Case Study on the Time Zero (T0) of Event Data Recorder (사고기록장치의 기록 시점에 대한 사례연구)

  • On December 19, 2015, as Article 29-3 (Installation of Accident Recording Devices and Provision of Information) of Motor Vehicle Management Act came into force, In Korea, the EDR (Event Data Recorder) reports are often used for the analysis of various traffic accident cases such as multiple collisions, traffic insurance crimes, and sudden unintended acceleration (SUA), and the others. So many investigators have analyzed the driver's behavior and vehicle situation by comparing the time zero in the EDR report to the actual crash time in dash-cam (or CCTV). Time zero (T0) is defined as the reference time for the record interval or time interval when recording an accident in Article 56-2, Enforcement rule of Performance and Standard for Automobile and Automotive parts. Also in the EDR report, time zero (T0) is defined as whichever of the following occurs first; 1. "wake-up" by an air-bag control system, 2. Continuously running algorithms (by monitoring of longitudinal or lateral delta-V), 3. Deployment of a non-reversible deployment restraint. We have already proposed the "Flowchart & Checklist" to adopt the EDR report for traffic accident investigation and the necessity of specialized institutions or courses to systematically educate or analyze the EDR data. Therefore, in this paper, we report to traffic accident investigators notable points and analysis methods based on some real-world traffic accidents that can be misjudged in specifying time zero (T0).

  • There was an amazing growth in the korean shipbuilding industry considering the industrial magnitude and the technical level. On the contrary of the growth, there are a number of accidents in the shipbuilding yard. According to the statistic data of the industrial accident in the last 7 years, the number of accidental fatalities in shipbuilding industries are 1.48 ‱ which are over more than doubles of the average in the Korean industries, 0.66 ‱. Thus, through an accident investigation of fire and explosion case during painting work in the tank of the crude oil carrier, it have been tried to diagnose actual conditions in the shipbuilding yards.

Structural Analysis for the Collapse Accident of Tower Crane (타워크레인 붕괴사고의 구조적 분석)

  • The tower cranes are the very useful construction machine in the high place works. But they are very susceptible to the load balance, the wind load and the hanging load because they are the very slender structures and those center of gravity is located in the upper part. Therefore, the collapse accidents of tower one have repeatedly happened during the assemble or disassemble works. The correcting frame may has often used in order to correct the error in the setting of foundation anchors. The goal of this study is that propose the methods preventible the collapse accident of tower crane which is constructed by using the correcting frame. In order to accomplish the goal of this study, the field survey, the reference investigation and the structure analysis were performed for the collapse accident of tower crane using the correcting frame. This study result in the methods preventible the same accident.

A Study on the Analysis Model for School Safety Accident (학교안전사고 분석모형에 관한 연구)

  • Low birthrate is causing a reduction in the number of students at kindergartens, elementary schools, middle schools and high schools nationwide and yet, school safety accidents are on a constant rise, which was reported to be 237 accidents a day on average in 2011. Such phenomenon is proving how the school safety policy is not doing what it was supposed to do. In order to decrease the school safety accidents, first, causes of the accidents should be analyzed and then, prevention measures should be designed. For that reason, the study looked into the present condition of the school safety accidents and safety accident theories and based on the results, "School Safety Accident Analysis Matrix Model" was proposed. With a matrix method of the accident types (17 of them) and hazard factors (9 of them) applied, the concerned model analyzed a total of 153 accident causes. In consideration of the results from the analysis, the study suggested that the education authority should open a safety organization and design a school safety policy that would systematically deal with safety education, prevention measures practice, accident investigation and analysis, and countermeasures practice as well.

  • Purpose: This study operation of Joint inter-agency Chemical Emergency Preparedness Center and contribute to the reduction of chemical accidents that occur continuously. Method: The Joint inter-agency Chemical Emergency Preparedness Center functions and Chemical accident statistics data of the ('13~'17) were utilized. Results: The number of chemical accidents is decreasing from 113 in '15, 78 in '16, 87 in '17(latest five years 469 chemical accidents). The Joint inter-agency Chemical Emergency Preparedness Center is located in the industrial complex that handling a large amount of chemical, and performs functions such as prompt response, probation & investigation, accident prevention training, safety patrol. It is believed that it contributes to the decreasing of chemical accident by local control accident prevention function. Conclusion: Decreasing the safety management according to the Chemicals control act('15.1.1). The Joint inter-agency Chemical Emergency Preparedness Center('14.1 set up manage organization), which is operated as a mission to prepare respond to chemical accidents, plays a role.

An Approach to Estimation of Radiological Source Term for a Severe Nuclear Accident using MELCOR code (MELCOR 코드를 이용한 원자력발전소 중대사고 방사선원항 평가 방법)

  • For a severe accident of nuclear power plant, an approach to estimation of the radiological source term using a severe accident code(MELCOR) has been proposed. Although the MELCOR code has a capability to estimate the radiological source term, it has been hardly utilized for the radiological consequence analysis mainly due to a lack of understanding on the relevant function employed in MELCOR and severe accident phenomena. In order to estimate the severe accident source term to be linked with the radiological consequence analysis, this study proposes 4-step procedure: (1) selection of plant condition leading to a severe accident(i.e., accident sequence), (2) analysis of the relevant severe accident code, (3) investigation of the code analysis results and post-processing, and (4) generation of radiological source term information for the consequence analysis. The feasibility study of the present approach to an early containment failure sequence caused by a fast station blackout(SBO) of a reference plant (OPR-1000), showed that while the MELCOR code has an integrated capability for severe accident and source term analysis, it has a large degree of uncertainty in quantifying the radiological source term. Key insights obtained from the present study were: (1) key parameters employed in a typical code for the consequence analysis(i.e., MACCS) could be generated by MELCOR code; (2) the MELOCR code simulation for an assessment of the selected accident sequence has a large degree of uncertainty in determining the accident scenario and severe accident phenomena; and (3) the generation of source term information for the consequence analysis relies on an expert opinion in both areas of severe accident analysis and consequence analysis. Nevertheless, the MELCOR code had a great advantage in estimating the radiological source term such as reflection of the current state of art in the area of severe accident and radiological source term.

A Study on the Fatigue Investigation Program for Marine Accidents (해양사고의 피로도 조사 프로그램에 관한 연구)

  • Recently, fatigue related to marine accidents was recognized to be very important contribution factor when marine accidents such as collision, aground, fire, etc. were investigated Many studies concerned about fatigue factors related to human error can be found. In this study, the analysis and examination of USCG Fatigue Investigation Program were carried out in order to investigate marine accidents happened in Korean waters. The Fatigue Investigation Program was established on the MAIFA(Marine Accident Investigators Forum in Asia) web site. Finally, the application plan of the program was discussed.
