Designing an Effective Pay-for-performance System in the Korean National Health Insurance

  • The challenge facing the Korean National Health Insurance includes what to spend money on in order to elevate the 'value for money.' This article reviewed the changing issues associated with quality of care in the Korean health insurance system and envisioned a picture of an effective pay-for-performance (P4P) system in Korea taking into consideration quality of care and P4P systems in other countries. A review was made of existing systematic reviews and a recent Organization for Economic Cooperation and Development survey. An effective P4P in Korea was envisioned as containing three features: measures, basis for reward, and reward. The first priority is to develop proper measures for both efficiency and quality. For further improvement of quality indicators, an electronic system for patient history records should be built in the near future. A change in the level or the relative ranking seems more desirable than using absolute level alone for incentives. To stimulate medium- and small-scale hospitals to join the program in the next phase, it is suggested that the scope of application be expanded and the level of incentives adjusted. High-quality indicators of clinical care quality should be mapped out by combining information from medical claims and information from patient registries.

What and Where for Publications by Cancer Registries in the Asian Pacific? - Roles for the APJCP in the Future

  • The absolute necessity of cancer registration for cancer control planning is well accepted. The registry at the national or local level can provide not only essential data for cancer incidence, mortality and survival but may also point to risk and protective factors and efficacy of interventions by conducting epidemiological research. Timely publication of research findings in PubMed indexed journals is of the essence, especially in examples that allow free access so that the widest dissemination of information can be achieved. The present commentary covers the scope of research in Asia or using Asian data the period 2008-2013, nearly 40% of a total of over 300 papers being published in the APJCP. In order to reach its full potential the registry should incorporate many skills. Cooperation for this purpose, whether it be national, regional, Asia-wide or international, is a high priority and the International Agency for Research on Cancer, together with the National Cancer Institute in Thailand and the APOCP/APJCP are staging an Asian Cancer Network Forum in Bangkok in February of 2014 to allow discussion of ways forward. It is hoped that representatives from all regions of Asia will decide to attend and a l so contribute country reports for publication in a special supplement of the APJCP.

Measures against Security Safety and Hallucinogenic Drug Crimes (환각성 약물범죄와 경호안전상의 대응방안)

  • Troublesome problems are being caused by hallucinogenic drugs which come not only from illegal narcotics but also from unprohibited general drug-kinds such as cough-remedy agents of textromerupan, namely, Lamina, Rubi-king and the likes. Abuse and misuse of narcotics permeated into and threatens mental health of socially-known peoples, actor, actress, talents and businessmen as well. This study especially dealt with problems related with Mesampetamin, Rurajaepam, Texstrometropan and Karisofrodol which affects mental behaviour, and considered counter-measures through analysis and developed appropriate system of goods circulation for guidance to be complied by the medicine professionalists. The guidance sought here is recommended to the government ministries for further enforcement of crack-down of the drug crimes. Arquesa in method is recommend as a method of identifying behaviors and facial expressions of abusers and misusers of narcotics at the spot in place. This study is directed to curtailing hallucinogenic drugs use for absolute preventive security by detecting out black dealers of narcotics supplying to the abusers and misusers. There is a need of special education program to cut off the flow of narcotics or drugs crimes into official security force as well as private security personnel. This study introduced a priority and emphasis of real augmentation to theory and practice of the modern scientific security which can protect the mental health of the guarded persons as well especially in the light of security safety curriculum at the school.

Exploration of Community Risk Factors for COVID-19 Incidence in Korea (코로나19 발생의 지역사회 위험요인 분석)

  • Background: There are regional variations in the incidence of coronavirus disease 2019 (COVID-19), which means that some regions are more exposed to the risk of COVID-19 than others. Therefore, this study aims to investigate regional variations in the incidence of COVID-19 in Korea and identify risk factors associated with the incidence of COVID-19 using community-level data. Methods: This study was conducted at the districts (si·gun·gu) level in Korea. Data of COVID-19 incidence by districts were collected from the official website of each province. Data was also obtained from the Korean Statistical Information Service and the Community Health Survey; socio-demographic factor, transmission pathway, healthcare resource, and factor in response to COVID-19. Community risk factors that drive the incidence of COVID-19 were selected using a least absolute shrinkage and selection operator regression. Results: As of June 2021, the incidence of COVID-19 differed by more than 80 times between districts. Among the candidate factors, sex ratio, population aged 20-29, local financial independence, population density, diabetes prevalence, and failure to comply with the quarantine rules were significantly associated with COVID-19 incidence. Conclusion: This study suggests setting COVID-19 quarantine policy and allocating resources, considering the community risk factors. Protecting vulnerable groups should be a high priority for these policies.

An Empirical Study on the Importance of Sales Agency in Apartment Sale by AHP and Fuzzy Analysis (AHP 및 Fuzzy 분석을 통한 분양대행사의 분양성 결정요인 중요도 분석)

  • The purpose of this study is to analyze the importance of the determinants of housing sales agency's role in the sale of apartment housing. For this purpose, a hierarchical decision model was constructed to understand the role and importance of the sales agency. The analytical variable items structured by the research model were set up through literature review, precedent research, and expert brainstorming. The questionnaire consisted of two comparisons for AHP analysis and the importance of absolute importance for fuzzy analysis. Afterwards, the work of correcting the importance was carried out. As a result of the analysis, it was found that the contractor prioritized the sale conditions and the sales agency had priority over the planning for the sale. As a result of analysis, planning of customer pre-sale counseling data, planning of client subscription and contract maximization plan, planning method of advertisement public media method were found to be the most important factors. The results of measurement of absolute importance(fuzzy) & relative importance(,AHP) showed similar tendency. Therefore, it can be seen that the timing of the model house operation is an important period in which the subscription rate depends on the role of the sales agency and the marketing strategy.

Study on the Network System between of the Outpatient and Central Treatment Department of Long Term Care Hospitals (요양병원 외래 및 중앙진료부의 의료영역 간 연결관계에 관한 연구)

  • Purpose: As our population ages and becomes an elderly society the number of elderly care hospitals is rapidly increasing. Because physical functions and spatial perception in the elderly decrease with age, these hospitals require more systematic and intelligent space designs. The design of these spaces are even more complex because they must accommodate medical programs to treat various different diseases and ailments and also because there are many first time patients and irregular short term patients that seek out outpatient treatment services. Also by analyzing the spatial configuration systems and systematic relationships between each of the functional spaces of the outpatient treatment service departments for hospitals specialized in care for the elderly by focusing on the hallway and corridor systems of these hospitals, the according characteristics and trends were examined. Methods: Based on preceding research, the types of hallway and corridor systems of these hospitals were categorized into five types, including gallery corridors, middle corridors, hall-type, mixed type and cyclic type corridors, and into six types according to function including by medical diagnosis, patient registration, examination, administration and convenience and shared common space to derive any interconnecting relationships between the corridor systems. Also by comprehensively examining the types and combined utilization of the corridor types and the integration and the intelligibility of the space syntax, any trends within the corridor system were derived. The elderly care hospitals examined in this research study were twelve hospitals that opened after the year 2000 in Korea with more that 150 sick beds with areas larger than $1000m^2$ and with all outpatient medical service related rooms located entirely on a single floor of the hospital. Results: The following results could be confirmed based on this research study. 1) The spaces where medical diagnosis and examination occurred were adjacent, and the movement lines for first time patients and re-visiting patients were taken into consideration by separating the treatment space. 2) This research study confirmed that the larger the size of the hospital was, there were more detailed categorizations of treatment services and that there was a tendency for treatment areas to be separated and independent from examination areas. 3) There was a tendency for integration and intelligibility to decrease the more complex and diverse the combination of hall types designed into the corridor systems of these hospitals was. cyclic type corridors dramatically decreased the intelligibility of the corridor systems of these hospitals. 4) The priority rank of these spaces were confirmed to be highest in the order of registration, diagnosis, examination, treatment, administration and shared common spaces. However it was confirmed for the local integration that the diagnosis scope had the highest priority rank. Implications: There were exceptional cases confirmed where the number of unit spaces did not have an absolute effect on integration and intelligibility. These results can be interpreted to mean that this can be overcome through efficient architectural planning.

  • Entering legal management, shareholders receive some payment. We call this phenomenon the deviations from absolute priority. This study focuses on incremental effect and determinants of equity to shareholders by wealth transfer from creditors to stockholders in the process of legal management. The main results of this study can by summarized as follows. First, the incremental effect of equity to shareholders is common in the sample of this study. The sample contains 46 non-listed firms that filed for legal management and had confirmed their reorganization plans. Second, the results of the regression model analyzing the determinants of incremental equity to shareholders in legal management show that it is negative related to the solvency(total debt/total asset), firm size, and weight of claims for secured creditors and banks significantly. but corporate reorganization period(from filing to confirmation)are not significant.

Comparison of Measurement Methods for Volatile Organic Compounds in Ambient Air Using Adsorbent Tubes and Canisters (흡착관과 캐니스터를 이용한 대기 중 휘발성유기화합물 측정방법의 비교 평가)

  • This study was carried out to evaluate the performance of two sampling methods, i.e., adsorbent tubes and canisters, for the measurement of ambient volatile organic compounds (VOCs). A total of 24 target VOCs were selected from a list of 48 priority hazardous air pollutants (HAPs) in Korea. The two sampling methods were investigated with a wide range of performance criteria such as repeatability, linearity, and lower detection limits. In addition, mean relative errors (MRE) and mean duplicate precisions (MDP) were estimated by inter-lab comparison studies for duplicate field samples. Precisions for the two methods appeared to be well comparable with the performance criteria recommended by USEPA TO-15 and TO-17 for canister and adsorbent methods, respectively. Correlations and variations between the VOCs concentrations determined by the two methods were generally good in most cases. However, MREs and MDPs for individual VOCs appeared to be widely ranged, depending on each VOC. This implies that the two methods have its own advantages and disadvantages in determining a variety of VOCs in ambient air, and neither of which has absolute superiority. Finally, 9 of 24 VOCs were found to be difficult to determine by either methods due to their unstability in a canister, and lack of appropriate standard materials. Thus, it is suggested that development of measurement methods for such unstable VOCs is an urgent task from a viewpoint of HAPs management.

  • The Analytic Hierarchy Process (AHP) has been widely used as a comprehensive evaluation method since it can include various evaluation standards of both the public and private sectors. It also provides the objective mathematics to process subjective and Personal preferences of an individual or a group in making a decision. This study tried to use AHP to determine the priority of roads investment, considering various effects in a hierarchy such as environmental effects, residential life, and regional development which has not been treated explicitly. As a case study, roads in Gyeongsangbukdo province have been chosen for the evaluation in this research. For the application, it used relative measurements to estimate the weight of upper level structure, and absolute measurement for low level structure instead of pairwise comparisons.

Normalizing interval data and their use in AHP (구간데이터 정규화와 계층적 분석과정에의 활용)

  • Entani and Tanaka (2007) presented a new approach for obtaining interval evaluations suitable for handling uncertain data. Above all, their approach is characterized by the normalization of interval data and thus the elimination of redundant bounds. Further, interval global weights in AHP are derived by using such normalized interval data. In this paper, we present a heuristic method for finding extreme points of interval data, which basically extends the method by Entani and Tanaka (2007), and also helps to obtain normalized interval data. In the second part of this paper, we show that the solutions to the linear program for interval global weights can be obtained by a simple inspection. In the meantime, the absolute dominance proposed by the authors is extended to pairwise dominance which makes it possible to identify at least more dominated alternatives under the same information.
