Comparative Study of Qi-sensational Movement of 'Hanalmomjit' and the Properties of Materia Medica ('한알몸짓'의 기감동작(氣感動作)과 한약의 약성(藥性)에 대한 비교 연구)

  • Directional words like 'ascending(升, sheng)', 'descending(降, jiang)', 'floating(浮, fu)', 'sinking(沈, chen) are technical terminologies of Oriental Medicine to express the directions of material medica functioning-at the phase of Qi-superiorly, inferiorly, exteriorly, and interiorly in human body, and are constituting one of the major principles in prescribing medicines. Lacking the objective methodology to measure or, at least determine Qi, However, we can not expect to secure meaningful data to verify those directional terms. This problematic situation caused chaotic discrepancy between theory and practice of Oriental Medicine. Thus, adopting the method of dynamic meditation 'Hanalmomjit' of Lux Vitae meditation society, by which various state of Qi-sensational movements can be expressed, we compared its moves with the directional terms of traditional canons in Oriental Medicine. In this thesis, the experiment of expressing materia medica via 'Hanalmomjit' is resulted out and compared with the terms, confirmed to have a certain meaningfulness. The new method showed these results below: Herba Menthae, described to have the disposition of ascending and floating, showed 60% of ascending movements in 'Hanalmomjit' experiment. Radix Rehmanniae Praeparata, described to have the disposition of descending and sinking, showed 50% of descending movements. Radix Glycyrrhizae, described to have plain disposition, showed 60% of central movements. Rhizoma Atractylodis and Pericarpium Citri Reticulatae, described to have the disposition of ascending and floating, showed also ascending movements. Cortex Magnoliae Officinalis, described to have the disposition of descending and sinking, showed also descending movements. In conclusion, qi-sensational movement of 'Hanalmomjit' for materia medica showed sufficient relations described in classics, proving itself to be useful method for objective demonstration of material medica.

Prognosis of Unrepaired Ascending Aorta after the Surgical Replacement of Bicuspid Aortic Valves

  • Background: The surgical threshold for bicuspid aortic valve (BAV)-related aortopathy is a matter of debate due to its uncertain etiology and prognosis. This study investigated the prognosis of unrepaired BAV aortopathy in patients undergoing surgical aortic valve replacement (SAVR). Methods: We retrospectively analyzed data from 720 patients (age, 60.8±11.5 years; 246 women) who underwent SAVR for BAV disease without aortic repair between 2005 and 2020 at Asan Medical Center. The clinical endpoints were defined as occurrences of sudden death, aortic dissection or rupture, and elective aortic repair. To estimate postoperative changes in the dimensions of the unrepaired aorta, the individual annual aortic expansion rate was calculated. Multiple linear regression models were used to evaluate the risk of aortic expansion. Results: The mean ascending aortic diameter was 39.5±4.6 mm, and 299 patients (41.5%) had a baseline ascending aorta diameter >40 mm. During 70.0±68.3 months of follow-up, the mean annual aortic expansion rate was 0.39±1.96 mm/yr, no aortic dissection or rupture was observed, and sudden deaths were reported in 12 patients (0.34% per person-year). Linear regression analysis revealed no significant correlation between the baseline ascending aortic diameter and postoperative aortic expansion (R2=0.004, β=-0.84, p=0.082). Conclusion: In selected patients undergoing SAVR for a BAV (<55 mm), the risk of adverse aortic events was very low. As this observation contradicts current practice guidelines advocating for proactive aortic replacement in dilated ascending aortas measuring >45 mm, the study results need further validation by studies involving larger populations or randomized controlled trials.

  • Let D be an integral domain and S a multiplicative subset of D. An ascending chain (Ik)k∈ℕ of ideals of D is said to be S-stationary if there exist a positive integer n and an s ∈ S such that for each k ≥ n, sIk ⊆ In. As a generalization of domains satisfying ACCP (resp., ACC on ∗-ideals) we define D to satisfy S-ACCP (resp., S-ACC on ∗-ideals) if every ascending chain of principal ideals (resp., ∗-ideals) of D is S-stationary. One of main results of this paper is the Hilbert basis theorem for an integral domain satisfying S-ACCP. Also we investigate the class of such domains D and we generalize some known related results in the literature. Finally some illustrative examples regarding the introduced concepts are given.

  • Anomalous origin of right pulmonary artery from ascending aorta is a rare congenital heart disease. We experienced a case of anomalous origin of right pulmonary artery from ascending aorta with associated patent ductus arteriosus and patent foramen ovale, which was diagnosed by angiocardiography and cardiac catheterization. The ductus was ligated just before bypass, and a Dacron-graft with a diameter of 16 mm was interpolated posteriorly to the aorta between the right pulmonary artery and the pulmonary trunk. The postoperative course was uneventful. The right heart catheterization and right ventriculography performed on postoperative twelfth day revealed widely patent anastomotic site between the right pulmonary artery and the pulmonary trunk without residual stenosis. She was discharged on postoperative fourteenth day.

  • Schwannomas are uncommon neoplasms arising from Schwann cells of the neural sheath. Gastrointestinal schwannomas are rare, accounting for 1% of all malignant gastrointestinal tumors. Colonoscopic biopsy with immunohistochemical (IHC) staining is useful for confirming this tumor. We report on a patient with schwannoma arising from the ascending colon, which was detected by colonoscopy and endoscopic submucosal dissection was attempted. A 41-year-old man presented with abdominal discomfort. The patient was diagnosed with a subepithelial tumor on colonoscopy. He underwent endoscopic submucosal dissection. Histopathology and IHC staining confirmed that the colonic lesion was a benign schwannoma. However, the resection margin was positive. Therefore, laparoscopic ileocolectomy was performed.

  • Anomalous origin of a pulmonary artery from the ascending aorta is a rare congenital cardiovascular anomaly which usually involves the right pulmonary artery. For operative reconstruction, the surgical technique of choice used to be a direct end-to-side anastomosis of the ectopic pulmonary artery to the main pulmonary artery. A case of right pulmonary artery arising from the ascending aorta associated with a contralateral patent ductus arteriosus is presented, with description of a new modified surgical technique. The operation was done on cardiopulmonary bypass with deep hypothermia. After closure of PDA, a side-to-side anastomosis between the RPA and MPA, roofed with Gore-Tex patch, was established. The postoperative course was excellent, and the postoperative angiography revealed complete anatomic correction.

One Stage Eepair of Interruption of Aortic Arch with VSD in Neonate (신생아에서 심실중격결손증을 동반한 대동맥궁 결손증의 일단계 완전 교정술 -3례 치험-)

  • Three neonates with interrupted aortic arch with VSD underwent one stage repair using revised technique of cardiopulmonary bypass with short period of circulatory arrest. A left posterolateral thoracotomy was made to permit mobilization of the descending aorta and placement of polytetrafluoroethylene[PTFE graft for distal aortic perfusion. Then the patient was placed in the supine position and a median sternotomy was performed to permit the proximal dissection, VSD repair, and direct anastomosis between the ascending aorta and descending aorta. This technique has advantages to facilitate direct anastomosis between the ascending aorta and the descending aorta, to lessen circulatory arrest time, and to prevent dangerous laceration and post-operative narrowing of the thin small ascending aorta at cannulation site. There was no operative mortality but postoperative stenosis developed in one case which was relieved with balloon aortoplasty.

  • A 40-year-old male patient who had ascending aortic pseudoaneurysm Involving right coronary artery obstruction and thoracic descending aortic pseudoaneurysm was successfully managed by two-stage operation. Repair of intimal tear of ascending aortic pseudoaneurysm with a patch of woven dacron vascular graft and right coronary artery bypass graft with great saphenous vein were performed in first stage operation. On 28 days postoperatively, Repair of intimal tear of descending aortic pseudoaneurysm with a patch of woven dacron vascular graft was done under the femorofemoral partial cardiopulri!onary bypass in second stage operation. The patient was discharged at postoperative 13th days without any evident.

A Study on a Minimizing Method of Baseline Wandering in ECG (심전도 기저선 변동의 최소화방법에 관한 연구)

  • In this paper, we propose a method to minimize the baseline wandering that make hard to detect R wave in ECG. This method uses a different signal between ECG and ascending slope tracing waves to minimize the baseline wandering. When the slope of ECG signal maintains the value or falls, the ascending slope tracing wave fellows ECG signal directly, and this wave holds that value of ECG signal when the slope begins to rises in a certain time(=hold time). After this hold time, this wave traces ECG signal again. To evaluate this minimizing method for baseline wandering, the experiments are carried out with 5 ECG data in the database of MIT/BIH. R waves in the proposed different signal are detected by using descending slope trace waves and compared with the annotation file. The results show that the proposed method Is sure to minimize the baseline wandering in ECG.

  • The purpose of this study is to explain the effect of Naegyeong on Lee Dong-yuan's medical thought. Lee is famous physician who studied the theory of spleen and stomach. His general idea of spleen and stomach could be thought as an result of study on Naegyeong. He distinguished between Eum disease and Yang disease and classified into internal injury and external affection. In the contents of Naegyeong, he took note of the four seasons' changes that are continued by the ascending and descending between heaven and earth, also he took note of the fact that movement of human body's organs is represent of ascending and descending. Human body is a site that a singular ascending and descending of Gi of the stomach.
