청각선별을 통과한 주간 보호와 언어재활 서비스 수혜 소아의 가청역치 (Hearing Threshold of Children with Hearing Screening-Passed in Day Care Center and Speech-Language Pathology Clinic)

  • 청각선별에서 반응 역치는 검사 환경 잡음, 청각기관 생리적 특성, 과도한 음원 노출 등에 따라 다르게 나타난다. 이 연구 목적은 청각선별을 통과한 소아의 주파수별 가청역치에 대한 정보를 획득하는데 있다. 연구 대상은 언어치료센터와 지역아동센터에 다니는 3.3세부터 16.3세($9.01{\pm}2.52$) 사이의 110명으로 하였고, 이들의 소아 청각 선별 결과를 후향적으로 분석하였다. 청각선별은 고막운동성계측(Tympanometry), 등골근반사(acoustic reflex threshold), 자동화 이음향방사(automated otoacoustic emission), 순음청각선별(pure tone screening)을 시행하였다. 대상자들은 모두 청각선별 정상 기준에 포함되었다. 주파수별 가청역치 차이는 반복측정 분산분석 하였다. 결과: 가청역치 평균은 500, 1,000, 2,000, 4,000 Hz의 순서로 우측 귀 $16{\pm}6.49$, $11.5{\pm}4.79$, $6.86{\pm}4.99$, $5.95{\pm}6.65$ dB HL, 좌측 귀 $15.68{\pm}6.01$, $9.95{\pm}5.24$, $5.72{\pm}5.21$, $5.63{\pm}7.04$ dB HL로 각각 관찰되었다. 주파수 사이에는 500Hz와 1,000, 2,000, 4,000 Hz(p=.000), 1,000과 2,000, 4,000 Hz(p=.000)로 유의한 차이가 있었다.

지하철 실내 소음 하에서 음향기기 사용에 따른 음향반사역치와 소음 속 어음인지 변화 (Changes of Acoustic Reflex Thresholds and Speech-In-Noise Perception Using Personal Listening Device Under Subway Interior Noise)

  • 환경 소음의 무절제한 노출이 청력 손실로 이어질 수 있다는 보고는 익히 알려져 있지만, 지하철 실내 소음과 휴대용 음향기기 음량에 따른 이중 소음 노출에 대한 탑승객들의 청력 손실 여부는 많은 연구들에서 여전히 진행 중이다. 본 연구는 지하철 실내 소음 하에서 탑승객들이 이어폰을 이용하여 개인음향기기 사용 시 음향 반사 역치와 소음속 어음인지도가 어떻게 변화하는 지 확인하고자 하였다. 20대의 젊은 건청 성인 40명을 대상으로 실험 그룹과 대조그룹으로 각각 20명씩 구분하였다. 실험 전 모든 대상자에게 5개의 주파수에서 음향반사역치검사와 0, -5 dB SNR에서 문장검사를 실시하였다. 실험 그룹은 60분간 73.45 dBA의 실제 서울도심지하철의 평균 실내 소음 강도를 들으며 개인별 선호 음량으로 이어폰을 통해 좋아하는 음악을 들었고, 대조그룹은 동일강도의 지하철 소음에만 노출된 채 신문 혹은 잡지를 읽었다. 60분간의 지하철 소음 노출 후 음향반사역치검사와 소음 속 문장검사를 재실시하여 기준선과 비교하였다. 소음 노출 전 후 비교 시, 두 그룹 간 음향반사역치는 유의미한 차이가 없었다. 그러나 소음 속 문장 검사결과에서는 실험 그룹의 어음인지도가 소음 노출 후 유의하게 하락하였다. 본 결과를 근거로 지하철 탑승객이 일정기간 동안 지하철 실내 소음에 노출된 채 반복적으로 음악을 듣는 습관이 지속될 시 일시적인 어음인지력 저하의 위험이 예상 될 것이라고 생각한다.

신생아 울음의 의사소통 의도와 관련된 음향학적 특성 (Acoustic and Physiologic Characteristics of Newborn Infants' Communication Intent via Crying)

  • The purpose of this study was to investigate the acoustic characteristics of crying infants according to the communication intents such as hunger and pain in terms of acoustic differences in the fundamental frequency ($F_0$), jitter, shimmer, noise-to-harmonic ratio(NHR), habitual pitch, and intensity. The subjects were 20 healthy, normal infants, less than seven days old, from the city of Seoul and were born after 38 to 42 weeks(full term) of pregnancy. The sound of crying was recorded for three minutes. The crying due to pain was induced by means of the inborn metabolism error test, whereas the crying due to hunger was verified by means of the rooting reflex by waiting for the designated eating time. The results were as follows: (1) the fundamental frequency, noise-to-harmonic ratio(NHR), and intensity of the infants' crying due to pain was higher than that by hunger, showing a significant difference between the mean values. (2) the infants' crying due to hunger and that by pain did not have a significant difference in the mean jitter and shimmer values but both of them were largely outside of the normal threshold values(jitter by 1.04% and shimmer by 3.81%). This study was significant in the sense that it showed the acoustic characteristics of infants' crying from hunger and pain were very different from each other according to the communication intents in terms of the six acoustic parameters.

만성 상악동염이 중이기능에 미치는 영향 (IMPEDANCE AUDIOMETRY IN RELATION TO CHRONIC MAXILLARY SINUSITIS)

  • 만성 상악동염은 동점막의 복잡한 변화를 일으키고, 만성증으로 지속되는 비분비물 및 비점막의 염증이 이관에 파급되어 중이에 영향을 미치며 그 치유경과를 늦추게 한다. 저자는 만성 상악동염에서 동점막의 비후정도가 이관 및 중이에 미치는 영향을 알아 보고자, 1986년 1월부터 3월까지 이화여자대학교 의과대학 부속병원 이비인후과에 비폐색 및 비루를 주소로하여 내원한 환자중 만성 상악동염으로 진단된 16세 이상의 성인 환자를 대상으로 검사하였다. 동점막 비후정도에 따라 비정상군을 경도(2~3mm), 중등도(4mm이상), 고도(상악동 전체혼탁) 3군으로나누어 각각 40예를 조사군으로 하고 동점막의 두께가 0~1mm인 경우를 정상대조군으로 하여 40예를 GSI 28 AUTO TYMP를 사용하여 screening audiometry, tympanometry, acoustic reflex threshold (ipsilateral and contralateral)를 시행하여 다음과 같은 결과를 얻었다. 1) 남녀의 비는 1 : 2.6이고 21세에서 30세군이 53예(33.0%)로 가장 많았다. 2) 증상발현기간은 6개월 이내가 42.5%로 가장 많았다. 3) Tympanogram의 분포에서 비정상적 type으로 나오는 비율이 정상군과 비정상군중에서 경도군이 각각 22.5%이었고 중등도군이 30.0%, 고도군이 50.0% 순이었다. 4) 등골근반사역치는 정상군과 비정상군 사이에 차이가 없었다. 5) 등골근무반사율 역시 정상군과 비정상군 사이에 차이가 없었다.

Cortical Deafness Due to Ischaemic Strokes in Both Temporal Lobes

  • Cortical deafness is a clinical rarity whereby a patient is unresponsive to all types of sounds despite the preserved integrity of the peripheral hearing organs. In this study, we present a patient who suddenly lost his hearing following ischaemic infarcts in both temporal lobes with no other neurological deficits. The CT confirmed damage to the primary auditory cortex (Heschl's gyrus) of both hemispheres. Initially, the patient was unresponsive to all sounds, however, he regained some of the auditory abilities during 10 months follow up. Pure tone threshold improvement from complete deafness to the level of moderate hearing loss in the right ear and severe in the left was observed in pure tone audiometry. Otoacoustic emissions, auditory brainstem responses, and acoustic reflex findings showed normal results. The middle and late latency potential results confirmed objectively the improvement of the patient's hearing, however, after 10 months still, they were somewhat compromised on both sides. In speech audiometry, there was no comprehension of spoken words neither at 3 nor at 10 months. The absent mismatch negativity confirmed above mentioned comprehension deficit. The extensive auditory electrophysiological testing presented in this study contributes to the understanding of the neural and functional changes in cortical deafness. It presents the evolution of changes after ischaemic cerebrovascular event expressed as auditory evoked potentials starting from short through middle and long latency and ending with event-related potentials and supported by neuroimaging.

Axonal sprouting in the dorsal cochlear nucleus affects gap-prepulse inhibition following noise exposure

  • One of the primary theories of the pathogenesis of tinnitus involves maladaptive auditory-somatosensory plasticity in the dorsal cochlear nucleus (DCN), which is assumed to be due to axonal sprouting. Although a disrupted balance between auditory and somatosensory inputs may occur following hearing damage and may induce tinnitus, examination of this phenomenon employed a model of hearing damage that does not account for the causal relationship between these changes and tinnitus. The present study aimed to investigate changes in auditory-somatosensory innervation and the role that axonal sprouting serves in this process by comparing results between animals with and without tinnitus. Rats were exposed to a noise-inducing temporary threshold shift and were subsequently divided into tinnitus and non-tinnitus groups based on the results of gap prepulse inhibition of the acoustic startle reflex. DCNs were collected from rats divided into three sub-groups according to the number of weeks (1, 2 or 3) following noise exposure, and the protein levels of vesicular glutamate transporter 1 (VGLUT1), which is associated with auditory input to the DCN, and VGLUT2, which is in turn primarily associated with somatosensory inputs, were assessed. In addition, factors related to axonal sprouting, including growth-associated protein 43 (GAP43), postsynaptic density protein 95, synaptophysin, α-thalassemia/mental retardation syndrome X-linked homolog (ATRX), growth differentiation factor 10 (GDF10), and leucine-rich repeat and immunoglobulin domain-containing 1, were measured by western blot analyses. Compared to the non-tinnitus group, the tinnitus group exhibited a significant decrease in VGLUT1 at 1 week and a significant increase in VGLUT2 at 3 weeks post-exposure. In addition, rats in the tinnitus group exhibited significant increases in GAP43 and GDF10 protein expression levels in their DCN at 3 weeks following noise exposure. Results from the present study provided further evidence that changes in the neural input distribution to the DCN may cause tinnitus and that axonal sprouting underlies these alterations.
