  • Purpose: The purpose of this study is to report the effect of Gami-sibjeon-tang with acupuncture and moxa on endometrioma. Methods: We treated two patients who had endometrioma. Western OB&GY doctors had recommended to operate or observe. But they refused the way western doctors cure the disease. Their chief complainment was severe pain during menstruation period. And they had lower abdominal or lower back pain and acnes on skin. We used Gami-sibjeon-tang on both patients with acupuncture and moxa. Results: After treated by Gami-sibjeon-tang with acupuncture and moxa, they experienced improvement of dysmenorrhea and pain of abdomen and back, and reduction of acnes on skin. And even the sizes of endometrioma had reduced or eliminated. Conclusion: We thought the dysmenorrhea of endometrioma was concerned with inflammation and adhension. So Gami-sibjeon-tang was used when the inflammation and adhension didn't healed because of deficiency of Qi and blood. We administerd Gami-sibjeon-tang with acupuncture and moxa and their dysmenorrhea and other symptoms was relieved.

In an attempt to relieve postoperative pain, 0.5 mg morphine sulfate was administrated into the lumbar subarachnoid space. Pruritus, a side effect of intraspinal morphine, was explored in detail. The results were as follows : 1) The incidence of pruritus was 67.4%, 65.5% in man and 71.0% in woman. 2) The time of onset of pruritus was between 30 and 120 minutes with an average of 79.1 minutes. 3) Pruritus primary occurred on the face(87.9%), especially on the nasal, perinasal and periocular areas. Other sites included the scalp, neck, chest, abdomen, shoulder, hip, thigh, flank, and whole body. 4) The severity of pruritus was classified as mild and moderate, but 4 cases(6.9%) were regarded as severe and were treated with naloxone. 5) The duration of pruritus was from 15 minutes to 19 hours with an average of 4.7 hours. 6) There was no significant difference in the prevention of pruritus between the group recieving diphenhydramine and the one which received normal saline.

The progress was evaluated with the Global Assessment Scale (GAS).Results The abdominal pain was reduced from Numeric Rating Scale (NRS)9 to NRS1 and headache was reduced from NRS5 to NRS3. The tremor was reduced to GAS 10. Unified Parkinson Disease Rating Scale (UPDRS) was reduced from 40 to 27. The difference of heat distribution between left and right side was reduced and the abdomen was more warmer on Digital Infrared Thermal Imaging (DITI).Conclusions Sasang constitutional medicine can be a solution for the non motor symptoms caused by Parkinson's disease.

  • Purpose: This is a retrospective study that investigated cancer patients' complaints of moderate or severe pain to analyze pain characteristics, pain relief interventions and their effects. Methods: The participants of this study were 363 patients who were hospitalized in the cancer ward for three to 30 days and scored 4 points or higher on the pain severity assessment. Results: The most frequent region of pain was the abdomen. The most frequent factor that exacerbated pain was movement. The most frequent pain alleviating factor was administration of analgesics. The most frequent pain type was breakthrough pain, and the most frequent non-pharmaceutical intervention for pain control was heat therapy. Among all, analgesics were routinely prescribed for 52.2% of the participants. Morphine sulfate was the most frequently used analgesic while Gabapentin was the most frequently used non-narcotic analgesic. At the time of discharge, 82.5% of the participants marked their pain intensity as 3 points or lower. Conclusion: For cancer patients complaining of moderate or severe pain, it is important to actively control pain from the beginning of admission. Thus, it is necessary to educate not only cancer patients using narcotic analgesic for pain control and their families but nurses about the effects and side-effects of drugs. Moreover, patients and their families need to learn how to assess and record pain at home to collect data that can be referred for future treatment.

  • An omental infarctions is an uncommon cause of an acute abdomen and a rare entity in children. The etiology is still unclear and the symptoms mimic acute appendicitis. We recently encountered a case of a 10-year-old boy who had a previous surgical history of a manual reduction for intussusception. He complained of abdominal pain and right upper quadrant tenderness without fever or anorexia. An abdominal ultrasonography suggested an omental infarction and computed tomography confirmed the diagnosis. Conservative management was adopted with a rapid and uneventful recovery. Non-operative treatment is a safe and effective treatment of choice for omental infarction.

  • Tuberculosis is not a common cause of liver abscess and it is rarely considered in the differential diagnosis of a patient with hepatic mass. We report a case of tuberculous abscess of liver and spleen in a 15-year-old boy who presented with abdominal pain, fever and weight loss. The ultrasonographic and computed tomographic scan of the abdomen revealed multiple cystic lesions in the liver and spleen. Mycobacterium tuberculosis was cultured from the ascitic fluid and biopsy specimen of lymph node. Follow up CT scan of the abdomen after anti-tuberculosis medication for eighteen months showed complete resolution of the cystic lesions with calcified nodules.

  • Lee Je Ma (1837~1900) wrote the DongEuSuSeBoWon in 1894. Constitutional Medicine what is called Sasang Medicine has originated from him. In this book he refers to mild, serious, intractable, incurable and fatal disease. In the sight of his Medical theory each state of disease deserves to be researched. 1. In the case of Soumin exhaustion of Yang, Taeyang-Kwolum, noxious Yin, Yang is kept in the exterior by excessive Yin in the interior, middle diabetes and swelling belong to intractable disease and solid mass in the right epigastric zone belongs to fatal disease. 2. In the case of Soyangin exhaustion of Yin, lower diabetes, afternoon fever due to deficiency of Yin, spitting blood and swelling belong to intractable disease and distention of abdomen with gas or fluid belongs to fatal disease. 3. In the case of Taeumin syncope due to chills, dryness-heat, diabetes, pain of chest and abdomen and swelling belong to intractable disease and apoplexy with dosed eyes belongs to fatal disease. 4. In the case of Taeyangin vertebral disease makes flaccidity of lower limbs and dysphagia with vomiting of frothy fluid belongs to fatal disease.

  • Celiac plexus block is performed to relieve intractable upper abdominal cancer pain. Generally, celiac plexus blocks have been performed under control of X-ray fluoroscopy to determine the position of the needle tip and the spread of contrast media. During the period from March 1992 to February 1994, we have performed 21 cases of neurolytic celiac plexus block to alleviate pain of intra-abdominal malignancy. We retrospectively evaluated the location of the needle tip and the spread of contrast media. P-A views of simple abdomen demonstrated the locations of the needle tip: 66.7% of the left needle tips were in upper 3/1 of L1 (6 cases) and $T_{12}-L_1$ interspace (8 cases), 50% of the right needle tips were in upper 1/3 of $L_1$, (6 cases) and $T_{12}-L_1$ interspace (4 cases). Contrast media from the right needle spread upward to middle 1/3 of $T_{10}$ (5 cases) and middle 1/3 of $T_{11}$ (5 cases), downward to middle 1/3 of $L_1$ (6 cases) and lower 1/3 of $L_1$ (3 cases). Contrast media from the left needle were spread upward to middle 1/3 of $T_{10}$ (5 cases) and evenly to other spaces, downward to middle 1/3 of $L_1$, (4 cases) and $L_1-L_2$ interspace (6 cases). We analyze the spread of contrast media according to distance from needle tip by authors score system. Contrast media of right needle spread upward 6.1 and downward 3.4, that of left needle spread upward 6.5 and downward 3.7.
