Surgical Treatment of Pulmonary Aspergillosis -One Case Report- (폐면균증(肺麵菌症)의 외과적(外科的) 치료(治療) -일례(一例) 보고(報告)-)

  • A 33 year old male patient was admitted with 20 years history of recurrent hemoptysis. On clinical examination, mild left chest discomfortness and foul odored sputum with occasional rusty hemoptysis were principal complaints noted. Chest X-ray film revealed moderately advanced active tuberculosis lesion on both upper lung fields, and hen-egg sized mass surrounded with linear crescent of air shadow in a cavity on his left upper lung field. On left thoracotomy, dense pleural adhesions on left apicoposterior segmental surface with multiple lymphnode enlargements were noted, and the soft encapsulated mass of $5{\times}5{\times}8cm$ was localized in the apicoposterior segment of the left upper lobe. Apicoposterior segment with anterior segment of the left upper lobe was resected. Cavity was opened to find a rusty grayish colored, fragile mass, which was confirmed as "fungus ball" of aspergillosis by histological section slide with Gomori staining. The authors report one case of pulmonary aspergilloma superinfected with previous long standing pulmonary tbc.

  • VATER is a complex anomaly of multiple organs and often combined with cardiac anomalies. However, it can be cured with active surgical intervention. We achieved successful result with aggressive surgery in the patient with VATER and report it with references.

Unexpected Severe Cerebral Edema after Cranioplasty : Case Report and Literature Review

  • This report details a case of unexpected, severe post-operative cerebral edema following cranioplasty. We discuss the possible pathological mechanisms of this complication. A 50-year-old female was admitted to our department with sudden onset of stuporous consciousness. A brain computed tomography (CT) revealed a subarachnoid hemorrhage with intracranial hemorrhage and subdural hematoma. Emergency decompressive craniectomy and aneurysmal neck clipping were performed. Following recovery, the decision was made to proceed with an autologous cranioplasty. The cranioplasty procedure was free of complications. An epidural drain was placed and connected to a suction system during skin closure to avoid epidural blood accumulation. However, following the procedure, the patient had a seizure in the recovery room. An emergency brain CT scan revealed widespread cerebral edema, and the catheter drain was clamped. The increased intracranial pressure and cerebral edema were controlled with osmotic diuretics, corticosteroids, and antiepileptic drugs. The edema slowly subsided, but new low-density areas were noted in the brain on follow-up CT 1 week later. We speculated that placing the epidural drain on active suction may have caused an acute decrease in intracranial pressure and subsequent rapid expansion of the brain, which impaired autoregulation and led to reperfusion injury.

  • Actinomycosis is a chronic infectious disease, which is produced by Gram-positive anaerobic organisms, actinomycetes, normally inhabit in the mouth, bowel and female genital tract. Primary endobronchial actinomycosis is relatively rare infection and can be misdiagnosed as endobronchial tuberculosis or malignancies. We experienced a case of primary endobronchial actinomycosis in a 49-year-old man presented with fifteen-days history of hemoptysis and cough. He had a past history of extraction of teeth because of dental caries six months ago. Chest X-ray showed irregular consolidation ad bronchoscopic findings revealed nearly obstruction by tumor mass with active bleeding in RLL. Pathologic finding of the bronchial mass showed sulfur granule with granulation tissue formation. Intravenous administration of penicillin G followed by oral ampicillin therapy for 6 months resulted in marked improvement in symptoms and chest X-ray findings. We report this case with review of literature.

  • Although a tracheal stent can be an option for inoperable tracheal stenoses there still are some troublesome side effects including overgranulation from foreign body irritation restenosis and patient's discomfort associated with the procedure. We report a successful case of a retrievable stent made of self-expandable 'shape memory' metal and polyurethane in a 24 year old female patient with respiratory distress and tight stenosis in the trachea and left main bronchus, The stent was inserted following a balloon dilatation and was successfully removed on the 7th days after the procedure. She regained a normal active life without any repiratory symptoms and a follow-up of 8 months showed satisfactory results.

  • The avulsion fracture of the calcaneal tuberosity is rare injury. Usually, it occurs from indirect trauma in old patients with osteoporosis or in patients with diabetic neuropathy. Especially, the bone and joint damage occurred in active patient with severe sensory loss or arthropathy related to nerve damage regardless of the cause is referred to neuropathic arthropathy. Generally, a patient with nondisplacement or minimally displacement is treated by conservative therapy and a patient with severe displacement is treated by open reduction and internal fixation. We experienced a 33 years-old woman with diabetes mellitus who had the displaced avulsion fracture of the calcaneal tuberosity without significant trauma and did not treat. We report upon this case at the 2 years follow-up.

Autologous Fat Grafting as a Last Resort for Unsustainable Pain in a Woman with Multiple Osteochondromas

  • Multiple osteochondromas (MO) is characterized by the formation of osteochondromas throughout the entire body. Although the evidence regarding its pathogenesis is well understood, no curative treatment for the disorder is available. Patients can be treated symptomatically by surgical removal of painful osteochondromas. Unfortunately, some patients still suffer from severe pain, even after surgery. We report on a case concerning a 48-year-old woman with a history of MO who presented with persistent pain after surgical removal of a symptomatic osteochondroma of the left scapula and multiple symptomatic osteochondromas of the left foot and trochanteric region. Several interventions to reduce the pain did not have any lasting effect. Subsequently, she was treated with autologous fat grafting (AFG). After each session she was pain-free for at least one year and reported only partial recurrence of the pain. This is the first case report describing AFG for the treatment of pain after both surgical removal of an osteochondroma and symptomatic osteochondromas in a patient suffering MO with promising results. The treatment is more effective and clearly continues to remain active longer than injection therapy or pain medication. Future studies are necessary to confirm our results.

  • One of the most common benign tumors of bone is the osteochondroma which is a cartilage-capped bony projection on the surface of a bone. It may occur in any bone that has been performed from cartilage, but the most common locations are the long bone at the metaphyseal region of the most active growth cartilage, that is, the lower end of femur, the upper end of the tibia, and the upper end of the humerus. Other bones often involved are the ilium, scapula, fibula, and phalanges of hands and feet. But, the vertebral column is very rare location for osteochondroma. Only one case of osteochondroma involving the lumbar spine has been previously reported in Korea. We report an unusual case of osteochondroma arising from the left lamina and inferior articular process of the 3rd lumbar spine causing spinal cord compression.

  • There are various modalities in the treatment of facial asymmetry, but in severe case with TMD by actively growing deformed condyle, the treatment choice is removing the condyle growth center and TMD symptom such as click or muscular discomfort. In our one case, the patient was complain of facial asymmetry. There are severe deformed condyle head with bird-head fashion and enlarged mandibular ramus and body vertically about 18 mm, overgrowthed Rt. mandible body horizontally about 20 mm. She had intermittent Lt. TMJ clicking and muscular discomfort. The author diagnosed it as Lt. hemimandibular hyper-plasia & R, hemimandibular elongation, a combination form with TMD. the condyle was in active growing state in scintigraphic analysis. So we extirpated the deformed condyle by intrasoral sagittal split ramus osteotomy and reshaped the condyle and mandibular distal fragment extraorally. The distal fragment was readapted in glenoid fossa and fixated. In Rt. mandibular body area, autogenous onlay bone graft on the inferior border of mandible was performed to correct the asymmetry. The clicking and facial asymmetry was corrected and we report this results with other literature findings.

The Clinical Case Report about Postoperative Rehabilitation for the Tear of Anterior Cruciate Ligament Treated with Korean Medical Treatment and Continuous Passive Motion (한방 치료와 연속 수동 운동(CPM)을 병행한 전방 십자인대 파열 환자의 수술 후 재활 치료에 대한 증례 보고)

  • Objectives : The objective of this study is to report the effect of Koran medical treatment and CPM exercise on recovery after reconstruction surgery of torn anterior cruciate ligament Methods : A 27-year-old male patient who had anterior cruciate ligament reconstruction surgery was treated by Korean medical treatment and CPM exercise from March 24th 2015 to April 16th 2015. Results : After treatments, VAS were generally decreased, ROM of Lt. knee got better from $95^{\circ}/10^{\circ}$(flexion/extension) to $120^{\circ}/-5^{\circ}$ (flexion/extension), Lysholm Score increased from 26 to 63. Conclusions : This study showed that Korean medical treatments and CPM exercise has meaningful effect on recovery after reconstruction surgery of anterior cruciate ligament. And it is considered that we need to apporoach recovery of these kinds of operation with more active Korean medical treatment, and further researches should be done steadily.
