Acute Respiratory Distress Syndrome With Alveolar Hemorrhage due to Strongyloidiasis Hyperinfection in an Older Patient

  • Strongyloides stercoralis is an intestinal nematode that occurs sporadically in temperate areas like Korea. People who are in the immunosuppressed state, over the age of 65 or under the corticosteroid therapy are at risk for developing Strongyloides hyperinfection syndrome. Acute respiratory distress syndrome (ARDS) with alveolar hemorrhage is a rare presentation of Strongyloides hyperinfection. A 78-year-old man had been irregularly injected corticosteroid on his knees, but did not have any immunosuppressive disease. He was initially diagnosed with ARDS and septic shock. Bronchoalveolar lavage (BAL) fluid was bloody and its cytology revealed helminthic larvae identified as S. stercoralis. Results of Cytomegalovirus polymerase chain reaction (PCR), Pneumocystis jirovecii PCR, and Aspergillus antigen testing of the BAL fluid were positive. The clinical progress quickly deteriorated with multiple organ failure, shock and arrhythmia, so he finally died. This is a rare case of ARDS in an older patient without any known immunosuppressive conditions, with alveolar hemorrhage and S. stercoralis being found via BAL.

Cardiac Disease Detection Using Modified Pan-Tompkins Algorithm

  • The analysis of electrocardiogram (ECG) signals facilitates the detection of various abnormal conditions of the human heart. The QRS complex is the most critical part of the ECG waveform. Further, different diseases can be identified based on the QRS complex. In this paper, a new algorithm based on the well-known Pan-Tompkins algorithm has been proposed. In the proposed scheme, the QRS complex is initially extracted by removing the background noise. Subsequently, the R-R interval and heart rate are calculated to detect whether the ECG is normal or has some abnormalities such as tachycardia and bradycardia. The accuracy of the proposed algorithm is found to be almost the same as the Pan-Tompkins algorithm and increases the R peak detection processing speed. For this work, samples are used from the MIT-BIH Arrhythmia Database, and the simulation is carried out using MATLAB 2016a.

  • The causes of sudden death after medullary infarction involve arrhythmia, central respiratory failure, and dysautonomia. Sudden cardiac arrest in a medullary infarction is uncommon. Most of these cases experienced sudden cardiopulmonary arrest within 2 weeks from stroke onset as the extent of lesion increased. Here, we report two cases of medullary infarction presenting as sudden cardiac arrest. These cases indicate that medullary infarction could be one of the causes of sudden cardiac arrest.

  • Prolongation of the QT interval may result in a potentially dangerous arrhythmia. The most commonly proposed mechanism for QT interval prolongation(LQT) by pharmaceuticals is inhibition of the rapid delayed rectifier potassium channel (I$\sub$Kr). The LQT potency of pharmaceuticals can be effectively evaluated by examining the effect on human ether-a go-go-related gene (hERG) channels expressed in CHO cells, known to be equal to I$\sub$kr/. We have transfected JERG into CHO cell lines transiently to express high levels of functional hERG channels. (omitted)

P Wave Detection based on QRST Cancellation Zero-One Substitution

  • Cardiac arrhythmias are common heart diseases and generally cause sudden cardiac death. Electrocardiogram (ECG) is an effective tool that can reveal the electrical activity of the heart and diagnose cardiac arrhythmias. We propose detection of P waves based on QRST cancellation zero-one substitution. After preprocessing, the QRST segment is determined by detecting the Q wave start point and T wave end point separately. The Q wave start point is detected by digital analyses of the QRS complex width, and the T wave end point is detected by computation of an indicator related to the area covered by the T wave curve. Then, we determine whether the sampled value of the signal is in the interval of the QRST segment and substitute zero or one for the value to cancel the QRST segment. Finally, the maximum amplitude is selected as the peak of the P wave in each RR interval of the residual signal. The average detection rate for the QT database was 97.67%.

  • This paper investigates a classification method of the electrocardiogram (ECG) into different disease categories. The features for the classification of the ECG are the coefficients of the discrete wavelet transform (DWT) of ECG signals. The coefficients are calculated with Haar wavelet, and after DWT we can get 64 coefficients. Each coefficient has morphological information and they may be good features when conventional time-domain features are not available. Since all of them are not meaningful, it is needed to reduce the size of meaningful coefficients set. The distributions of each coefficient can be the rules to classify ECG signal.   • 본 논문에서는 조기 심실 수축과 조기 심방 수축을 검출함에 있어 정밀한 QRS 구간의 폭, 정확한 P파와 T파의 크기 및 위치를 크게 요구하지 않고, 데이터의 가공과 복잡한 알고리즘의 사용에 의해 발생하는 ECG 데이터의 변형과 손실을 최소화할 수 있으며, 또한 개인차 때문에 발생할 수 있는 오류를 최소화하기 위한 알고리즘을 제안한다. 이를 위해 ECG 신호를 각각의 단위 파형으로 분리한 후, 정상 R-R 간격을 가지는 파형을 기준으로 기준파형을 만들어, 각 파형과 기준파형사이의 패턴 대조 및 유사도 분석을 통해 조기 심실수축과 조기심방수축을 검출할 수 있도록 하였다.

  • Myocardial bridging, a congenital coronary anomaly, is present when a segment of a major epicardial coronary artery, runs intramurally through the myocardium. So with each systole, the coronary artery is compressed. It has been associated with angina, arrhythmia, myocardial infarction and sudden cardiac death. This is a case of a 39-year-old woman who was diagnosed myocardial bridge. She complained of recurrent chest pain, palpitation. We diagnosed her as Gyesimtong(JiXiTong, 悸心痛), and prescribed Jeongkicheonhyang-tang(正氣天香湯). After treatment, all of the symptoms had improved and have not recurred for 18 months. This case suggests that oriental medicine therapy can be applicable to improve in symptoms of myocardial bridge.

  • Mangeum-disease is important syndrome in soyangin's pathology taking urgent diarrhea. In this case study, we diagnose one patient as mangeum-disease. She had a cold before and took a medical treatment with antibiotics. During the treatment she had a diarrhea, palpitation, arrhythmia and edema. We diagnose it as mangeum-disease. So we dose her with hyungbangjihwangtang and we had a good result-all symptoms are disappeared in two months.

  • 최근 사물인터넷(IoT) 기기가 활성화됨에 따라 웨어러블 장치 환경에서 장기간 모니터링 및 수집이 가능해짐에 따라 생체 신호 처리 및 ECG 분석 연구가 활성화되고 있다. 그러나, ECG 데이터는 부정맥 비트의 불규칙적인 발생으로 인한 클래스 불균형 문제와 근육의 떨림 및 신호의 미약등과 같은 잡음으로 인해 낮은 신호 품질이 발생할 수 있으며 훈련용 공개데이터 세트가 작다는 특징을 갖는다. 이 논문에서는 ECG 1D 신호를 2D 스펙트로그램 이미지로 변환하여 잡음의 영향을 최소화하고 전이학습과 메타학습의 장점을 결합하여 클래스 불균형 문제와 소수의 데이터에서도 빠른 학습이 가능하다는 특징을 갖는다. 따라서, 이 논문에서는 ECG 스펙트럼 이미지를 사용하여 2D-CNN 메타-전이 학습 기반 부정맥 분류 기법을 제안한다.
