Acoustic Analysis for Thermal Environment-related Vocalizations in Laying Hens (산란계의 열환경별 특이음에 대한 음성학적 분석)

  • The aim of this study was to divide vocalizations of laying hens (Hy-Line Brown) into general vocalizations (GVs), heat stress-related vocalization (HSV), and cold stress-related vocalizations (CSVs) and to determine if they are classified by the discriminant function analysis method. Thirty laying hens, 65-wk-old, were recorded using digital video recorders 2 times from 10:00 to 14:00 h in each thermal environment (thermoneutral: $22.0{\pm}1.8^{\circ}C$, too hot: $32.0{\pm}2.0^{\circ}C$, too cold: $8.0{\pm}1.9^{\circ}C)$ after a 7 day acclimation period. When the laying hens were not recorded, they were kept in thermoneutral conditions. The GVs, HSV, and CSVs were divided based on the shapes of spectrums and spectrograms. The GVs, HSV, and CSVs were identified as 5, 1, and 3 types, respectively. Pitch, intensity, duration, formant 1, formant 2, formant 3, and formant 4 among the thermal environment-related vocalizations were significantly different (P<0.001). The discrimination rate determined by discriminant function analysis was 86.2%. These results suggest that HSV and CSVs are present and may be used as an indicator of the thermal environment.

Cardiac valve replacement: a 7-year long-term evaluation (심장조직판막치환: 7 년간의 술후 장기성)

  • Six hundred fourteen consecutive cases of bioprosthetic cardiac valve replacement performed during the period from March 1976 through December 1982 were reviewed. A total of 748 tissue valves [534 Ionescu-Shiley valves, 144 Hancock valves, 46 Angell-Shiley, and 24 Carpentier-Edwards] were implanted in 610 patients. Of these, 477 had single valve replacements [403 mitral, 60 aortic, and 14 tricuspid] including three REDO MVR and one REDO AVR. The remaining 129 had double valve replacements [95 AVR and MVR and 34 MVR and TVR] and 8 had triple valve replacement.592 cases were evaluated. Overall early mortality rate [within 30 days of operation] was 7.1% [6.2% in single valve replacement, 10.2% in double valve replacement, and 16.7% in triple valve replacement]. Leading causes of mortality were low cardiac output or myocardial failure and ventricular arrhythmias. The follow-up period was from one month to 7 years with a cumulative follow-up of 906.6 patient-years [mean 1.53 years]. The late mortality was 1.6%, 3.9%, 0%, 2.6%, 6.6% and 2.0% per patient-year for MVR, AVR, TVR or triple valve replacement, AVR+MVR, MVR+TVR and total, respectively. Actuarial analysis of late results including early mortalities indicates an expected survival rate of 87.6+1.8% at 3 years and 85.92.4% at 7 years for all cases. We also analyzed actuarial survival rate between groups of each valve replacement [AVR, TVR, Double valve, and Triple valve] and the tissue valve groups in MVR. We experienced 7 cases [0.77% per patient-year] of confirmed endocarditis, two of which were fatal. Valve failure-free rates calculated according to the confirmed cases were 97.5% at 4 years, 87.5% at 7 years, and 88.3% at 6 years for Ionescu-Shiley, Hancock and Angell-Shiley valves, respectively. The occurrence rate of thromboembolism was 2.0% per patient-year in total cases, although almost all the patients were given anticoagulant therapy for one year. The occurring rate in MVR was 1.5% and 2.7% per patient-year for Ionescu-Shiley and Hancock valve groups, respectively. The difference in actuarial rate free from thromboemboli between Ionescu-Shiley and Hancock groups was statistically significant [P value less than 0.001]. Thromboembolic events beyond the period of anticoagulation therapy mainly occurred in patients with atrial fibrillation. The actuarial thromboemboli free survival was 95.71.4% at 3 years and 80.17.3% at 7 years. The incidence of hemorrhagic complications was 1.2% per patient-year [fatality 0.55% per patient-year] for anticoagulated patients. Although our clinical data favorably compares with results from other reports, our results suggest that anticoagulant therapy be given on a short-term basis or not at all to hemodynamically stable patients. Long-term therapy with antiplatelet drugs is probably inevitable with patients who have thromboembolic risk factors [such as atrial fibrillation].

Randomized controlled trial to compare oral analgesic requirements and patient satisfaction in using oral non-steroidal anti-inflammatory drugs versus benzydamine hydrochloride oral rinses after mandibular third molar extraction: a pilot study

  • Background: Third molar extraction is associated with considerable pain and discomfort, which is mostly managed with oral analgesic medication. We assessed the analgesic effect of benzydamine hydrochloride, a topical analgesic oral rinse, for controlling postoperative pain following third molar extraction. Methods: A randomized controlled trial was conducted in 40 patients divided into two groups, for extraction of fully erupted third molar. Groups A received benzydamine hydrochloride mouthwash and group B received normal saline gargle with oral ibuprofen and paracetamol. Oral ibuprofen and paracetamol was the rescue analgesic drug in group A. Patients were evaluated on the $3^{rd}$ and $7^{th}$ post-operative days (POD) for pain using the visual analogue score (VAS), trismus, total number of analgesics consumed, and satisfaction level of patients. Results: The VAS in groups A and B on POD3 and POD7 was $4.55{\pm}2.54$ and $3.95{\pm}1.8$, and $1.2{\pm}1.64$ and $0.95{\pm}1.14$, respectively and was statistically insignificant. The number of analgesics consumed in groups A and B on POD3 ($5.25{\pm}2.22$ and $6.05{\pm}2.43$) was not statistically different from that consumed on POD7 ($9.15{\pm}5.93$ and $10.65{\pm}6.46$). The p values for trismus on POD3 and POD7 were 0.609 and 0.490, respectively and those for patient satisfaction level on POD3 and POD7 were 0.283 and 0.217, respectively. Conclusions: Benzydamine hydrochloride oral rinses do not significantly reduce intake of oral analgesics and are inadequate for pain relief following mandibular third molar extraction.

Cerebrospinal Fluid Lumbar Tapping Utilization for Suspected Ventriculoperitoneal Shunt Under-Drainage Malfunctions

  • Objective : The diagnosis of shunt malfunction can be challenging since neuroimaging results are not always correlated with clinical outcomes. The purpose of this study was to evaluate the efficacy of a simple, minimally invasive cerebrospinal fluid (CSF) lumbar tapping test that predicts shunt under-drainage in hydrocephalus patients. Methods : We retrospectively reviewed the clinical and radiological features of 48 patients who underwent routine CSF lumbar tapping after ventriculoperitoneal shunt (VPS) operation using a programmable shunting device. We compared shunt valve opening pressure and CSF lumbar tapping pressure to check under-drainage. Results : The mean pressure difference between valve opening pressure and CSF lumbar tapping pressure of all patients were $2.21{\pm}24.57mmH_2O$. The frequency of CSF lumbar tapping was $2.06{\pm}1.26times$. Eighty five times lumbar tapping of 41 patients showed that their VPS function was normal which was consistent with clinical improvement and decreased ventricle size on computed tomography scan. The mean pressure difference in these patients was $-3.69{\pm}19.20mmH_2O$. The mean frequency of CSF lumbar tapping was $2.07{\pm}1.25times$. Fourteen cases of 10 patients revealed suspected VPS malfunction which were consistent with radiological results and clinical symptoms, defined as changes in ventricle size and no clinical improvement. The mean pressure difference was $38.07{\pm}23.58mmH_2O$. The mean frequency of CSF lumbar tapping was $1.44{\pm}1.01times$. Pressure difference greater than $35mmH_2O$ was shown in 2.35% of the normal VPS function group (2 of 85) whereas it was shown in 64.29% of the suspected VPS malfunction group (9 of 14). The difference was statistically significant (p=0.000001). Among 10 patients with under-drainage, 5 patients underwent shunt revision. The causes of the shunt malfunction included 3 cases of proximal occlusion and 2 cases of distal obstruction and valve malfunction. Conclusion : Under-drainage of CSF should be suspected if CSF lumbar tapping pressure is $35mmH_2O$ higher than the valve opening pressure and shunt malfunction evaluation or adjustment of the valve opening pressure should be made.

Antioxidative Effects of Mugwort (Artemisia vulgaris L.) Extracts Diet on ICR Mouse Skin (애엽 추출물 섭취가 ICR 마우스의 피부조직에 미치는 항산화 효과)

  • The feeding effects of mugwort (Artemisia vulgaris L.) extracts (ME) on the anti-oxidative actions of ICR mouse skin was investigated. To study the antioxidative effects of ME on ICR mouse skin, female ICR mice were grouped into basic diet group (control), ascorbic acid diet group (AA-2.5, AA-5.0, AA-10.0 and AA-20.0 mg/kg BW/day) as a positive control and experimental diet group (mugwort extract; ME-25, ME-50, ME-100, and ME-200 mg/kg BW/day) and fed for 10 weeks. Protein contents in ME-50, ME-100, and ME-200 feeding group were increased ($3.1%{\sim}11.1%$) and hydroxyl radical contents were significantly decreased ($10.4%{\sim}17.4%$) compared to control group. Oxidative stress signals and oxidized protein contents were significantly reduced to the range of 15.3 to 17.1% in ME-100 and ME-200 groups. Also, superoxide dismutase (SOD) activity was significantly increased to the range of 15.0% to 23.3% in ME-100 and ME-200 groups. Catalase activities were significantly increased ($14.0%{\sim}36.9%$) in all groups in a dose-dependent pattern. Antioxidative ability of ME showed similarity to that of ascorbic acid.

Feasibility of Emotional Freedom Techniques in Patients with Posttraumatic Stress Disorder: a pilot study

  • Objectives: Posttraumatic stress disorder (PTSD) is a prevalent mental health condition, and techniques using sensory stimulation in processing traumatic memories have gained attention. The Emotional Freedom Techniques (EFT) is a psychotherapy that combines tapping on acupoints with exposure to cognitive reframing. This pilot study aimed to assess the feasibility of EFT as a treatment for PTSD by answering the following research questions: 1) What is the compliance and completion rate of patients with PTSD with regard to EFT protocol? Is the dropout rate reasonable? 2) Is the effect size of EFT protocol for PTSD sufficient to justify a future trial? Methods: Thirty participants diagnosed with PTSD were recruited. They received weekly EFT sessions for five weeks, in which they repeated a statement acknowledging the problem and accepting themselves while tapping the SI3 acupoint on the side of their hand. PTSD symptoms were evaluated using the PTSD Checklist for DSM-5 (PCL-5) before and after the intervention. Results: Of the 30 PTSD patients (mean age: 34.1 ± 9.1, 80% female), 96.7% showed over 80% compliance to the EFT sessions, and 86.7% completed the entire study process. The mean PCL-5 total score decreased significantly after the intervention, with a large effect size (change from baseline: -14.33 [95% CI: -19.79, -8.86], p < 0.0001, d = 1.06). Conclusion: The study suggests that EFT is a feasible treatment for PTSD, with high session compliance and low dropout rates. The effect size observed in this study supports the need for a larger trial in the future to further investigate EFT as a treatment for PTSD. However, the lack of a control group and the use of a self-rated questionnaire for PTSD symptoms are limitations of this study. The findings of this pilot study can be used to plan a future trial.

The Development of the Korean Evaluation Scale for Hearing Handicap (KESHH) for the Geriatric Hearing Los (노인성난청을 위한 청각장애평가지수(KESHH)의 개발)

  • The hearing impairment is the representative disorder that affects the quality of the routine life of the aged period. This study was aimed to develop the Korean evaluation scale for hearing handicap(KESHH) with which we can evaluate social and psychological effects of the hearing impairment. Applying this scale clinically, we can analyze the geriatric hearing loss specifically and improve the quality of the aural rehabilitation that can help the hardness of the hearing impairment. Data were collected from 288 participants(176 hearing aid users and 112 non-hearing aid users) and the average age of the participants was 67.4 years old ( 60.15 for the hearing aids users and 78.9 for the non hearing users). The composition ratio of the male and female participants were 58.0% and 42.0% and extrovert and introvert personality were 49.3% and 50.7% showing balanced formation. The tentative draft of KESHH measurements were produced with 30 items and following 5 subscales. Using factor analysis, 6 items were erased and 4 subscales - social effect, psycho/emotional effect, interpersonal effect, and perception of hearing aids - were identified. As each subscale consisted of 6 items, 24 items were corrected and remained totally. Conclusively, the KESHH was developed with 24 items and 4 subscales including 6 items on each subscale. In addition, the KESHH was divided into type-1 and 2 depending on hearing aid users and non hearing aid users. The results of this study can be summarized as the following 5 parts. Firstly, the reliabilities of the KESHH were proved to be high because the subscales' Cronbach alpha values were from 0.723 through 0.895. Secondly, the KESHH showed systematically increasing score as the hearing impairment increased. The lowest score was 24 and the highest score was 117 and the average scores of the hearing impaired and non-hearing impaired are 72.06(SD=15.67) and 66.98(SD=20.94) showing 5.08 increased score for the hearing impaired. Depending on the degree of the hearing loss, the scores recorded 52.63 at the below of the mild hearing loss, 67.29 for the moderate hearing loss, 71.89 for the moderately severe hearing loss, and 75.57 for the severe hearing loss The comparison of the scores by hearing levels indicated that the higher the hearing levels were, the higher the scores of the KESHH with statistical significance(p<0.001). Thirdly, the correlation among 4 subscales was 0.384~0.880(p<0.001). Also, the pure tone average, personality, and the four subscales correlations showed statistical significance with 0.148~0.880 except for the pure tone average and personality and the pure tone average and perception of hearing aids. Fourthly, the total variances explained for the independent subscles were analyzed with multiple regression. The social effect was explained 17.4% with pure tone average, personality, and the status of hearing aid use variances. The psycho/emotional effect was explained 14.4% with puretone average, personality, and age variances. The interpersonal effect was explained 11.2% with pure tone average, personality, and the status of hearing aid use variances. The perception of hearing aids effect was explained 2.2% with only personality. Finally, test-retest reliability was proved to be high with 0.791(p<0.001). Conclusively, the KESHH that was developed considering Korean culture can be a useful instrument for expressing the hearing handicaps of the Korean aged hearing impaired in scores for both hearing aid users and non-users. Also, it is thought that the KESHH is useful clinically for identifying the changes of the hearing handicap scores before and after wearing hearing aids and aural rehabilitation at diverse situations.

Analyzing the Importance and Performance of Sanitation Management within Childcare Center Foodservice Facilities in Gyeongbuk Province (경북 지역 보육 시설 급식소의 위생 관리에 대한 중요도-수행도 조사)

  • The purpose of this study was to analyze the importance and performance of sanitation management within childcare center foodservice facilities in Gyeongbuk province. The survey involved 248 people who participated in the food hygiene and safety education for childcare center managers in Gyeongbuk province from June to July 2010. A total of 236 survey papers were analyzed statistically by SPSS program. By the method, t-test and importance-performance analysis (IPA) was performed. Among the respondents, facility managers are 40s (41.9%) which accounted the most percentage, and for the type of facility, the largest amount was home (39.9%). The number of preschool children who are under 20 people accounted for 45.3% and food service staffs were accounted for 36.7%. No significant differences were observed for the four items on the IPA, except for the topics "the immediate deal with trash and leftover food" and "minimizing time (within 2 hours) for distribution after cooking" (p<0.001). The average score on the IPA was 4.14 points, and the average score for the importance of the evaluation was 4.49 points out of 5. Finally, childcare facility managers are aware of the importance that is lower than can be done. The results suggest that continuing education is necessary to administrators to manage the health care facility effectively.

Nutrient Composition of Benthic Diatom Cultured on Plate at Four Areas in Spring (봄철 해역별 파판배양 부착규조류의 영양성분)

  • 봄철 해역에 따른 부착미세조류의 영양성분 변화를 알아보기 위하여 본 실험을 수행하였다. 실험은 울진, 부안, 완도, 부산 지역에서 봄철인 2~4월 동안 실시되었으며 실험간의 오차를 줄이기 위해 실험 시작일의 차이는 최대 10일 이내로 하였다. 배양을 위한 실험해역의 기초 조사로 배양지역 연안해수의 수온, 염분, pH, 총질소, 총인, Chlorophyll-a의 측정을 하였고 각 지역의 부착미세조류를 수확하여 영양분석을 하였다. 실험에 사용된 파판은 전복 종묘생산에 이용되는 규조배양용 파판과 틀을 이용하였고 실험기간은 총 8주 동안 실시하였다. 2주마다 파판을 수거한 뒤 부착된 미세조류를 부드러운 솔로 분리하여 원심 분리한 후 지방산과 아미노산의 측정을 하였다. 또 각 시료의 일정량을 Sedwick-rafter counting chamber를 이용하여 검경ㆍ계수한 후 이를 단위면적당 개체수(cells/$\textrm{cm}^2$)로 환산하여 biomass를 구하였다. 봄철 지역간의 수온의 범위는 13.9~16.6$^{\circ}C$를 보였으며 울진이 가장 높고 부안지역이 가장 낮았다. 염분은 부안지역의 염분이 평균 2$\textperthousand$ 이상 낮았으며 그 외의 지역은 큰 차이가 없었다. pH는 7.77~8.21의 범위를 보이고 있었으며 질산염 농토는 부산이 0.265mg/$\ell$로 최대값을 보였고 울진이 0.032mg/$\ell$로 제일 낮았다. 인산염 농도는 완도지역이 0.018mg/$\ell$로 최대치를 보였고 부안은 검출되지 않았다. Chlorophyll-a의 농도는 울진지역이 0.66mg/$\ell$으로 제일 낮았으며 부산이 2.69mg/$\ell$, 부안지역이 2.27mg/$\ell$ 로 높은 값을 보였다. 봄철 지역에 따른 부착미세조류의 아미노산 분석결과는 울진이 실험지역 중 제일 높은 총 아미노산과 필수아미노산의 함량을 나타냈으며 각 지역별로 울진은 4주에 20.3%, 부안은 8주째 8.1%, 완도는 8주째 4.7%, 부산은 6주째 10.4%로 서로다른 시기에 높은 아미노산의 함량을 나타냈다. 실험지역 부착미세조류의 지방산 결과를 보면 전 실험기간동안 지방산 중 대부분은 $C_{18}$이하의 포화 또는 불포화 지방산이 주를 이루고 있었으며 palmitic acid( $C_{16:0}$)와 palmitoleic acid( $C_{16:0}$)의 양이 전체 지방산 함량중 37.47~75.94%로 대부분을 차지하고 있었다. 그리고 n-3HUFA(high unsaturated fat쇼 acids)인 EPA는 부안지역이 2주째 20.85%로 전체 지역중 함량이 제일 높았고, 울진은 실험기간중 6주가 14.76%로 제일 높은 함량을 보였다. 완도는 6주에만 10.97%의 함량을 보였으며 부산지역의 시료에서는 EPA의 검출이 없었다. 봄철 각지역에 설치된 파판에서의 부착미세조류 출현량을 보면 2주째의 부착미세조류량은 부안이 8.3$\times$$10^3$ cells/$\textrm{cm}^2$로 가장 높았고 부산이 0.6$\times$$10^3$ cells/$\textrm{cm}^2$로 가장 낮았다. 울진의 경우 0.7$\times$$10^3$ cells/$\textrm{cm}^2$로 부산과 비슷하게 낮은 출현량을 보였고 완도는 2.4$\times$$10^3$ cells/$\textrm{cm}^2$였다. 4주째의 출현량은 울진지역이 471.1$\times$$10^3$ cells/$\textrm{cm}^2$로 가장 높았으며 부안, 완도, 부산 순으로 출현량이 낮았고 특히 부산은 1.2$\times$$10^3$ cells/$\textrm{cm}^2$로 가장 낮았다. 6주째의 출현량은 울진 지역이 368.6$\times$$10^3$ cells/$\textrm{cm}^2$로 가장 높았으며 부안 89.8$\times$$10^3$ cells/$\textrm{cm}^2$, 완도 25.1$\times$$10^3$ cells/$\textrm{cm}^2$, 부산 5.9$\times$$10^3$ cells/$\textrm{cm}^2$ 순이었다. 8주째의 경우에도 울진, 부안, 완도, 부산이 각각 255.8$\times$$10^3$ cells/$\textrm{cm}^2$, 157.8$\times$$10^3$ cells/$\textrm{cm}^2$, 44.3$\times$$10^3$ cells/$\textrm{cm}^2$, 1.1$\times$$10^3$ cells/$\textrm{cm}^2$로 4주, 6주의 경우와 같은 경향을 보였다.보였다.

Investigation on Characteristics of Swine Manure of Optimum Volume for Escalator Reversing Composting Facility (돼지분뇨 특성에 따른 기계교반 퇴비화시설의 적정용적 산정 연구)

  • This study was carried out to investigate evaporation rate of moisture per surface area and degradation rate of organic matter in full scale escalator reversing composting facility were analyzed to develope a computer program for the computation of an optimum volume of composting facility according to handling methods of swine farm, moisture levels of manure, degradation rate of organics and evaporation rate of moisture during composting. The obtained results can be followed as bellow; The temperature in full scale escalator reversing composting facility during composting reached $70^{\circ}C$ in 4 days and maintained until 11 days. Reduction rate of moisture and density was average 1.20% and 29.7%, respectively. Annual degradation rate of organic matter was 3.53%, showing lowest rate in winter as 3.23%. These seasonal degradation rate could be a factor to be considered for proper management and installation of composting facility. When computed with the amount of feces, urine, slurry and manure plus wastewater produced, the optimum volumes of composting facility for slurry and manure plus wastewater including each 95% moisture was $229m^3$ and $277m^3$, respectively, showing 21% ($48m^3$) difference.
