『상한론(傷寒論)』변병진단체계(辨病診斷體系)에 근거하여 계지인삼탕(桂枝人參湯)을 투여한 아토피 피부염 치험 1례 (A Case Report of Atopic dermatitis treated by Kyejiinsam-tang based on shanghanlun provisions)

  • Objective : This study reports the effect of Kyejiinsam-tang on Atopic Dermatitis. Methods : This is a case report on a patient who has been suffering from Atopic Dermatitis. According to Diagnostic system based on Shanghanlun provisions, Kyejiinsam-tang was provided for 250 days. The severity of Atopic Dermatitis was evaluated by SCORAD index and pictures. Results : When analyzing the patient's history and symptoms, it can be found in article No. 163 in Shanghanlun. The area and intensity of Atopic Dermatitis were reduced when viewed photos findings, and SCORAD index decreased 58.1 to 7.1. So diagnosis and therapies showed valid results in Atopic Dermatitis. Conclusions : Kyejiinsam-tang is efficient to patient of this case report as a treatment.

하행 괴사성 종격동염 -1례 보고- (Descending Necroting Mediastinitis -1 case report-)

  • Descending necrotizing mediastinitis(DNM) is one of the most lethal form of mediastinitis originating from an oropharyngeal infection. It requires an early and aggressive sugical treatment, but the operative approach and optimal form of mediastinal drainage remains controversial. We report a case of DNM in a 45-year-old male who underwent right cervicomediastinotomy to drain the deep neck space, upper mediastinum and anterior mediastinal drainage was accomplished through a subxiphoid approach. After this procedure, he steadily improved and was dischrged on hospital day 36. We report this case with a brief review of the literature.

  • Background: Patients with chronic ITP (idiopathic thrombocytopenia) frequently do not require comprehensive medication for daily life. Usually, it had been regarded that postoperative bleeding after a simple or surgical extraction is easily controlled by simple local measures even in patients with ITP. This lack of regular medication usage can sometimes lead practitioners or patients to underestimate the potential life-threatening risk of ITP. There had been no report on postoperative hemorrhage in a patient with ITP related to dental implant surgery. Case presentation: This report presented a life-threatening postoperative hemorrhage after dental implant surgery in an adult with chronic ITP and subsequent emergency management after severe bleeding and airway compromise. Conclusion: The presented case emphasizes the thorough hematological evaluation of the patients even for patients who do not take any specific medications for asymptomatic, chronic ITP.

  • This is to report a case of traumatic bone cyst which occurred in the left condylar head of the mandible. It is a very rare case and the prognosis after operation seems to be fair, therefore we report as follows. 1. The pt, a 49 year-old female, has been suffering from difficulties of mastication with disfiguration of face due to deviation of mandible to the right side for 20 years. 2. Histopathologic findings showed the figure of multilocular bony cysts contained with fibrous tissue, osteoid tissue and many hematopoietic cells 3. Roentgenographic features showed well circumscribed radiolucent lesion with some radiopaque area. 4. After clinical evaluation, condylectomy was done to remove the lesion. 5. After the operation, the esthetic problem and masticatory functions were improved.

Cystic Echinococcosis of the Pelvic Bone with Recurrences: A Case Report

  • Hydatid cysts commonly affect the liver and the lung. However, they rarely involve bones with vertebral column. We hereby report a case of a female patient with cystic echinococcosis of the hip bone and ilium. She presented with a long history of frequent recurrences highlighting the dismal prognosis at this rare site. Resection of the hydatid cyst from the sacroiliac region was done with allograft and autograft (rib graft) with lumbosacroiliac fixation. Follow-up of the patient at 6 months showed no detectable abnormality on radiology and the patient was doing well.

한약치료를 통해 관해가 유도된 궤양성대장염 환자: 증례보고 (Herbal Medicine Treatment for Induction and Maintenance of Remission in Ulcerative Colitis: A Case Report)

  • This case report details induction and maintenance of remission in a 34-year-old female patient with ulcerative colitis (UC) after Korean herbal medicine treatment. The patient diagnosed as UC after a series of examinations including endoscopy in a western medical hospital mainly presented bloody diarrhea and dyspepsia and symptoms were persistent even with medications. She was given individualized Korean herbal medicine prescriptions for 7 months and the symptoms were monitored during treatment and no adverse events were reported. After clinical remission, endoscopic remission was also confirmed by colonoscopy. Clinical remission was maintained for 30 months after treatment without Western medication for UC. This case report shows that Korean herbal medicine treatment may have the potential for complete remission of UC and further research is warranted.

  • Treatment of metastatic carcinoma of the oral cavity can be a challenge due to its rarity. The current case report details metastatic carcinoma of the mandible originating from follicular thyroid carcinoma, which is the second most prevalent tumor of the thyroid. As the mandibular lesion developed, the primary thyroid lesion of thyroid. Here, we report a case of metastatic carcinoma of the mandible in a 67-year-old female that was treated with partial mandibulectomy and fibular free flap.

  • Pulmonary malakoplakia is a rare lung lesion more frequently found in immunocompromised patients than in immunocompetent individuals. In this study, we report the challenging case of a young immunocompetent patient with an irregular pulmonary nodule with peripheral cysts who, after undergoing surgery, was diagnosed with malakoplakia. Due to the rarity of the disease and the similarity of this condition to malignant neoplasms, cytopathological or histopathological examinations are necessary for the correct diagnosis. A description of pulmonary malakoplakia with peripheral cysts has not been previously published in the literature.

아스피린과 NSAIDs 과민성 풍열형(風熱型) 두드러기에 대한 방풍통성산가미방(防風通聖散加味方)의 치험례 (A Case Report on a Patient with Wind-Heat Type Urticaria Sensitive to Aspirin and NSAIDs Using Bangpoongtongsung-san Gamibang)

  • Objectives : The purpose of this case study is to report the effectiveness of Bangpoongtongsung-san gamibang on a patient with wind-heat type urticaria sensitive to aspirin and NSAIDs. Methods : We treated a 48-year-old man who had aspirin and NSAIDs sensitive wind-heat type urticaria with Bangpoongtongsung-san gamibang, pharmacopuncture(Hwangnyunhaedok-tang), acupuncture(needle remaining time : 20 min) and photodynamic therapy. Results : The Urticaria Activity Score(UAS) started at 5 and ended with 0, and the Visual Analog Scale(VAS) of heating sensation on whole body started at 10 and ended with 1-3. Conclusions : This study has demonstrated that Bangpoongtongsung-san gamibang could be effective to treat aspirin and NSAIDs sensitive wind-heat type urticaria.

Guillain-Barré Syndrome-like Neurological Symptoms after COVID-19 Vaccination Treated with Traditional Korean Medicine: A Case Report

  • Objective: To report a clinical case of Guillain-Barré syndrome-like neurological symptoms, including limb weakness, phantosmia, and nausea/vomiting after COVID-19 vaccination (AstraZeneca) that was improved by traditional Korean medicine (TKM) treatment. Methods: A 73-year-old male complained of extreme limb weakness, severe phantosmia, and nausea/vomiting after COVID-19 vaccination. No abnormalities had appeared in various radiological and laboratory tests, but the symptoms had continued to worsen for three months before visiting our clinic. Results: The patient was diagnosed with neurological complications suspicious of Guillain-Barré syndrome after COVID-19 vaccination. The patient was treated with acupuncture, moxibustion, herbal drugs (Banhabakchulchunma-tang), and nasal inhalation therapy with Aquilariae Lignum. Three weeks after Korean medicine treatment, his neurological symptoms had improved. Nausea/vomiting and phantosmia continued to show improvement, and muscle strength was gradually recovered in both lower limbs. Conclusion: Traditional Korean medicine could be a choice for the treatment of neurological complications after COVID-19 vaccination.
