Purpose: The purpose of this study was to describe nurses' experiences of accidents in patient safety. Methods: Data were collected from October 8, 2011 to January 31, 2012 through in-depth interviews with seven nurses who had worked on wards or in the ICU in a university hospital. Data were analyzed by applying Colaizzi's phenomenological methodology. Results: The following six categories were extracted: Fear of the patient's condition caused by the accident, Conflict in the accident report, Blame on others and circumstances, Feeling guilty and sorry as the patient's condition is improving, Being disappointed with the unfavorable atmosphere in dealing with the accident, After the accident, being sensitive in performing nursing duties and being faithful to the principles. Conclusion: The results indicate that the organizational culture in the hospital related to accidents in patient safety is still closed and punitive, and such an atmosphere causes nurses to feel seriously hurt, but through this experience nurses are likely to mature as nursing professionals. Programs on prevention of accidents in patient safety and a system to guard against these accidents should be established. Also the organizational safety culture should be improved.
After the Fukushima Daiichi accident, there has been an increasing preference for passive safety features in the nuclear power industry. Some passive safety systems require limited active components to trigger subsequent passive operation. Under very serious accident conditions, passive safety features could be rendered inoperable or damaged. This study evaluates (i) the performance and effectiveness of the passive safety features of iPOWER (innovative Power Reactor), and (ii) whether a severe accident condition could be reached if the passive safety systems are damaged, namely the case of heat exchanger tube rupture. Analysis results show that the reactor coolant system remains in the hot shutdown condition without operator actions or electricity for over 72 h when the passive auxiliary feedwater systems (PAFSs) are operable without damage. However, heat exchanger tube rupture in the PAFS leads to core damage after about 18 h. Such results demonstrate that, to enhance the safety of iPOWER, maintaining the integrity of the PAFS is critical, and therefore additional protections for PAFS are necessary. To improve the reliability of iPOWER, additional battery sets are necessary for the passive safety systems using limited active components for accident mitigation under such extreme circumstances.
The basic concept of analysis of human error that induced railway accident is that errors are consequences, not causes. But in most cases, it is likely that direct causes of the accident could be concluded as real causes, which make it difficult to find out root causes. Design, training, staffing, culture and condition are general category applied to investigate human error. In order to examine how those approach could help accident analysis, this paper studied accident investigation reports of UK RAIB(Rail Accident Investigation Branch). Rather than consider specific investigation method, we focus investigation result on how to describe causal factors and how to indicate recommendations to prevent similar accident. The reports show that they try to find out causes more in organizational, environmental and job factors, which implies the necessity to improve investigation process of human error accident in Korea.
The design, construction, and operation of a light water reactor (LWR) are subject to compliance with safety criteria specified for accident situations, such as loss-of-coolant accident (LOCA) and reactivity-initiated accident (RIA). Because reactor fuel is the primary source of radioactivity and heat generation, such a criterion is established on the basis of the characteristics and performance of fuel under the specific accident condition. As such, fuel behavior under accident situations impact many aspects of fuel design and power generation, and in an indirect manner, even spent fuel storage and management. This paper provides a comprehensive review of: the history of the current LOCA criteria, results of LOCA-related investigations on conventional and new classes of fuel, and status of on-going studies on high-burnup fuel under LOCA situations. The objective of the paper is to provide a better understanding of important issues and an insight helpful to establish new LOCA criteria for modem LWR fuels.
This paper describes result of a study on safety against a fire of charging cable for mobile phone for vehicle. Combustion on the USB cable in the car was happened while driving. Gas coming from the burning USB cable could be a reason which can make a secondary car accident since the driver also can be embarrassed while driving. In order to prevent a secondary car accident connected on the road, to research a reason why USB cable can emit gas and be burned in charging. We did simulation test with abnormal fault condition for the electronic component on the board in the USB cable. So we get the result from abnormal fault condition simulation test, for instance, shorted test for output terminal of 8 pin switch, shorted test for chip resistor after thermal aging in the condition $25^{\circ}C$, 93 % RH during 48 hours. To analysis the result of all test, Combustion on the USB cable was not the 8 pin but other electrical component such as a chip resistor. Therefore we guess that the reason for USB cable combustion in charging in a car was not 8 pin and a LED but another defective component.

The rehabilitation for the workplace accident victims is basically aimed to get them back to their working place through the secondary care processes which have some more special and more professional rehabilitation than any others, after the primary treatments like operation in an acute hospital since the accident. They need more concentrative rehabilitation efforts according to classified injury types during the period of acute, sub-acute and convalescent. This study is for the partial research of rehabilitation hospital for workers' accident victims and presents design data which can be used for the appropriate area calculation and spatial composition. Drawing documents analysis, interviews, observation were used to examine the condition of existing rehabilitation hospitals.

Although there are still controversial opinions and uncertainty on application of SiCf/SiC composite cladding as next-generation cladding material for its great oxidation resistance in high temperature steam environment and other outstanding advantages, it cannot deny that SiCf/SiC cladding is a potential accident tolerant fuel (ATF) cladding with high research priority and still in the engineering design stage for now. However, considering its disadvantages, such as low irradiated thermal conductivity, ductility that barely not exist, further evaluations of its in-pile behaviors are still necessary. Based on the self-developed code we recently updated, relevant thermohydraulic and mechanical models in FROBA-ATF were applied to simulate the cladding behaviors under normal and accident conditions in this paper. Even through steady-state performance analysis revealed that this kind of cladding material could greatly reduce the oxidation thickness, the thermal performance of UO2-SiC was poor due to its low inpile thermal conductivity and creep rate. Besides, the risk of failure exists when reactor power decreased. With geometry optimization and dopant addition in pellets, the steady-state performance of UO2-SiC was enhanced and the failure risk was reduced. The thermal and mechanical performance of the improved UO2-SiC was further evaluated under Loss of coolant accident (LOCA) and Reactivity Initiated Accident (RIA) conditions. Transient results showed that the optimized ATF had better thermal performance, lower cladding hoop stress, and could provide more coping time under accident conditions.

The number of car accident is Recently on the increase in Korea because of the explosive increase of cars, the poor road condition, the lack of safety facility, and others. The insurant with a accident has to decide whether receiving a insurance or not. In this paper, we represent a reasonable decision support material by calculating the approximate insurance fee based on the discount rate and premium additive rate, which is changed by the accident type and the accident expenditure. Practically, there is difference in the standard insurance rate and premium additive rate according to the accident type and the accident expenditure in Korea. The premium additive rate is assessed considering the number of accident, the pattern of accident, and the reason of accident for 3 years. In this paper, we represent a decision making method considering not only the first-time car accident but also the future car accident. For considering the repeated accident, we analyzed the real data accumulated until the year of 1996 from S Insurance Company, and estimated the probability density function between the first and the second-time accident, and executed the goodness of fit test using ARENA and STATISTICA software. Using this conditional PDF, we can calculate the insurance fee next 3 years and compare the insurance fee with the equivalent present value of cash flows. The program performing this analysis is represented, and written in VISUAL BASIC Language. We tried to suggest an accurate guideline for the insurant to decide the insurance coverage rationally, and tried to correct a wrong idea of dependence on the car insurance only by the amount of the accident expenditure. And we expect this study can generally be applied to many different accident types under the uncertain circumstances in our daily life.
Unlike in general manufacturing process, safety management in laboratory-based research area is complicated because the latter generally involves trying untested methods or handling unusual substances in small amounts. Laboratory accidents in South Korea have recently shown an increasing trend. Unfortunately, statistics on such accidents are not officially published by any domestic public agencies. In this study, multivariate analysis was performed on the relationships between variables to develop effective strategies for preventing laboratory accidents. A Cross-Tabulation Analysis of accident-related factors in 179 accident cases revealed that the laboratory type, accident type, and unsafe-act type are all statistically significant, whereas the unsafe condition and management factors differ with the statistical criteria. Furthermore, the results of a Multiple-Correspondence Analysis showed that accidents can be divided largely into three groups having different accident causes and injury types; this confirms the necessity of different strategies to prevent accidents of each type. The findings also reveal differences between the distribution of accident types mentioned in the accident case collection books and actual reported cases. This suggests that an official statistical system administered by a public institution would be necessary for effective prevention of laboratory accidents.

I propose a good service through circulation harmony by remodeling the system of the structure of cash supply and demand. And I propose accident prevention countermeasure by removing a caxh accident by the simplification, specialization and soientification of money business. I discuss the present condition and the points of cash transportation structure in Chapter 2 and the improvement plan of cash transportation structure in Chapter 3, and conclude in Chapter 4.
