양측성 외전신경 마비를 보인 파열성 후교통동맥 동맥류 - 증례보고 - (Ruptured Posterior Communicating Artery Aneurysm Causing Bilateral Abducens Nerve Paralyses - Case Report -)

  • 뇌 지주막하출혈에서 흔치않은 임상양상인 양측성 외전신경마비와 두통을 주소로 내원한 46세 여자환자에서 혈관조영술을 시행하여 좌측 후교통동맥 동맥류, 전맥락동맥 동맥류, 그리고 우측 후교통동맥 동맥류등 다발성 뇌동맥류가 발견되어 수술시행 하였다. 양측성 외전신경마비 이외에 시력 및 시야장애는 보이지 않았다. 수술시 상당한 뇌압증가의 양상을 보였고, 좌측 후교통동맥 동맥류 출혈흔적과 기저조에 소량의 혈종이 있었으며, 첫번의 좌측 수술 후 2주 정도에 안구운동장애가 서서히 회복되어 정상화되었다. 저자들은 뇌동맥류 출혈에서 드물게 보는 양측성 외전신경마비 환자를 경험하였기에 문헌고찰과 함께 증례보고 한다.

전침치료를 통한 교통사고로 유발된 외전신경마비로 인한 사시 환자의 치험 1례 (The One Case Report of Strabismus of Traffic Accident Induced Abducence Nerve Paralysis Treated with Electro-acupuncture)

  • Objectives The purpose of this case is to report the effect of Korean Medicine, especially Electro-acupuncture on strabismus of traffic accident induced abducens nerve paralysis. Methods Patient received Korean Medicine treatment such as acupuncture, electro-acupuncture & Herb-medicine. Patienst was treated with the electro-acupuncture at Right eye paralytic lateral rectus muscle lesions. The case was enforced 10 minutes using the PG-306 electro-acupuncture and applyed the low consequence wave of 1-8Hz. The improvement of the strabismus was observed. Results During ten weeks of treatment, patient's strabismus and diplopia were improved. Conclusions In this case, Electro-acupuncture on strabismus and diplopia of traffic accident induced abducens nerve paralysis was effective. The effectivity is depended on the time stimulation is performed.

  • We report one case with paralysis of the sixth cranial nerve, which was diagnosed at Kyung Hee Unversity Hospital. The patient was happened with traffic accident. He showed disorder of both abduction eye movement, headache, double vision and dizziness. We have experienced good improvement to recover paralytic strabismus by the Oriental Medicine.

Dissecting Aneurysm of Vertebral Artery Manifestating as Contralateral Abducens Nerve Palsy

  • Isolated abducens nerve paresis related to ruptured vertebral artery (VA) aneurysm is rare. It usually occurs bilaterally or ipsilaterally to the pathologic lesions. We report the case of a contralateral sixth nerve palsy following ruptured dissecting VA aneurysm. A 38-year-old man was admitted for the evaluation of a 6-day history of headache. Abnormalities were not seen on initial computed tomography (CT). On admission, the patient was alert and no signs reflecting neurologic deficits were noted. Time of flight magnetic resonance angiography revealed a fusiform dilatation of the right VA involving origin of the posterior inferior cerebellar artery. The patient suddenly suffered from severe headache with diplopia the day before the scheduled cerebral angiography. Neurologic examination disclosed nuchal rigidity and isolated left abducens nerve palsy. Emergent CT scan showed high density in the basal and prepontine cistern compatible with ruptured aneurismal hemorrhage. Right vertebral angiography illustrated a right VA dissecting aneurysm with prominent displaced vertebrobasilar artery to inferiorly on left side. Double-stent placement was conducted for the treatment of ruptured dissecting VA aneurysm. No diffusion restriction signals were observed in follow-up magnetic resonance imaging of the brain stem. Eleven weeks later, full recovery of left sixth nerve palsy was documented photographically. In conclusion, isolated contralateral abducens nerve palsy associated with ruptured VA aneurysm may develop due to direct nerve compression by displaced verterobasilar artery triggered by primary thick clot in the prepontine cistern.

국내 한의학 학술지에 게재된 사시 관련 논문들의 경향성 분석 (The Analysis on Trend of Articles about Strabismus in Journal of Korean Medicine)

  • Objectives : Strabismus is a disease that can be observed and treated directly from the outside. Therefore, it is very important in Korean medicine ophthalmology in accessibility to treatment. This study is designed to investigate the trends of strabismus-related studies published in Journal of Korean medicine. Methods : 5 Internet databases were selected to search for the study subject. The following 9 keywords were used; Strabismus, Heterophoria, Binocular vision, Visual acuity, Diplopia, Paralytic, Cover test, Oculomotor nerve paralysis, Abducens nerve paralysis. Through this process, a total of 45 studies were found and analyzed into 5 categories ; publication year, publication journal, number of author, type of article, analyzation of case report. Results : There were 2 review articles, 1 original article and 42 case reports. 5 studies(11.1%) were published in 2004 year. Journal of Korean Oriental Medical Ophthalmology & Otolaryngology & Dermatology were the most common with 40%. The authors were often 2 and 3 (each 22.2%). In the case report study, the treatment period was most often 1-30 days (35.7%). Conclusions : 42 studies were on paralytic strabismus and 1 study was on paralytic and non-paralytic strabismus. These results represent that the studies are concentrated in case reports about paralytic strabismus, but this seems to be an area to be improved in future research.

Oral findings and dental management of a patient with Moebius syndrome: a case report

  • Moebius syndrome is a rare, congenital neurological disease involving facial paralysis and limitation of eye movements. It results from maldevelopment of the sixth and seventh cranial nerves. Dental features of this syndrome include micrognathia, microstomia, tongue deformity, cleft palate, hypoplasia of the teeth, and congenital missing teeth. A 7-year-old female with Moebius syndrome was referred from a local dental clinic for caries treatment. She presented with facial paralysis and microstomia. Oral findings included multiple caries with enamel hypoplasia, congenital missing teeth, and tongue deformity. Dental treatments including restorative and preventive procedures were performed. Oral findings and management aspects of Moebius syndrome for this case are discussed. Early evaluation and multidisciplinary care are needed for children with Moebius syndrome.

외전신경마비의 한의학 치료에 대한 체계적 문헌 고찰 (Systematic Review of Korean Medicine for the Treatment of Sixth Cranial Nerve Palsy)

  • Objectives: This research aimed to confirm the efficacy of oriental medicine in sixth cranial nerve palsy and to evaluate the quality of the studies. Methods: Using the medical subject heading (MeSH) keywords "abducens nerve palsy", "abducens nerve paralysis", "sixth nerve palsy", "herbal medicine", "Chinese medicine", "oriental medicine" and "acupuncture", we collected the research papers through an electronic database search in Oriental Medicine Advanced Searching Integrated System (OASIS), National Discovery for Science Leaders (NDSL), PubMed, Cochran Library, and China National Knowledge Infrastructure (CNKI). Results: A total of 136 papers were searched from the databases. Among these, 15 case reports and 8 randomized controlled trials (RCTs) met our criteria. 17 articles were found on the use of acupuncture as the main intervention, 11 on the use of herbal medicine as the main intervention, and 9 on the use of electroacupuncture as the main intervention. The frequently used acupoints were LI4, EX-HN5, GB1, BL2, TE23, LR3, BL1, ST2, GB20, and ST36. All 23 studies confirmed the efficacy of Korean medicine. According to the assessment using the risk of bias, the overall quality of the RCTs was low. Conclusions: Twenty-three papers suggested that Korean medicine treatment for abducens nerve palsy was effective in many cases. Analyzing the potential bias was difficult.

  • Stabismus refers to a muscle imbalance that results in improper alignment of the visual axes of the two eyes. It may be divided into paralytic and nonparalytic strabismus. Paralytic strabismus is primarily a neurologic problem ; nonaralytic strabismus is more strictly an ophthalmologic problem. Characteristic clinical disturbances result from lesions of the third, fourth, and sixth cranial nerves. Lesions of the sixth nerve result in a paralysis of lateral or outward movement and a crossing of the visual axes. We present a case of paralytic strabismus in 24-year-old female who recovered by Oriental medicine, including acupuncture and herbal medicines.
