낚시어선 개념의 재정립과 법제 정비에 관한 연구 (Redefinition of the Concept of Fishing Vessel and Legislation Adjustment)

  • 낚시어선 제도를 도입하게 된 근본 배경에는 평상시 어선으로 순수 어업활동을 영위하다 특정한 시기(금어기 등)에 한해 낚시어선업을 겸업할 수 있도록 하여 영세 어업인의 경제활동에 대한 보조 수단적 자격을 부여하기 위한 것으로 하고 있다. 그리고 여기에서의 낚시어선은 「어선법」에 따라 등록된 어선을 사용하여 유선(遊船) 행위를 할 수 있도록 한 것이므로 낚시어선의 형태 또한 관련 규정에 따라 실질적으로 현장에서 어로활동을 하기에 용이한 일반 보편적인 구조를 가지고 있어야 한다. 그러나 현재 대부분의 낚시어선업자는 소득증대에만 중점을 두면서 일반적인 어선 본래의 용도에 맞게 합당한 형태로 낚시어선을 건조하기보다는 낚시어선업에 치우친 편향된 선체구조를 가지는 등 편법에 준하는 비정상적인 선형을 선호하고 있다. 그 결과, 전체 어업활동 중 낚시어선업을 일부 겸업(부업) 정도로만 여기고 있는 어선 세력들과의 갈등[정부 지원책(면세유 공급 등)에 대한 상대적 형평성 훼손 및 생계형 어족자원 고갈 등]은 물론이고 안전관리에 있어서도 심각한 문제를 일으키고 있는 실정이다. 한편, 이 같은 문제를 야기 시키는 가장 근본적인 원인은 현행 「낚시 관리 및 육성법」에서 낚시어선의 개념을 「어선법」에 따라 등록된 어선으로 제한하고, 또한 이에 따른 검사기준 등을 적용하는 것에서 비롯되고 있다 할 것이다. 이에 따라 본 논문에서는 낚시어선의 분포 현황, 구조적 특성, 낚시어선의 운용실태 및 정부의 낚시진흥정책 등에 대한 분석을 통해 낚시어선에 대한 개념을 현실정에 맞게 관련 법제(규정) 등을 재정비하여 현재의 낚시어선을 어선으로부터 완전히 분리시켜 낚시전용 선박으로 운용하기 위한 개선방안을 제시하고자 한다.

새만금호의 수질 특성 및 영양상태지수를 이용한 부영양화 평가 (Assessment of Eutrophication Using Trophic State Index and Water Quality Characteristics of Saemangeum Lake)

  • 조류의 이상증식을 일으키는 새만금호 부영양화를 평가하기 위해 Carlson 지수를 적용하였다. 연구를 위해 2021년 월별로 새만금호내 총 7개 정점에서 수질조사를 실시하였다. Chl.a의 농도는 동계에는 만경수계가 약간 높았고, 춘계와 하계에는 동진수계가 약간 높게 나타났으나, 일부 시기를 제외하고는 전체적으로 호소 수질환경기준 3등급과 비슷하거나 낮은 농도를 나타내었다. COD는 만경수계와 동진수계 모두 하계와 추계에 호소 수질환경기준 4등급과 유사하거나 상회하는 수질을 나타내었다. TOC는 모든 지점에서 3등급 이내의 수질을 보였다. 총인 농도는 호수 수질환경기준 4등급을 초과하였고, 월별로는 1월과 강우 후 8월에 높게 나타났다. 수질인자간의 상관성 분석에서 염분 농도에 대한 유기물, 총인, 총질소의 상관성이 상대적으로 높게 나타나 배수갑문을 통한 해수유입과 상류 하천을 통한 담수 유입에 의한 담수역, 기수역, 해수역의 수질 특징을 반영하고 있었다. 영양상태지수에 의한 새만금호의 부영양화 변동 특성을 보면, Chl.a와 SD, TN의 지수에서 부영양화 초기 단계의 수질을 보였으며, TP 지수의 경우 심각한 부영양화 상태를 나타내고 있었다. 수질인자간 부영양화 지수의 크기는 모든 수계에서 TSI(TP) > TSI(TN) > TSI(SD) > TSI(CHL)의 순으로 나타났다. TSI(CHL)에 대한 TSI(TP) 및 TSI(SD)와의 편차를 2차원 평면으로 나타낸 사분면 분석 결과를 보면, 조류 성장에 대한 총인이 영향에서는 모든 수계에서 대부분의 총인(TP)에 의한 제한적 영향은 나타나지 않았으며, 빛 감쇠에 영향을 미치는 인자는 외부로부터 유입되는 적은 입자상 물질에 의한 영향이 크게 나타나는 것으로 평가할 수 있다.

  • 북동 멕시코 만에서 particulate organic carbon(POC)의 시/공간적 변화를 조사하기 위하여, 1997년 11월부터 2000년 8월까지 총 9번의 관측이 이루어 졌으며, 같은 기간 동안 위성자료(해색, 표층수온, 해면고도이상, 표층바람)와 주요 강들 의 유출량이 조사되었다. POC 농도는 내 대륙붕과 미시시피 하구 역에서 높은 값을 (>100 $mg/m^3$) 나타내고 대륙붕과 대륙사면으로 가면서 감소된다. POC의 경년 변화는 상대적으로 1997과 1998년(El $Ni\tilde{n}o$)이 1999과 2000년(La $Ni\tilde{n}a$) 보다 상대적으로 증가되어 나타난다. 이런 현상은 미시시피강 및 다른 주요 강들에 영향을 주는 강우량 변화에 따른 담수 유 입의 변화와 직접적으로 연관되어 있으며, 강우량 변화는 ENSO 현상과 같은 세계 기후변화와 관련이 있는 것으로 사료 된다. 북동 멕시코 만 연안으로 유입되는 주요 강들의 방류량은 초봄에 증가되어 여름과 가을에 감소되지만, 공간적으로 확장된 높은 농도의 POC 분포는 여름 조사기간에서 관측되고, 낮은 농도 및 제한된 확산은 가을과 초 봄 관측에서 나타 난다. 여름철 동안 상대적인 강의 유출량은 봄에 비하여 현저하게 감소하지만, 증가된 표층수온은 수층을 강하게 성층화 시키고 표층에서 부력을 증가시킨다. 이런 조건에서 고농도의 POC를 함유하는 저염수는 상부 대륙사면까지 확장되고 이 는 Loop Current와 Loop Current Eddies의해서 조절된다. 봄과 가을 동안 유출량은 보통이거나 이상을 보이지만, 증가된 바람과 낮은 표층수온으로 인하여 수직적 혼합을 유발하고 이는 높은 농도의 POC를 내 대륙붕에 제한 시키는 것으로 사료된다.

경증의 임상 소견을 보이는 COVID-19 환자들의 흉부 CT 소견 (Chest CT Findings of COVID-19 Patients with Mild Clinical Symptoms at a Single Hospital in Korea)

  • 목적 경증의 임상 소견을 보이는 coronavirus disease 2019 (이하 COVID-19) 환자들의 흉부 CT 소견에 대하여 후향적으로 평가하고자 한다. 대상과 방법 경증의 임상 소견(발열 < 38℃, 호흡 곤란 없음)을 보이는 87명의 COVID-19 환자의 흉부 CT 소견을 평가하였다. 결과 39명(44.8%)의 환자는 정상 CT 소견을 보였으며, 48명(55.2%)은 비정상 CT 소견을 보였다. 폐 실질 병변이 있는 48명의 환자 중 17명(35.4%)이 일측성 분포 보였으며, 31명(64.6%)이 양측성 분포를 보였다. 1명(2.1%)의 환자는 흉막하 분포를 보였고, 9명(18.8%)은 기관지 혈관 주위 분포를 보였으며, 38명(79.2%)은 흉막하 및 기관지 혈관 주위 분포를 보였다. 22명(45.8%)의 환자는 경화 없는 순수 간유리 음영 병변을 보였고, 17명(35.4%)은 간유리 음영이 우세한 혼합 밀도의 병변을 보였고, 9명(18.8%)은 경화가 우세한 혼합 밀도의 병변을 보였다. 간유리 음영 없는 경화 병변을 보인 환자는 없었다. 결론 경증의 임상 소견을 보이는 COVID-19 환자의 가장 흔한 CT 소견은 흉막 하 및 기관지 혈관 주위 분포를 보이는 양측성 다발성 간유리 음영 우세 병변이었다. 경증의 임상 소견을 보이는 COVID-19 환자의 거의 절반의 초기 흉부 CT상 폐 실질 병변을 보이지 않았다. 경증의 COVID-19는 상대적으로 중증의 COVID-19과 비교하여 일측성 병변의 비율이 높고, 순수 간유리 음영이나 간유리 음영 우세 병변의 비율이 높으며, 공기기관지 음영의 비율은 낮다.

담낭 절제술 후 총담관 직경의 장기 변화에 대한 CT 평가 (CT Evaluation of Long-Term Changes in Common Bile Duct Diameter after Cholecystectomy)

  • 목적 본 연구는 담낭절제술 후 보상적 총담관 확장의 빈도와 정도를 조사하고, 담낭절제술과 common bile duct (이하 CBD) 확장 사이의 시간을 평가하고, 폐쇄성 CBD 확장을 암시하는 잠재적으로 유용한 CT 소견을 식별하는 것을 목표로 한다. 대상과 방법 2009년에서 2011년 사이에 단일 센터에서 담낭절제술 전후에 여러 차례 CT 스캔을 받은 담도 폐쇄가 없는 121명의 환자를 대상으로 한 후향적 연구를 진행하였다. 또한 담낭절제술 후 CBD 결석 또는 팽대부 종양으로 인해 초기에 연구에서 제외되었던 11명의 환자의 임상 및 CT 소견을 조사하여 폐쇄성 및 비폐쇄성 CBD 확장의 특징을 확인하였다. 결과 121명의 환자의 평균(표준편차) 단축 최대 CBD 직경은 담낭절제술 전 축면에서 5.6(± 1.9) mm였지만 담낭절제술 후 7.9 (± 2.6) mm로 증가했다(p < 0.001). 담낭절제술 전 축성 CBD 직경이 8 mm 미만인 106명의 환자 중 39명(36.8%)이 담낭절제술 후 ≥ 8 mm의 CBD 확장을 보였다. 장기(> 2년) 연속 추적 CT 스캔을 받은 17명의 환자 중 6명(35.3%)은 결국 모두 담낭 절제술 후 2년 이내에 축성 CBD 직경이 유의미하게(> 1.5배) 증가한 것으로 나타났다. 폐색 또는 관련 증상이 없는 121명의 환자 중 단 1명(0.1%)만이 담낭 절제술 후 intrahepatic duct (이하 IHD) 확장 > 3 mm를 보였던 반면, CBD 폐쇄가 있는 11명의 환자 모두 복통과 비정상 검사실 지수가 있었고 81.8% (9/11)가 IHD 및 CBD의 상당한 확장을 보였다. 결론 보상적 비폐쇄성 CBD 확장은 일반적으로 폐쇄성 확장과 비슷한 정도로 담낭절제술 후에 발생한다. 그러나 담낭절제술 후 담관 폐색과 관련 증상이 있거나, 의미 있는 IHD 확장 또는 2-3년 후 추가적인 CBD 확장이 발생하는 경우 CBD 폐쇄를 의심해야 한다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. The frame and leaflet of the polymer valve were made from polyurethane, To evaluate the hemodynamic performance of the polymer valve a comparative study of flow dynamics past a polymer valve and a St. Jude Medical prosthetic valve under physiological pulsatile flow conditions in vitro was made. Comparisons between the valves were made on the transvalvular pressure drop, regurgitation volume and maximum valve opening area. The polymer valve showed smaller regurgitation volume and transvalvular pressure drop compared to the mechanical valve at higher heart rate. The important parameters of this process are the conduction loss to laser power absorption (defined as the conduction-to-laser power parameter, Nk), the convection heat transfer at the surface to conduction loss (defined as Bi), dimensionless extinction coefficient (defined as Br.), and dimensionless irradiation time (defined as Fo).   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. We use combination of MLP and fuzzy filter for pattern recognition to estimate the direction of a muscle and Hogan's method for the force estimation. EMG signals are acquired by using a amputation simulator and 2 dimensional joystick motion. The simulation results of proposed prosthetic arm control system using the EMf signals show that the arm is effectively followed the desired trajectory depended on estimated force and direction of muscle movements. 30638 T00401030638 ^x A new neural network architecture for the recognition of patterns from images is proposed, which is partially based on the results of physiological studies. The bedside monitor consists of a main unit and module cases with various parameter modules. The main unit includes a 12.1" TFT color LCD, a main CPU board, and peripherals such as a module controller, Ethernet LAN card, video card, rotate/push button controller, etc. The main unit can connect at maximum three module cases each of which can accommodate up to 7 parameter modules.   • 토양(土壤)의 몇가지 물리적(物理的), 화학적성질(化學的成質)이 연초식물(煙草植物)의 생육(生育) 및 Ionic Balance에 어떤 영향을 미치고 있는가를 연구(硏究)하기 위하여 폿트실험을 하였다. 공시(供試)된 토양(土壤)은 pH를 3.5와 5.8인 두가지로 조정하였고 이에 2종류의 음(陰)이온을 ($NH_4Cl$의 Cl, $(NH_4)_2SO_4$$SO_4$) 처리하였고, 4수준(水準)의 pF치(値)(1.5-2.0-2.5-3.5)를 두었다. 그 결과(結果)를 요약(要約)하면 다음과 같디. 1. 산도(酸度)가 높은 토양(土壤)(pH3.5)에 $NH_4Cl$이 가(加)해진 경우 연초(煙草) Cl의 함량(含量)이 높았으며, $NH_4$와 Cl의 해작용(害作用)뿐아니라, Mn의 독작물(毒作用)까지 받았으므로 거의 생장(生長)하지 못하고 대단히 비정상적(非正常的)인 생육(生育)을 하였다. 한편 ($(NH_4)_2SO_4$를 많이 흡수하므로 Mg와 Ca의 결핍증(缺乏症)을 보였다. 그러나 산도(酸度)가 낮은 토양(pH5.8)에 가(加)해진 $NH_4-N$은 질산화작용(窒酸化作用)에 의하여 모두 $NO_3-N$로 변하였으며, $NH_4-N$와 함께 가(加)해진 Cl이나 $SO_4$에 관계없이 연초(煙草)는 건전한 생육(生育)을 하였으며 Cl 보다는 $SO_4$가 가(加)해진 토양(土壤)에서 자란 식물(植物)이 건물중(乾物重)도 더 높고 성숙도(成熟度)도 더 빨랐다. 2. 토양(土壤)의 수분함량(水分含量)은 식물(植物)이 인산(燐酸)을 흡수하는데 제한인자(制限因子)가 되는 것으로 생각되며, 지상부(地上部)와 뿌리의 건물수량(乾物收量)은 pF 1.5>2.0>2.5>3.5>의 순서이었고 pF3.5은 연초(煙草)의 일시계절점으로 생각되었다. 3. 연초식물(煙草植物)의 화학분석치(化學分析値)와 건물수량(乾物收量)은 이 식물(植物)이 정상적(正常的)인 (C-A) 개념(槪念)에 잘 적용된다는 것을 보여주었으며, 정상적(正常的)인 생육(生育)을 하고 있는 연초식물(煙草植物)에 있어서는 지상부(地上部)의 Carboxylate의 함량(含量)은 (C-A) 값과 거의 비슷함을 보여 주었다. 만약 식물체중(植物體中)에 $NH_4$함량(含量)이 상당히 많은 양(量)으로 존재(存在)한다면 $NH_4$를 분석(分析)하여 그 값을 (C-A)의 값을 계산하는데 사용해야 할것이다. Al은 가동성(可動性)이 매우 낮은 물질이므로 식물(植物)의 지상부(地上部)의 해(害)를 줄정도의 양(量)으로는 존재(存在)하지 않고 많은 양(量)이 뿌리의 내부나 그 주위에 존재(存在)한다. 식물체(植物體)가 Al을 많이 함유(含有)하고 있을 경우에도 (C-A) 값의 산출에 Al의 함량(含量)을을 고려해야 한다.

기관지 암의 진단에서 형광기관지 내시경검사의 유용성 (Usefulness of LIFE in diagnosis of bronchogenic carcinoma)

  • 배경 : 폐암은 13%만이 완치할 수 있는 예후가 불량한 질환으로 조기진단이 중요한 것으로 알려져 있다. 상피내암이나 미세침윤성 기관지암 등의 조기폐암은 조기에 효과적으로 치료할 수 있어 그 진단이 중요하며, 최근에는 전구암 병변도 조기에 진단 가능하므로써 그 예방적인 치료가 시도되고 있다. 조기진단을 위한 객담 세포진 검사나 기존의 기관지 내시경검사는 조기폐암과 전구암 병변에 대한 진단율이 떨어져 진단에 어려운 실정이다. 최근에는 정상과 암세포의 자기형광 (autofluorescence)을 이용한 형광 기관지 내시경(lung imaging fluorescence endoscope device : LIFE)을 통하여 조기 폐암과 전구암 병변의 진단에 효과적인 것으로 알려져 있다. 목적 : 본 연구는 기존의 내시경을 시행하는 환자를 대상으로 형광기관지 내시경 검사를 시행하여 기관지 점막에 이상이 있는 경우 조직검사를 통하여 두 검사 사이에 민감도와 특이도의 차이를 비교하였다. 방법 : 대상환자는 총 35명으로 평균연령 59.3(27-77)세였으며, 남자 26명, 여자 9명이었다. 흉부 방사선상에 폐암 환자 9명, 폐구엽성 무기폐환자 4명, 폐렴환자 3명, 객담에서 악성세포 양성을 보인 2명, 원인 모르는 흉수 환자 2명, 혈담환자 9명, 만성 기침환자 6명이였다. 대상환자 중 악성종양으로 진단된 환자는 15명이였으며, 기존의 백색빛 광원의 기관지 내시경검사와 형광 기관지 내시경 검사를 시행하였으며, 두 검사에 중에 한 검사에서도 이상병변이 있는 경우 조직검사를 시행하여 총 79조직을 얻어 두 검사간에 민강도와 특이도를 비교하여 다음과 같은 결과를 얻었다. 결과 : 1) 총 79조직은 정상조직이 8, 상피세포 증식이 21, 상피세포의 이형성이 23, 침윤성 폐암이 27조직이었다. 2) 상피세포의 이형성인 23조직에서 백색빚 기관지내시경은 14조직에서 양성으로 60.9 % 였으나, LIFE는 19조직에서 양성으로 82.6 %로 높은 경향을 보였다. 3) 27조직의 침윤성 폐암에 대한 백색빛 기관지내시경는 27조직 모두 양성(100%)인 반면에, LIFE는 19조직이 양성으로 70.3 %의 양성율을 나타냈다. LIFE에서 음성으로 보인 8조직의 침융성 폐암은 선암 7조직 중 3조직, 소세포암 8조직 중 5조직이었으며, 이들은 모두 상피조직은 정상이나 암조직이 점막이하에 침윤되어 있었던 조직이었다. 반면에 상피조직에 이상이 있던 7조직의 편평상피 세포암 조직, 1조직의 대세포암, 2조직의 유암과 2조직의 전이성 앙은 모두 양성이었다. 결론 : 이상의 결과로 LIFE는 전구암 병변과 기관지 기관지 폐암증에 상피세포에 이상이 있는 기관지 폐암의 진단에는 백색빛 기관지내시경보다 민감도가 높아 기관지 상피세포에 이상이 있는 전구암 병변과 조기폐암에는 유용할 것으로 사료되나, 기관지 폐암중에 정상의 기관지 점막하에 칩윤하는 기관지 폐암의 진단에는 한계가 있을 것으로 사료된다.

Tourette씨병의 Serotonin계와 정신병리와의 상호관계에 관한 연구 (STUDY ON THE RELATIONSHIP BETWEEN SEROTONIN SYSTEM AND PSYCHOPATHOLOGY IN TOURETTE'S DISORDER)

  • 틱장애 아동들의 생화학적인 원인을 규명하고, 동반 질환과의 관계를 규명하기 위하여 87명의 틱장애 아동과 30명이 대조군을 대상으로 하여 다음과 같은 결과를 얻었다 87명 중 45명은 Tourette씨병(TS)이었고, 22명은 만성틱장애(chronic motor tic disorder, CMT), 20명은 일과성틱장애(transient tic disorder, TTD)이었다. 공존질병에 따른 분류로는 순수한 틱군(PT)은 43명, 틱과 주의력결핍 과잉운동장애(attention deficit hyperactivity disorder, T+ADHD)가 동반된 군이 28명, 틱과 강박장애(obsessive compulsive disorder, T+OCD)가 동반된 군이 16명이었다. 이들 대상군과 대조군에 대하여 혈장 5- HT(5-hydroxytryptamine)와 5-HIAA(5-hydroxyindoleacetic acid)를 측정하여 다음과 같은 결과를 얻었다. 1) 혈장내 5-HT함량과 5-HTAA함량 간에는 통계적으로 의미있는 상관성이 관찰되었다(Pearson correlation coefficient, 0.77, p<0.05). 2) 대상군에서 연령과 5-HT또는 연령과 5-HIAA함량과는 통계적으로 의미있는 상관성이 관찰되지 않았다. 3) 대조군에서 연령과 5-HT또는 연령과 5-HIAA함량과는 통계적으로 의미있는 상관성이 관찰되지 않았다. 4) 혈장 내의 5-HT함량은 TS, CMT, TTD그리고 대조군간에 통계적으로 의미있는 차이가 관찰되었으며 (F=34.48, df=3.113. p<0.01), 사후검증에서는 대조군과 TS군, 대조군과 CMT군, 대조군과TTD군간에는 의미있는 차이가 관찰되었으나, TS군과 CMT군 TS군과 TTD군, 그리고 TTD군과CMT군 간에는 의미있는 차이가 없었다. 5) 혈장 내의 5-HIAA함량은 TS군, CMT군, TTD군 그리고 대조군 간에 통계적으로 의미있는 차이가 관찰되었으며(F=26.48, df=3,113, p<0.01), 사후검증에서는 대조군과 TS군, 대조군과 CMT군, 대조군과 TTD군 간에는 의미있는 차이가 관찰되었으나. TS군과 CMT군 TS군과 TTD군, 그리고 TTD군과 CMT군 간에는 의미있는 차이가 없었다. 6) 공존질병의 생화학적 소견에 미치는 영향에 대하여는 혈장 내의 5-HT와 5-HIAA함량은 PT군, T+ADHD군, T+OCD군 그리고 대조군간에 통계적으로 의미있는 차이가 관찰되었으며 (5-HT, F=37.59, df=3, 113, p<0.01, 5-HIAA, F=27.37, df=3, 113, p<0.01), 사후검증에서는 대조군과 PT군. 대조군과 T+ADHD군, 대조군과 T+OCD군간에는 통계적으로 의미있는 차이가 관찰되었으나, PT군과 T+ADHD군, PT군과 T+ADHD군, PT군과 T+OCD군, 그리고 T+ADHD군과 T+OCD군간에는 의미있는 차이가 관찰되지 않았다. 이상의 결과로 미루어 serotonin계의 개체발생적인 과정은 대상군과 대조군에서 차이가 없으나, serotonin계의 기능저하가 틱장애의 발병과 관련이 있다고 할 수 있으며, 공존질병이 생화학적인 소견에 미치는 영향은 극미하다고 할 수 있다. 향후의 연구에 있어서는 틱장애에 대하여 serotonin계의 기능항진을 일으키는 약물 투여의 효과에 대한 연구가 시행되어야 하며 분자생물학적인 방법을 사용하여 그 기전을 규명하여야 한다.
