Change of Recommended Energy Intake for Korea (한국인의 에너지 섭취권장량 변화)

  • This research examined the amounts and methods change of recommended energy intake(REI) from 1962's recommended dietary intakes for Korean to 2010's dietary reference intakes for Koreans. REI is composed of 3 factors, such basal metabolic rate(or Resting Energy Expenditure, REE), activity energy, and thermogenic effect of foods. The first 1962 calculation formula of REI was weight based formula, that of 95's was the weight based REE multiplied by activity coefficient, and the recent one of 2005's(Estimated Energy Requirement: EER) was age, height. weight, and the activity level applying formula derived from the energy expenditure data by doubly labeled water technique(DLW). During the 50 years or so, REIs were reduced in all age groups, according to the activity(labor) strength and hour were reduced. The individual REI calculation method was introduced in 1995, and individual REI calculation was recommended since to prevent obesity. For the better REI estimation for Koreans, REI calculation formula derived from our peoples' DLW energy expenditure data is required.

A survey on daily physical activity level, energy expenditure and dietary energy intake by university students in Chungnam Province in Korea (충남지역 대학생의 신체활동수준, 에너지소비량 및 에너지섭취량 조사)

  • This study investigated the daily physical activity level, energy expenditure, energy balance, and body composition and their relationship with university students. The participants were 130 male students ($19.5{\pm}0.5$ yrs) and 139 female students ($19.5{\pm}0.3$ yrs) at a university in Chungnam province. Physical activity level was evaluated by an equation based on 24 hr-activity record and dietary nutrient intake was evaluated using the food record method during a three-day period consisting of two week days and one weekend. Body composition was measured using Inbody 430 (Biospace Co., Cheonan, Korea). As a result, mean body mass index (BMI) of subjects indicated that they had normal weight, however mean body fat ratio was $19.1{\pm}5.4%$ for males and $28.4{\pm}5.0%$ for females, indicating that they had higher than normal weight. Daily mean physical activity level was 1.55 for males and 1.47 for females, which was regarded as 'low active', respectively. Females had more light activity than males (p<0.01). Daily mean energy expenditure was $2,803.5{\pm}788.9$ kcal/d for males and $1,915.4{\pm}510.2$ kcal/d for females (p<0.001). Daily mean dietary energy intake was $2,327.0{\pm}562.5$ kcal/d for males and $1,802.1{\pm}523.6/d$ for females (p<0.001), and daily mean energy balance was $-476.5{\pm}955.9$ kcal/d for males and $-113.3{\pm}728.1$ kcal/d for females (p<0.01). Daily mean dietary intake of protein, vitamins, and minerals, except Ca, satisfied recommended nutrient intake. Daily energy expenditure was positively related to body weight (p<0.01), BMI (p<0.01), and fat free mass ratio (p<0.05), but was negatively related to body fat ratio (p<0.01). In conclusion, subjects had a negative energy balance and low physical activity. They had a normal weight by BMI but had a more fat than normal weight by body fat ratio. This appears to be related to their low physical activity. Thus, nutrition education should be provided for university students in order to increase their physical activity for maintenance of normal weight by body composition and health promotion.

A Comparative Study of Relationships among Energy Intakes, Energy Expenditure, Physical Activity and Cardiovascular Disease Related Factors in Vegetarians and Non-Vegetarians (채식인과 비채식인의 섭취열량, 소비열량 및 활동량과 심혈관질환 관련인자와의 관련성에 관한 비교연구)

  • The purpose of this study is to verify the relation among vegetarian diet, physical activity and the risk factors of cardiovascular disease. Two hundred and forty five buddhist nuns (age : 23~79 yrs) were selected as vegetarians. For control subjects, 235 healthy female adults (age : 23~79 yrs) were selected. Study period was from October 1996 to February 1997. The contents are consist of food consumption survey, anthropometric measurement, amount of energy expenditure, physical activity and clinical examination. Results were summarized as follows : The average ages of vegetarians were $44.2{\pm}17.3\;yrs$ and those of non-vegetarians were $40.5{\pm}18.4\;yrs$. Average body mass indices (BMI) of vegetarians and non-vegetarians were $22.5{\pm}2.7\;and\;21.1{\pm}2.5$, WHRs were $0.9{\pm}0.1\;and\;0.8{\pm}0.1$, and the average duration of vegetarian diet of the vegetarians was 13.1{\pm}12.9 years. All the nutrient intakes of both groups were over RDA's except calcium and vitamin A intakes of non-vegetarians that were below RDA's Fiber and vitamin C intakes of the vegetarians were significantly higher than those of non-vegetarians (p<0.01). Average energy intakes of vegetarians and non-vegetarians were $2177.1{\pm}420.6\;kcal/day\;and\;2103.2{\pm}534.9\;kcal/day$, total daily energy expenditure were $1789.9{\pm}325.8\;kcal/day\;and\;1703.8{\pm}344.8\;kcal/day$, those by physical activity were $507.8{\pm}360.9\;kcal/day\;and\;400.0{\pm}247.8\;kcal/day$. Serum lipid level, blood sugar, systolic blood pressure of vegetarians were significantly lower than those of non-vegetarians, but ratio of HDL-cholesterol/total-cholesterol was higher than those of non-vegetarians. Levels of total serum cholesterol of vegetarians and non-vegetarians were 161.4{\pm}33.3 mg/dL and 189.6{\pm}33.6mg/dL, HDL-cholesterol were $48.2{\pm}11.4\;mg/dL\;and\;50.8{\pm}12.2\;mg/dL$, LDL-cholesterol were $86.6{\pm}26.9\;mg/dL\;and\;111.1{\pm}17.0\;mg/dL$, atherogenic indice(AI) were $2.4{\pm}0.6\;and\;2.9{\pm}0.7$, fasting blood sugar were $90.8{\pm}14.3\;mg/dL\;and\;103.6{\pm}24.6\;mg/dL$, systolic blood pressure were $107.5{\pm}14.9\;mmHg\;and\;119.3{\pm}15.3\;mmHg$. Energy expenditure by physical activity was negatively correlated with levels of serum total-cholesterol and LDL-cholesterol and atherogenic indice(AI). Therefore, physical activity and vegetarian diet effectively helped reduce blood lipids related to cardiovascular disease.

Energy expenditure measurement of various physical activity and correlation analysis of body weight and energy expenditure in elementary school children (일부 초등학생의 대표적 신체활동의 에너지소비량 측정 및 에너지소비량과 체중과의 상관성 분석)

  • Purpose: There is a lack of data on the energy cost of children's everyday activities, adult values are often used as surrogates. In addition, the influence of body weight on the energy cost of activity when expressed as metabolic equivalents (METs) has not been vigorously explored. Methods: In this study 20 elementary school students 9~12 years of age completed 18 various physical activities while energy expenditure was measured continuously using a portable telemetry gas exchange system ($K_4b^2$, Cosmed, Rome, Italy). Results: The average age was 10.4 years and the average height and weight was 145.1 cm and 43.6 kg, respectively. Oxygen consumption ($VO_2$), energy expenditure and METs at the time of resting of the subjects were 5.41 mL/kg/min, 1.44 kcal/kg/h, and 1.5 METs, respectively. METs values by 18 physical activities were as follows: Homework and reading books (1.6 METs), playing game with a mobile phone or video while sitting (1.6 METs), watching TV while sitting on a comfortable chair (1.7 METs), playing video game or mobile phone game while standing (1.9 METs), sweeping a room with a broom (2.7 METs) and playing a board game (2.8 METs) belong to light intensity physical activities. By contrary, speedy walking and running were 6.6 and 6.7 METs, respectively, which belong to high intensity physical activities over 6.0 METs. When the effect of body weight on physical activity energy expenditure was determined, $R^2$ values increased with 0.116 (playing a game at sitting), 0.176 (climbing up and down stairs), 0.246 (slow walking), and 0.455 (running), which showed that higher activity intensity increased explanation power of body weight on METs value. Conclusion: This study is important for direct evaluation of energy expenditure by physical activities of children, and it could be used directly for revising and complementing the existing activity classification table to fit for children.

Search of hemodialysis nursing behaviors and Estimation of hemodialysis nursing costs at a tertiary hospital (일개 3차 의료기관의 혈액투석 간호행위규명 및 간호원가 산정)

  • The purpose of this study is searching for hemodialysis nursing bahaviors by hemodialysis room nurses and analyzing them. Then, it estimates hemodialysis nursing costs and obtains basic data for development of proper nursing costs. First, it searched for hemodialysis nursing behaviors at a tertiary hospital hemodialysis room in Seoul and classified them. After the content validity was verified by 6 experts, Tool of hemodialysis nursing behaviors was developed. patients who recived hemodialysis were classified by dialysis patient classification tool. The searcher observed hemodialysis nursing behaviors applied to classified patients per 5 minutes. Then hemodialysis nursing hours spent to classified patients were calculated respectively. The direct expenditures and indirect expenditures were estimated. Ultimately, hemodialysis nursing costs were estimated. The results of the study were as follows ; 1. hemodialysis nursing behaviors were grouped by the same knowledge and skills. then, the content validity of them was verified by evaluation tool of nursing intervention classification by expert groups. They consisted of 9 hemodialysis activity domains and 71 hemodialysis nursing behaviors. The predialysis activity domain included 15 nursing behaviors, the activity domain of start-dialysis included 12 nursing behaviors, the activity domain of during- dialysis included 9 nursing behaviors, the activity domain of finish-dialysis included 5 nursing behaviors, the activity domain of after-dialysis included 5 nursing behaviors, the nursing documentation & undertaking and transfering included 5 nursing behaviors, the supply, drug, equipment & environment management activity domain included 7 nursing behaviors, the patient emotional support & education activity domain included 4 nursing behaviors, the emergency activity domain included 9 nursing behaviors. 2. The acute hemodialysis nursing hours were 106.42 minutes per a dialysis and the chroni hemodialysis nursing hours were 72.23 minutes per a dialysis. 3. The direct expenditure was 11.971 won per hour and indirect expenditure was 288won. 4. Finally, the cost of acute hemodialysis was 21,745 won and that of chronic hemodialysis was 14,759 won. By search of hemodialysis nursing behaviors, they will be used as hemodialysis nursing care standard and will be tended toward high qualitative care. Estimation of hemodialysis nursing costs will be used as fundamental data for development of proper nursing costs.

The Impact of Technology Innovation Activity on Managerial Efficiency: An Inverted U shaped Model (기술혁신활동이 경영효율성에 미치는 영향 : Inverted U Shaped 모형)

  • Purpose: This study addressed the relationship between technological innovation activity and management efficiency of Korean automobile firms. We tested the hypothesis of non-liner relationship of innovation activity in relation to management efficiency. Methods: We discussed prior literature in the firm innovation strategy and management efficiency studies to provide better understanding of relationships between technological innovation activity and management efficiency. As a result, we developed develop and tested a model (Inverted-U shaped) capturing the non-linear impact of technological innovation activity. While we used R&D expenditure and patent registration data for measuring firms' innovation activity, management efficiency was evaluated by using DEA(Data Envelopment Analysis). Results: Main findings of our empirical analysis indicated that the relationships between technological innovation activity and management efficiency was inverted U shaped. This implied that the relationship between technological innovation and management efficiency is inverted U-shaped non-linear, with management efficiency increasing up to a point, beyond which higher levels of R&D and patent registration activities led to a decrease in management efficiency. Conclusion: This study empirically assessed the inconclusive findings of previous research in the area of effects of innovation activities in relation to firm performance. The paper also provided theoretical and practical implications for firms who explore efficient strategy to promote the management performance through technological innovation activities. Future research directions with the limitation of the study was discussed.

Determinants of the Utilization of Oriental Medical Services by the Elderly (한방의료 이용현황 및 이용결정요인에 관한 연구 - 고령화 패널을 이용하여 -)

  • Objective: This study aimed to analyze the utilization of Oriental medical services and its determinants among the elderly. Method: Data from a Korean longitudinal study of aging was used. Regression analysis was used to find the determinants of the utilization of medical care. Results: People with low education and low income were more likely to use Oriental medical services. Determinants of using Oriental health service were sex, marriage, income, subjective health condition, activity restriction due to pain, and chronic disease. Among them, only subjective health condition and activity restriction due to pain were significant determinants of frequency of and expenditure on Oriental medical services. Especially, activity restriction due to pain was a significant factor in the use of Oriental medical services, but not in the use of Western medical services. Conclusion: Treatment related to pain was closely associated with Oriental medical services. These treatments need to be developed with scientific and clinical evidence.

Serum 25-Hydroxyvitamin D Status and Associated Factors in Premenopausal Working Women (폐경전 성인직장여성의 혈청 25-Hydroxyvitamin D상태 및 관련인자에 관한 연구)

  • This study was performed to estimate serum 25-hydroxyvitamin D level and to evaluate the relationship between serum 2S-hydroxyvitamin D level and associated factors. The subjects were 61 premenopausal working women aged 30 - 49 y in Busan. The serum 25-hydroxyvitamin D level was measured by radioimmunoassay. Data for physiological characteristics, lifestyle factors, physical activity and nutrient intake were assessed by questionnaire including information about outdoor activity time, daily activity diary and 24 hr recall method. The mean vitamin D intake was 3.12 ug, which corresponded to $62.5\%$ of the Korean RDA. The mean level of serum 25-hydroxyvitamin D was 31.0 ng/mL. Low 25-hydroxyvitamin D (<25 nmol/L) was not found in the subjects. The serum 2S-hydroxyvitamin D level showed positive significant correlations with exercise hours, daily energy expenditure, hours of outdoor activity per weekdays (p < 0.001, P < 0.05, P < 0.05). Exercise hours were found to be the most important determinant of serum 25-hydroxyvitamin D level. Therefore nutritional education for increasing hours of physical activity including indoor and outdoor exercise, is needed for premenopausal working women to increase vitamin D status.

Dietary calcium intake, physical activity, and bone mineral density in elderly men (노년기 남성의 칼슘 영양 상태, 육체적 활동량과 골격 대사에 관한 연구)

  • The relationships between nutrients intake, physical activity and bone mineral density were investigated in 19 elderly men aged 71-80 years. A trained nutritionist interviewed usual dietary intake and daily activity with a questionnaire, and bone mineral density was measured at the lumbar spine and three regions of the proximal femur (femur neck, Ward's triangle and trochanter) with a Luna DP3 dual photon absorptiometry. The correlations between dietary calcium intake and bone mineral density at the lumbar spine and trochanteric region were significant at P<0.05 and P<0.01 level respectively. the significant correlations were also found between vitamin A(P<0.005), riboflavin(P<0.01), and ascorbic acid(P<0.05) intake and bone mineral density at these sites. Higher physical activity was associated with greater bone mineral density of four sites, but this was not significant. But there were significant relationships between total energy expenditure and bone mineral density of the lumbar spine(P<0.01), femur neck (P<0.05) and Ward's triangle(P<0.05). In this study the results revealed that bone mineral density of the lumbar spine and trochanteric region were associated with dietary calcium intake. And bone mineral density of the femur neck and Ward's triangle were related to physical activity but not to nutrients intake. In conclusion, dietary calcium intake seems to be a important factor for greater bone mineral density. Further evidence will be needed that physical activity protects against bone fracture and osteoporosis in the edlerly.
