Quality Assessment of Hatchway Using QFD & FMEA (QFD와 FMEA를 이용한 화물창구의 품질 평가)

  • The quality of a product/system is getting more important concept nowadays. 'Performance', as one of the quality characteristics, means how well a product carries out its given functions and is the most adjacent characteristics to the customer satisfaction. To an engineer, however, who deals with large structures such as ships, 'Safety' is actually getting more important because of its direct relationship with failures of a product/system itself and human injuries when an accident occurs. In this study, therefore, we consider both performance and safety recognized the most important elements in dealing with structures and present a quality analysis method based on customer requirements by using QFD for performance analysis and FMEA for safety analysis respectively. Applying these methods to the hatchway of a bulk carrier, we could find 8 and 12 important parts based on performance and safety respectively. Among these, only 7 parts were pointed out commonly critical. From these, we can suggest that designers should pay more attention to these 7 parts and thus give a high priority of concerns to them when trying to improve the quality of system.

A CLINICAL STUDY OF THE ORAL AND MAXILLOFACIAL FRACTURE

  • With today's social and cultural personal interactions, greater leisure time and participation in sports activities, and growing traffic volume, the risk of physical trauma has increased markedly. This is a clinical and retrospective study of patients exposed to oral and maxillofacial trauma. We clinically observed 72 patients with trauma in the Department of Oral and Maxillofacial Surgery, Kyunghee University Dental Hospital, from June 2006 through November 2007. The following data was obtained: 1. The male:female ratio of patients having experienced physical trauma was 6.2:1, with most patients in their twenties. 2. Traffic accident (37.5%) was the most common cause of trauma. 3. The highest incidence of fracture occurred to the zygomatic arch(22.1%) among mid-facial fractures and angle(37.5%), symphysis(35.4%) in mandible fractures. 4. Open reduction (88.9%) was the most frequently used form of treatment. Closed reduction was performed on the remaining 11.1% of cases. 5. Teeth and alveolar bone damage occurred in 23.6% of all cases. 6. Other injuries that were related to mid-face fracture occurred in 27.8% of all cases. 7. Post-operative complications occurred in 31.9% of cases, and the highest complication was the nerve injury.

Impact of Subsidies for Labor and Management-directed Health Promotion Activities on Industrial Accidents Prevention (노·사주도 건강증진활동 비용지원사업의 산재예방 효과)

  • Purpose: This study was conducted to compare the rates of industrial accidents in workplaces that adopted the 2011 Workplace Health Promotion Subsidy Scheme before and after the implementation. Methods: The study analyzed the raw data of industrial accidents of 304 workplaces which received subsidies for health promotion activities in 2011. The raw data covered the period from February 2010 to July 2012, based on the dates of industrial accidents. Results: Workplaces subsidized for health promotion activities reported fewer occurrences in staff injuries and illnesses than before the subsidization, as the total number of industrial accident victims dropped from 0.35 to 0.24. The rate of industrial accidents also dropped from 0.49 to 0.35, with the number of working days decreased from 35,433 to 23.867, about 33%. Conclusion: The study showed that financial support for health-promoting activities contributed to the decease in industrial accidents, which is an important indication advocating the need for corporate and government investment on workers health promotion programs. Furthermore, this study is also significant as it is the first research conducted in Korea to examine the impact of a workers health promotion project using a direct indicator, the rate of industrial accidents.

Estimation of Incidence Rate and Factors Related to Occurrence of Traumatic Spinal Cord Injury in Seoul (서울지역 외상성 척수손상환자의 발생률 추정 및 발생관련요인)

  • For the purpose of disclosing estimated annual incidence rate and causative factors of traumatic spinal cord injury and obtaining basic data for the establishment of effective measures to prevent this injury, the medical record of 204 cases of Seoul residents with spinal cord injury which admitted in 46 general hospitals and National rehabilitation center located in Seoul, including Sam Yook Rehabilitation Center and Jung Ang Hospital in Kyungi do and occurred in 1995 were analyzed. The results were as follows: 1) The estimated annual incidence rate of traumatic spinal cord injury in 1995 of Seoul residents was 20.5 per million population. This incidence rate was the highest in the 40th years (34.1), followed by in the 50th years (26.5) and in the 20th years (25.6) in descending order. 2) The incidence rate ratio between male and female was 3.5:1. 3) The leading cause of injury was the highest in traffic accidents (42.2%), followed by falls from elevation (29.4%) and falls on the same level (9.8%). 4) Traffic accident mainly occured from 20th years to 40th years and falls from elevation in 50th years. 5) The level of spinal cord injury was the highest in cervical cord (63.2%), followed by the thoracic cord (21.1%) and lumbosacral cord (15.7%). Traffic accidents mainly caused in cervical cord injury whereas thoracic cord injury and lumbosacral cord injury were caused from falls from elevation. Above theconsiderations in mind, it suggested that in order to prevent traumatic spinal cord injuries, safety education of falls from elevation should be focused on the 20th years and 50th years male, moreover safety education of traffic accidents will be needed in the 30th years and 40th years male.

  • The spine is one of the most important skeletal joints, and it strongly affects the health of the musculoskeletal system. A normal spine has an S-shape, and it is very important to maintain this shape. Recently, spinal diseases such as low back pain have increased rapidly, especially among the elderly. Some of these diseases are caused by congenital spinal disorders and sporting and accident injuries as well as by bad postures. Improper spinal postures could generate excessive disc pressure, which is related to degeneration and pain. Therefore, in this study, we investigated the three-dimensional kinematic parameters of the thoraco-lumbar joint in several bad postures using a motion capture analysis technique. Different bad postures created a significant amount of flexion/extension, side bending, and axial rotation angle compared with neutral postures. Further study is necessary to investigate the disc pressure and ligament force due to the increase in joint rotation from the bad postures.

A Clinical Study of The Nasal Bone Fracture According to Stranc Classification (비골골절의 Stranc 분류에 따른 임상적 연구)

  • Purpose: The nasal bone fracture is known as the most common facial fracture. Several authors reported the classification of nasal bone fracture. Stranc classified the type of nasal fractures based on a pattern of impaction and level of injuries. Stranc classification proposed here is based on careful clinical observation and relates to both treatment and prognosis. The aim of this study is to determine any predictive value to the preoperative classification of nasal fracture, using the description by Stranc and Robertson. Methods: We reviewed 310 patients with nasal bone fracture treated at our hospital for last two years. Results: Lateral impact type of nasal bone fracture predominated more than frontal impact in the ratio of 2.3:1. The most common type of Stranc classification was lateral impact plane I(48.4%). Male predominated more than female in the ratio of 3:1. The most frequent age group was first decade(27.1%), second decade in frontal impact(30.5%), first decade in lateral impact (30.7%). The most common etiology was violence (31.3%) followed by slip down(21.3%), and traffic accident(18.1 %).The most common associated fracture was orbital(22.9%) followed by zygoma(10%), and maxilla (6.1%). The most common complication was septal deviation(20.0%) in frontal impact, and nasal deformity (26.0%) in lateral impact. The incidence of nasal deformity in lateral impact(26.0%) was more higher than frontal impact(15.8%) Conclusion: By assessing the pathomechanics and resultant degree of injury to the nasal skeleton, a better understanding of the treatment plan and prognosis was obtained. Using this information, satisfactory informed patient consent can be obtained.

Innominate Artery Rupture after Blunt Chest Trauma (흉부 둔상 후에 발생한 무명동맥 파열)

  • A rupture of an innominate artery caused by blunt trauma is relatively rare because this artery is short and protected by the chest bony cage. This report describes a 25-year-old man who suffered a traffic accident, that resulted in an innominate artery rupture, which was detected by a chest computed tomogram and angiogram. This patient underwent urgent surgery through a right clavicular incision and median sternotomy without a cardiopulmonary bypass due to multiple injuries. An approximately 3 cm sized injury was found from the innominate artery to the proximal right subclavian artery and the origin of the common carotid artery. The injured lesion was repaired with a saphenous vein patch. After surgery, he was discharged from hospital without complications.

Study on the Characteristics of Bus Traffic Accidents by Types Using the Decision Tree (의사결정나무를 활용한 업종별 버스 교통사고 특성 연구)

  • PURPOSES : This study was initiated to analyze the characteristics of bus traffic accidents, by bus types, using the decision tree in order to establish customized safety alternatives by bus types, including the intra-city bus, rural area bus, and inter-city bus. METHODS : In this study, the major elements involved in bus traffic accidents were identified using decision trees and CHAID algorithm. The decision tree was used to identify the characteristics of major elements influencing bus traffic accidents. In addition, the CHAID algorithm was applied to branch the decision trees. RESULTS : The number of casualties and severe injuries are high in bus accidents involving pedestrians, bicycles, motorcycles, etc. In the case of light injury caused by bus accidents, different results are found. In the case of intra-city bus accidents, the probability of light injury is of 77.2% when boarding a non-owned car and breaching of duty to drive safely are involved. In the case of rural area bus accidents, the elements showing the highest probability of light injury are boarding an owned car, vehicle-to-vehicle accidents, and breaching of duty to drive safely. In the case of intra-city bus accidents, boarding owned car, streets, and vehicle-to-vehicle accidents work as the critical elements. CONCLUSIONS : In this study, the bus accident data were categorized by bus types, and then the influential elements were identified using decision trees. As a result, the characteristics of bus accidents were found to be different depending on bus types. The findings in this study are expected to be utilized in establishing effective alternatives to reduce bus accidents.

Multiple Rib Fracture: analysis of 100 cases (다발성 늑골골절 치험 100례)

  • During the period of 4 years from August 1, 1975 to August 1, 1979, authors have experienced 100 cases of multiple rib fractures by nonpenetrating injury at Department of Thoracic Surgery, Paik Foundation Hospital in Seoul, Korea. 1. The ratio of male to female patients with multiple rib fracture was 2.6:1 with male predominance and 84% of the total cases were between 20 and 50 years of age. 2. The most common cause of multiple rib fracture was traffic accident and falls accounted for the next largest group. 3. The most common site of rib fracture was 4th rib to 7th rib level on both hemithorax [52%]. 4. Associated injuries were cerebral contusion in 26%, clavicular fracture in 22%, long bone fracture in 22%, pelvic bone fracture in 10%, and scapular fracture in 8%. 5. Early complications and/or result of the multiple rib fractures were lung contusion in 23 cases, subcutaneous emphysema in 21 cases, hemothorax in 21 cases, hemopneumothorax in 6 cases, and flail chest in 12 cases. 6. The flail chests were managed by strapping the chest with adhesive plaster, external traction of flail segment with towel clip, ventilatory assistance for marginal clinical indications, and in cases of complicated with intrathoracic hemorrhage, wire fixation of flail segment through open thoraco-tomy. 7. The principles of therapy for hemothorax and/or pneumothorax were rapid reexpansion of the lungs by thoracentesis [11%] and closed thoracostomy [22%], but open thoracotomy had to be done on 3 cases because of massive bleeding or intrapleural hematoma and diaphragmatic rupture. 8. The over all mortality was 4% [4 among 100 cases] and the cause of all deaths was head injury.

Aortic Arch Rupture due to Compression Injury of the Thorax - A case report - (흉부 압박손상에 의한 대동맥궁 파열 - 1예 보고 -)

  • Traumatic rupture of the thoracic aorta is the second most common cause of death from motor vehicle accidents after head injury. About 85% of these patients do not survive to reach the hospital. The most common mechanism for this is deceleration injury, as occurs in a high speed motor vehicle accident. The aortic isthmus is the site of disruption for about 95% of all blunt thoracic aortic injuries. Another mechanism is crush injury which causes compression of the aorta between the displaced sternal body or manubrium and the thoracic vertebral column. These forces tear the inner layer of the aortic wall at an unusual location. We report here on a case of aortic arch dissection where the injury clearly occurred due to a crush injury and not because of deceleration. The surgical repair was delayed for 10 days after administering intensive medical therapy. The ascending aorta and aortic arch were replaced with an artificial graft with the patient under circulatory arrest and cerebral protection.
