복부 피하조직으로의 폐흡충증 이소기생 치험례 (Paragonimiasis in the Abdominal Subcutaneous Tissue: A Case Report)

  • Purpose: Paragonimiasis is infectious disease occurred by Paragonimus Westermani, which invades into human body as a final host. Habitual eating the freshwater crab or crawfish unboiled is one of the reason of infection. Paragonimiasis raged in 1970s in Korea, Japan, China and other Asian countries but the incidence decreased rapidly. Once people eat infected second host, parasite penetrates the duodenal wall and migrates to the lung. During this migration period, the parasite can migrate to other organ, such as brain, spinal cord, liver and subcutaneous tissue, but the cases are rarely reported. The objective of our study is to present our experience of the ectopic migration of parasite to the subcutaneous tissue of the abdomen, which was easily treated with excision and Praziquantel medication. Methods: A 59-year-old woman who likes eating unboiled freshwater crab was diagnosed as Paragonimiasis 15 months ago. Her symptoms were fever and cough, and she was treated with Praziquantel medication. 3 months after discharge, she visited our hospital with left pleuritic chest pain, cough with fever, and palpable mass formation on left lower quadrant of the abdomen. Wedge resection of the left lung and Praziquantel medication was maintained for a week. Nevertheless, fever persisted after the treatment. The patient received total excision of the abdominal soft tissue mass, and the fever was relieved. Results: Pathologic findings of the mass showed multiple cyst and abscess formation with crystal structure which is suspicious parts of the parasite or calcified egg shells. Uncontrolled fever was relieved after the operation, and there was no evidence of recurrent Paragonimiasis and ectopic migration for 1 year follow up period. Conclusion: Ectopic migration of Paragonimus is rare, but multiple organ can be involved. Patient with Paragominiasis who was refractory in fever control after Praziquantel medication or surgical evaluation of the lung should be considered as ectopic migration of the Paragonimiasis.

  • Chylous mesenteric cyst is a rare variant of mesenteric cystic lesions. Pathologically there is lack of communication of the main lymphatic vessels, resulting in cystic mass formation. Clinical presentation is diverse and can range from an incidentally apparent abdominal mass to symptoms of an acute abdomen. A 5-year-old girl presented with abdominal distension without pain. CT scan showed a huge and thin-walled cystic mass without solid portion. Laparotomy showed a $20{\times}18cm$ sized huge mesenteric cyst containing chylous fluid. Pathological diagnosis was cystic lymphangioma.

개의 종괴형 간세포암종에 대한 진단 영상 소견에 대한 고찰 (Diagnostic Imaging of Massive Hepatocellular Carcinoma in 5 Dogs)

  • Hepatocellular carcinoma (HCC) is a primary hepatic neoplasms in dogs. Three types of HCC such as massive, nodular and diffuse form were reported. Massive HCC is most common and has relatively better prognosis than other forms because this type of HCC can be removed surgically and has low frequency of relapse or metastasis. Diagnostic image can provide useful information for shape and internal structure of the hepatic mass as well as the location and adhesion or invasiveness of the mass to establish surgical plan to remove the mass safely. In this study, we investigated diagnostic features of massive HCC in 5 dogs. Radiography showed soft tissue mass in cranial abdomen in 3 dogs. On ultrasonography, all dogs had a solitary hepatic mass with mixed echo pattern with anechoic cysts, which represented necrosis and hemorrhage. The radiographic and ultrasonographic findings of affected hepatic lobe were compared with the macroscopic findings through laparotomy. Computed tomography was performed to check metastasis and figure out the origin of the mass in two dogs. Ultrasonography is useful and relatively sensitive examination for diagnosis and planning for surgery in canine massive HCC.

  • Fetus-in-fetu (FIF) is a rare congenital condition in which a fetiform mass is detected in the host abdomen and also in other sites such as the intracranium, thorax, head, and neck. This condition has been rarely reported in the literature. Herein, we report the case of a fetus presenting with abdominal cystic mass and ascites and prenatally diagnosed as meconium pseudocyst. Explorative laparotomy revealed an irregular fetiform mass in the retroperitoneum within a fluid-filled cyst. The mass contained intestinal tract, liver, pancreas, and finger. Fetal abdominal cystic mass has been identified in a broad spectrum of diseases. However, as in our case, FIF is often overlooked during differential diagnosis. FIF should also be differentiated from other conditions associated with fetal abdominal masses.

Clinical Features and Outcomes of Primary Omental Lipoma in a Dog

  • A 12-year-old intact male Cocker spaniel dog was presented for evaluation of lethargy and abdominal distension. Clinical examination revealed a round, palpable mass in the middle of the abdomen. Abdominal computed tomography showed a round soft tissue mass ($width{\times}height{\times}length$, $25{\times}13{\times}15cm^3$) without regional invasion and distant metastasis. Cytologic evaluation of the mass revealed adipose tissue-derived cells having vacuolated cytoplasm, indistinct borders, large nucleus and ropy chromatin pattern with variable sized lipid droplets. Complete surgical resection of the mass was performed and the mass was histopathologically diagnosed as primary omental lipoma. The dog has been recovered well without any additional clinical signs, and there was no relapse over the 8 months follow-up period. The clinical features and prognosis of the dog with primary omental adipocytic tumors have been described in this report.

Successful Postoperative Management of Gastrointestinal Stromal Tumor in a Dog

  • A 10-year-old spayed female Maltese with a history of vomiting and lethargy was referred to the hospital. Physical examination revealed dehydration and severe pain following abdominal palpation. A large mass was observed in the cranial abdomen through radiography and ultrasonography. Laparotomy was performed to find the origin of the mass. The mass was about 8 cm originating from the cecum and subsequently removed. Histopathologic evaluation revealed that the cecal mass was suspected to be a mesenchymal-derived tumor. Through immunohistochemistry, the mass was diagnosed as a gastrointestinal stromal tumor (GIST) based on the c-kit expression. Given its recurrence, postoperative preventive therapy was initiated with masitinib mesylate, which is a tyrosine kinase inhibitor. The animal did not show any side effects during the medication period. After 6 months of therapy, it was well controlled without any recurrence. In this case, we introduced a novel postoperative management of GIST using masitinib mesylate.

복부대동맥류 1례 보고 (A Case of Abdominal Aortic Aneurysm)

  • The incidence of abdominal aortic aneurysm is very rare in this country as other disease of the aorta. Aneurysm can be cased by a variable cause but mainly by arteriosclerosis. It is the disease of aged and degeneration. So recently it may be prevalent due to increase of aged people. In general the natural course of an abdominal aortic aneurysm is very grave so surgical intervention is indicated as soon as possible after the diagnosis. Recently a case of abdominal aortic aneurysm was seen in this clinic with abdominal pain and pulsating mass on the abdomen. This case was confirmed by aortography and treated by graft replacement of the aneurysm with favorable result.

양측성 부신 신경아세포종 (Bilateral Adrenal Neuroblastoma)

  • Neuroblastoma is the most common solid malignancy in childhood. However, a neuroblastoma presenting as bilateral and multifocal is very rare. A male newborn weighing 2.7 kg was born by normal vaginal delivery at 39 weeks' gestation to a 27-year-old mother who had a normal pregnancy. He was in good condition at birth but presented palpable masses in the both upper abdomen and both side of the neck. Ultrasound examination showed the lesions to be $3{\times}3cm$ sized calcified mass in right suprarenal area and $5{\times}3cm$ sized homogenous mass in left suprarenal area. The abdominal mass was also examined by computed tomography with similar findings. With the impression of bilateral neuroblastoma or metastatic spread, the laparotomy was performed on the 13th day of life. Frozen section of biopsy of the left neck mass was obtained, which showed neuroblastoma. Intraoperative findings revealed bilateral adrenal masses which were distinct and anatomically separate. Bilateral adrenalectomy was accomplished and the initial postoperative course was uneventful. The patient was discharged for hopeless and expired at age 45 days. We present bilateral adrenal neuroblastoma considered to be simultaneous occurrence rather than metastases from one site to another.

  • Based on 11 Classics of Materia Medica designated by Ministry of health and welfare bulletin 1995-15 as legal basis in scope and preparation of herbal prescription, we reviewed indication, property and processing of Rhus Verniciflua Stokes which appear extensively in literature since earlier Joseon Dynasty. Following is the conclusion of this review after comparing with 'Korean Pharmacopoeia: commentary on herbal product standard'. 1. The properties of Rhus Verniciflua Stokes in 'Korean Pharmacopoeia :commentary on herbal product standard' should be changed as 'Tonifying the middle, restoring the muscle, fulfilling marrow, breaking old static blood, tonifying and activating after removing mass, unblocking the meridian, killing worms' and indication should be changed as 'Old static blood, deep rooted mass, wind-cold-dampness arthralgia, cough, 9 types of chest pain, abdominal or flank accumulation of stagnated qi[氣], amenorrhea, hernia mass, small bowel or bladder colic pain, abdominal pain due to worm accumulation'. 2. The processing of Rhus Verniciflua Stokes in 'Korean Pharmacopoeia: commentary on herbal product standard' should include 'natural drying or steaming drying followed by grinding and stir-baking until ripened or smoking appears'.

$^{18}F$-FDG PET/CT로 진단된 충수 선암종 ([$^{18}F$]-FDG PET/CT Images of Appendiceal Adenocarcinoma)

  • A 53-year -old man underwent $^{18}F$-FDG whole body PET/CT because of the detected liver mass on abdominal CT. The PET/CT showed a huge liver mass ($9{\times}9cm$, SUV: 12.12 ) in the right lobe and a focally hypermetabolic lesion in the right lower quadrant of abdomen (SUV: 9.12). At first, we suspected that the focal hypermetabolic lesion in RLQ was the physiologic uptake of ureter or a metastatic lesion of small bowel. We repeated the abdominal PET/CT next day. The focally hypermetabolic lesion was identified as the appendiceal mass. He underwent right hemicolectomy and right lobectomy of the liver. It was confirmed that the lesion was appendiceal adenocarcinoma with liver metastasis. Cancer of the appendix is an uncommon disease that is rarely suspected before surgery. But, we suggest that PET/CT is useful to identify the small lesion like appendiceal malignant mass.
