Implementation on ADHD Diagnostic Expert System based on DSM Diagnostic Criteria (DSM 진단 기준을 이용한 ADHD 진단 전문가시스템 구현)

  • In this paper, we design and implement an expert system for diagnosing ADHD. As a result of the analysis with DSM-IV-TR, the ADHD diagnostic criteria are changed according to the age group. With this analyzed diagnostic, objects and their values are set and rules are created. We design a diagnostic system consisting of 'ADHD diagnostic system engine' and 'user query response program'. The ADHD diagnostic system engine is a rule-based reasoning engine that is implemented in the Prolog language and receives INPUT from the user query response program. By INPUT, the rule is executed based on the ADHD diagnostic criteria and the OUTPUT is sent back to the 'user query response program' by inferring the diagnostic result. The 'user query response program' is implemented in the Python language and serves as an interface for handling conversation with the user. The bridge between 'ADHD diagnostic system engine' and 'user query response program' is performed through the Pyswip library. As a result, the ADHD Diagnostic Expert System will help you plan your treatment with reduced diagnostic costs and use-complexity.

A STUDY OF THE DEVELOPMENT AND STANDARDIZATION OF ADHD DIAGNOSTIC SYSTEM (전산화된 주의력장애 진단시스템의 개발 및 표준화 연구)

  • Objectives:Present study developed the computerized ADHD Diagnostic System(ADS) in order to diagnose ADHD and evaluate treatment effect of it, and conducted a standardization study for ADS. Methods:The normative group was composed of 847 children and adolescents between the age of 5 and 15(boy 429, girl 418) living in the areas of Seoul, Kyunggi-do, and Kangwon-do. 30 ADHD children with age ranged 7 to 9 years were participated present study to evaluate the validity of ADS. To establish the norms for diagnosing ADHD, the means and standard deviations of normative group were used to calculate T-scores for each age group. Results:The reliability coefficient of ADS(Cronbach's ${\alpha}$) was .85. There were significant differences in the measures of ADS except commission error between the normal and the ADHD groups. Three factors were extracted through factor analysis of ADS, which were labelled 'inattention', 'slow information processing' and 'impulsivity'. Discriminant analysis showed that ADS significantly discriminate the normal and the ADHD groups. Percentage of correct classification by ADS variables was 96.7%. Conclusion:These results put together strongly support the reliability and validity of ADS as a diagnostic instrument for ADHD.

CLINICAL EVALUATION OF CHILDREN WITH INATTENTION AND HYPERACTIVITY IN A PSYCHIATRIC CLINIC (주의산만과 과잉운동을 주소로 하는 정신과 내원 아동들의 임상 평가)

  • The aim of this study is to examine the diagnostic profiles and related clinical variables of children with attention and hyperactivity in psychiatric outpatient clinic. Seventy one children with age range of 5 to 14 were diagnosed by DSM-IV, and assessment battery including KEDI-WISC, KPI-C, ADS(ADHD Diagnostic System) were completed. The subjects were divided into 3 diagnostic groups:ADHD only(n=17), ADHD comorbid(n=27), Other diagnosis(n=27). The results were as follows:In ADHD comorbid group, tic disorder, developmental language disorder, borderline intellectual function, oppositional defiant/conduct disorder, and learning disorder were combined in descending order. Other diagnosis group consisted of tic disorder, borderline intellectual function, depression/anxiety, oppositional defiant/conduct disorder, and others. There were significant differences in IQ, PIQ, and VIQ among the three groups, and ADHD only group showed higher scores of IQ and VIQ than ADHD comorbid group. On the KPI-C, there were no significant differences in all subscales among the three groups. On the visual ADS, omission error and sensitivity showed significant differences among the three groups, and ADHD comorbid group represented higher omission error and lower sensitivity than other diagnostic group. The findings indicated that the inattention and hyperactivity symptoms could be diagnosed into diverse psychiatric disorders in child psychiatry, and ADHD children with comorbidity will show more problems in academic performance and school adjustment.

Case Series Reporting 4 Cases of Attention Deficit Hyperactivity Disorder (ADHD) Treated with Galgeunhwangryeonhwanggeum-tang based on Disease Pattern Identification Diagnostic System by Shanghanlun (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 갈근황연황금탕(葛根黃連黃芩湯) 투여 후 호전된 성인, 소아 ADHD 4례)

  • Objectives : This study reports on the cases of four ADHD patients who treated with herbal medication based on Disease Pattern Identification Diagnostic System (DPIDS). Methods : Two children and two adults were diagnosed with ADHD according to DSM-V, and Galgeunhwangryeonhwanggeum-tang was administered. The parents of the two children completed the Korean ADHD Rating Scale (K-ARS) before and after treatment, while the other patients used the Korean ADHD Self Rating Scale (K-ASRS). The patients' somatic symptoms and other issues were evaluated through interviews. Results : In the first case, the K-ARS scale exhibited a change from 28 to 3. The patient demonstrated improvements in attention, concentration, and social behavior, along with the elimination of self-injurious behavior. In the second case, the K-ARS scale showed a change from 41 to 9. The patient in the third case demonstrated improved chronic fatigue, increased attention, work efficiency, and enhanced social skills, leading to maintaining employment for over six months for the first time in their life after the treatments. Additionally, the patient in the fourth case demonstrated improved work concentration and relief in nocturia. Conclusions : In this study, four patients exhibited improvements in symptoms of attention deficit, hyperactivity, and impulsivity before and after herbal treatment, leading to enhanced academic and social relationships. As the patients progressed in their health restoration, symptoms related to the genitourinary system also showed improvement.

  • Objective : We investigated that ADHD categorical diagnosis and the dimensional tools for the evaluation of ADHD, widely used in the clinical field, such as the child behavior check list- Korean version (K-CBCL), Korean personality inventory for children (KPI-C), computerized Attention Diagnostic System (ADS). Method : The DSM-IV clinical diagnosis applied by child psychiatrist. K-CBCL, KPI-C, ADS are used. Ultimately, totally 161 ADHD children and 161 controls were evaluated. Subject group are consist of 202 boys (62.7%) and 120 girls (37.3%), and the mean age was $9.5{\pm}2.0$ years old. Results and Conclusion : Social problem, and attention problem in the K-CBCL, correct response time standard deviation in the computerized ADS were statistically significant different and attention problem in the K-CBCL, hyperactivity subscale in the KPI-C were significant trait, between subject group and control group. The ROC value of attention problem in the K-CBCL, hyperactivity subscale in the KPI-C, and ADS were .78, .93, .86. Finally, we found that K-CBCL, KPI-C, ADS were significant corelation with the ADHD categorical diagnosis.

Effects of Intelligence Ability on Continuous Performance Test (지적 능력이 연속수행과제(CPT) 수행에 미치는 영향)

  • Objectives : The study was conducted to investigate the effect of intelligence ability on attention using Continuous Performance Test (CPT). Methods : 56 children with ADHD (52 boys, 4 girls) and 41 children in normal (28 boys, 13 girls) were sampled, their age range was 7 to 15. They performed IQ test and ADHD Diagnostic System (ADS) in order to examine intelligence and attention. Participants were divided into normal group and ADHD group, average IQ level children and superior IQ level children. Then ADS variables (omission error, commission error, reaction time, reaction time deviation, response sensitivity, and response criterion) were analyzed. Results : There was no significant interaction effect between group (normal, ADHD) and intelligence (average, superior). But there was significant difference between normal group and ADHD group in omission error, commission error, reaction time deviation, and response sensitivity. Also average level IQ group had significantly showed more omission, greater reaction time deviation, and lower response sensitivity than superior level IQ group. Conclusion : ADHD group has attention deficit than normal group, and CPT is available tool to detect attention problems. These findings indicate that intelligence can contaminate inattention and cognitive impulsivity thus it compensates for attention deficit. And it suggests that intelligence effect is considered in analyzing CPT in ADHD children.

ADHD, New Developed or Newly Found : Historical Review (주의력결핍 과잉행동장애, 과거에도 있었을까? - 역사적 고찰을 중심으로 -)

  • Introduction : Attention-deficit hyperactivity disorder (ADHD) was a newly coined term for a disease that existed prior to its naming in the mid 20th century. The issue about whether ADHD is a new disorder or merely a new name for an existing disorder is still controversial. The authors tried to find the clues to the answer for this question through reviewing historical documents for traces of ADHD. Contents : We could find literatures and medical records that contain possible ADHD symptoms. In particular, in 1845, Heinrich Hoffmann's 'fidgety Philip' or 'Johnny Look-in-the-air' nearly satisfies today's criteria for ADHD. Methylphenidate was approved as a promising chemical for inattention in 1957 before the establishment of the concept of ADHD. In 1968, ADHD was first officially introduced as "Hyperactivity Reaction of Childhood" by DSM-II. In 1980, DSM-III, 'Attention Deficit Disorder (ADD)' was adopted as an official diagnostic term and changed to 'ADHD' since the creation of DSM-III-R in 1987. Conclusion : As stated above, ADHD has existed since long ago and became familiar via an advanced diagnostic system and therapeutic options.

Dopamine Transporter Density of the Basal Ganglia in Children with Attention Deficit Hyperactivity Disorder Assessed with I-123 IPT PECT (I-123 IPT SPECT를 이용한 주의력결핍 과잉행동장애 아동에서의 methylphenidate 투여 전후의 기저 신경절 도파민 운반체 밀도 변화 측정)

  • Purpose: Attention deficit hyperactivity disorder (ADHD) has been known as psychiatric disorder in childhood associated with dopamine dysregulation. In present study, we investigated changes in dopamine transporter (DAT) density of the basal ganglias using I-123 N-(3-iodopropen-2-yl) -2-carbomethoxy-3beta-(4-chlorophenyl) tropane [I-123 IPT] SPECT in children with ADHD before and after methylphenidate treatment. Materials and Method: Nine drug-naive children with ADHD and seven normal children were included in the study. We peformed brain SPECT two hours after the intravenous administration of I-123 IPT and made both quantitative and qualitative analyses using the obtained SPECT data, which were reconstructed for the assessment of soecific/nonspecific DAT binding ratios in the basal ganglia. All children with ADHD reperformed [123I]IPT SPECT after treatment with methylphenidate(0.7mg/kg/d) during about 8 weeks. SPECT data reconstructed for the assessment of specific/nonspecific DAT binding ratio of the basal ganglia were compared between before and after treatment methylphenidate. We investigated correlation between the change of ADHD symptom severity assessed with ADHD rating scale-IV and specific/nonspecific DAT binding ratio of basal ganglia. Results: Children with ADHD had a significantly greater specificinonspecific DAT binding ratio of the basal ganglia comparing to normal children(Right : z = 2.057, p = 0.041 : Left : z : 2.096, p = 0.032). Under treatment with methylphenidate in all children with ADHD, specificinonspecific DAT binding ratio of both basal ganglia decreased significantly greater than before treatment with methylphenidate (Right : t = 3.239, p = 0.018 ; Left : t = 3.133, p = 0.020). However, no significant correlation between the change of ADHD symptom severity scores and specific/nonspecific DAT binding ratio of the basal ganglia were found. Conclusions: These findings support the complex dysregulation of the dopaminergic neurotransmitter system in children with ADHD.

  • Objectives : Attention Deficit Hyperactivity Disorder(ADHD) is a psychiatric disorder, characterized by the primary symptoms of inattention and/or impulsivity and hyperactivity. the aim of this study is to investigate the clinical efficiency of Various Oriental Medical Therapy combinated with Learning therapy in the treatment of ADHD. Methods : This study is a clinical report of 1 ADHD child treated with Various Oriental Medical Therapy(Herb-med, acupunture, etc.) in Combination with Learning therapy. CAR(Conner's abbreviated Rating Scale), K-CBCL, ADS(ADHD Diagnostic System) were compared between before and after treatment. Results : The results show the Various Oriental Medical Therapy in Combination with Learning therapy is efficient in the treatment of ADHD. Conclusions : Various Oriental Medical therapy including herb-med, pharmacopuncture, electroacupuncture is efficient in improvement of ADHD child. also In case of combination with Learning therapy, it is more effective in child's school life and school work. therefore, it is practical useful in the treatment of ADHD.

Common Features of Attention Deficit Hyperactivity Disorder and Epileptic Disorder in Childhood and Early Adolescence (소아와 조기청소년에서 보이는 주의력결핍 과잉행동장애와 간질의 공통적 특성)

  • Objectives:We conduct this study to investigate the common features between Attention Deficit Hyperactivity Disorder(ADHD) and epileptic patients compared to normal control. Methods:Epileptic patients were recruited from the department of pediatic in Jesus Hospital. ADHD patients were recruited from the department of neuropsychiatry in Jesus Hospital. We excluded mental retardation or brain organic pathology. We use ADHD Diagnostic System and Korean-Child Behavior Checklist(K-CBCL) to assess features of ADHD. Electroencephalogram(EEG) of ADHD, epileptic patients and normal control were analyzed and compared. Results:Compared to normal control group, inattention, reaction time deviation were increased in both ADHD and epilepsy group. EEG abnormalities(control 13.8%, epilepsy 97.1%, ADHD 40%) in three groups were reported. Conclusion:There are common features of ADHD and epileptic patients.
