Use of abdominal compression device in colonoscopy: a systematic review and meta-analysis

  • Background/Aims: Colonoscopy for screening is associated with unpleasant experiences for patients, and abdominal compression devices have been developed to minimize these problems. However, there is a paucity of data supporting the therapeutic benefits of this strategy. This study examined the effects of using an abdominal compression device during colonoscopy on the cecal intubation time (CIT), abdominal compression, patient comfort, and postural changes. Methods: We searched PubMed and Scopus (from inception to November 2021) for randomized controlled trials that assessed the effects of an abdominal compression device during colonoscopy on CIT, abdominal compression, patient comfort, and postural change. A random-effects meta-analysis was performed. Weighted mean differences (WMDs) and Mantel-Haenszel odds ratios (ORs) were calculated. Results: Our pooled analysis of seven randomized controlled trials revealed that abdominal compression devices significantly reduced CIT (WMD, -0.76 [-1.49 to -0.03] minutes; p=0.04), abdominal compression (OR, 0.52; 95% confidence interval [CI], 0.28-0.94; p=0.03), and postural changes (OR, 0.46; 95% CI, 0.27-0.78; p=0.004) during colonoscopy. However, our results did not show a significant change in patient comfort (WMD, -0.48; 95% CI, -1.05 to 0.08; p=0.09) when using an abdominal compression device. Conclusions: Our findings demonstrate that employing an abdominal compression device may reduce CIT, abdominal compression, and postural change but have no impact on patient comfort.

The Effect of Abdominal-Compression Belt on Balance Ability with One Leg Standing

  • Objective: The aim of this study was to determine the effect of abdominal-compression belt in one leg standing on balance in normal adult. Background: With the effects of increased intra-abdominal pressure, the abdominal-compression belt is contributing to a static balance control. However, specific study is still insufficient. Method: Forty subjects were randomly allocated to two groups: control(n=20) and experimental group(n=20), respectively. The experimental group used an abdominal-compression belt, whereas the control group did not that. All subjects were educated using pressure biofeedback unit and ultrasound imaging for exact application by abdominal-compression belt. Main outcome measurement was used a general stability index, fourier harmony index, weight distribution index, and fall index in tetrax balance system. Results: Experimental group improved significantly on general stability, only 2 factors(eyes closed with head turned forward and eyes closed with head turned backward) among fourier harmony index, and fall index, However, weight distribution index did not revealed significant difference. Conclusion: The findings suggest that application of abdominal-compression belt could be effective on improving balance ability in one leg standing of normal adults. Application: The results of the abdominal-compression belt might help to control balance in workers.

한 발 서기 시 복부 압박 벨트가 요통 환자의 정적 균형에 미치는 영향 (Effect of Abdominal Compression Belt on Static Balance During One Leg Standing in Low Back Pain Patients)

  • Purpose: This study aimed to measure static balance of low back pain patients while one-leg standing in abdominal compression belts. Methods: The study included 40 adult males and females at J university, divided into a low back pain patient group and a normal group through the Oswestry disability questionnaire (ODQ). The subjects were instructed to hold a one-leg standing posture for 15 seconds on a balance measurement plate while wearing an abdominal compression belt. Shifting distance (0.1 cm), mean velocity (cm/s), pressure, and contact area were analyzed using BioRescue (BioRescue, RMINGEIEIRIE, Rodez, France). The average value was used to measure the result 3 times for each condition. Results: Both normal and low back pain groups significantly decreased in the speed of sway while wearing the abdominal compression belt. Furthermore, the pressure of the center of motion significantly decreased in the low back pain groups while wearing abdominal compression belt. However, there were no significant differences in the speed of sway or the pressure of center of motion between groups after wearing the abdominal pressure belt. Conclusion: These results suggest that abdominal compression belts are one option for improving balance temporarily. However, balance after wearing abdominal compression vests depends on onset of back pain, age, and symptoms of pain in the groups with low back pain. Further research is needed to investigate muscle activity, dynamic balance, and the effect of the period of wearing abdominal compression belts in the variety of low back pain patients.

Effect of Pelvic Compression Belt on Abdominal Muscle Activity, Pelvic Rotation and Pelvic Tilt During Active Straight Leg Raise

  • Background: Uncontrolled lumbopelvic movement leads to asymmetric symptoms and causes pain in the lumbar and pelvic regions. So many patients have uncontrolled lumbopelvic movement. Passive support devices are used for unstable lumbopelvic patient. So, we need to understand that influence of passive support on lumbopelvic stability. It is important to examine that using the pelvic belt on abdominal muscle activity, pelvic rotation and pelvic tilt. Objects: This study observed abdominal muscle activity, pelvic rotation and tilt angles were compared during active straight leg raise (ASLR) with and without pelvic compression belt. Methods: Sixteen healthy women were participated in this study. ASRL with and without pelvic compression belt was performed for 5 sec, until their leg touched the target bar that was set 20 cm above the base. Surface electromyography was recorded from rectus abdominis (RA), internal oblique abdominis (IO), and external oblique abdominis (EO) bilaterally. And pelvic rotation and tilt angles were measured by motion capture system. Results: There were significantly less activities of left EO (p=.042), right EO (p=.031), left IO (p=.039), right IO (p=.019), left RA (p=.044), and right RA (p=.042) and a greater right pelvic rotation angle (p=.008) and anterior pelvic tilt angle (p<.001) during ASLR with pelvic compression belt. Conclusion: These results showed that abdominal activity was reduced while the right pelvic rotation angle and anterior pelvic tilt angle were increased during ASLR with a pelvic compression belt. In other words, although pelvic compression belt could support abdominal muscle activity, it would be difficult to control pelvic movement. So pelvic belt would not be useful for controlled ASLR.

  • PURPOSE: This study examined the muscle activity of the abdominal muscle when lifting with abdominal hollowing with visual feedback and lifting with a pelvic compression belt. This study suggests how to lift an object safely in the workplace for people who bend their backs repeatedly. METHODS: The study was conducted on healthy men in their 20s and 30s. When lifting a 7kg object, lifting with abdominal hollowing with visual feedback, and lifting an object with a pelvic compression belt were performed three times in random order. The muscle activities were measured rectus abdominis (RA), external oblique (EO), internal oblique/transverse abdominis (IO/TrA) muscles, and abdominal hollowing exercises, and box lifting exercises were carried out in advance before the experiment. One-way ANOVA was used to compare muscle activities, and a Tukey HSD was used for post-analysis. The level of significance was set to .05. RESULTS: According to the study, there was no significant difference in muscle activity of the RA muscle depending on the lifting method (p > .05). There were significant differences between the EO and IO/TrA muscle (p < .05). The IO/TrA muscle activity showed the largest increase in lifting an abdominal hollowing with visual feedback (p < .05). The EO muscle activity increased in pelvic compression belt lifting (p < .05). The muscle activity was increased in RA, but there was no significant difference (p < .05). CONCLUSION: Abdominal hollowing lifting with visual feedback increases the muscle activity of the IO/TrA muscle, which is higher than normal, and affects the core stability of the body.

간세포암 환자에서 ABCHES와 복부압박장비의 적용한 호흡동조방사선 치료의 유용성 비교 (Comparison with ABCHES and Abdomen Compression Device in Respirational Radiation Therapy on Patients in Hepatocellular Carcinoma)

  • 호흡조절 방사선치료는 호흡이나 장기의 움직임을 극복하기 위하여 환자의 호흡주기를 획득하여 종양조직에 처방선량을 부여하며 동시에 주위 정상조직에는 적은 방사선량을 부여하는 방법이다. ABCHES를 이용한 호흡조절 방사선 치료는 호흡을 얕은 호흡으로 유도하며 종양조직의 움직임을 최소화 하는데 도움을 줄 수 있는 부속 장비이다. 한편 복부 압박 기구는 환자의 복부에 압박을 실시함으로 호흡에 제한을 두는 치료보조용 기구이다. 본 논문에서는 ABCHES를 단독으로 사용한 것과 ABCHES와 복부압박 장비를 이용하여 간세포암 환자의 종양의 움직임을 정량적으로 분석하고 그에 해당하는 치료효과를 선량체적곡선으로 분석하려 하였다. ABCHES를 사용한 경우와 ABCHES와 복부압박기구를 동시에 사용한 경우를 비교해본 결과 상하 방향과 앞뒤 방향 그리고 좌우 향으로 각각 평균 1.0 mm, 0.2 mm, 0.2 mm 정도로 그 움직임을 제한 할 수 있음을 확인할 수 있었다. 체적 감소율 또한 ABCHES를 사용한 경우 HPTV와 LPTV가 체적의 변화율을 $16{\pm}2%$ 정도로 줄일 수 있었고, LPTV의 경우 $15.8{\pm}0.8%$의 체적을 제어할 수 있었다. 선량체적 곡선을 분석한 경우 ABCHES만을 사용한 경우보다 ABCHES와 복부압박 기구를 동시에 사용한 경우가 종양조직에는 처방선량에 가까운 선량값을 보였으며 정상조직인 동측 폐, 콩팥, 정상 간조직에는 보다 적은 선량이 부여된 것을 확인 할 수 있었다. 본 논문에 결과 ABCHES를 단독으로 사용하는 것보다 복부압박기구와 ABCHES를 동시에 사용한 경우가 종양 및 정상조직에 부여된 선량 기준에 적합하다고 판명 되었고, 향후 ABCHES와 복부압박 기구에 대한 제한점 즉, 복부의 압박강도에 따른 환자의 불편감, 호흡주기가 정확하지 않은 환자들에게 적용하여야 하는 방법 등이 추후에 논의 되어야 할 것으로 사료된다.

Successful Treatment of Abdominal Cutaneous Entrapment Syndrome Using Ultrasound Guided Injection

  • There are various origins for chronic abdominal pain. About 10-30% of patients with chronic abdominal pain have abdominal wall pain. Unfortunately, abdominal wall pain is not thought to be the first origin of chronic abdominal pain; therefore, patients usually undergo extensive examinations, including diagnostic laparoscopic surgery. Entrapment of abdominal cutaneous nerves at the muscular foramen of the rectus abdominis is a rare cause of abdominal wall pain. If abdominal wall pain is considered in earlier stage of chronic abdominal pain, unnecessary invasive procedures are not required and patients will reach symptom free condition as soon as the diagnosis is made. Here, we report a case of successful treatment of a patient with abdominal cutaneous nerve entrapment syndrome by ultrasound guided injection therapy.

비만환자의 위장조영술에 있어 복와위 압박법의 적용에 관한 연구 (A study on the Application of Prone Compression Study for Obese Patients in Upper Gastrointestinal Series)

  • The aim of this study is to measure the application of prone compression study using compression paddle for obese patients in upper gastrointestinal series. Prone compression study using compression paddle was performed in fifty patients, who were not examined completely erect compression study for obesity. The radiographs of stomach were classified into the lower, middle, and high body and then we gave five points included 'very poor', 'poor', 'suspicious', 'good', and 'complete' according to level of detection for area gastrica and mucosal fold. Statistic analysis was performed using T-test and ANOVA, and confidence rate was fixed in 95%(P<0.05) for the significance. The results were as follows : 1. The compression marks on high body was 'poor' grades in erect and prone compression study. The points were 1.64 and 1.86, respectively. 2. The compression marks on middle body was 1.68 in erect compression study, and 'suspicious' in prone compression study. 3. The compression marks on lower body was 'poor' in erect compression study, and 'good' in prone compression study. 4. There was a high statistic signification between the two study on middle and lower body except for high body(P<0.01). 5. The average abdominal thickness of subjects was 23.98 centimeter. There was no statistic signification between the difference of average marks by the abdominal thickness(P>0.05). As these results, the prone compression study in upper gastrointestinal serie seem to be an useful study for obese patients, because it decreases pain and the feeling of uneasiness, and improve compression efficiency remarkably.

Effects of therapeutic ultrasound for contour asymmetry after liposuction, a case study

  • This is a case report involving a novel setting of sonographic scan for thickness of superficial abdominal fat and a non-invasive approach for reduction of the fat thickness. A 38-year-old woman complained of her abdominal contour asymmetry after the laser-assisted liposuction performed two years ago. Compression-free sonographic scanning confirmed that her superficial abdominal fat was thicker in left side than that of right side. After 9 sessions of 40KHz therapeutic ultrasound, the contour asymmetry was reduced.

  • Background: Improvement of lumbo-pelvic stability can reduce the compensatory action of the quadratus lumborum (QL) and selectively strengthen the gluteus medius (GM) during side-lying hip abduction (SHA). There are abdominal draw-in maneuver (ADIM) and abdominal bracing (AB) as active ways, and pelvic compression belt (PCB) as a passive way to increase of lumbo-pelvic stability. It is necessary to compare how these stabilization methods affect the selective strengthening of the GM. Objects: To investigate the effects of ADIM, AB, and PCB during SHA on the electromyography (EMG) activity of the GM, QL, external oblique (EO) and internal oblique (IO), and the GM/QL EMG activity ratio. Methods: A total of 20 healthy male adults participated in the study. The subjects performed three conditions in side-lying in random order: SHA with ADIM (SHA-ADIM), SHA with AB (SHA-AB), and SHA with PCB (SHA-PCB). To compare the differences among the three conditions, the EMG activities of the GM, QL, EO and IO, and GM/QL EMG activity ratio were analyzed using one-way repeated ANOVA. Results: The EMG activity of the QL was significantly higher in SHA-AB than in SHA-ADIM and SHA-PCB. The GM/QL activity ratio was significantly higher in SHA-PCB than in SHA-ADIM and SHA-AB. In addition, the figure for SHA-ADIM was significantly higher than that for SHA-AB. In the case of the EO, the figure for SHA-AB was significantly higher than corresponding values for the other two conditions. The figure for SHA-ADIM was significantly higher than that for SHA-PCB. The EMG activity of the IO was significantly higher in SHA-AH than in SHA-PCB. Conclusion: It can be suggested that wearing the PCB can more selectively strengthen the GM than to perform ADIM and AB during SHA. In addition, the ADIM can be recommended when there is a need to strengthen abdominal muscles during SHA.
