Delayed Rupture of Flexor Pollicis Longus by Plate for Fracture of the Distal Radius (요골골절에 사용된 금속판에 의한 장무지 굴곡건의 지연성 파열)

  • Purpose: Delayed rupture of flexor pollicis longus as a sequelae of the plate inserted for distal radius fracture is a very rare. This is the first case reported and published in Korea. Methods: A 48 years old female patient visited hospital, complaining flexor disturbance of interphalangeal joint of left thumb, which suddenly occurred without any external wound. We found that she had received operation of fixing plate for fracture of left distal radius 10 years ago. As operational opinion, we have checked that flexor pollicis longus tendon has been ruptured with oblique ways being stimulated by extended plate to palmar side over long period. Results: Authors performed tenorrhaphy of flexor pollicis longus without tendon graft and presented a successful active flexion of the left thumb interphalangeal joint 1 year after the operation. Conclusion: If the extruded part of the end plate is observed during the operation or follow-up, it is considered to be necessary to get rid of the plate as early as possible after the fracture healing.

  • Acetyl-CoA carboxylase (ACC) is an excellent candidate for antibiotics drug target, which mediates malonyl-CoA synthesis from acetyl-CoA through acetylation process. It is also involved in the committed step of fatty acid synthesis which is essential for living organisms. We have determined the three dimensional structure of C terminal domain of HP0371, biotin-carboxyl carrier protein of H. pyroli, in solution state using heteronuclear multi-dimensional NMR spectroscopy. The structure of HP0371 shows a flatten b-sheet fold which is similar with that of E. coli. However, the sequence and structure of protruding thumb are different with that of E. coli and the thumb shows different basis of structural rigidity based on backbone dynamics data.

Review on the Quantitative Standards for Twirling Supplementation and Draining (염전보사의 정량적 기준에 대한 고찰)

  • Objectives and Methods : The aim of this study was to investigate twirling and supplementation and draining by analyzing ancient acupuncture literature and modern research papers, and to discuss the quantitative standards for the procedure. Results : Twirling supplementation and draining techniques were first described in Huang do neijing, after which they were recorded in various other ancient acupuncture texts. Not only were these techniques used in the active treatment of patient historically, but they also form the foundations of modern-day acupuncture treatment. Twirling supplementation and draining techniques described in ancient literatures can be divided into Yin and Yang. Examples of Yang characteristics include Up, Male, Morning, Hands, the governor vessel, Outside, and Left. The left rotation technique is a reinforcing method performed by moving the thumb forward while rotating the needle. Examples of Yin characteristics include Down, Female, Afternoon, Foot, the conception vessel(CV), Inside, and Right. The right rotation technique is a reducing method performed by moving the thumb backward while rotating the needle. Twirling supplementation and draining has been divided for four ways by classical literature and research that worked before. the direction of acupuncture, the stimulus-quantity of acupuncture, the performance of doctor(the uses of both hands, left and right hand) and the feeling of patient. Conclusions : Therefore, continuous research is required to implement Twirling supplementation and draining in a consistent manner. In addition, more active clinical trials using the techniques are necessary as well.

A Study on Pain Characteristics in Parts of the Hand among Estheticians (피부미용사의 손 부위 통증에 관한 연구)

  • Objectives: The purpose of this study was to investigate estheticians' self-reported pain symptoms and pain levels according to the parts of the hand. Methods: Data were collected through a questionnaire survey among 116 estheticians from 31 skin care centers and spas. Information about general characteristics, job characteristics and work characteristics were based on data from the self-administered questionnaire. To determine the self-reported pain symptoms and pain levels according to the parts of the hand, a Cornell Musculoskeletal Discomfort Questionnaire was used. Results: The subjects had the greatest self-reported pain symptoms in the thumb(Right: 53.4%, Left: 46.6%) and felt the highest level of pain in the wrist(Right: 4.87 points, Left: 4.71 points). Physical burdens and age had statistically significant effects on the pain level in the right hand, while physical burdens and the manager rather than the director had statistically significant effects on the pain level in the left hand. Conclusions: Since the thumb and wrist have the greatest subjective pain symptoms in the hand, the use of proper tools is recommended in order to reduce the usage of the thumbs during massage, as is regular stretching to protect the wrists.

  • In this paper an anthropomorphic robot hand called SKKU Hand IIl is presented, which has a miniaturized fingertip tactile sensor. The thumb is designed as one part of the palm and multiplies the mobility of the palm. The fingertip tactile sensor, based on polyvinylidene fluoride (PVDF) and pressure variable resistor ink, is physically flexible enough to be deformed into any three-dimensional geometry. In order to detect incipient slip, a PVDF strip is arranged along the direction normal to the surface of the finger of the robot hand. Also, a thin flexible sensor to sense the static force as well as the contact location is fabricated into an arrayed type using pressure variable resistor ink. The driving circuits and the tactile sensing systems for the SKKU Hand II are embedded in the hand. Each driving circuit communicates with others using CAN protocol. SKKU Hand II is manufactured and its feasibility is validated through preliminary experiments.

Bowel Perforation Due to Immobilization after Resurfacing Thumb with Anterolateral Thigh Free Flap in an Elderly Diabetic Woman

  • Inevitable immobilization after surgery on lower extremities can induce chronic constipation. Elderly diabetic women usually express ambiguous gastrointestinal symptoms and signs. We present here a case of panperitonitis developed from severe fecal impaction in an elderly diabetic woman after hand reconstruction using material harvested from the lower extremities. A 68-year-old diabetic female underwent anterolateral thigh free flap and wound revision twice on the left thumb. Three weeks after surgery, she complained about mild abdominal pain though she had daily defecation. Despite encouraging ambulation, her compliance was low. Resection of the sigmoid colon and colostomy were performed after diagnosis with bowel perforation. However, the patient went into septic shock and died with multiorgan failure after the guardians issued a DNR (do not resuscitate) order. For preventing bowel perforation, increased uptake of dietary fiber and early ambulation postoperatively should be encouraged, after even hand surgeries.
