Isolated Tricuspid Regurgitation: Initial Manifestation of Cardiac Amyloidosis

  • Amyloid deposits in the heart are not exceptional in systemic amyloidosis. The clinical manifestations of cardiac amyloidosis may include restrictive cardiomyopathy, characterized by progressive diastolic and eventually systolic biventricular dysfunction; arrhythmia; and conduction defects. To the best of our knowledge, no previous cases of isolated tricuspid regurgitation as the initial manifestation of cardiac amyloidosis have been reported. We describe a rare case of cardiac amyloidosis that initially presented with severe tricuspid regurgitation in a 42-year-old woman who was successfully treated with tricuspid valve replacement. Unusual surgical findings prompted additional evaluation that established a diagnosis of plasma cell myeloma.

  • 심장의 활동을 전기적 변위로 표현되는 심전도 신호는 심장병 진단에 중요한 임상적 파라미터들을 제공한다. 특히 심전도 신호에서 P, QRS Complex,, T 특징점들로 대표되는 파형 변곡점들의 시간상 위치와 크기 및 형태학적 모양은 심장의 이상 리듬을 나타내는 부정맥여부를 검출하는데 핵심적인 역할을 한다. 본 연구에서는 특히 QRS complex 구간에 대한 첨도치의 연산 해석을 통하여 정상적인 심전도 리듬과 심실조기수축 부정맥 리듬을 구분하는 방법을 제시하고 또한 스마트폰을 기반으로 하는 심전도 모니터링 시스템에 적용하고자 하였다.

  • 본 연구에서는 마이크로프로세서 및 스마트폰 기반의 초소형, 저전력 심전도 계측 시스템에 적용하기 위한 심박동 검출 및 매트릭스 추정 기법을 이용한 실시간 이상 심전도 판별 알고리즘을 구현하였다. MIT-BIH 표준 데이터베이스를 이용하여 실시간 심전도 분석 알고리즘의 성능 평가를 수행한 결과 이상 심전도가 포함되어 있는 7개 레코드에서 심박동 검출 성공률은 99.63%, 매트릭스 추정기법을 이용한 이상 심전도 검출은 92.46%로 우수한 성능을 나타내었다.

  • This paper describes an algorithm of P-wave detection by significant point extraction method with digital differentiation. P-waves are detected by pattern matching algorithm with significant points. By means of defecting onset and offset points of P-waves, this algorithm can measure the PR intervals exactly which is important to diagnosis the atrial arrhythmia. Because P-wave duration is changed according to heart-rates, we calculate the R-R interval prior to the determination of P-wave duration threshold values in order to actively adapt to the change of P duration The results obtained from the proposed algorithm have defected successfully P-wave almost more than 90%.

  • 본 논문은 유전알고리즘을 이용하여 부정맥 진단의 최적화된 입력을 구성하는 방법을 제시한다. 심전도 신호의 특징을 추출하기 위해 웨이블릿 변환이 널리 사용되고 있지만, 추출된 특징들의 선택과 최적화의 문제에 대해서는 명쾌한 해결책을 제시하지 못하고 있다. 심전도 신호는 연속 웨이블릿 변환을 이용해 5레벨로 분해되었으며, 각 서브밴드에서 추출된 계수들은 부정맥 진단을 위한 특징으로 쓰이게 된다. 웨이블릿 변환을 통해 추출된 특징들(feature)은 유전자 알고리즘과 중회귀 분석을 동하여 부정맥 진단을 위한 최적화된 특징조합이 결정되었다. 본 연구를 통해 특정레벨의 어떤 계수가 부정맥 진단에 크게 영향을 미치는지 판단할 수 있었으며 입력의 차원감소는 연산시간의 축소를 가져왔고 분류정확도를 향상시켜 분류기의 성능을 증대시켰다.

  • In this paper, we propose an effective compression method for electrocardiogram(ECG) signals. 1-D ECG signals are reconstructed to 2-D ECG data by period and complexity sorting schemes with image compression techniques to Increase inter and intra-beat correlation. The proposed method added block division and mean-period normalization techniques on top of conventional 2-D data ECG compression methods. JPEG 2000 is chosen for compression of 2-D ECG data. Standard MIT-BIH arrhythmia database is used for evaluation and experiment. The results show that the proposed method outperforms compared to the most recent literature especially in case of high compression rate.

  • This paper describes the design of a hardware system for ECG feature extraction based on pipeline processor consisting of three computers. ECG data is acquisited by 12 bit A/D converter with hardware QRS triggred detector. Four diagnostic parameters-heart, axis, and ST axis, and ST segment are used for the classification and the diagnosis of arrhythmia. The functions of the main CPU were distributed and processed with three microcomputers. Therefore the effective data process and the real time process using microcomputer can be obtained. The interconnection structure consisting of two common memory units is designed to decrease the delay time caused by data transfer between processors and designed by which the delay time can be taken 1% of one clock period.

  • The venom gland of Toad contains large quantities of cardiac glycosides, and toad venom poisoning is similar to digitalis toxicity and carries a high mortality. Sometimes after ingestion of aphrodisiac pills which contain dried toad, a patient develops gastrointestinal symptoms and bradycardia, psychoneurologic symptoms. We have experienced 2 cases of toad venom intoxication, who ingested asian toads. Patients were presented to our ED with nausea, vomiting, and abdominal pain. The patients were peformed monitoring and conservative treatment and were fully recovered. Toad venom intoxication should be considered in patients with clinical manifestation of gastrointestinal irritation, cardiac arrhythmias, hyperkalemia, and detectable serum digoxin level without current medication of digoxin.

  • Tuberculosis developed in the pericardium usually occurs as diffuse constrictive pericarditis or effusive pericarditis which contains much pericardial effusion. But types such as localized abscess or tuberculoma are very rare. Myocardial tuberculosis is also very rare and mainly extended directly from hilar lymph node or spreaded hematogenously in miliary tuberculosis. It is known to be able to make arrhythmia or heart failure by invasion of conduction system or myocardial muscle mass, but it is usually discovered by incidental postmortem autopsy and rarely concerned by clinical basis. Recently we have experienced a case of localized intrapericardial tuberculous abscess which extended to myocardium and operated that successfully. So we would report them with reference study.

Surgical treatment of primary heart tumor -Report of 22 cases- (원발성 심장 종양의 수술적 치료)

  • During 22-year period ending in June 1988, operation was performed on 22 patients with primary heart tumor at Yonsei University College of Medicine. Mean age was 38.8*3.03[mean \ulcornerEM] ranging from 14 to 63 years old. Twenty cases were myxomas. Others were fibromyxoma and rhabdomyosarcoma. All patients complained of dyspnea on exertion. Nine cases had palpitation. Other constitutional symptoms were weight loss [7 cases], headache [4 cases], generalized edema [3 cases] and cough [3 cases]. Five cases had neurological symptoms and signs preoperatively. Preoperative NYHA Class was poor [Class II; 8 cases, III; 9 cases and IV; 5 cases]. The most common site of tumor origin was fossa ovalis limbus [16 cases; 72.8%]. Because of severe mitral regurgitation and of recurrent myxoma, 2 cases were reoperated for mitral valve replacement. Postoperative complications were postoperative mitral regurgitation [5 cases; 22.8%] and arrhythmia [4 cases; 18.2%], one of which was supra-His bundle block. All patient survived operation. Follow-up of 15 patients [mean 28.95*8.3 months] was good as functional class 1[8 cases] or II [7 cases]. More effective adjuvant therapy will be necessary to improve long-term prognosis for malignant primary heart tumor.
