호흡주기에 따른 방사선입체조형치료법의 개발 (Development of Conformal Radiotherapy with Respiratory Gate Device)

  • 목적 : 호흡주기에 따른 위치변동 감지센서를 이용하여 종양의 위치가 일정워치에 있을 때만 방사선을 치료하는 호흡 동기치료기구를 제작하고 일정한 호흡주기 상태에서 수행된 CT simulation과 3차원 입체조형치료계획에 따라 방사선을 치료하는 시스템을 개발하고자 하였다. 호흡유무에 따른 종양의 치료 마진(margin)을 측정하고 계획용표적체적(planning target volume:PTV)의 크기에 따른 선량체적표(dose volume histogram:DVH)와 종양억제확률(tumor control probability:NTCP), 건강조직손상확률(normal tissue complication probability:NTCP) 및 선량 통계자료를 통하여 치료성과를 평가하고 선량증강 범위를 예측하고자 하였다. 대상 및 방법 : 종양이 비교적 작고 전이가 없는(T1N0M0) 5명의 폐암환자를 선택하여 X-선 조준장치를 이용하여 횡격막의 이동거리를 측정하는 방법으로 내부장기의 운동을 평가하였다. 호흡동기치료기구는 끌어당김 센서가 부착된 허리띠 모양으로 구성되었으며 이를 흉곽 또는 복부에 부착하여 호흡주기에 의한 흉곽의 크기변동에 따라 센서의 회로가 개폐되고 이것을 선형가속기의 조종간에 연결하는 간단한 기구로서 감도와 재현성이 높았다. 호흡을 배기한 후 일시적 호흡이 정지된 상태에서 Spiral-CT (PQ-5000)로 3차원 영상을 획득하고 Virtual CT-simulator (AcQ-SIM)에 의하여 종양의 위치와 주위 장기들을 확인 도시하였으며 3차원 치료계획장치(Pinnacle, ADAC Co.)를 이용하여 3차원 입체조형치료를 계획하였다. 치료계획의 평가는 호흡동기치료기구의 사용유무에 따른 PTV의 크기에 따라 최적 선량분포를 구사하였으며 각각의 DVH, TCP, NTCP 및 선량통계자료를 도출 비교 검토하였다. 결과 : X-선 simulation에서 폐암환자의 횡격막 이동은 약 1 cm에서 2.5 cm로서 평균 1.5 cm로 측정되었고 자유호흡시 PTV는 CTV (clinical target volume)에 약 2 cm 마진을 주었으며 호흡동기치료기구를 사용하였을 때는 0.5 cm 마진이 적당한 것으로 측정되었다. 종양의 PTV는 연장 마진의 거의 자승비로 증가하였으며 TCP의 값은 마진 범위 $(0.5\~2.0\;cm)$에 관계없이 거의 일정하였고 NTCP의 값은 마진 크기에 따라 평균 $65\%$로 급속히 증가하였다. 결론 : 호흡주기에 따른 위치변동 감지센서를 이용한 호흡동기치료기구는 종양의 위치가 일정할 때만 방사선이 조사되는 간단하고 정확한 장치로서 3차원 입체조형치료 및 강도변조방사선치료에서 매우 유용한 장치임을 확인할 수 있었다. 또한 호흡조절 방사선입체조형치료방법의 기술과 시술절차를 확립시키고 정량적인 선량평가를 위하여 DVH, TCP, NTCP 등의 정량분석과 종양의 투여 선량 증가량(dose escalation)을 예측하는 기초자료를 제공할 수 있었다.

전산화단층촬영 검사의 방사선 선량에 관한 연구 (A Study on the Radiation Dose in Computed Tomographic Examinations)

  • 본 연구는 전산화단층촬영 검사의 방사선 선량에 관하여 조사하고자 하였다. 이를 위하여 문헌고찰을 실시하였으며, 여러 기관으로부터 자료를 수집하였고, 각 의료기관의 CT장비에서 선량을 측정하였으며, 병원에서 시행하고 있는 Protocol에 입각하여 피폭선량을 계산하였다. 이 연구의 주요 결과는 다음과 같다. 1. 두부팬텀을 이용하여 측정된 100 mAs 당 $CTDI_W$ 값은 4 slice MDCT 장비가 24.20 mGy로 가장 높았다. 장비의 세대별 구분에 따른 $CTDI_W$ 값은 통계적으로 유의한 차이가 있었다(p < 0.01). 2. 복부팬텀을 이용하여 측정한 100 mAs 당 $CTDI_W$ 값은 4 slice MDCT 장비가 13.58mGy로 가장 높았으며, $CTDI_W$ 값은 장비 세대별로 통계적으로 유의한 차이가 있었다(p<0.01). 3. 두부검사에서의 환자선량은 조영제를 사용하지 않을 경우에 가장 높은 장비는 16 slice MDCT로 $818.83\;mGy{\cdot}cm$이었다(p < 0.05). 조영제를 사용하는 경우는 4 slice MDCT 장비에서 $1460.77\;mGy{\cdot}cm$로 가장 높게 측정되었으며, 평균 환자선량은 장비 종류 사이에 유의한 차이가 있었다(p < 0.1). 4. 흉부검사에서의 환자선량은 조영제를 사용하지 않을 경우에 16 slice MDCT에서 평균은 $521.63\;mGy{\cdot}cm$로 가장 높았으며, 장비종류별로 유의한 차이가 있었다(p < 0.05). 조영제를 사용하는 경우에 8 slice MDCT 장비에서의 환자선량이 평균 $1,174.70\;mGy{\cdot}cm$로 가장 높았으며, 장비별로 평균 환자선량에는 통계적으로 유의한 차이는 없었다. 5. 복부-골반검사에서 조영제를 사용하지 않을 경우에 환자선량이 가장 높은 장비는 16 slice MDCT 장비로 평균은 $856.27\;mGy{\cdot}cm$이었으며, 평균 환자선량은 장비 사이에 유의한 차이가 있었다(p < 0.05). 조영제를 사용하는 경우 환자선량이 가장 높은 장비는 16 slice MDCT로 평균 $1,720.64\;mGy{\cdot}cm$이었다. 평균 환자선량은 장비 사이에 유의한 차이가 있었다(p < 0.05). 6. 간 검사에서 조영제를 사용하지 않을 경우에 환자선량이 가장 높은 장비는 8 slice MDCT로 평균 환자선량은 $612.07\;mGy{\cdot}cm$이었고, 환자선량은 장비사이에 유의한 차이가 있었다(p < 0.05). 조영제를 사용하는 경우의 환자선량은 8 slice MDCT 장비의 평균이 $2,197.93\;mGy{\cdot}cm$로 가장 높았으며, 장비사이에 유의한 차이가 있었다(p < 0.1). 조사대상 의료기관의 76.2%에서 조영제를 투여하는 검사의 반복횟수가 $3{\sim}4$회로 방사선피폭에 의한 위험이 있을 것으로 추정되었다.

배곧은근의 혈관 주행에 관한 시신해부 및 컴퓨터단층촬영 (Cadever dissection and Dynamic CT for Vascular Anatomy of Rectus Abdominis Muscle)

  • Purpose: Pedicled transverse rectus abdominis myocutaneous(TRAM) flap has been a gold standard for breast reconstruction and one of surgical techniques preferred by many surgeons. The authors examined the course of deep epigastric artery focusing on distance from margins of rectus abdominis to pedicle and location of choke vessels to get minimal muscles during pedicled TRAM flap operation. Methods: Eleven rectus abdominis muscle from nine cadavers were used in this study. Rectus abdominis was separated from the cadavers, deep inferior and superior epigastric artery were isolated and then 8 anatomical landmarks in medial and lateral margins of rectus abdominis were designated. Distance to a pedicle meeting first horizontally was measured and vertical location from umbilicus to choke vessel was determined. In addition, 32 rectus abdominis images of 16 women(average age: 37.2 years old) from 64 channel abdomen dynamic computerized tomography were also examined with the same anatomical landmarks with those of cadavers. Results: Average distance from four landmarks on lateral margin of rectus abdominis to pedicle was 1.9 - 3.4cm and 1.8 - 3.8 cm on medial margin. Choke vessel was located between middle and inferior tendinous intersection in all cases and average distance between two tendinous intersection was 6.7 - 7.0 cm on medial margin and 6.2 cm on lateral margin. Location of inferior tendinous intersection was on umbilicus or superior of it in all cases and its average distance from umbilicus was 1.8 - 5.6 cm on medial margin and 2.7 - 6.2 cm on lateral margin. Conclusion: Distance from medial and lateral margins of rectus abdominis muscle to pedicle was the shortest in inferior tendinous intersection and that was averagely 1.8 cm on medial margin and 1.9 cm in average on lateral margin. All choke vessels were located between middle and inferior tendinous intersection.

Contrast Media in Abdominal Computed Tomography: Optimization of Delivery Methods

  • Objective: To provide a systematic overview of the effects of various parameters on contrast enhancement within the same population, an animal experiment as well as a computer-aided simulation study was performed. Materials and Methods: In an animal experiment, single-level dynamic CT through the liver was performed at 5-second intervals just after the injection of contrast medium for 3 minutes. Combinations of three different amounts (1, 2, 3 mL/kg), concentrations (150, 200, 300 mgI/mL), and injection rates (0.5, 1, 2 mL/sec) were used. The CT number of the aorta (A), portal vein (P) and liver (L) was measured in each image, and time-attenuation curves for A, P and L were thus obtained. The degree of maximum enhancement (Imax) and time to reach peak enhancement (Tmax) of A, P and L were determined, and times to equilibrium (Teq) were analyzed. In the computed-aided simulation model, a program based on the amount, flow, and diffusion coefficient of body fluid in various compartments of the human body was designed. The input variables were the concentrations, volumes and injection rates of the contrast media used. The program generated the time-attenuation curves of A, P and L, as well as liver-to-hepatocellular carcinoma (HCC) contrast curves. On each curve, we calculated and plotted the optimal temporal window (time period above the lower threshold, which in this experiment was 10 Hounsfield units), the total area under the curve above the lower threshold, and the area within the optimal range. Results: A. Animal Experiment: At a given concentration and injection rate, an increased volume of contrast medium led to increases in Imax A, P and L. In addition, Tmax A, P, L and Teq were prolonged in parallel with increases in injection time The time-attenuation curve shifted upward and to the right. For a given volume and injection rate, an increased concentration of contrast medium increased the degree of aortic, portal and hepatic enhancement, though Tmax A, P and L remained the same. The time-attenuation curve shifted upward. For a given volume and concentration of contrast medium, changes in the injection rate had a prominent effect on aortic enhancement, and that of the portal vein and hepatic parenchyma also showed some increase, though the effect was less prominent. A increased in the rate of contrast injection led to shifting of the time enhancement curve to the left and upward. B. Computer Simulation: At a faster injection rate, there was minimal change in the degree of hepatic attenuation, though the duration of the optimal temporal window decreased. The area between 10 and 30 HU was greatest when contrast media was delivered at a rate of 2 3 mL/sec. Although the total area under the curve increased in proportion to the injection rate, most of this increase was above the upper threshould and thus the temporal window was narrow and the optimal area decreased. Conclusion: Increases in volume, concentration and injection rate all resulted in improved arterial enhancement. If cost was disregarded, increasing the injection volume was the most reliable way of obtaining good quality enhancement. The optimal way of delivering a given amount of contrast medium can be calculated using a computer-based mathematical model.

Head and neck extra nodal NHL (HNENL) - Treatment Outcome and Pattern of failure - A Single Institution Experience

  • Background: Extra nodal lymphoma (ENL) constitutes about 33 % of all non-Hodgkin's lymphoma. 18-28% develops in the head and neck region. A multimodality treatment with multi-agent chemotherapy (CT) and radiotherapy (RT) is considered optimum. Materials and Methods: We retrieved the treatment charts of patients of HNENL treated in our institute from 2001-2012. The charts were reviewed and the demographic, treatment details and outcome of HNENL patients were retrieved using predesigned pro-forma. Results: We retrieved data of 75consecutive patients HNENL. Median age was 47years (Range: 8-76 years). Of the 75 patients 51 were male and 24 were female. 55patients were evaluable. The patient and tumor characteristics are summarized in Table 1. All patients were staged comprehensively with contrast enhanced computed tomography of head, neck, thorax, abdomen, pelvis and bone marrow aspiration and biopsy 66 patients received a combination multi-agent CT with CHOP being the commonest regimen. 42 patients received 4 or lesser number of cycles of chemotherapy whereas 24received more than 4 cycles chemotherapy. Post radiotherapy, 41 out of 42 patients had a complete response at 3 months. Only 21patients had a complete response after chemotherapy. All patients received radiation (mostly involved field radiation) as a part of the treatment. The median radiation dose was 45 Gray (Range: 36 Gray-50 Gray). The radiation was planned by 2D fluoro simulation based technique in 37cases and by 3 Dimensional conformal radiation therapy (3DCRT) in 36 cases. Two patients were planned by the intensity modulated radiation therapy (IMRT) technique. IMRT was planned for one thyroid and one nasal cavity primary. 5 patients experienced relapse after a median follow up of 19 months. The median survival was not reached. The estimated two and three year survival were 92.9% (95%CI- 68.6- 95.35) and 88% (95%CI- 60.82 - 92.66) respectively. Univariate analysis revealed higher stage and poorer baseline performance status to be significantly associated with worse progression free survival. 5 patients progressed (relapse or primary disease progression) after treatment. Of the 5 patients, two patients were primary orbital NHL, two patients had NHL nasal cavity and one was NHL thyroid. Conclusions: Combined modality treatment in HNENL confers excellent disease control with acceptable side effects.

한국인에 있어 깊은아래배벽동맥(Deep Inferior Epigastric Artery)의 천공지(Perforator)에 관한 해부학적 연구 (Anatomical Study for Vascular Distribution of the Perforator of Deep Inferior Epigastric Artery in Koreans)

  • Purpose: The pedicle of transverse rectus abdominis myocutaneous(TRAM) flap and deep inferior epigastric arterial perforator flap is deep inferior epigastic artery (DIEA) and accurate anatomic knowledge about perforator of DIEA is very important for the elevation of these flap. The authors investigated a detailed vascular network of perforator of DIEA in Koreans. Methods: 24 fresh cadavers were studied. Among them, 15 were examined based on the plain X-ray examination for the distribution and location of perforator of DIEA. And 9 fresh cadavers were examined based on the 3-dimensional computed tomography(CT) study for the distance between ending point of perforator of DIEA and mother artery, the distance between most medial mother artery and midline, the distance between most lateral mother artery and midline, and the running type of perforators of DIEA. Results: Based on the plain X-ray examination, suitable(external diameter$${\geq_-}0.5mm$$) perforators of DIEA are located between the level of umbilicus and 8 cm below it. Based on the 3D-CT study, average distance between the ending point of perforator of DIEA and the mother artery is 30.26 mm on the left, 28.62 mm on the right, respectively. The average distance between most medial mother artery and midline is 17.13 mm on the left, 15.76 mm on the right, respectively. The average distance between most lateral mother artery and midline is 56.31 mm on the left, 50.90 mm on the right, respectively. The main running course of suitable perforators of DIEA is type a, which is a direct musculocutaneous perforator vessel from main vascular axis passing outward to join the subdermal plexus, directly. Conclusion: 3-dimensional computed tomography study as well as plain X-ray examination provided more accurate and detail informations about perforators of DIEA in Koreans. These informations will help us understand the detailed vascular anatomy and operation with ease and safe in the lower abdomen of Koreans.

췌장염 환자의 복통 호전을 위한 전침 및 이침 치료 예비 임상연구 프로토콜 (A Clinical Study of Electroacupuncture and Auricular Acupuncture for Abdominal Pain Relief in Patients with Pancreatitis: A Pilot Study)

  • Objectives : The purpose of this study is to evaluate the feasibility of further acupuncture research as an effective alternative and safe treatment for abdominal pain control in patients with pancreatitis. Methods and Results : This study is an open-label, assessment-blind, parallel designed pilot clinical trial. Thirty participants will be assigned to the acupuncture group(n=15) and usual care group(n=15). All patients will receive the conventional standard-of-care(SOC) therapy, but only the experimental group will receive acupuncture therapy six times a week, and the duration of acupuncture therapy will be held up to 12 weeks or until the pains are to be resolved. For the conventional SOC therapy, painkiller will be given. In treatment group, the subjects will receive the identical SOC therapy in combination with electroacupuncture therapy on twelve acupuncture points(LI4, PC6, SP6, GB39, ST36, ST37), and auricular acupuncture therapy on five auricular acupuncture points(Sympathetic, Shen Men, Abdomen, Pancreas gall, and Spleen). The primary outcome will be measured using the visual analogue scale(VAS), and the secondary outcome will be measured using the painkiller demand, quality of life index and severity of pancreatitis by abdominal computed tomography(CT). Assessments will be made at baseline and at week 1, 4, 8 and 12. Results of abdominal CT will be evaluated at baseline and at week 12. Conclusions : The result of this trial will provide a basis for the effectiveness and safety of acupuncture treatment for abdominal pain in patients with pancreatitis.

고지방 식이로 유도된 비만 마우스에서 대두 부산물인 순물과 침지수의 항비만 효과 (Anti-Obesity Effect of By-Product from Soybean on Mouse Fed a High Fat Diet)

  • 본 연구는 대두 부산물인 순물과 침지수가 고지방 식이에 의해 비만이 유도된 마우스의 지방 제거에 미치는 영향을 조사하였다. 침지수가 포함된 일반 사료를 섭이한 실험군의 체중 및 간과 부고환 지방 조직 내 지방 축적은 현저히 적은 것으로 나타났으며, 복부 내 내장 지방과 피하지방도 크게 발달하지 않은 것으로 나타나 침지수는 체내 축적된 지방량을 효과적으로 감소시키는 것으로 조사되었다. 침지수 식이군의 혈중 AST와 ALT의 활성은 대조군 수준으로 유지되었으며, 콜레스테롤 함량과 중성 지질 농도가 감소되는 것으로 나타나 침지수가 고지방 식이로 인한 혈중 효소 활성과 지질농도를 개선하는 것으로 조사되었다. 특히 침지수는 metabolic sensor 단백질인 AMPK와 ACC의 인산화를 촉진시켜 체내 지방산 산화에 영향을 주는 것으로 확인되었다. 또한 침지수는 복부 내 피하지방과 내장지방의 축적을 억제하는 것으로 확인되었다. 따라서 대두 부산물 중 침지수는 비만이 유도된 마우스의 혈중 지질 함량을 개선할 뿐만 아니라 체내 조직의 지방 축적을 완화시키거나 제거하는 데 효과적일 것으로 사료된다.

Hookworm Infection Caused Acute Intestinal Bleeding Diagnosed by Capsule: A Case Report and Literature Review

  • Hookworm infections are rare causes of acute gastrointestinal bleeding. We report a middle aged man with primary nephrotic syndrome and pulmonary embolism. During the treatment with steroids and anticoagulants, the patient presented acute massive hemorrhage of the gastrointestinal tract. The results of gastroscopy showed red worms in the duodenum. Colonoscopy and CT angiogram of abdomen were unremarkable. Capsule endoscopy revealed fresh blood and multiple hookworms in the jejunum and ileum. Hookworms caused the acute intestinal bleeding. The patient responded well to albendazole. Hematochezia was markedly ameliorated after eliminating the parasites. Hence, hookworm infection should be considered in the differential diagnosis of a patient with obscure gastrointestinal bleeding. Capsule endoscopy may offer a better means of diagnosis for intestinal hookworm infections.

Sweet BV가 복부비만에 미치는 영향 (Clinical Studies of Sweet Bee Venom to The Effect of Abdominal Fat Accumulation)

  • Objective The purpose of this study was to investigate the effects of Sweet Bee Venom to the abdominal fat accumulation clinically. Methods The 20 healthy women volunteers who showed the notice of this study by the home page of Sangji University were treated with Sweet Bee Venom(SBV) during twenty times. To investigate the effects of Sweet Bee Venom of the abdominal fat accumulation, abdominal CT, LFT, Thermography, BMI, Inbody 3.0 etc. were performed during clinical trials. And statistical analysis was carried out the data of 10 volunteers who performed all the schedule of this study. Results Following results were obtained from the clinical studies Sweet Bee Venom showed the effect of decreased the body weight, thickness of abdominal skin and fat layer, BMI, and increased abdominal heat, but they are not showed statistical significant. Conclusions These results suggest that treatment Sweet Bee Venom on the abdomen was effective to decrease fat tissue but for the treatment of obesity was performed with right diet program and exercise.
