A Study on the Effects of Quadriceps Femoris Flexibility Exercise on the Functional Ability of the Patients with Degenerated Arthritis (대퇴사두근 유연성 운동이 퇴행성 슬관절염 환자의 기능활동정도에 미치는 영향)

  • The purpose of this study was to evaluate the effects of quadriceps femoris flexibility exercise which would improve the degree of knee flexion range of motion, extension torque, and the activities of daily living (ADL) in elderly subjects with degenerative knee arthritis. Fourteen elderly patients (two men and twelve women) with degenerative knee arthritis participated and had a quadriceps femoris flexibility exercise intervention program in this study. The mean age of the patients was 70.00 years for men and 71.16 years for women. This study carried out the experimental study of one group pretest-posttest design, which evaluated the degree of knee flexion range of motion, extension torque, the ADL ability of the patients before and after applying the exercise intervention for five weeks. The results of this study are as follow: 1. The knee flexion ranges of motion of the patients were measured before and after the intervention and the ranges increased significantly both in the left and right knee flexion range of motion (p < 0.05). 2. The peak torque of the knee muscle, the peak torque/body weight, and average power of the patients showed significant increases in both in the left and right knee after applying the intervention (p < 0.05). 3. The intervention produced a significant reduction in pain of the patients (p < 0.05). Their functional activities of ADL improved significantly compared with before the intervention (p < 0.05). It has been shown that the quadriceps femoris flexibility exercise intervention program increased significantly the knee flexion range of motion, and extension torque, as well as an increase in performance of functional activities of ADL of the patients. Thus, the quadriceps femoris flexibility exercise should be considered as one of the therapeutic exercises for the elderly patients with degenerative knee arthritis applied.

A Study of Activities do Daily Living Performance on Arthritic Elderly Patients (노인의 퇴행성 슬관절염과 일상생활 수행력에 관한 연구)

  • To know the effect of index of social ability (ISA) and function of social life (FSL) with activities of daily living (ADL) and instrumental activities of daily living (IADL), we carried out a study on the elderly with osteoarthritis daily living in Daegu city for 4 months, from April to July 2002. The followings were observed: 1. The score of functional disability of knee, ADL and ISA were better at lower age (p < 0.01). 2. The younger they are, the better FSL they have. The lower age group has good FSL (p < 0.01). 3. Weight affects more to the under 49Kg group than 50-59Kg group and 60-69Kg group in functional disability of knee, while it doesn't make much difference for the other groups (p > 0.05) and other assessment index doesn't show any relationship with weight. 4. Regarding relat ionship with height , taller group has better knee functions: it's proved by post hock examination that the over 160cm group has much higher knee function than the other groups (p < 0.05). In case of ADL, the over 160cm group and under 149cm group were better than 150$\∼$159cm group. (p < 0.01) . 5. ISA was also different depending on height . It was proved that FSL was the better for the over 160cm group than for the others as a result of post hock examination (p < 0.05). 6. In regard of how many months have passed since the attack of functional disability of knee, knee joint function was better for the 12mon$\∼$36mon group than for the others and the score of ADL was lower for the 12mon$\∼$36mon group than for the others (p < 0.01). ISA was lower for the 12mon$\∼$36mon group and the under 6mon group than for the other groups (p < 0.01). FSL was lower for the 12mon$\∼$36mon group and the under 6mon group than the other groups. (p < 0.01). 7. Comparing left knee with right one, the problem with left knee has lower score of functional disability in all items (p < 0.01). 8. Considering friction noise, knee joint function was slightly better for non-friction noise-group, but it's not statistically significant though. ADL and ISA were better for non-friction noise-group (p < 0.01), while no statistical significance was found in FSL, 9. Knee joint function was better for the group who doesn't have limitation on range of knee flexion motion (p,0.05) but no other difference was found in other items from this group. 10. Limitation on range of knee flexion motion has no statistical difference in ADL and FSL. However, ISA was better for the non-limitation group (p < 0.05) . 11. Knee joint function score has high correlation with ISA (r=0.812, p < 0.01) and FSL (r=0.732, p < 0.01) , which shows the better knee joint function, the higher the score. 12. ISA and FSL were highly correlated: the better ISA, the better FSL (r=0.893, p < 0.01), while ADL has no correlation with either knee joint function or ISA or FSL.

The Effects of Dual-task Action Observation Physical Training on the Walking Ability and Activities of Daily Living in Chronic Stroke Patients (이중과제 동작관찰 신체훈련이 만성 뇌졸중 환자의 보행 능력과 일상생활 활동에 미치는 영향)

  • PURPOSE: The purpose of this study was to determine the efficacy of dual-task action observation training (AOT) and single-task AOT related with daily living task on walking ability and ADL performance in chronic stroke patients. METHODS: Twenty-seven chronic stroke subjects were included in the study. They were randomly assigned to three task categorieds as follows: whole dual-task AOT or partial dual-task AOT or single-task AOT rehabilitation. Whole dual-task AOT observed the movement at once and partial dual-task AOT observed the movement divided into 4 parts related functional gait and activities of daily living task for 2 minutes 30 seconds. Single-task AOT observed the movement related functional gait for 2 minutes 30 seconds. Both groups had physical training session for 12 minutes 30 seconds. The study was conducted for four weeks, with three training sessions a week, for twelve weeks. All subjects were evaluated for their walking ability and activities of daily living through devices, 10m walking test (10MWT), dynamic gait index (DGI), and Korea-Modified Barthel Index (K-MBI). RESULTS: A significant improvement of walking ability and ADL performance happened among dual-task AOT subjects, compared with a single-task AOT subjects, during the 4-weeks course treatment. The results of the study showed statistically significant differences in 10MWT (p<0.05) and DGI (p<0.05), and K-MBI (p<0.05). CONCLUSION: Our results indicated that dual-task AOT has a positive additional impact on recovery of walking ability and ADL performance in chronic stroke patients.

The Influences of Visual and Hearing Impairment on Activities of Daily Living for the Community Dwelling Elderly (재가노인의 시청각기능장애가 일상생활수행능력에 미치는 영향)

  • Purpose: The aim of this study is to identify the influence of visual and hearing impairment on the activities of daily living of community dwelling elderly. Methods: Data were collected by home visiting interviewers from 452 older people aged 65 years or older living in community. Resident Assessment Instrument MDS-HC(2.0version) was used for data collection. Data analysis for descriptive statistics, Chi-square test and multiple regression was made by SAS 6.2 Results: 34.7% of the subject had hearing impairment and 64.3% had visual impairment Among IADL. one half of them were dependent in ordinary house work and meal preparation. In the case of ADL. 13.9% of subjects were dependent in bathing and 8.9% in personal hygiene. There was significant difference in IADL performance by visual and hearing impairment On the other hand, ADL performance showed the significant difference. only in the case of hearing impairment. As the result of input of visual and hearing impairment in the process of regression. variances were increased from 3% to 11%. Conclusions: Large proportions of older people living in the community have visual and hearing impairment. It could be confirmed that hearing and vision were significant factors influencing on IADL performance of older people. Intervention and support policy for elderly needs to focus on improvement of visual and hearing impairment.

The Effects of Evjenth-Hamberg Stretching on Upper-Extremity Function and Activities of Daily Living in Subacute Stroke Patients (에비안스-함베르크 스트레칭이 아급성 뇌졸중 환자의 상지기능과 일상생활활동에 미치는 영향: 단일 사례연구)

  • Objective : The purpose of this study is to assess effects of evjenth-hamberg stretching on upper-extremity function and ADL performance ability of patient with subacute stroke patients Methods : The object of this study K-hospital located in daegu, 39-year-old man who have receiving inpatient treatment with stroke. The subject during the period of four weeks, five times a week, and thirty minutes for one session, evjenth-hamberg stretching exercise of affected side. Before and after the intervention in order to compare upper-extremity function was assessed using manual function test(MFT), motor activity log(MAL) and ADL performance ability was assessed by functional independence measure(FIM). Results : Upper-extremity function of the object score improved from each 3 in MAL, the ADL performance ability score increased from 73 to 94. Conclusion : The result of this study evjenth-hamberg stretching in patients with subacute stroke upper-extremity function and ADL performance ability to promote was found that the effect. The evjenth-hamberg stretching in the occupational therapy, if appropriately utilized more on the functional recovery of patients is expected to be helpful.

Fall Prediction Model for Community-dwelling Elders based on Gender (지역사회 노인의 성별에 따른 낙상 예측모형)

  • Purpose: This study was done to explore factors relating to number of falls among community-dwelling elders, based on gender. Methods: Participants were 403 older community dwellers (male=206, female=197) aged 60 or above. In this study, 8 variables were identified as predictive factors that can result in an elderly person falling and as such, supports previous studies. The 8 variables were categorized as, exogenous variables; perceived health status, somatization, depression, physical performance, and cognitive state, and endogenous variables; fear of falling, ADL & IADL and frequency of falls. Results: For men, ability to perform ADL & IADL (${\beta}_{32}$=1.84, p<.001) accounted for 16% of the variance in the number of falls. For women, fear of falling (${\beta}_{31}$=0.14, p<.05) and ability to perform ADL & IADL (${\beta}_{32}$=1.01, p<.001) significantly contributed to the number of falls, accounting for 15% of the variance in the number of falls. Conclusion: The findings from this study confirm the gender-based fall prediction model as comprehensive in relation to community-dwelling elders. The fall prediction model can effectively contribute to future studies in developing fall prediction and intervention programs.

The Effects of Individualized Cognitive Program on LOTCA-G and ADL in Elderly with Dementia and Mild Cognitive Impairment (맞춤형 인지프로그램이 치매와 경도인지손상노인의 LOTCA-G 및 일상생활동작에 미치는 영향)

  • Purpose : The purpose of this study was to evaluate the influence of LOTCA-G and ADL by individualized cognitive program in elderly with vascular dementia, alzheimer dementia and mild cognitive impairment. Method : The subjects of this study, old man and woman diagnosed with vascular dementia, alzheimer dementia and mild cognitive impairment, 24 patients were picked up, who were agreed with this research and were having hospital care for 3weeks at nursing care centers. Individualized cognitive program was applied to 8 patients of vascular dementia, 8 patients of alzheimer dementia and 8 patients of mild cognitive impairment. Cognitive function measured by LOTCA-G and performance measured by FIM. The SPSS Ver. 22.0 statistical program was used for data processing. The significance level for statistical inspection was set as 0.05. Result : In comparison of LOTCA-G was significant increased after intervention and among three groups were significant difference. But FIM was no significant difference after intervention and among three groups were no significant difference. Conclusion : Therefore, the individualized cognitive program is useful to improve the cognitive function in elderly with dementia and mild cognitive impairment.

The Effect of Closed Kinetic Chain Exercise Using PNF on Hand Function and Activities of Daily Living of Patient with Incomplete Spinal Cord Injure : Single Subject Design (PNF를 이용한 닫힌사슬운동이 불완전 경수 손상 환자의 손 기능과 일상생활동작에 미치는 영향 : 단일사례연구)

  • Purpose: Deficits in upper-extremity function in individuals with tetraplegia are primarily due to the loss of motor pathways. The purpose of this case report is to describe the effect of closed kinetic chain exercise using PNF on hand function and ADL of patient with incomplete spinal cord injure. Methods: Patient was a 53-year-old man with C4 tetraplegia and hand and upper extremity impairment who participated in this training intervention for 10 weeks. Results: Patient demonstrated improvements in upper extremity strength, hand function and performance of ADL. Outcome measures(ASIA motor scale, Hand power, Jebsen-Taylor Hand Function test, SCIM) were measured before and after the training program. Conclusion: The results of this case suggests that an closed kinetic chain exercise program in incomplete spinal cord injury patient may induce Hand function and ADL.

Comparative Study on Reliability-Based Topology Optimization (신뢰성 기반 위상최적화에 대한 비교 연구)

  • Reliability-based Topology optimization(RBTO) is to get an optimal design satisfying uncertainties of design variables. Although RBTO based on homogenization and density distribution method has been done, RBTO based on BESO has not been reported yet. This study presents a reliability-based topology optimization(RBTO) using bi-directional evolutionary structural optimization(BESO). Topology optimization is formulated as volume minimization problem with probabilistic displacement constraint. Young's modulus, external load and thickness are considered as uncertain variables. In order to compute reliability index, four methods, i.e., RIA, PMA, SLSV and ADL(adaptive-loop), are used. Reliability-based topology optimization design process is conducted to obtain optimal topology satisfying allowable displacement and target reliability index with the above four methods, and then each result is compared with respect to numerical stability and computing time. The results of this study show that the RBTO based on BESO using the four methods can effectively be applied for topology optimization. And it was confirmed that DLSV and ADL had better numerical efficiency than SLSV. ADL and SLSV had better time cost than DLSV. Consequently, ADL method showed the best time efficiency and good numerical stability.

Reliability and Validity of the Korean Version of Melbourne Low-Vision ADL Index (한국판 맬버른 저시력 일상생활지수(Melbourne Low-vision ADL Index: MLVAI)의 신뢰도 및 타당도)

  • Objective: The purpose of this study was performed to modify the performance-based assessment tool, Melbourne Low-Vision ADL Index (MLVAI) had been developed in Australia to suit the Korea culture to verify the reliability and validity. Methods: The subjects were only over the age of 20 living in communities, 26 with low-vision and 42 normal persons. The Korean MLVAI was completed through the expert translation verification and validation of the configuration tool. The validity of the Korean MLVAI was established through the content, discriminant, and convergent validity. Also, the reliability was analyzed through internal consistency reliability for the items, test-retest, and interrater reliability. Results: The Content Validity Index(CVI) was more than .78. There was a statically significant low in low-vision. Also, there was a statically significant low in low-vision. The convergent validity was determined the correlation coefficient of .751 analyzing LVQOL and Korean MLVAI total score, had a significant correlation(p<.05). Cronbach's ${\alpha}$ coefficient indicated an internal consistency of .983(p<.05). Test-retest reliability had a high, significant correlation of .976 and interrater reliability had a high, an intraclass correlation coefficient of .91(p<.05). Conclusion: The results of this study mean that the Korean MLVAI which was modified to fit the Korean is the ADL assessment tool with both validity and reliability. Based on this study, the Korean MLVAI can be used as a useful ADL assessment for OT interventions in low vision.
