Center of Pressure and Ground Reaction Force Analysis of Task-oriented Sit-to-stand in Stroke Patients (뇌졸중 환자의 과제지향적 일어서기 시 신체압력중심과 지면반발력 특성 )

  • PURPOSE: This study examined the center of pressure (COP) and ground reaction force (GRF) characteristics during each task-oriented sit-to-stand in stroke patients. METHODS: Twenty stroke subjects were included in this study. The task consisted of sit-to-stand (SS), sit-to-stand for reaching (SR), and sit-to-stand for walking (SW). The response time, COP, and GRF were measured during each task. The COP and GRF data were obtained using a two-force plate. The force plates were placed on a chair (below the buttock) and floor (below the feet). RESULTS: Significant differences were observed between SS (1.48 ± .48 s) and SR (2.09 ± 0.82 s) and between SS and SW (2.27 ± .72 s) in the preparatory phase time during each sit-to-stand exercise (p = .002) and showed significant differences between SS (13.90 ± 6.44 cm) and SW (34.62 ± 39.38 cm) and between SR (16.14 ± 8.04 cm) and SW in the mediolateral COP range during each sit-to-stand exercise (p = .013). CONCLUSION: These findings suggest that more complex task-oriented sit-to-stand exercise requires a high-level motor programming process than a simple sit-to-stand task. Therefore, a variety of tasks-oriented sit-to-stand exercises will be useful training to achieve better ADL ability for stroke patients.

Motor and cognitive function according to level of physical activity in stroke patients (뇌졸중환자의 신체활동수준에 따른 운동기능과 인지기능)

  • Background: In the rehabilitation of stroke patients, regular physical activity is very important not only as a treatment for maximal functional recovery but also as a strategy to prevent the recurrence of stroke. The purpose of this study was to objectively measure the amount of physical activity in people with stroke, and to examine the differences in motor and cognitive function according to a level of physical activity. Design: A cross-sectional study. Methods: Physical activity (GENEActiv), motor function (Fugl-Meyer Assessment), cognitive function (Montreal Cognitive Assessment-Korean version), and the Korean version of Modified Barthel Index were evaluated in adult stroke patients with hemiplegia. Results: There was no statistically significant difference in the level of physical activity according to the motor and cognitive function. There was no statistically significant difference in motor and cognitive function according to the level of physical activity, but there was a statistically significant difference in the MBI (p<.01). Conclusion: As a result of the difference in the MBI according to the level of physical activity, it was found that the more moderate to vigorous physical activities are performed, the higher the independence in daily living. These results can be interpreted as that the more often you participate in physical activities such as physical therapy (gait training), the better your independence in ADL. Since regular physical activity participation of adult stroke patients can improve daily living performance, it is considered necessary to participate in physical activities such as continuous physical therapy.

Development of Dementia Care Model in a Community (지역사회 치매관리 모형 개발 : 광명시의 경우)

  • There has been a dramatic increase in public awareness regarding dementia during recent years. However, dementia remains a family affair and patients do not receive adequate care in Korea. This study aims to assist patients and their caregivers by establishing Home and Community based Long-Term Care in a city. The data collected for analysis include five main categories: dementia prevalence, limitations of daily activities of patients, burden of caregivers, the services that patient's family want to utilize, the resources that handle dementia in the community. Major findings can be summarized as follows: 1)The prevalence rate of dementia for elderly people is 13.1 per 100 persons. Alzheimer's disease amount to 38.9% of dementia patients and vascular dementia account for 36.7% of them 2)Eight out of ten patients have mild dementia. Almost all patients have normal ADL. IADL, however, shows different picture. In every items of IADL, about 60% of patients reveals some limitations. 3)The proportion of patients who had medical diagnosis is as low as 20%. Families of patients think dementia as normal aging process and medical doctors in the community do not give special concern to dementia patients. 4)Caregivers does not have proper social support. They suffer from long care time, experience large obstacles in respect of health, daily living, and social activity. 5)Health center and Community welfare center have launched some programs-consultation, home-visiting nursing, day care center, voluntary force mobilization and so on-for dementia patients. But they do not perform expected roles and functions because of lack of skilled personnels and inadequate coordination of relevant organizations for dementia care. 6)Families of dementia patients prefer home helper and home-visiting nurse to hospitalization. For the future, however, demand for institution-based long-term services will increase. We develope community dementia care model based on above findings as follows: 1)Health center execute community cardiovascular control program for the prevention of vascular dementia. 2)Refer to epidemiologic characteristics of patients and preference of family, the most urgent task for dementia care in this city is to expand and organize Home and Community based Long-Term Care. 3)For the continuous and comprehensive care, care plan for a patient must be prepared. Case management team should be builded to prepare this plan and coordinate relevant resources. 4)Special long-term care unit for dementia will be needed in a near future. This unit should have multiple functions, such as day-care center, short stay facility, training center for relevant personnels, besides long-term nursing home considering effective care of dementia and efficient operation of the facility. 5)Voluntary workers deserve their due efforts. Incentive mechanisms must be developed to activate voluntary activities.

  • Background: This research has been carried out to analyze presupposition factors which influence factors on Rehabilitation Exercise practice in Elderly limited Activities of Daily-Living an Anlaysis based on Theory of Planned Behavior to support to induce Rehabilitation Exercise behavior. Method: From the geriatric hospitals in Pusan and Kyungnam, 316 elderly who has ADL Disability participated in the study. We did 1st questionnaire investigation. We provided them 4 weeks of Rehabilitation Exercise training, and 2 weeks later we collected 2nd questionnaire sheet to confirm their Rehabilitation Exercise behavior practice and analysed. Results: In this research relative influence to behavioral intention for Rehabilitation Exercise headed by attitude toward the behavior(.61), perceived behavior control(-.56) and subjective norm(.27)these results were slightly different from other research, in case of perceived behavior control was low but on attitude toward the behavior it showed high in results. It's older have strong subjectivities and has not change their mind, the old's behavioral intention is expressed as an attitude toward the behavior. Rehabilitation Exercise Practice of Elder should prioritize to have the health belief which is positive, As a result it will be regarded to expect the maximum effect.

Evaluation of Human Body Effects during Activities of Daily Living According to Body Weight Support Rate with Active Harness System (동적 하네스 체중지지율에 따른 일상생활 동작 시 인체영향평가)

  • In this paper, we measured human body signals in order to verify a active harness system that we developed for gait and balance training. The experimental procedure was validated by tests with 20 healthy male subjects. They conducted motions of Activities of Daily Living(ADL)(Normal Walking, Stand-to-Sit, Sit-to-Stand, Stair Walking Up, and Stair Walking Down) according to body weight support rates (0%, 30%, 50% of subjects' body weight). The effectiveness of the active harness system is verified by using the results of foot pressure distribution. In normal walking, the decrease of fore-foot pressure, lateral soleus muscle and biceps femoris muscle were remarkable. The result of stand-to-sit results motion indicated that the rear-foot pressure and tibialis anterior muscle activities exceptionally decreased according to body weight support. The stair walking down show the marked drop of fore-foot pressure and rectus femoris muscle activities. The sit-to-stand and stair walking up activities were inadequate about the effect of body weight support because the velocity of body weight support system was slower than male's activity.

The Relationship Between Self-Concept and Self-Care Activities of the Cerebral Palsied Adolescents (뇌성마비 청소년의 자아개념과 자가간호수행의 관계)

  • The purpose of this study was to investigate the relationship between self-concept and self-care activities of the cerebral palsied adolescents and to gain the baseline data for development of effective rehabilitation nursing intervention program of the cerebral palsied adolescents. The design of this study was a descriptive correlational study. The subjects of the study were 160 cerebral palsied adolescents attending at special schools located in Seoul and Kyonggi and rehabilitation centers located in Seoul, Kyonggi and Kyongnam province. The data was collected from May 20 to July 20, 2000. The instrument used for this study were the self-concept scale(50items 4point scale) and self-care activities scale(29items 4point scale). Self-Concept Scale had developed by Fitt(1965), which was standardized by Chung(1968) and modified by Kim(1984). Self-Care Activities Scale developed by the researcher through out the consulting of expert and pilot study on the basis of ADL check list developed by Kang(1984) and LDSQ-3(Lambeth Disability Screening Questionnair-3) developed by Na et al. (1995). The data was analyzed by the SPSS/PC+program using frequency, percentage, mean, standard deviation, t-test, ANOVA & Scheffe test and Pearson's correlation coefficient. The results of this study were as follows; 1. The mean score of self-concept was $138.55{\pm}17.20$(range: 50-100), which the item mean score was $2.77{\pm}0.34$(range: 1-4). The score of subarea of the self-concept was the highest score in family self ($3.01{\pm}0.54$) and the lowest score in physical self ($2.52{\pm}0.42$). 2. The mean score of self-care activities was $95.25{\pm}21.69$ (range: 29-116), which the item mean score was $3.28{\pm}0.75$ (range: 1-4). The score of subarea of the self-care activities was the highest score in feeding($3.75{\pm}0.59$) and the lowest score in walking($2.64{\pm}1.21$). 3. There were statistically significant difference in the score of self-concept according to the age (F=3.24, P=.04), the grads (F=4.36, P=.01), and types of cerebral palsy (F=2.42, P=.03). 4. There were statistically significant difference in the score of self-care activities according to the age (F=8.29, P=.00), the grads (F=16.05, P=.00), types of living place (F=6.46, P=.00), types of cerebral palsy (F=48.92, P=.00), whether or not receiving a rehabilitation therapy (t=-3.64, P=.00), whether or not receiving a vocational training (t=2.14, P=.03), and whether or not using a device (t=-7.42, P=.00). 5. There was not significant correlation between self-concept and self-care activities (r=.081, P=.311).

Evaluation on the Practicum Using Standardized Patients for Nursing Assessment to Articular Disease (표준화 환자를 이용한 관절질환 간호사정 실습교육의 평가)

  • Purpose: This study was performed to evaluate of practicum by using standardized patient(SP) for nursing assessment. Method: This study had 2 steps. The 1st-step was pre-intervention stage including selection of a learning title, formation of case scenario, training of SP and developing the evaluation tools for students' clinical competence to assessment, 6 categories 29 items. The 2nd-step consisted of intervention and evaluation stages. 34 nursing students divided 2 groups participated in assessing the SP. Evaluation of each group was performed by 2 nursing professors. All students recorded their feelings after assessing the SP. The SP also evaluated about nursing students' attitude toward the SP. Results: ICC(Interclass correlation coefficient) between 2 groups was over 0.7 all categories. Students' assessing score(range 0-1) was muscular-joint function status(0.41), nutritional status(0.39), history taking(0.38), IADL(0.18), ADL(0.15), and emotional status (0.07). The mean scores of the nursing students' attitude by SP was 4.03(range 1-6). Also most students showed positive reactions to the education using SP because they had the chance to experience what they could not practice in clinical setting. Conclusion: The evaluation tool revealed high reliability. Nursing students' clinical competence was below average. But they took a good attitude to SP. We recommended further research using SP with various disease.
