Letter to Editor : A case of Seganmeongmok-tang for choroidal melanoma-related eye pain and headache (맥락막 흑색종으로 유발된 안구통증과 두통이 세간명목탕으로 호전된 환자 1례)

  • Objectives : choroidal melanoma is a rare solid tumor and has been known that thers is few effective drugs for the treatment. The purpose of this case report is to report a case of reduction of choroidal melanoma-related eye pain and headache treated with Seganmeongmok-tang. Methods : A patient with choroidal melanoma-related eye pain and headache was treated with Seganmeongmok-tang. The changes in clinical findings were evaluated. Results : The symptoms of patients were improved after about 1 week taking Seganmeongmok-tang. Conclusion : This case report suggests that Seganmeongmok-tang may have an effect on reduction of choroidal melanoma-related eye pain and headache. Further rigorous studies are necessary to investigate the therapeutic effects of Seganmeongmok-tang on choroidal melanoma.

  • Introduction: This case report assessed the effects of Korean medicine treatments on a chronic migraine patient who did not respond to triptan. Case Presentation: The patient received Korean medicine treatments, such as acupuncture, moxibustion, cupping, and herbal medication, during a hospitalization period. The effects of treatments were assessed with the Numerical Rating Scale (NRS) and migraine attack frequency. The intensity and attack frequency of migraines were significantly reduced after the complete treatment. Conclusions: For migraines, Korean medicine such as acupuncture and herbal medication could be an alternative to conventional therapy.

  • objective The purpose of this case report is to review the possibility of utilizing Jeokseokji-uyeoryang-tang as a therapy of seborrheic dermatitis, and to consider clinical using possibility of Shanghanlun.method Patient's Diagnosis and progress of seborrheic dermatitis was evaluated based on the patient's chart and picture. And diagnosis based on Shanghanlun was organized by table.results When analyzing the patient's history and symptoms, it can be found in article No. 159 in Shanghanlun. The area and intentsiy of erythema were reduced when viewed photos findings, so diagnosis and therapies used in this case showed valid results in seborrheic dermatitis.conclusion Jeokseokji-uyeoryang-tang is efficient to patient of this case report as a treatment.

  • Dirofilariasis is a common parasitic disease in both domestic and wild animals around the world, with canines as the principal reservoir host and mosquitoes as the vector. Human ophthalmic dirofilariasis is an uncommon condition, but there have been reports from many parts of the world, including Africa, Australia, the Americas, Europe, and Asia. Ophthalmic involvement with Dirofilaria may present itself as periorbital, subconjunctival, orbital, or intraocular infections. In this report, we present a case of orbital dirofilariasis with lateral rectus muscle involvement. To our knowledge, this is the first orbital dirofilariasis case reported in Iran. Although debulking of the tumor usually leads to resolution, our patient showed an episode of recurrence after biopsy. Complete recovery was achieved after a spontaneous discharge, without the need for systemic medication.

  • Herein, we describe the case of a 20-year-old woman who presented with dysphagia of 2 months' duration associated with vomiting, moderate abdominal pain, decreased oral intake, and significant weight loss. During the past 3 years, the patient experienced intermittent mild abdominal pain with infrequent vomiting. Endoscopy at Jordan University Hospital showed a mass in the esophagus, and endoscopic biopsies were performed. The preliminary histopathological report excluded malignancy. Two days after endoscopy, the patient presented to the emergency department complaining of severely worsening pain and total dysphagia. The pain persisted despite intravenous paracetamol administration, which was concerning for esophageal perforation; therefore, an urgent surgical intervention was performed. The mass was removed surgically, along with a para-esophageal lymph node. The final histopathological results of the endoscopic and resected specimens supported the diagnosis of pseudomyogenic hemangioendothelioma (PMHE). This is the first case reporting esophageal involvement of PMHE.

  • Bacterial infection of skeletal muscle can lead to the formation of abscesses. Primary pyomyositis is typically seen in tropical countries, and Staphylococcus aureus is the commonest causative organism. We present a case of acute adductor magnus muscle abscess (pyomyositis) with spread to adjacent thigh compartments via the perforators without iliopsoas muscle involvement. Due to the involvement of the entire thigh compartment, systemic antibiotic treatment alone was insufficient, whereas surgical drainage improved the clinical picture. The aetiological organism was S. aureus. Herein, we report the case of a patient who had primary pyomyositis, rather than a secondary type, that spread to the posterior and lateral aspect of the thigh through the second and third perforators, which pierce the adductor magnus muscle belly before entering the femur.

A Clinical Case of Lymphadenopathy and Hearing Loss after Covid-19 Vaccination (COVID-19 백신 접종 후 발생한 라력(瘰癧)과 이롱(耳聾)에 대한 치험 1례)

  • Objectives: The purpose of this study is to report a case of lymphadenopathy and hearing loss in a patient after coronavirus disease 2019(COVID-19) vaccination. Methods: The patient diagnosed as lymphadenopathy and hearing loss(sensorineural hearing loss) after the first and second rounds of the Pfizer COVID-19 vaccination. We applied Korean medical treatment including herbal medicine and acupuncture for lymphadenopathy and SSNHL. We used VAS(Visual Analog Scale), CT-scan and Pure Tone Audiometry to estimate they symptoms. Results: After the treatment, general symptoms of lymphadenopathy and hearing loss including aural fullness showed big improvement. Conclusions: This case report shows effect of Korean medicine on lymphadenopathy and SSNHL and suggests a possibility of Korean medicine to treat adverse events following COVID-19 vaccination.

Pulmonary Blastoma -A Case Report- (폐아세포종 -수술치험 1례-)

  • We experienced a very rare case of pulmonary blastoma in a 29 year old female. She complained of left chest pain and dyspnea for 1 month. The characteristic feature of this tumor is it`s biphasic pattern consisting of a spindle cell stroma containing glandular structures. A serial check of simple chest X-ray and computed tomography revealed a growing huge lung mass occupying whole left thorax. We successfully removed the upper lobe of left lung with a huge tumor mass. Pathologic study revealed this tumor as pulmonary blastoma. We report a case with review of literature.

Conservative treatment of corpus callosum hemorrhage due to a falling coconut in Indonesia: a case report

  • The potential for traumatic brain injury resulting from falling coconuts is frequently overlooked. These incidents can cause focal lesions in the form of brain hemorrhage. Corpus callosum hemorrhage due to blunt trauma from a falling object is rare and typically associated with poor prognosis. The purpose of this report is to detail a case of corpus callosum hemorrhage caused by a coconut fall and to discuss the conservative management approach employed. We report the case of a 54-year-old woman who was admitted to the hospital with symptoms of unconsciousness, headache, and expressive aphasia after being struck by a falling coconut. Notably, hemorrhage was detected within the body of the corpus callosum, as revealed by imaging findings. The patient received intensive monitoring and treatment in the intensive care unit, including oxygen therapy, saline infusion, an osmotic diuretic, analgesics, and medication to prevent stress ulcers. The patient demonstrated marked clinical improvement while undergoing conservative treatment. Despite the typically unfavorable prognosis of these rare injuries, our patient exhibited meaningful clinical improvement with conservative treatment. Timely diagnosis and appropriate interventions were crucial in managing the patient's condition. This report emphasizes the importance of considering traumatic brain injury caused by falling coconuts and highlights the need for further research and awareness in this area.

  • A schwannoma is a benign neurogenic tumor derived from Schwann cells. A rare case of a large painful schwannoma in the foot with metatarsal deformity was presented. Due to suspicion of malignancy, amputation had been recommended previously. We report on a rare case of a large forefoot schwannoma causing pain and paresthesia of the forefoot.
