소아에서 급성 복증으로 오진될 수 있는 복성 간질 (Abdominal Epilepsy Misdiagnosed as Acute Surgical Condition in Childhood)

  • 저자들은 발작적인 복통 혹은 주기적인 구토를 주소로 내원한 8~11세의 소아 4명에서 임상 및 뇌파 소견을 종합하여 '복성간질'로 진단하고 항경련제로 치료하여 증상이 호전되어 문헌고찰과 함께 보고한다. 소아에서 원인을 알 수 없는 발작적인 복통 혹은 구토가 계속될 때 복성 간질을 고려하여 뇌파 검사를 시행하고 항경련제의 투여를 고려해야 할 것으로 사료된다.

  • Epilepsy is a chronic cerebral. disease resulting from a variety of factors, which is a syndrome with chief complaint of recurrent seizure caused by abnormal electrical discharge of cerebral nerve cell, while the clinical result showed that epilepsia occurs more frequently in pediatrics. A Child having the chief complaint of recerrent vomiting for 2 months undergone the brain and abdominal C.T, MRI, and gastric endoscopy, when the child was normal. While vomiting gas too serious to administer a medicine, a cyclic vomiting repeated with its discontinuation for 2-3 days after continuation for 10 days. As a result, the child visited the ambulatorium of our hospital and then was hospitalized and treated over two times, when there was a remarkably improvement. There was no any particular complaint for 1 year and 2 months, then, the child complained moderate headache. For thjs reason, we performed again electroencephalography, when there appeared in epilepsia opinion. Since then, we have experienced a case of autonomic nervous seizure which was significantly improved by anticonvulsant, and a therapy by using the phlegm resolvents and central stimulants for treating epilepsy(豁痰醒腦治癎法) as an oriental medicine. Therefore, it is to report both treatment view of oriental medicine and analysis on Oriental and Western medical literatures.

In Dong-Eui-Bo-Gam, Ondamtng & its Gagambang are applied for the sudden palpitation, epilepsy, the insomnia of exhaustion and an acute convulsion which are caused by the asthenia of heart and gall bladder(心膽虛怯). 2. In Dong-Eui-Bo-Gam, Guibitang & its Gagambang are applied for amnesia, amenorrhea, upper abdominal pain, the itching sign the pubic region, the hernia of the vulva which are caused by the asthenia of heart and spleen(心脾兩虛). 3. In Dong-Eui-Bo-Gam, Soyosan & its Gagambang are applied for epilepsy, the hemorrhage in time of coughing, the intermittent fever, metrorrhagia, various woman's diseases which are caused by the depression of vital energy of liver(肝氣鬱結).

A Case of Cecal Volvulus Presenting with Chronic Constipation in Lissencephaly

  • Cecal volvulus is uncommon in pediatric patients and there are few reports of cecal volvulus with cerebral palsy. Here, we report the case of a 19-year-old male patient who presented with abdominal distension, a history of cerebral palsy, refractory epilepsy due to lissencephaly, and chronic constipation. An abdominal x-ray and computed tomography without contrast enhancement showed fixed dilated bowel intensity in the right lower abdomen. Despite decompression with gastric and rectal tube insertion, symptoms did not improve. The patient underwent an exploratory laparotomy that revealed cecal volvulus. Cecal volvulus usually occurs following intestinal malrotation or previous surgery. In this patient, however, intestinal distension accompanying mental disability and chronic constipation resulted in the development of cecal volvulus. We suggest that cecal and proximal large bowel volvulus should be considered in patients presenting with progressive abdominal distension combined with a history of neuro-developmental delay and constipation.

아로마 복부경락마사지가 입원 뇌병변 장애아동의 변비 완화에 미치는 효과 (Effects of Abdominal Meridian Massage with Aroma Oils on Relief of Constipation among Hospitalized Children with Brain related Disabilities)

  • Purpose: This study was done to evaluate the effects of 3 times/week and 5 times/week abdominal meridian massage with aroma oils (AMMAO) on the relief of constipation among hospitalized children with disabilities involving the brain lesions (cerebral palsy, epilepsy, and others). Methods: The participants were 33 hospitalized children with a disability involving the brain (15 were in the 5 times/week of AMMAO group and 18 were in the 3 times/week of AMMAO group). Data were collected from March 21 to May 1, 2011. Chi-square test, t-test, and repeated measures ANOVA with SPSS 18.0 were used to evaluate the effects of AMMAO. Results: While there was no significant difference between the two groups, there was a significant difference within groups between baseline and the end of the intervention period for the following, frequency of suppository use or enemas, amount of stool, and number of bowel movements. Conclusion: The results of this study indicate that AMMAO is an effective nursing intervention in relief of constipation for hospitalized children with a disability involving the brain. Therefore it is recommended that AMMAO be used in clinical practice as an effective nursing intervention for relief of constipation to these children.
