Effects of Neurofeedback Training and Computer-Assisted Cognitive Rehabilitation on Cognition and Upper Extremity Function in PostStroke. (신경되먹임 뇌파 훈련과 컴퓨터보조 인지 재활훈련이 뇌졸중 환자의 인지와 상지기능에 미치는 영향)

  • Objective : This study was to evaluate the effects of a Neurofeedback(NFB) and Computer Assisted Cognitive Rehabilitation(CACR) training to improve on cognition and affected arm function in stroke subjects. Methods : Participants were randomly allocated to three groups: NFB(n=14), CACR(n=14) and control(n=16). All groups received occupational therapy and physical therapy for 5 session 30 minutes per week during 6 weeks. Also NFB and CACR group practiced additional NeuroComp training and RehaCom training for 30 sessions 30 minutes during 6 weeks. Results were evaluated by cognition, affected arm function. Results : There were significantly increased by CACR training that outcomes of MMSE-K(p<.05). And there weren't significantly difference by NFB and CACR training that outcomes of the affected arm function. And a difference between three groups wasn't found. Conclusion : The NFB and CACR training improves cognitive function. These results suggest that NFB and CACR training is feasible and suitable for individuals with stroke.

The Evaluation of Attenuation Difference and SUV According to Arm Position in Whole Body PET/CT (전신 PET/CT 검사에서 팔의 위치에 따른 감약 정도와 SUV 변화 평가)

  • Purpose: For better PET imaging with accuracy the transmission scanning is inevitably required for attenuation correction. The attenuation is affected by condition of acquisition and patient position, consequently quantitative accuracy may be decreased in emission scan imaging. In this paper, the present study aims at providing the measurement for attenuation varying with the positions of the patient's arm in whole body PET/CT, further performing the comparative analysis over its SUV changes. Materials and Methods: NEMA 1994 PET phantom was filled with $^{18}F$-FDG and the concentration ratio of insert cylinder and background water fit to 4:1. Phantom images were acquired through emission scanning for 4min after conducting transmission scanning by using CT. In an attempt to acquire image at the state that the arm of the patient was positioned at the lower of ahead, image was acquired in away that two pieces of Teflon inserts were used additionally by fixing phantoms at both sides of phantom. The acquired imaged at a were reconstructed by applying the iterative reconstruction method (iteration: 2, subset: 28) as well as attenuation correction using the CT, and then VOI was drawn on each image plane so as to measure CT number and SUV and comparatively analyze axial uniformity (A.U=Standard deviation/Average SUV) of PET images. Results: It was found from the above phantom test that, when comparing two cases of whether Teflon insert was fixed or removed, the CT number of cylinder increased from -5.76 HU to 0 HU, while SUV decreased from 24.64 to 24.29 and A.U from 0.064 to 0.052. And the CT number of background water was identified to increase from -6.14 HU to -0.43 HU, whereas SUV decreased from 6.3 to 5.6 and A.U also decreased from 0.12 to 0.10. In addition, as for the patient image, CT number was verified to increase from 53.09 HU to 58.31 HU and SUV decreased from 24.96 to 21.81 when the patient's arm was positioned over the head rather than when it was lowered. Conclusion: When arms up protocol was applied, the SUV of phantom and patient image was decreased by 1.4% and 9.2% respectively. With the present study it was concluded that in case of PET/CT scanning against the whole body of a patient the position of patient's arm was not so much significant. Especially, the scanning under the condition that the arm is raised over to the head gives rise to more probability that the patient is likely to move due to long scanning time that causes the increase of uptake of $^{18}F$-FDG of brown fat at the shoulder part together with increased pain imposing to the shoulder and discomfort to a patient. As regarding consideration all of such factors, it could be rationally drawn that PET/CT scanning could be made with the arm of the subject lowered.

  • Shingles is a latent viral infection of the sensory ganglia. It can be accompanied by a variety of neurologic complications, including polyradiculitis and myelitis. A 66-year-old man with diabetes mellitus presented with progressive weakness, hypethesia and neuralgic pain in his right arm after herpes zoster infection in right C5 dermatome. He was diagnosed with zoster polyradiculomyelitis and treated with intravenous acyclovir and corticosteroid. It is a rare case of zoster neurologic complication in spite of oral acyclovir treatment.

  • We propose statistical methods for evaluating the efficiency of CAPTCHA. Most people unfairly assumed that machines are not capable at reading precisely. This fact leads to the invention of CAPTCHA, a distorted word or short phase, which is designed to thwart computers and separate human from machines. However, advances in image recognition technologies mean that machines are constantly getting better at recognizing CAPTCHA. This forces CAPTCHA designers to design even more difficult CAPTCHAs to prevent their systems from being gamed by malicious bots. However, this arm race has an unintended side effect on the common users. Many CAPTCHAs are now so hard that many people are unable to read them. This obviously conflicts with the original purpose that CAPTCHA was invented in the first place. Our analysis shows that some CAPTCHAs are more users friendly. In particular, Yahoo-style CAPTCHA is the most friendliness. This suggests that a good CAPTCHA could be a simple text with some distortion that prevents machines from correctly segmenting characters.

Spinal Cord Compression as Initial Presentation of Follicular Thyroid Carcinoma

  • Follicular thyroid carcinoma with metastasis rarely manifests as spinal cord compression without any previous symptoms of its malignancy. This report describes a 64-year-old man with follicular thyroid carcinoma who presented initially with left arm motor weakness. Magnetic resonance images demonstrated severe cervical cord compression by a mass with destruction of C4 vertebra. Corpectomy of C4 and anterior interbody fusion was carried out. Histopathological study revealed a metastatic follicular carcinoma of the thyroid. We present our case, especially focused of its possible pathophysiology, with review of pertinent literatures.

  • In this paper, the design and characteristics of a ${\pi}-shaped$ ultrasonic motor which is applicable to optical zoom operation of lenssystem for mobile phone are investigated. Its desi후 and simulation of performances are carried out by FEM (finite element method) commercial software. As a simulation result, by applying voltage with single phase, a combined vibration is produced at the surface of an arm of stator. The prototype of motor is fabricated and its outer size is 8*4*2 $mm^3$ including the cylindrical steel rod of 2 mm in diameter as rotor. The motor exhibits a maximum speed of 500 rpm and a power consumption of 0.3 W when driven at 20 Vpp and 64 kHz.

Q-Switching of a Diode-Pumped Nd:YVO4 Laser with a Closely Folded Resonator

  • A diode-pumped $Nd:YVO_4$ laser is developed. The laser has a closely folded resonator. The laser beam in two arms of the folded resonator passes the same acousto-optic modulator. The laser output power and pulsewidth dependence on a Q-switching frequency were measured. It is found that the newly proposed geometry provides an effective means for reducing the pulsewidth while maintaining almost the same output power compared with the usual folded resonator, where the beam in one arm of the resonator passes the acousto-optic modulator.

  • A laboratory experiment was conducted to investigate the effects of working posture. work/rest ration, and task type on the physical workload in automobile assembly jobs. Ten healthy male university students participated as subjects in the experiment. East subject performed 18 experimental session with combination of 3 working posture 3 work/rest ratio and 2 task types. To examine the effect of various working conditions on workload, DMG analysis was performed on 4 different muscles in the neck an arm area, Also RPE scale and heart rate were taken as response variables. The result showed that all working conditions considered in the experiment had significant effects on the workload concluding that labor intensity in auto assembly jobs should be adjusted with respect to different levels of working posture, work/rest ratio, and task type. Detailed analysis are presented in the paper.

Statistical Analysis of the Performance Reliability Data for Stroke Patients (뇌졸중 환자 운동신뢰성 측정치의 통계적 분석)

  • Purpose: The purpose of this study was to evaluate performance reliability of stroke patients using kinematic analysis. Methods: A protocol to evaluate performance reliability was performed for three tasks on 20 stroke patients and 10 normal people. The tasks include hand to head (HH) task, hand to mouth (HM) task, and hand to target (HT) task. Results: The affected arms showed smaller joint angle, slower peak velocity, longer time to peak velocity for task performances than control group. Also, slower peak velocity and longer movement time for task performance in unaffected arm of stroke patients were obtained compared with the control group. Conclusion: Kinematic analysis is very useful quantitative tool to provide understanding on upper extremity function of stroke patients.

  • In this paper, developed a uninterruptible hotline tools and method for replacement of strain pole crossarm on overhead distribution line. For practical use of developed tools, following direct hot-line methods were proposed: 'Direct hot-line method for replacement of pin pole crossarm', 'Direct hot-line method for alteration of crossarm' and 'Crossarm replacement method in uninterruptible cable displacement area'. These methods were proposed more simple and safe working on hotline job.
