그라우팅(C.G.S)에 의한 구조물 기초 보강 (Reinforcement of the Structure Foundation using Grouting(C.G.S))

  • The use of Compaction Grouting evolved in 1950's to correct structural settlement of buildings. Over the almost 50 years, the technology has been developed and is currently used in wide range of applications. Compaction Grouting, the injection of a very stiff, 'zero-slump' mortar grout under relatively high pressure, displaces and compacts soils. It can effectively repair natural or man-made soil strength deficiencies in variety of soil formations. Major applications of Compaction Grouting include densifying loose soils or fill voids caused by sinkholes, poorly compacted fills, broken utilities, improper dewatering, or soft ground tunneling excavation. Other applications include preventing liquefation, re-leveling settled structures, and using compaction grout bulbs as structural elements of minipiles or underpinning. In this paper, on the basis of the case history constructed in this year, a study has been performed to analyze the basic mechanism of the Compaction Grouting. Also, the effectiveness of the ground improvement and the bearing capacity of the Compaction Pile has been verified by the Cone Penetration Test(CPT) and Load Test. Relatively uniform Compaction grouting column could be maintained by planning the Quality Control in the course of grouting. And, the Quality Control Plan has been conceived using grout pressure, volume of grout and drilling depth.

Protocol Based Real-Time Continuous Electroencephalography for Detecting Vasospasm in Subarachnoid Hemorrhage

  • A continuous electroencephalography (cEEG) can be helpful in detecting vasospasm and delayed cerebral ischemia in aneurysmal subarachnoid hemorrhage (SAH). We describe a patient with an aneurysmal SAH whose symptomatic vasospasm was detected promptly by using a real-time cEEG. Patient was immediately treated by intraarterial vasodilator therapy. A 50-year-old woman without any significant medical history presented with a severe bifrontal headache due to acute SAH with a ruptured aneurysm on the anterior communicating artery (Fisher grade 3). On bleed day 6, she developed a sudden onset of global aphasia and left hemiparesis preceded by cEEG changes consistent with vasospasm. A stat chemical dilator therapy was performed and she recovered without significant neurological deficits. A real-time and protocol-based cEEG can be utilized in order to avoid any delay in detection of vasospasm in aneurysmal SAH and thereby improve clinical outcomes.

The Treatment of Left Atrial Appendage Aneurysm by a Minimally Invasive Approach

  • Left atrial appendage (LAA) aneurysm is a rare, pathologic condition that may lead to atrial tachyarrhythmia or thromboembolic events. A 49-year-old man presented with aggravated palpitation and dizziness. He suffered from refractory atrial fibrillation despite a previous history of radiofrequency catheter ablation. Echocardiography revealed a 57-mm LAA aneurysm. Surgical ablation was performed through a right mini-thoracotomy, and the LAA aneurysm was obliterated with a 50-mm AtriClip (Atricure Inc., Westchester, OH, USA). However, follow-up computed tomography showed residual communication, so the patient is still taking warfarin. We report that a minimally invasive strategy for treating LAA aneurysm can be considered, but incomplete closure may occur; thus, caution is needed.

Compaction Grouting, the injection of a very stiff, 'zero-slump' mortar grout under relatively high pressure, displaces and compacts soils. It can effectively repair natural or man-made soil strength deficiencies in variety of soil formations. Major uses of Compaction Grouting include densifying loose soils or fill voids caused by sinkholes, poorly compacted fills, broken utilities, improper dewatering, or soft ground tunneling excavation. Other application include preventing liquefaction, re-leveling settled structures, and using compaction grout bulbs as structural elements of minipiles or underpinning. The technique replaced slurry injection, or 'pressure grouting', as the preferred method of densification grouting. There are several reasons for the increased use of Compaction Grouting which can be summarized in one word: CONTROL. The low slump grout and injection processes are usually designed to keep the grout in a homogeneous mass at the point of injection, while acceptable in some limited applications, tends to quickly get out of control. Hydraulic soil fracturing can cause extensive grout travel, often well beyond the desired treatment zone. So, on the basis of the two case history constructed in recent year, a study has been peformed to analyze the basic mechanism of the Compaction Grouting and verify the effectiveness of the ground improvement using some test methods.

  • A 38-year-old female with arthralgia in right elbow joint for 6 months was referred for a bone scan which showed diffuse uptakes of $^{99m}Tc$-MDP in the liver and spleen without hepatosplenomegaly. She had a history of hepatitis B vaccination 3 days ago. These uptakes were disappeared on the follow-up bone scan after 4 months. We suggest this transient diffuse hepatic uptake after vaccination of hepatitis B might be due to aluminum component within the hepatitis B vaccine as adjuvant.

거대 표피 낭종의 치험례 (A Case of Giant Epidermal Inclusion Cyst)

  • Purpose: The epidermal cyst is a very common skin lesion which usually occurs in the hairy regions. They are generally small but rarely reach more than 5 cm in diameter. We present a patient with a giant epidermal cyst on buttock area. Methods: A 50-year-old man with a slowly enlarging, huge mass in his left buttock was examined. There was no history of trauma in this area. Physical examination revealed a soft, nontender, $15{\times}15cm$-sized mass in his left buttock. T1-weighted magnetic resonance images demonstrated a well-circumscribed, multilocular cystic lesion with homogeneous, slightly high signal intensity. On T2-weighted images the lesion had wide areas of high signal intensity. The mass was totally excised. Results: A histopathological finding revealed that the cystic wall was lined with whole layers of stratified squamous epithelium. Keratin layers from the surface of the epithelium were seen to be sloughing into the cystic lumen. Multinucleted giant cells were found outside the cystic wall. Conclusion: Herein we report a rare case of giant epidermal cyst occurring on the buttock.

  • Cowden 증후군으로 진단되어 추적 관찰 중인 21세 여자에서 위장관 용종의 정기적 관찰을 위하여 시행한 내시경 검사 중 캔디다 식도염이 발견되어 문헌 고찰과 함께 보고한다. 면역학적 이상, 진균 식도 감염의 선행 질환, 투약의 병력이 없었고, 뚜렷한 식도 감염의 임상 증상도 없었다.

  • The calcaneal insufficiency avulsion fracture usually occurs in an area of fused apophysis in adults without significant history of trauma or overuse activities. It is an uncommon injury which has been described in patients with complicated diabetes, Charcot neuroarthropathy, amyloidosis with neuropathy, severe osteoporosis, and other conditions. Discussion of the issue of fracture location is still not sufficient. We report on a case of a 50-year-old male who experienced a non-traumatic diabetic calcaneal insufficiency fracture. Intraoperatively, a biopsy specimen was obtained from the exposed fracture site for histological study. We assume that the calcaneal fused apophyseal line is the weak point of failure due to various incomplete mixtures of trabecular bone, woven bone, and cartilaginous tissues, and may fail when repeated tensile stress is imposed.

A case of anemia caused by combined vitamin B12 and iron deficiency manifesting as short stature and delayed puberty

  • Anemia caused by vitamin B12 deficiency resulting from inadequate dietary intake is rare in children in the modern era because of improvements in nutritional status. However, such anemia can be caused by decreased ingestion or impaired absorption and/or utilization of vitamin B12. We report the case of an 18-year-old man with short stature, prepubertal sexual maturation, exertional dyspnea, and severe anemia with a hemoglobin level of 3.3 g/dL. He had a history of small bowel resection from 50 cm below the Treitz ligament to 5 cm above the ileocecal valve necessitated by midgut volvulus in the neonatal period. Laboratory tests showed deficiencies of both vitamin B12 and iron. A bone marrow examination revealed dyserythropoiesis and low levels of hemosiderin particles, and a cytogenetic study disclosed a normal karyotype. After treatment with parenteral vitamin B12 and elemental iron, both anemia and growth showed gradual improvement. This is a rare case that presented with short stature and delayed puberty caused by nutritional deficiency anemia in Korea.

  • Spontaneous spinal epidural hematoma, an idiopathic accumulation of blood in the vertebral epidural space without identifiable predisposing factors, is a rare condition. The diagnosis can be made from a careful history and neurological examination, but clinical diagnosis is often difficult because of its non-specific symptomatology and it can be confused with myocardial infarct, musculoskeletal pain, vasculitis and acute dissection of an aortic aneurysm. For a favourable outcome, early decompressive laminectomy and evacuation of hematoma are necessary. We report a 50-year-old female who presented with acute paraparesis and back pain on thoracic area. Diagnosis of spinal epidural hematoma which extended over $T_4-T_9$ was made by magnetic resonance imaging. We performed immediately decompressive laminectomy and evacuated the epidural hematoma. No cause for bleeding was evident. Postoperatively, neurological symptoms disappeared within six weeks.
