Quality of life and the related Factors in Cerebrovascular Surgery Patients (뇌혈관 수술 환자의 삶의 질과 관련요인)

  • Purpose: The purpose of this study was to investigate the quality of life and related factors of CVA surgery patients. Method: The subjects were 64 CVA surgery patients who had discharged and received follow up care at the OPD. Data were collected using William's SS-QOL instrument and IADL. And Data were analyzed with SPSS WIN program in which frequency, percentage, t-test, Pearson's correlation and stepwise regression were used to examine the quality of life and related factors. Result: General characteristics related to QOL were marital status and economical status. Depression was the most important factor with influence on QOL in CVA disease patients after surgery. There were further positive correlation between patient's IADL and QOL and between patient's social support and QOL. Meanwhile the patient's depression level was negatively correlated with QOL. Conclusion: Depression was the most important factor with influence on QOL in CVA patients after surgery. Therefore active nursing intervention to decrease depression and to improve patient's physical functional status in needed. And the patient's family should be included in all nursing intervention and patient education so that the patient's quality of life is prompted by the maintenance of optimal wellbeing.

The Effects of Health Counseling Programs by Occupational Health Nurses on the Unemployed Visiting the Employment Welfare Plus Center (고용복지플러스센터 이용 구직자를 대상으로 산업간호사가 제공한 건강상담 프로그램의 효과)

  • Purpose: The present study was conducted to examine the effects of a health counseling program by occupational health nurses on the unemployed visiting the Employment Welfare Plus Center. Methods: The one-group pretest-posttest design was used. The study presented a secondary analysis of the data collected in a health counseling program run by occupational health nurses for the unemployed visiting the Employment Welfare Plus Center. Kolmogorov-Smirnov, Shapiro-Wilk test, ${\chi}^2$ test and Wilcoxon signed rank test were used to analyze 158 data collected between October 5, 2015 and January 5, 2016. Results: After the intervention, the participants showed a significant decrease in the level of stress index (uBioMacpa), stress (discomfort), musculoskeletal pain and depression. In addition, the participants showed a significant increase in self-rated general health, exercise and regular eating behavior. Conclusion: The health counseling program offered by occupational health nurses is an effective intervention to improve the physical and mental health of the unemployed individuals visiting the Employment Welfare Plus Center.

A Concept Analysis of the Quality of Life for the Children (아동의 삶의 질에 대한 개념 분석)

  • Purpose: The purpose of this study was to attempt an accurate understanding of the quality of life for the children and provide basic materials for application to nursing research, intervention and further theoretical development. Method: Data obtained from such research methods as the lexical and literature review were analysed according to Walker and Avant's(1995) process of concept analysis. Results: As a result of analysis, it was found that the attributes of the concept of the quality of life for the children included value/acceptance, family relationship, safety, good relationship with friends, right of children, school life, parent relationship, leisure, economical stability, not child abuse, and health. There was the coexistence between antecedent element and the outcomes of subjective judgement - health, family, economic strength, education, friends, welfare, social-cultural environment which indicates the coexistence of positive and negative emotions. Conclusion: nursing intervention should be provided for children by concentrating focus on the coordination of internal and external situations in order to prompt them to express their life with positive emotions. Experiential reference was used to suit the purpose of this study based on the use of objective and subjective indicators, and the development of the instrument considering the children were proposed because of the absence of the unified measurement instrument.

The Effects of Foot Reflexology on Pain and Depression of Middle-aged Women with Osteoarthritis (발반사마사지가 골관절염을 가진 중년여성의 통증과 우울에 미치는 효과)

  • Purpose: This study was to examine the effects of foot reflexology on pain and depression of middle-aged women with Osteoarthritis Method: The subjects were 41 osteoarthritis patients resided in the Jinju city from March to May, 2005. The foot reflexology was applied to the experimental group 3 times a week for 4 weeks, 30 minutes eachs. For the data analysis, $X^2$-test was conducted to verify the homogeneity of general characteristics, and t-test was done to verify the homogeneity of pain and depression. To examine the relative efficacy of the intervention, t-test, paired t-test and repeated measures ANOVA were conducted. Results: After foot reflexology, the subjects in experimental group showed significant improvement in pain (F=155.77, p=.000) and depression ((F=20.00, p=.000). Conclusion: The results suggest that the foot reflexology is effective in relieving of pain and depression. Therefore, it is necessary to develop foot reflexology as an independent nursing intervention.

Perception of Touch Therapies Among Nurses Who Take Care of Cancer Patients (암환자를 돌보는 간호사의 접촉 관련 중재에 대한 인식)

  • Purpose: The objectives of this study were to investigate perception of touch therapies among nurses who looking after cancer patients, ultimately to probe possibility of utilization of touch therapies as nursing interventions. Method: A descriptive study conducted from Dec. 2005 to Jan. 2006. 270 nurses taking care of cancer patients in 2 urban hospital were completed self-report questionnaire about general characteristics, utilization of touch therapies, willingness to adopt touch therapies in their practice, and perception of safety and effectiveness of touch therapies. Result: The participants were average 27.97 year-old, 60% of them had bachelor's degree, most of them were staff nurses(83.0%), and 34.6% of them were 3-5 years experienced in oncology nursing. Among 5 touch therapies(therapeutic touch, massage, reflexology, acupressure, and acupuncture), massage was the most positively perceived therapy not only the aspects of safety(67.1%) and effectiveness(65.9%), but also willingness to utilize in their practice(71.2%). There was no significant difference in willingness to utilize in their practice per age and years of working in oncology nursing. Older and more experienced groups perceived more positively to massage and reflexology. Conclusion: Massage might be readily adopted as a nursing intervention in oncology nursing practice. Strategies to encourage the utilization of touch therapies as nursing intervention should be established.

Effects of Exercise Intervention Programs on Recovery of Functional and Quality of Life in Breast Cancer Patients: A Systematic Review and Meta Analysis (유방암 환자 대상의 운동 중재 프로그램이 기능회복과 삶의 질에 미치는 효과: 체계적 문헌고찰 및 메타분석)

  • Purpose: This study was carried out to evaluate the efficacy of exercise program on recovery of functional and quality of life in breast cancer patients through systematic literature review and meta-analysis. Methods: The following databases were used to search the literature: RISS, KISS, KMbase, and KoreaMed. Keywords included 'breast cancer', 'mastectomy' 'exercise', 'rehabilitation', 'intervention' and the evaluated articles were published from 2009 to 2018. The statistical was used R program, the effect size of shoulder functionality, grip strength, upper limb, pain and QOL were calculated by random-effects model. Results: 7 RCT studies were meta-analyzed and were evaluated for the risk of bias by the RoB of Cochrane Collaboration; the overall risk of bias was low. The effect sizes of exercise program was flexion (g=1.11), abduction (g=0.97) as indicated by a "large effect size". Conclusion: It is necessary to study the develop and apply a exercise programs to improve the function and quality of life in breast cancer patients.

The Systematic Review on Clinical Studies of Traditional Korean Medicine Treatment for Obesity in Menopausal Women (폐경 여성의 비만에 응용되는 한방치료에 대한 문헌 고찰)

  • Objectives: The purpose of this study is to review clinical studies of herb medicine and acupuncture treatment on obesity in menopausal women. Methods: Key words "Obesity", "Menopause", "Herb medicine", "Acupuncture", "Moxibustion", "Catgut embedding" were searched on 9 database systems (PubMed Central, Cochrane Controlled Register of Trials, Embase, China Academic Journals, Korean Traditional Knowledge Portal, Oriental Medicine Advanced Searching Integrated System, Korean Studies Information Service System, National Digital Science Library, DBpia) on April 30th 2019. Results: 1 case report and 17 clinical trials were collected in accordance with the selection and exclusion criteria. Among the 17 trials, 6 were randomized controlled trials, 1 was controlled clinical trial, and 10 were single-arm trials. The types of intervention were herb medicine, electroacupuncture, acupuncture, auricular acupuncture, warm needle acupuncture, moxibustion, laser acupuncture, and catgut embedding. The study design, study results and method of intervention were analyzed. Conclusions: 1 case report described the effectiveness of pharmacopuncture, 4 trials described the effectiveness of herbal medicine, 2 of electroacupuncture, 1 of laser acupuncture, and 2 of catgut embedding. Among the 17 trials, 2 studies showed that herbal medicine treatment was more effective than no treatment or selective serotonin reuptake inhibitors, and 1 study showed that electroacupuncture was more effective than hormone therapy. All of 18 selected studies reported the effectiveness of weight reduction and abdominal obesity reduction after the traditional Korean medicine treatment for obesity in menopausal women.

A Literature Review of Return-to-Work Interventions for Cancer Survivors (암 생존자 직장복귀 중재 프로그램 연구 고찰)

  • Purpose: Cancer patients are at a higher risk of being unemployed or facing difficulty in returning to work (RTW) than individuals without health concerns. The aim of this study is to identify and describe interventions developed specifically to assist cancer patients to RTW after treatment. Methods: A comprehensive search was conducted from September to October 2018 in different international databases: PubMed, Medline, and Embase. Studies using qualitative, quantitative, or mixed designs were included if they satisfied the following criteria: (a) described an intervention to assist cancer patients to RTW during or after treatment; (b) conducted on patients aged 18 and over and diagnosed with cancer; (c) written in English; (d) published in peer-reviewed journals. Results: Fourteen studies met the inclusion criteria. Counseling with physical activity intervention, behavioral training to reduce fatigue, and multi-disciplinary rehabilitation programs were found to be effective. Conclusion: Interventions adopting a multidisciplinary approach were effective for RTW in cancer patients. This literature review emphasizes the need for more tailored interventions based on survivors' needs and characteristics in the RTW field.

Etiology of Sex Offences and Intervention Strategy (성폭력 범죄의 원인과 개입전략에 대한 고찰)

  • This paper is a preliminary literature review to give lights on the importance of academic research on sex offences. First, definition and etiological factors of sex offences were introduced and then it was attempted to classify offensive behaviors. Furthermore, it was explored on which aspects of our criminal justice system the research results in the Western societies could be applied. Related to recidivism research, risk factors were explored again and intervention strategies were investigated to reduce the future risk of sex offenders. Finally, these findings were evaluated for the applicability to Korean criminal justice system.

The Effect of Mother-fetus Interaction Promotion Program of Talking and Tactual Stimulation on Mother-fetus Interaction and Mother-infant play Interaction (태담과 촉각 자극의 모-태아 상호작용 증진 프로그램이 모아 상호작용에 미치는 효과)

  • Mother and infant relationship has a great influence on child's developments. Expecially, mother and infant relationship is affected by mother and infant interactions during one year after birth and an omen of mother-infant interaction after birth is mother-fetus interaction. This study was conducted to develop mother-fetus interaction promotion program of talking and tactual stimulation aimed at enhancing sensitivity of primiparas and to evaluate the effect of the nursing intervention program on mother-fetus interaction and mother-infant play interaction. Non-eguivalent control group posttest design was used, and Blumer's symbolic interaction theory and Barnard's mother-infant interaction model was used a conceptual framework of this study. Fifty primiparas and infant(26 mother-infant dyads for intervention group and 24 dyads for control group) were recruited from three general hospital and an OBGY clinic located in Kwang Nung city. Data was collected from January 30th to December 20th in 2001. For the intervention group, programed education which focused on mother-fetus interaction in the 1st trimester. Telephone counselling was provided with interval of two weeks. For two groups, home visiting for data collection of mother- fetus interaction was conducted at 36 to 38 weeks of gestational age. And mother-infant play interactions were assessed at 4 to 6 weeks after birth using videotapes. Play situations were videotaped and two trained observers analyzed the tapes. The data were analyzed using chi-square test and Fisher's Exact Test to test the equivalence of two groups, and the effect of intervention progrom was determined with t-test and Wilcoxon Rank Sum test. The results were as follows: 1. The significant difference was found in mother-fetus interaction between two groups(t=2.085, P=0.0425). It indicated that intervention progrom was effective in improving mother-fetus interaction. 2. The significant difference was found in mother-infant play interaction between two groups(W=347.5, P=0.0001). In subscale analysis, three subscales showed significant differences between the groups: interactional behavior of mother(t=5.921, P=0.0001), interactional behavior of infant(t=5.736, P= 0.0000), and synchronic interactional behavior of mother and infant(t=7.940, P= 0.0000). In conclusion, this study has shown that the applied nursing intervention aimed at enhancing sensitivity of primiparas promoted mother-fetus interaction and mother-infant play interaction. Therefore, this study suggests that this nursing interventions to increase maternal sensitivity to the fetus's movement should be broadly applied to primiparas, which can be beneficial for formation of mother - infant relationship, and for promotion the social, affective, and cognitive developments of their children.
