Comparison of Systemic Accident Investigation Techniques Based on the Sewol Ferry Capsizing

  • Objective: This study aims to survey and compare three systemic accident investigation techniques of Accimap, STAMP and FRAM, based on the application studies of the Sewol ferry accident. Background: Traditional accident investigation methods such as domino models, FTA, etc. work well for losses caused by physical component failures or actions of human in relatively simple systems, but are unable to depict mechanisms generating errors and violations in the current complex socio-technical systems. For better understanding the structure and behavior of the socio-technical systems, systemic techniques have been developed and used. Method: This study was mainly based on survey of literatures through surfing webpages of ScienceDirect and Google, and ergonomics relevant journals. The key words of Sewol, Sewol ferry, Sewol ferry accident, etc. were used in the survey. Results: Three systemic accident investigation methods included similar actors in the Sewol ferry accident including government, Ministry of Ocean and Fisheries, Korean Coast Guard, Korean Register of Shipping, Korea Shipping Association, Chonghaejin Marine Company, crew members. The methods graphically represented each level's failures or performance variabilities of relevant functions and relationships between them. It was shown that the systemic methods consider the entire system, ranging from the environment in which the accident occurred, to the role of government in shaping the system of work. Each method has its own comparative pros and cons, but the Accimap has advantages in terms of time of analysis, data required, model complexity and degree of comprehensiveness. Conclusion: This study reviewed and compared three systemic accident investigation methods, which showed that there are systemic characteristics and pros and cons in the methods. Application: The results would be used as a guideline when selecting accident investigation methods.

Text-mining based Cause Analysis of Accidents at Workplaces in Korea (텍스트 마이닝 기법을 활용한 우리나라 산업재해의 원인분석)

  • The analysis of the causes of accidents in workplaces where machines and tools are used is essential to improve the effectiveness and efficiency of safety prevention policies in places of employment in Korea. The causes of workplace accidents are not fully understood mainly due to difficulties in analyzing available descriptive information. This study focuses on the automated accident cause analysis in workplaces based on the accident abstracts found in industrial accident reports written in an unstructured descriptive format. The method proposed in this paper is based on text data mining and uses the keyword search function of Excel software to automate the analysis. The analysis results indicate that the primary reason for the frequency of accidents is related to technical aspects at a stage in which dangerous situations occur in the workplace. Accidents due to managerial causes are typically observed when danger exists in the workplace; however, managerial actions play a more important role in reducing accident severity. A small company tends to use unsafe machines and devices, leading to further accidents due to technical causes, whereas managerial causes are more conspicuous as the company grows. To preclude the occurrence of accidents due to inadequate knowledge, the implementation of safety management and the provision of safety education to elderly workers at the early stage of their employment are particularly important for small companies with less than 100 workers.

Evaluating the Application of Portable Safety Equipment in Nuclear Power Plants using Multi-unit PSA (다수기 PSA 기반 원자력 발전소 이동형 안전 설비 활용성 평가)

  • Following the Fukushima accident, portable equipment employed as accident mitigating systems have been installed and operated to reduce core damage and large early release frequencies. In addition, the establishment of an accident management strategy has gained importance. This study investigated the current status of portable equipment including the international portable equipment FLEX (diverse and flexible coping strategies), and domestic portable equipment multi-barrier accident coping strategy (MACST). Research on optimal utilization of MACST remains insufficient. As a preliminary study for establishing an optimal strategy, sensitivity studies were conducted to facilitate the priority of use on portable equipment, number of portable equipment, and dependency of operator actions based on a multi-unit probabilistic safety assessment model. The results revealed the conditions that reduced the multi-unit and site conditional core damage probabilities, indicating the optimal strategy of MACST. The results of this study can be used as a reference for establishing an optimal strategy that utilizes domestic safety equipment in the future.

  • The converted accident rates that included in Pre-Qualification (PQ) are to evaluate the results of accidents which have occurred. Therefore, it is demanded the development of proactive evaluation contents to evaluate the safety management activities and efforts on construction companies. The purpose of this study is to provide basic data for the establishment of proactive evaluation contents at public construction project in Korea through the collected and analyzed the PQ documentation applied by public agencies in Los Angeles, USA. The results indicated that diverse reactive safety evaluations are implemented to examine accidents which occurred in past projects such as occupational health insurance, regulation violation. And also, when the accidents occurred, it is evaluated the proactive safety management actions such as safety education, accident prevention programs, and safety management plans are reflected on the evaluation. Thus, if proactive safety related evaluation contents are established and reflected in PQ examinations together with diverse existing reactive safety evaluations considering converted accident rates, not only the existing function of converted accident rates can be reinforced but also businesses' active safety management activities can be induced and decreases in the construction industry's accident rate can be expected.

  • This paper present a new dynamic HRA (Human Reliability Analysis) method and its application for Quantifying the human error probabilities in implementing an accident management action. For comparisons of current HRA methods with the new method, the characteristics of THERP, HCR, and SLIM-MAUD, which are most frequently used methods in PSAs, are discussed. The action associated with the implementation of the cavity flooding during a station blackout sequence is considered for its application. This method is based on the concepts of the quantified correlation between the performance requirement and performance achievement. The MAAP 3.0B code and Latin Hypercube sampling technique are used to determine the uncertainty of the performance achievement parameter. Meanwhile, the value of the performance requirement parameter is obtained from interviews. Based on these stochastic distributions obtained, human error probabilities are calculated with respect to the various means and variances of the timings. It is shown that this method is very flexible in that it can be applied to any kind of the operator actions, including the actions associated with the implementation of accident management strategies.

Development of a Fully-Coupled, All States, All Hazards Level 2 PSA at Leibstadt Nuclear Power Plant

  • This paper describes the development process, the innovative techniques used and insights gained from the latest integrated, full scope, multistate Level 2 PSA analysis conducted at the Leibstadt Nuclear Power Plant (KKL), Switzerland. KKL is a modern single-unit General Electric Boiling Water Reactor (BWR/6) with Mark III Containment, and a power output of $3600MW_{th}/1200MW_e$, the highest among the five operating reactors in Switzerland. A Level 2 Probabilistic Safety Assessment (PSA) analyses accident phenomena in nuclear power plants, identifies ways in which radioactive releases from plants can occur and estimates release pathways, magnitude and frequency. This paper attempts to give an overview of the advanced modeling techniques that have been developed and implemented for the recent KKL Level 2 PSA update, with the aim of systematizing the analysis and modeling processes, as well as complying with the relatively prescriptive Swiss requirements for PSA. The analysis provides significant insights into the absolute and relative importances of risk contributors and accident prevention and mitigation measures. Thanks to several newly developed techniques and an integrated approach, the KKL Level 2 PSA report exhibits a high degree of reviewability and maintainability, and transparently highlights the most important risk contributors to Large Early Release Frequency (LERF) with respect to initiating events, components, operator actions or seismic component failure probabilities (fragilities).

Study on Importance of Safety Management Factors in Aircraft Using Business (항공기 사용사업에서의 안전관리요소의 중요도에 관한 연구)

  • 2016, in total 53 corporations operate 160 aircraft. Since 2000, 52 accidents occurred in these corporations. This number takes 20.2% out of in total 257 accidents. Especially in 2016, in two(2) accidents, two(2) aircraft and people onboard were damaged. According to accident reports of aircraft using service, in most cases actions against common sense were connected to accidents. This means that attentions of managers or pilot would have protected accidents. On the basis of such background, this research analyses accidents cases of corporations operating aircraft by utilizing ahp. According to this anlysis, unlike scheduled and unscheduled airlines, pilots in command (0.109) and assisting crew (0.105) in Liveware have taken the most importance. Operational procedure (0.100) in Software and a controlling system (0.086) in Hardware have shown the second most importance. This result demonstrates that in case of corporations operating aircraft require safety management at different level than airlines.

  • Safety accidents at industrial sites can occur anytime, anywhere. Recently, research on the industrial safety management system based on the IoT has been actively conducted. However, most of the studies are studies of actions after safety accidents or simply monitoring. In this paper, the safety equipment was introduced before safety accidents occurred, and the subjects could be managed not only by workers but also by visitors. Also, it implements a system to prevent the accident by detecting the user's motion and situation periodically before a safety accident occurs. The implemented system is a system that attaches the device for safety management to the hard hat and can identify the safety situation by using the Bluetooth beacon device attached to the user's smartphone and the industrial site.

  • The purpose of this study is to understand how to affect the safety behavior to the organization's trustees to investigate the level of safety awareness, safety awareness via the distribution center workers prepare the logistics industry disaster prevention measures. The survey was analyzed data to target by the clothing distribution center employees of 182 people. Analysis First, the level of safety awareness showed on a positive effect on organization trust. Second, it was found on the safety level of consciousness a positive effect on the safety behavior. Third, the level of safety awareness affect the safety behavior showed that the mediating role of organization trust. That is a higher level of safety awareness, such as the strict practice of business activities, investments, safety rules on safety education and training emphasis on safety increases confidence in the organization. It was confirmed the higher the confidence in the safety organization appears behavior such as actively participating in the work carried out in a safe manner, safety education and training. In addition, the effect was mediated by the organizational impact on confidence in the safety of personnel safety awareness actions. Based on that research results prepared safety awareness was raised through the industrial accident prevention measures.

A Study on the Improvement of the Blind Zones in View of Safety Laws and Regulations for the New Multi­purpose Business (신종 다중이용업소의 안전기준 사각지대 개선에 관한 연구)

  • Currently, the legal framework for installation of safety facilities has not been implemented for new multi-purpose business excluding 23 multi-purpose business types, and therefore, there is a high likelihood of large number of casualties in case of fire accident. At present, the new multi-purpose business can start its operation only with administrative actions of the licensing agencies and registration to the tax agency, and it is difficult that the fire department, which is a competent safety management department, makes intervention them in the same way as before. In addition, as the licensing agency has no authority to restrict the business due to insufficient implementation of safety facilities, a lot of civil complaints occur, which causes difficulties in changing concerned laws and regulations rapidly. In this study, it is considered to analyze the legal system, status of safety management of multi-purpose business, type of new multi-use facility and characteristics of fire risk. Based on such analysis results, this study will suggest the improvement plan to solve the blind zones in the safety management, which enables the multi-purpose business to be operated within the legal framework.
