30~70대 여성의 골밀도, 생화학적 특성 및 영양소 섭취 분석 -2011년 국민건강영양조사를 중심으로- (Analysis of Bone Mineral Density, Biochemical Index and Nutrient Intakes of 30-70 Years Old Women -Based on 2011 KNHANES-)

  • Objectives: This study was carried out to investigate bone mineral density (BMD), blood pressure, age and biochemical index and nutrient intakes, and to analyze the relationship between BMD, blood pressure, biochemical index, nutrient intakes of Korean women. Methods: The study subjects were 499 women with a age range of 30-79 years living in Seoul and Kyunggi area who participated in 2011 KNHANES. The study subjects were divided into 5 age groups : 30 years (145), 40 years (110), 50 years (102), 60 years (85) and 70 years (57), and 3 BMD groups: normal 258 (50.4%), osteopenia 163 (32.9%) and osteoporosis 78 (16.7%). Results: The average waist circumference, BMI and body fat increased with age, but fat free mass decreased with age. Average BMD and T-score was decreased from 0.84, $0.74g/cm^2$ in 30 years to 0.05, $-1.05g/cm^2$ in 70 years. The rates of osteopenia and osteoporosis increased significantly 22% in 30~40 years, 47% in 50 years and 56% in 60~70 years. Systolic blood pressure and cholesterol were significantly increased with age. The rates of hypertension was significantly increased 2.1% in 30 years, 30.4% in 50 years 89.5% in 70 years. Average nutrient intakes such as protein, calcium, iron, vitamin A and C were significantly decreased with age. There were significant negative correlations between age and calcium, systolic blood pressure, triglyceride. Conclusions: In this study, we found the risk of osteoporosis and hypertension were significantly increase with over 50 years age. Calcium and protein intake decreased with age. Therefore, in order to prevent osteoporosis and hypertension, adult women need to be educated regarding the importance of protein, calcium and other nutrients in their diet.

서울 및 근교에 거주하는 한국인의 연령별 식생활 비교 및 평가: (1) 영양소 섭취 비교 (Comparative Analysis and Evaluation of Dietary Intakes of Koreans by Age Groups: (1) Nutrient Intakes)

  • It is necessary to compare the dietary characteristics among different age groups because different nutritional problems require different dietary applications. This study was performed to identify the nutritional characteristics of Korean diet among different age groups. The study subjects consisted of preschool children(PC), elementary school children(EC), high school students(HS), college students(CS), and adults over 30 years old living in Seoul and the surrounding areas. The subjects over 30 years old were classified to 30-49 years, 50-69 years, and over 70 years. A dietary survey was conducted using 24-hour recall method and data were collected from 2392 subjects. Energy intake was as much as 1771kcal for PC, 2139kcal for EC, 2475kcal for HS, 2276kcal for CS, 1860kcal for 30-49 years, 1715kcal for 50-69 years, 1326kcal for over 70 years. The subjects of PC, EC, HS, and CS groups got about 25% of energy from fat, but the subjects in 30-49 years, 16%, those in 50-69 years, 15%, and those over 70 years, 13%. The energy consumption from protein was about 15% in each age group. Percent of energy intake as Korean RDA was greater than 100% for PC, EC, HS, and male CS groups. The percent intake for RDA of calcium, iron, vitamin A, and vitamin B$_2$was low in most age group. Iron intake was inadequate in female of PC and CS groups, and subjects in 30-49 years and over 70 years. Mean adequacy ratio(MAR) was 0.9 in PC, and decreased with increase of age. Therefore MAR did not reach to 0.7 among subjects over 70 years. Index of nutritional quality(INQ) for calcium, iron, vitamin A, and vitamin B was below 1 in most age groups. From these results, major nutritional problems of each age group were overall inadequate intakes of calcium and iron, excess consumption of protein and fat for PC, EC, HS, and CS groups, and inadequate nutrients intake of subjects over 70 years old. 대장암 검진-50세 이하부터 시작하는 것은 필요한 것인가? (Colon Cancer Screening-Is It Necessary to Start under the Age of 50?)

  • Colorectal cancer is the third most common cancer worldwide. The occurrence of colon cancer can be prevented by removing precursor lesions. Several countries are making efforts to prevent the occurrence of colon cancer via screening programs. Korea is also following suite by screening individuals < 50 years of age. Currently, the incidence of colon cancer among the young is increasing globally, and Korea has a high colon cancer incidence rate among individuals in their 20s and 40s. Therefore, it may be necessary to start the screening individuals < 50 years of age to detect the cancer's manifestation early. Moreover, advanced adenomas associated with poor prognosis can be detected early, the overall screening rate can be increased, and death from colon cancer at a young age can be prevented. Although the period of colorectal cancer screening has been expanded, the overall colorectal cancer screening rate has not. Additionally, increasing the screening rate among individuals > 50 years could reduce the mortality rate at a lower cost. Although the incidence of colon cancer is increasing in younger individuals, the evidence to conclude that screening at the age of < 50 years has a meaningful impact on the incidence and survival rates of colon cancer remains insufficient. Therefore, rather than following the trend and performing screening early, an approach that actively selects cases where tests for colorectal cancer diagnosis are required is warranted.

정시안의 조절반응량에 관한 연구 (A Study of Accommodative Response on Emmetropia)

  • 본 연구 목적은 조절반응량을 측정할 수 있는 양얀개방형 자동굴절검사기(auto-refractometer)를 이용하여 정시안의 실제 조절반응량을 알아보고자 하였다. 나안시력이 1.0 이상이며 등기구면굴절력이 ${\pm}0.50D$ 이하이고, 난시도수가 -0.75D 이하인, 10세부터 59세사이(평균 $25.8{\pm}12.4$) 377안(남 146안, 여 231안)을 10세 간격의 연령군으로 나누어 조절반응량, 원근거리 난시도수와 난시축 변화를 분석하였다. 연령별 조절반응량은 전체 평균은 1.47D, 10대 1.63D, 20대 1.57D, 30대 1.71D, 40대 0.97D, 50대 0.05D로 10대에서 30대까지는 약간 증가를 보였으나, 40대 이상에서 현저한 저하를 보이다가 50대에는 거의 반응하지 않는 것으로 나타났다. 성별 차이는 남자는 평균 연령 $26.4{\pm}12.3$에 조절반응량은 1.56D, 여자는 $25.5{\pm}12.5$에 1.43D로 남자가 약간 높게 나타났다. 원근거리에서 난시도수의 변화는 전체 평균은 +0.12D, 10대 +0.06D, 20대 +0.16D, 30대 +0.09D, 40대 +0.05D, 50대 +0.06D, 남자 +0.16D, 여자 +0.09D로 모든 군에서 의미 있는 차이는 없었지만, 원거리보다 근거리에서 모두 군에서 난시가 높게 나타났다. 난시축 변화는 원거리 난시축에 비해 ${\pm}10^{\circ}$ 미만으로 변한 경우가 103안(33%), 시계방향으로 $10^{\circ}$ 이상 회전이 108안(34.6%), 반사계방향으로 $10^{\circ}$ 이상 회전이 101안(32.4%)으로 나타나 근거리에서 난시축의 변화가 있는 것으로 분석되었다.

50세 이상 천식 환자의 인플루엔자 백신 접종률 및 관련 요인 (The Estimated Proportion for Influenza Vaccination and Related Factors in Korea Adults Aged 50 and Older with Asthma)

  • Objectives: This study aims to investigate the estimated proportion for influenza vaccination and to identify factors associated with influenza vaccination in Korean adults aged 50 and older with asthma. Methods: Data from 2010-2015 Community Health Survey(n=23,662) was used. A chi-square test was performed to investigate the estimated proportion for influenza vaccination, and a multiple logistic regression analysis was used to identify the factors associated with self-reported influenza vaccination. Results: In men, 42.3-49.5% of asthma patients 50-64 years of age, and 78.5-90.2% of elderly (over 65 years of age) asthma patients received influenza vaccination. In women, 49.7-61.9% of asthma patients 50-64 years of age, and 82.7-89.7% of elderly asthma patients received influenza vaccination(p<0.0001). Low education level, non-smoking, non-drinking, hypertension, diabetes and use of public center were related to high influenza vaccination in 50-64 years old men with asthma. Low education level, non-smoking, bad health status, hypertension, diabetes and use of public center were related to high influenza vaccination in 50-64 years old women with asthma. Non-smoking and use of public center were related to high influenza vaccination in over 65 years old men with asthma. White-collar jobs, smoking, absence of hypertension and use of public center were related to low influenza vaccination in over 65 years old women with asthma. Conclusions: Influenza vaccination remains relatively low in asthma patients 50-64 years of age. It is necessary to recommend vaccination to asthma patients, provide them with information, and devise other strategies to improve vaccination.

10년간 우리나라 운항승무원의 혈중 지질과 관련요인 분석 (10-Year Analysis of Blood Lipid Profile and Other Risk Factors Among Aircrew Members in Korea)

  • This study was conducted to analyze blood lipid profile and other risk factors among crew members who are currently active and had more than 10 years of experience as crew members. Data was analyzed using medical record files in an airline medical department. The results are as follows. 1. The total number of crew members studied was 392. Among age groups, those below age 40 consisted of 26(6.9% of the total), those in age $41{\sim}50$ were 135 (34.4%) and those over age 51 were 230 (58.7%). 263 were former air force pilots consisting 66.9%. Those who had captain status numbered 21 I comprising 53.7% of the total. The type of aircraft most of the crew members involved were large size aircraft which totaled 268 (68.2%). With respect to the number of working years as crew members, 488(48.9%) comprised the largest group with $11{\sim}15$ years. 2. The rate of smoking among crew members has shown gradual decrease with 50.3% smoking in 1983 to 33.6% in 1993. Among $41{\sim}50$ age group the rate has shown a decrease from 20.9% to 13.3% In those group over age 51 it decreased from 25.5% to 16.6%. But group below age 40 were within the range of 3.6~3.8% with no significant change in the rate of smoking. 3. Body Mass Index in age group over 51 was slightly higher than other age groups. On the whole, BMI over 25 was not found. 4. The total cholesterol levels of those below age 40 were $196.9{\pm}38.5mg/dl,\;216.2{\pm}39.2mg/dl$ in ages $41{\sim}50$, and $225.1{\pm}42.5mg/dl$ in age group over 51. No significant difference was found among age groups. 5. HDL-cholesterol levels of over age 50 were higher than other age group and ranged from $40{\sim}55mg/dl$. 6. LDL-cholesterol levels of those over 51 were $126.7{\pm}37.7mg/dl$ higher than other age groups. But there were no significant changes in all age during 10 years of follow up. 7. Cardiac index of age group below age 40 was 3.8, 4.3 in age group $41{\sim}50$ and 4.5 in those over age 51 group. No significant changes among groups were found during the follow up period. 8. Triglyceride levels of age group below age 40 was $142.2{\pm}70.1,\;167.3{\pm}77.5$ in age group of $41{\sim}50$ and $113.6{\pm}897$ in age group over 50 showing that triglyceride levels increased with age. No significant changes in pattern were noted.

성인의 연령별 설탕 섭취상태와 혈중 지질과의 관련성 (Relation between Sugar Intake and Serum Lipids in Korean Adults according to Age)

  • The purpose of this study was to estimate sugar intake and relation between it and blood lipid profiles in Korean adult according to age. The 346 subjects were measured anthropometric assessment, dietary intake using 24-hour recall method, and serum lipid profiles. The average age of the subjects of 20-49, 50-64, over 65 years were 40.8, 57.0, 70.2 years, respectively. The mean height and weight were significantly decreased with increment of age(p<0.001, p<0.01). The mean energy, food, and sugar intakes of the age groups were decreased with increment of age(p<0.05, p<0.001, p<0.001), that is, 1649.8 ㎉/1253.6 g/36.1 g for 20~49 years, 1555.4 ㎉/1157.0 g/31.4 g for 50~64 years, 1404.4 ㎉/893.5 g/17.1 g for over 65 years. The major foods consumed sugar of the age groups were watermelon, sugar, ice cream for 20-49 years, watermelon, sugar, peach for 50-64 years, watermelon, sugar, kimchi for over 65 years. Carbohydrate intake of the subject adjusted age provided significantly negative correlation with serum HDL-cholesterol(p<0.05). However, there was not significant correlation between sugar intake and serum lipid profile. Based on these results, further studies on effects on blood lipid of sugar and carbohydrate intakes were needed for proper carbohydrate intake.

서울 일부지역 성인여성의 연령에 따른 골밀도에 영향을 미치는 요인 분석 (Analysis of Factors Affecting Bone Mineral Density with Different Age among Adult Women in Seoul Area)

  • Weight control diets induce reducing women' bone mineral density (BMD) that has a close relationship to risk in osteoporosis. This study was carried out to identify bone density risk factors affecting women's BMD, and to examine the relationship age, lifestyle and dietary habits for bone health by physical measurement and questonnaies. The subjects of this study were 194 women living on the Seoul area. When the subjects were divided into 4 age groups, BMI was the highest in the 50 years group (24.8) and the lowest in the 20 years group (21.63). Average T-score, which is BMD of forearm bone and calcaneus was the highest in the 40 years (-0.07) and the lowest in the 20 years (-0.59). The rate of eating breakfast was shown significantly higher in the 50 years group than that in the younger group. The frequencies of eating out, fried food intakes, and alcohol drinks were shown significantly different by age (p < 0.01). In conclusion, the risk rate of BMD was high in the 20syears and 50 years groups. It may due to the 20s' weight-control diet. Breakfast eating, exercise, intakes of anchovies, radishes, carrots, zucchinis and tomatoes were significantly important factors to prevent bone density risk.

전라북도 지역의 치과적 생체 연령추정 사례의 분석 (Analysis of Age Estimation Cases of Living Body Based on Dental aspect in Jeollabuk-do)

  • 2000년 1월부터 2007년 12월까지 전북대학교병원 치과진료처 구강내과에 의뢰된 연령추정 121례를 분석 및 평가하여 다음과 같은 결론을 얻었다. 1. 연령추정 의뢰인의 성별 분포는 남성(58.7%)이 여성(41.3%)보다 많았으며, 연령별로는 50대 이상이 전체의 과반수(55.4%)를 차지하였다. 2. 호적 연령과 주장 연령의 차이는 5세 이상(34.1%) 차이가 가장 많았고, 이는 50대 이상이 대부분이었다. 3. 연령을 늘리려는 의뢰인(64.2%)이 줄이려는 의뢰인(35.8%)보다 많았으며, 이는 남녀 모두 유사하게 나타났다. 10세 미만과 20대는 나이를 줄이려는 경우가, 10대, 30대이상에서는 나이를 늘리려는 경우가 많았다. 4. 복지를 이유로 연령정정을 원하는 경우가 가장 많았다. 복지혜택과 관련해서는 50대 이상이, 직장과 관련해서는 40대와 50대가, 교우관계와 관련해서는 30대와 40대가, 가족 관계와 관련해서는 50대가, 취업과 관련해서는 10대와 20대가 가장 많았다. 5. 연령추정 방법은 20세 이상에서는 영구치의 교모도와 치수강 퇴축도를 사용하였으며, 10세 이상 20세 미만에서는 치근단공 폐쇄시기를, 10세 미만에서는 영구치 석회화 정도를 각각 사용하였다. 20세 이상에서 영구치 교모도와 치수강 퇴축도를 사용하기 어려운 경우, 영구치의 상실시기와 하악각의 증령적 변화를 이용하여 연령을 추정하였다. 6. 각 방법에 의해 추정된 연령의 77.7%는 의뢰인이 주장하는 연령에 근접하였으나, 22.3%는 호적상의 연령에 근접하여 의뢰인의 주장과 일치하지 않았다.

30~60대 여성의 골밀도와 비만도, 폐경, 혈압, 식생활 관련 요인 분석 (Analysis of BMI Menopose, Blood Pressure and Dietary Habits Affecting Born Mineral Density of 30~60 Years Women)

  • Reducing women's bone mineral density (BMD) has close relationship to risk in osteoporosis. This study was carried out to identify bone density risk factors affecting women's BMD, and to analyze the relationship of age, between BMD and menopause, BMI, blood pressure lifestyle and dietary habits for bone health by physical measurement and questionnaires. The study subjects, 128 women living in Seoul, were divided into 4 age group; 30~39 years (17), 40~49 years (54) and 50~59 years (36) and 60~69 years (21). There were significant differences in mean height, BMI, systolic blood pressure, menache and menopause age and menopause ratio and BMD T-score among the 4 age groups. BHD was significantly decreased according to increasing age and BMI and menopause. The rate of BMD risk subjects was significantly different increasing with age and BMI. The rate of risk group were 0%, 13%, 22.2% and 71.4%, by age groups and 0%, 16.2%, 33.3% and 52.9% by BMI respectively. BMD of menopause groups was significantly decreased from 1.23 (40 years) to 1.34 (60 years). p < 0.001. Also systolic blood pressure were significantly increased from 116.5 mmHg (30 years) to 130.81 mmHg (60 years). The T-score of normal group also decreased significantly from 1.27 to 0.13 (60years) with age. There were significant negative correlation between BMD and age (r = -0.409) menopause (r = -0.346), BMI (r = -0.218) systolic blood pressure (r = -0.193), salty taste eating out (r = -0.185) (p < 0.05). There were significant positive correlation between BMD and meat fish and walking time. In conclusion, bone density decreased with age. Most of the 50 years' subjects were in risk group. BMI, menopause, systolic blood pressure, frequency alcohol consuming. To prevent osteoporosis, over 40 years needed to be educated to maintain normal weight and the improvement of eating and living habits.
