms such as weakness and any adverse event. ■ Results After treatment, numeric rating scale of limb numbness was improved to 5 from 8 at admission without any adverse event. However, we could not find any significant differences on hand circumference and limb weakness. ■ Conclusion This case suggests that Ssanghap-tang and acupuncture, moxibustion therapy could be effective in improving paresthesia of subarachnoid hemorrhage patient.

A clinical study on a patient with hypothyroidism (생간건비탕가미방(生肝健脾湯加味方)을 이용한 갑상선기능저하증 치험1예(例))

  • Objectives : Hypothyroidism is a common endocrine disorder in which the thyroid gland does not produce enough thyroid hormone. It can cause a number of symptoms, such as tiredness, poor ability to tolerate cold, and weight gain. The purpose of this study was to report the clinical effects of herbal medicine on hypothyroidism. Methods : We employed oriental medical treatments; herbal-medication (Saenggangeonbi - tang gamibang), acupuncture and moxibustion. At the same time, the patient started to exercise. We treated the patient two or three times a week with oriental therapy method. She took medicine three times a day after a meal. During taking medicine, we let the patient avoid fatty food, flour based food, and alcohol. The symptoms and normalization of the thyroxine and TSH levels are important points of evaluating the patient's condition. So the patient measured the body weight and took a blood test a time per two months and compared the results with previous results. Results : After taking treatment - acupuncture and moxibustion during 6 months - and taking herbal-medicine, the level of TSH, fT4, T4 and T3 became normalized. The body weight was decreased about 18 pounds. In advance, the symptom of tiredness, edema was much improved. Conclusion : Herbal medicine (Saenggangeonbi-tang gamibang) was effective in the treatment of hypothyroidism and it helped to normalize the level of TSH, fT4, T4 AND T3.

Three Cases of Postpartum Hyperhidrosis Patients Treated with Korean Medical Treatment Including Sanhuhyulpung-tang (산후혈풍탕을 포함한 한방치료로 호전된 산후다한 치험 3례)

  • Objectives: The purpose of this study is to report the effects of Korean medical treatment including Sanhuhyulpung-tang on three postpartum patients who suffered from hyperhidrosis. Methods: We treated three patients complained of postpartum symptoms including hyperhidrosis. The patients were treated by Traditional Korean Medicine including herbal medicine, acupuncture, moxibustion, and cupping therapy. Sanhuhyulpung-tang is a herbal medicine prescription which is used in clinical practice for the treatment of postpartum sweating, anxiety, flushing, and joint pain. Three patients were hospitalized for treatment for 6, 8, and 19 days respectively. During hospitalization, patients took Sanhuhyulpung-tang three times a day, and acupuncture, moxibustion, and cupping were performed once a day. To evaluate the degree of improvement, we used VAS score of the patient's chief complaint during hospitalization. Results: After the treatment, symptoms including hyperhidrosis were improved. The degree of excessive sweating decreased to 30% in two patients, and 55% in one patient. Combined symptoms including lower coldness, burning sensation, chilling, and multiple site pain also improved on discharge day. Conclusions: This case report shows that Korean medicine treatment including Sanhuhyulpung-tang can improve postpartum symptoms including hyperhidrosis.

A Case Report of Quadriparesis due to Subarachnoid Hemorrhage with Intraventricular Hemorrhage due to Cerebral Aneurysm Rupture Improved after Treated with Korean Medicine Treatment Including Tonggyuhwalhyeol-tang-gagambang (통규활혈탕가감방을 포함한 한의복합치료로 뇌동맥류 파열에 의한 뇌실내출혈을 동반한 지주막하출혈 환자의 사지마비에 개선을 보인 증례보고 1례)

  • This study reported the case of a patient with quadriparesis due to cerebral aneurysm rupture, subarachnoid hemorrhage, and intraventricular hemorrhage (IVH) treated with Korean medicine. The patient was treated with acupuncture, herbal medicine (mainly Tonggyuhwalhyeol-tang-gagam), Western medicine, moxibustion, cupping, and rehabilitative therapy for 75 days, and improved after administration. After treatment, the Manual Muscle Test grade improved from 4/4-/3+/3 to 4+/4+/4+/4+, the Korean version of the modified Barthel Index score improved from 9 to 100, the National Institute of Health's Stroke Scale score improved from 2 to 0, the Global Deterioration Scale score improved from 3 to 2, and the Korean version of the Mini-Mental State Examination score improved from 22 to 30. During administration, the patient did not show seizures, shock, or loss of consciousness, and the vital signs were stable in the normal range. We followed up the brain computed tomography findings for 2 times and found that there was no definite evidence of intracranial hemorrhage or IVH or re-rupture or rebleeding after Korean medicine treatment. This study suggests that Korean medicine treatment with blood-invigorating and stasis-removing herbs could be a safe and effective intervention option for improving quadriparesis due to cerebral aneurysm rupture and subarachnoid hemorrhage.

Meridian Massage Therapy for Treating Constipation in Patients with Stroke: a Systematic Review (중풍 환자의 변비에 대한 경락 마사지의 효과: 체계적 문헌 고찰)

  • 목적 : 이 연구의 목적은 중풍 환자의 변비에 대한 증상완화요법으로서의 경락 마사지에 대해 그 임상적인 근거를 평가하기 위함이다. 방법 : 본 연구에서는 언어적인 제한 없이 12가지의 데이터베이스에서 개시부터 2010년 4월까지의 문헌을 검색했다. 변비가 있는 중풍 환자를 경락 마사지로 치료한 모든 형태의 임상 연구를 대상으로 하여, 플라시보 대조군 연구나 약물요법 또는 무처치 대조군 연구의 경우를 본 연구에 포함하였으며, 증례 보고나 사례군 연구는 제외하였다. 모든 임상 연구의 방법론적인 질은 Cochrane의 risk of bias analysis를 이용하여 평가하였다. 결과 : 전체적으로 세 편의 비무작위 배정 대조군 연구와 한 편의 비대조군 관찰 연구가 포함되었다. 모든 연구들은 높은 수준의 risk of bias를 보였다. 세 편의 비무작위 배정 대조군 연구들은 경락 마사지의 효과를 무처치군과 비교하였는데, 중풍 환자의 변비에 대해 경락 마사지의 긍정적인 효과를 나타내었으며, 한편의 비대조군 관찰 연구 역시 긍정적인 결과를 보였다. 하지만 모든 연구들은 bias에 노출되어 있었다. 결론 : 경락 마사지가 중풍 환자의 변비에 대해 효과적인 치료법이라는 것을 보여주기에는 현재의 근거가 부족하며, 모든 연구들은 높은 수준의 risk of bias를 가지고 있기 때문에 더욱더 엄격한 연구가 요구된다.

Case report: Dizziness due to Hippocampal Sclerosis (해마경화증 환자의 어지럼증 치험 1례)

  • Objectives: The aim of this study was to evaluate the effect of Korean medicine on a patient with dizziness due to hippocampal sclerosis. Methods: A 51-year-old woman with dizziness, diagnosed with hippocampal sclerosis, was treated with herbal medication, acupuncture, and moxibustion. Her improvement was evaluated by a balance test, NRS, and the percentage of dizziness severity compared to that on her admission day. Results: Improvement in the balance test result and NRS, and a decrease of percentage of dizziness severity, were observed after treatment with Korean medicine. Conclusions: This study suggests that treatment with Korean medicine may be an effective therapy for dizziness due to hippocampal sclerosis.
