Simple Radiographic Analysis of Chronic Shoulder Pain in Patients 50 Years and Older (만성 견관절 동통을 가진 50세 이상 환자의 단순 방사선 사진 분석)

  • Purpose: To evaluate a usefulness of the simple radiograph in the patients with chronic shoulder pain 50 years and older. Material and method: 1152 patients with chronic shoulder pain and 100 asymptomatic individuals were involved in this study. All patients were 50 years and older. We excluded patients who had a history of fracture or dislocation. Radiographic interpretation was performed on a shoulder AP view, an axillary view and a supraspinatus outlet view. For statistical analysis, a chi-square test was performed. A p value of <0.05 was considered statistically significant. Results: Abnormal radiologic findings were identified in 369(32%) out of 1152 patients with a shoulder pain: greater tuberosity sclerosis, acromial sclerosis, subacromial osteophytes are common abnormal radiologic findings. A rotator cuff tear or impingement syndrome was identified on a final diagnosis in 61(85.2%) out of the 76 patients with radiologic abnormalities in both greater tuberosity and acromion (p<0.05). Abnormal radiologic findings were identified in 18% of the asymptomatic individuals. Conclusion: Simple radiographic analysis is an important primary diagnostic tool in patients (50 years and old) with chronic shoulder pain.

The High-risk Groups According to the Trends and Characteristics of Fatal Occupational Injuries in Korean Workers Aged 50 Years and Above

  • Background: Due to an increasing number of workers aged 50 years and above, the number of those employed is also on the rise, and those workers aged 50 and over has exceeded 50% of the total fatal occupational injuries. Therefore, it is necessary to implement the selection and concentration by identifying the characteristics of high-risk groups necessary for an effective prevention against and reduction of fatal occupational injuries. Methods: This study analyzed the characteristics of high-risk groups and the occupational injury fatality rate per 10,000 workers among the workers aged 50 and over through a multi-dimensional analysis by sex, employment status of workers, industry and occupation by targeting 4,079 persons who died in fatal occupational injuries from January 2007 to December 12. Results: The share of the workers aged 50 years and above is increasing every year in the total fatal occupational injuries occurrence, and the high-risk groups include 'male workers' by sex, 'daily workers' by worker's status, 'craft and related-trades workers' by occupation, and 'mining' by industry. Conclusion: The most frequent causal objects of fatal occupational injuries of the workers aged 50 years and above are found out to be 'installment and dismantlement of temporary equipment and material on work platforms including scaffold' in the construction industry and 'mobile crane, conveyor belt and fork lifts' in the manufacturing industry.

Some Factors Affecting Serum Lipid of Korean Rural Women (농촌지역 성인 여성들의 혈청 지질 수준에 영향을 미치는 요인 분석 연구)

  • Dietary and other factors affecting serum lipid levels of 103 rural women aged 30-76 years were assessed. Data for dietary intakes were obtained by 24-hour recall method. Body weight, height and blood pressure were measured and BMI was calculated from the anthropometric data. Serum samples were collected and analyzed for TG and lipoprotein fractions. Relation of the factors with serum lipid concentration was analyzed by Pearson's correlation coefficient(r). The results were summarized as follows: The weight, hight and BMI of the subjects were 56.8kg, 152.4cm and 24.5k/==, respectively. 31.8% of the subjects under 50 years of age($\leq$49yr group) and 61.0% of the subjects from 50 years up(==50yr group) were classified as hyperlipidemia. Most of the subjects had normal blood pressure but 8.5% were hypertensive. Total food intake of hyperlipidemic subjects was more than those of normal subjects in both age groups. Nutrients intake also tended to be higher in hyperlipidemic subjects of $\geq$50yr group. Intake of some foods like nuts, milk, or meat affected serum lipid profile even though the effects was somewhat different between two age groups. Body weight was positively related with serum TG and VLDL-cholesterol in $\leq$49yr group, and body weight as well as height and BMI affected serum lipid level in $\geq$50yr group. In summarization, it appeared that hyperlipidemia was a serious health problem in rural women. Hypertriglyceridemia due to sharp increase after 50 years old was remarkable and further research should be performed to determine the related factors in the near future.

Colon Cancer Screening-Is It Necessary to Start under the Age of 50? (대장암 검진-50세 이하부터 시작하는 것은 필요한 것인가?)

  • Colorectal cancer is the third most common cancer worldwide. The occurrence of colon cancer can be prevented by removing precursor lesions. Several countries are making efforts to prevent the occurrence of colon cancer via screening programs. Korea is also following suite by screening individuals < 50 years of age. Currently, the incidence of colon cancer among the young is increasing globally, and Korea has a high colon cancer incidence rate among individuals in their 20s and 40s. Therefore, it may be necessary to start the screening individuals < 50 years of age to detect the cancer's manifestation early. Moreover, advanced adenomas associated with poor prognosis can be detected early, the overall screening rate can be increased, and death from colon cancer at a young age can be prevented. Although the period of colorectal cancer screening has been expanded, the overall colorectal cancer screening rate has not. Additionally, increasing the screening rate among individuals > 50 years could reduce the mortality rate at a lower cost. Although the incidence of colon cancer is increasing in younger individuals, the evidence to conclude that screening at the age of < 50 years has a meaningful impact on the incidence and survival rates of colon cancer remains insufficient. Therefore, rather than following the trend and performing screening early, an approach that actively selects cases where tests for colorectal cancer diagnosis are required is warranted.

A clinical study of allergic rhinitis (알레르기 비염에 관한 임상적 연구)

  • As recent developments of Immunology and Nuclear medicine, serum IgE and IgG values are helpful in the diagnosis and evaluation of the therapeutic effects of nasal allergies. But in Korea, air pollution and the increased use of food additives have become leading factors in nasal allergies, It seems to be induced by environmental change, especially industrialization and urbanization, so allergic rhinitis in our environment has changed in accordance with the changes made in the living environment. Therefore this study is attempted in order to observe a clinical analysis which places more importance on allergic rhinitis. We studied 200 patients who had visited Kyunghee Oriental Medical Center with allergic rhinitis from January 1, 1999 to December 31, 1999 The results were as follows: 1. The sex distribution was 114 males(57%) and 86 females(43%). In age distribution, the average age was 25. In males, ages ranged from 3 to 66 years old and the average was 23.81. In females, ages ranged from 4 to 67 years old and the average was 28.57. The peak age was 30~39 years old(24%); under 9 years old and 10~19 years old were each 18%; 20~29 years old was 22%; 40~49 years old was 11 %; over 50 years old 6.5%. The gulf between males and females showed a statistically significant difference(P<0.025). 2. In the age of onset, male' s maximum was 62.5, minimum was 0.25; female s maximum was 59.5, minimum was 0.2. Under 9 years old was the most with 34%(male 24%, female 10%), 10~19 years old was 18%, 20~29 years old was 22.5%, 30~39 years old was 13.50%, over 40 years old was 12%, The gulf between males and females were showed statistically significant difference.(p<0.014) 3. The average duration of the disease was 5.67 years. In male and female, the maximum was 30, the minimum was 0.05; under 5 years old was the most with 62%(male 34.50%, female 27.50%); 6~10 years old was 23%. So, under 10 years old was 85%. There was no statistically significant difference in the duration of disease. 4. Regarding type of residence, 47.50% of patients with allergic rhinitis lived in apartments, 52.50% lived in houses. In males, 29.50% lived in apartments, 27.50% lived in houses. In females, 18% lived in apartments, 25% lived in houses. There was no statistically significant difference in the residence by T-test and chi-test. 5. In the distribution of season, spring is the most with 29.5% of patients, winter 28%, fall 25.5%, and summer 17%. But there was no statistically significant difference. 6. After observing 200 patients with allergic rhinitis, classifying main symptoms into 5 types, sneezing was the main symptom in 177cases(88.50%), nasal obstruction in 176cases(88%), rhinorrhea in 169cases(84.5%), post nasal discharge in 87cases(43.50%), and itching in I04cases(52%). The Cumulus ration is 98.50% and symptoms overlapped with an average 3.57±0.1 times but in an analysis of variance of these symptoms, the gulf between males and females was not recognized as statistically significant by T-test and ANOVA. 7. Patients whose families have allergic diseases account for 90 cases(45%) : 49cases(24.50%) male and 41cases(20.50%) female. There were 4 cases (71.11 %) whose families have allergic rhinitis, 9cases(10%) of asthma, and 7.78% with allergic dermatitis. There were 61 (67.80%) cases of patients whose parents have allergic diseases; cases wherein the patient s child had allergic diseases numbered 13 (14.45%); and cases with a sibling with allergic diseases totalled 16cases (17.80%). There was no statistically significant difference in allergic disease regarding sex, parents, or siblings by chi-test. 8. Blood type: For males, type A is the most common, with 37cases(18.5%), followed by type B with 32cases(16%), type O 28cases(l4%) and type AB 13cases(6.5%). For females, type B is the most common, with 30cases(15%), followed by type O with 23cases(l1.5%), type A with 18cases(9%) and type AB with 13cases(6.5%). There was no statistically significant difference in blood type by chi-test. 9. In the selection of prefered food, most patients prefer cool food, with 98 such cases(49%), tepid food in 54cases(27%) and warm food in 48cases(24%). These showed a statistically significant difference in the selection of prefered food between males and females by chi-test(p<0.009). 10. The state of Past History was classified into II types. chronic hypertrophic rhinitis is the most common with 11cases (18.64%), tonsil and adenoid hypertrophy is 8cases(l3.56%), sinusitis is 6cases(10.17%), nasal septum deviation is 4cases, nasal polyp is 2cases, others are 10cases(l6.95%). No statistically significant difference in past history between males and females was shown, but a statistically significant difference was shown when males and females were compared with total cases by T-test(p<0.002, P<0.0008). 11. Regarding complications, 37 patients (28.91%) had sinusitis: 22cases(17.19%) in male, 15cases(11.72%) in female. Chronic hypertrophic rhinitis was found in 15cases(11.72%). Others are under 10%. There was no statistically significant difference in the type of complications between males and females, but a statistically significant difference was shown when males and females were compared with total cases by T-test(P<0.00l, P<0.007). 12. In the treatment, medication was used 1691 times, an average of 2.58 times. No.34 was used 370 times for 124 cases, an average of 2.98 times. No. 152 was used 318 times for 106 cases, an average of 3.00 times. No.151 was used 307 times for 97cases, an average of 3.16 times. No. 31 was used 117 times for 33 cases, an average of 3.55 times. No 25 was used 116 times for 33 cases, an average of 3.52 times. 13. In the duration of treatment, the most frequent is 1 week(69cases, 34.50%), the maximum is 20weeks, and the minimum is 1week. A treatment period of 2~3 weeks accounted for 32% of cases, a period of 4~5weeks accounted for 13.5%. The gulf between males and females showed a statistically significant difference in the duration of treatment.(p<0.01). There was a statistical significance when the males were compared with total cases by ANOVA(P<0.03). 14. A comparison between before-treatment and after-treatment showed a statistically significant difference in treatment by T-test (p<0.01) and F-test (p<0.0058).

Comparative Analysis and Evaluation of Dietary Intakes of Koreans by Age Groups: (1) Nutrient Intakes (서울 및 근교에 거주하는 한국인의 연령별 식생활 비교 및 평가: (1) 영양소 섭취 비교)

  • It is necessary to compare the dietary characteristics among different age groups because different nutritional problems require different dietary applications. This study was performed to identify the nutritional characteristics of Korean diet among different age groups. The study subjects consisted of preschool children(PC), elementary school children(EC), high school students(HS), college students(CS), and adults over 30 years old living in Seoul and the surrounding areas. The subjects over 30 years old were classified to 30-49 years, 50-69 years, and over 70 years. A dietary survey was conducted using 24-hour recall method and data were collected from 2392 subjects. Energy intake was as much as 1771kcal for PC, 2139kcal for EC, 2475kcal for HS, 2276kcal for CS, 1860kcal for 30-49 years, 1715kcal for 50-69 years, 1326kcal for over 70 years. The subjects of PC, EC, HS, and CS groups got about 25% of energy from fat, but the subjects in 30-49 years, 16%, those in 50-69 years, 15%, and those over 70 years, 13%. The energy consumption from protein was about 15% in each age group. Percent of energy intake as Korean RDA was greater than 100% for PC, EC, HS, and male CS groups. The percent intake for RDA of calcium, iron, vitamin A, and vitamin B$_2$was low in most age group. Iron intake was inadequate in female of PC and CS groups, and subjects in 30-49 years and over 70 years. Mean adequacy ratio(MAR) was 0.9 in PC, and decreased with increase of age. Therefore MAR did not reach to 0.7 among subjects over 70 years. Index of nutritional quality(INQ) for calcium, iron, vitamin A, and vitamin B was below 1 in most age groups. From these results, major nutritional problems of each age group were overall inadequate intakes of calcium and iron, excess consumption of protein and fat for PC, EC, HS, and CS groups, and inadequate nutrients intake of subjects over 70 years old. Comparative Study on Nutrition Attitudes, Food Purchase Behaviors, and Dietary Habits of Housewives Living in Seoul and Kyunggi Area (주부들의 영양태도, 식품구매 및 식생활 평가의 차이에 관한 비교 연구 -서울특별시와 경기도 일부 지역에 거주-)

  • This study focused on the nutrition knowledge, food purchase behaviors, and dietary habits of 213 housewives (41 women under 39 years, 98 women 40~49 years, and 74 women over 50 years). Younger housewives held more special jobs, whereas older housewives worked as housewives or had no jobs ($p$<0.05). The average monthly income of the three family groups was 3,000,000~5,000,000 won, and the monthly dietary expenditure for all three groups was 200,000~500,000 won. Familial frames for the three groups was nuclear family (over 80%), but housewives younger than 39 years (19.5%) lived together with their parents and let them take care of their children. Housewives (over 50 years) prepared a balanced diet for every meal, utilized less drastic cooking methods to prevent loss of nutrients, and shopped at many places in order to save money on various food materials. Further, they purchased food materials that resulted in the least amount of left overs. On the other hand, housewives under 39 years visited large supermarkets and purchased cooked foods more than other groups. Housewives under 39 years considered nutrients content as the most important factor when purchasing food materials, whereas 74.5% of housewives between 40~49 years and 51.4% of housewives over 50 years put the most importance on flavor ($p$<0.05). Housewives under 39 years were interested in manufacture date, nutrient content, and food price, in decreasing order, whereas housewives between 40~49 years and over 50 years were interested in manufacture date, food prices, and nutrient content. Older housewives were interested in brown rice and fruits, whereas younger housewives under 39 years were interested in meat, fish, eggs, fried foods, and fast food. Older housewives over 50 years did not consume fried foods more than two times per week and consumed three regular meals more often than younger housewives. Accordingly, for younger housewives, it is necessary that healthy foods be cooked for their families.

Seral Changes in Environmental Factors and Recovery of Soil Fertility during Abandoned Field Succession after Shifting Cultivation (회전 후 묵밭의 식생 천이 진행에 따른 환경요인의 변화와 토양 비옥도의 회복)

  • Seral changes in environmental factors and recovery of soil fertility during abandoned field succession after shifting cultivation were investigated in eastern Kangwon-Do, Korea. Relative light intensity of herb and shrub layer decreased gradually until 50 years and increased slightly thereafter. Amount of litter and nutrients derived from it were depicted as a parabola form showing the gradual increment during the first 50 years and slight decrease thereafter. Organic matter, pH value, total-N and Mg of soil were plotted as an early depletion-mid pinnacle form showing the extrems depletion during the first 10 years, abrupt increase in about 20 or 50 years and gradual decrease thereafter. Ca, Al, Mn and Na of soil were depicted as a pinnacle form showing the peak in about 20 or 50 years. Thickness and field capacity of soil increased gradually, but K and total-P did not show any tendency as succession proceeded. The soil fertility, overall capacity of soil nutrients and water for plant growth, was plotted as the early depletion-mid pinnacle form.

Influencing Factors on Osteopenia and Osteoporosis in Korean Aged 50 Years and Above (우리나라 50세 이상 성인의 골감소증과 골다공증 영향요인)

  • Purpose: The purpose of this study was to identify the influencing factors on osteopenia and osteoporosis in Korean aged 50 years and above. Methods: Data from the Fifth Korean National Health and Nutrition Examination Survey (KNHANES V-1, 2010) were obtained and 2,773 subjects who got bone density test and were classified "normal, osteopenia or osteoporosis" were selected. Data were analyzed $x^2$ test, ANOVA, $Scheff{\acute{e}}$ test and logistic regression using the SAS 9.4. Results: Gender, age, days of flexibility exercise, vitamin D in blood and weight were found to be influencing factors for osteoporosis in Korean aged 50 years and above. Gender. age and smoking were found to be influencing factors for osteopenia in Korean aged 50 years and above. Conclusion: This study suggested that nursing intervention should include exercise promotion, intake vitamin D, smoking cessation and weight control program in order to reduce the prevalence of osteoporosis and osteopenia.

Relationship among Pack-Years of Smoking, Metabolic Biomarkers, and Diet Quality in Male Adults: From the Korean National Health and Nutrition Examination Surveys, 2007~2009 (성인 남성에서 흡연력이 건강 관련 대사적 지표 및 식사의 질에 미치는 영향: 2007~2009 국민건강영양조사 자료를 바탕으로)

  • The purpose of this study was to investigate the relationship between pack-years of smoking, metabolic biomarkers, and diet quality in male adults. We analyzed data from the combined 2007-2009 Korean National Health and Nutrition Examination Survey (KNHANES). The extent of smoking was quantified in pack-years (py), with 1 py equivalent to 20 cigarettes per day for 1 year. Subjects (n=1,318) were recruited and divided into two groups according to age (30~49 years age group, n=695; ${\geq}50$ years age group, n=623). In ach age group, subjects were divided into three groups according to the extent of smoking, which included non-smokers (30~49 years, n=156; ${\geq}50$ years, n=217), light smoker (30~49 years, n=269; py<16; ${\geq}50$ years, n=189, py<31), and moderate smoker (30~49 years, n=270; $py{\geq}16$; ${\geq}50$ years, n=217, $py{\geq}31$). The Nutrient Adequacy Ratio (NAR) and the number of foods (Dietary Variety Score, DVS) were analyzed by using the data from the 24-recall method. The results showed that moderate smokers were significantly more likely to drink alcohol compared with the other two groups. Triglyceride and cholesterol levels were significantly higher in smoker groups. The non-smoker group consumed significantly higher levels of vitamin C and potassium compared to the smoker groups. In the diet quality, vitamin C NAR of non-smoker group was significantly higher than those of smoker groups. In the ${\geq}50$ age group, the MAR of nonsmoker, light, and moderate smoker groups were 0.81, 0.78, and 0.78 respectively. The smoker group consumed significantly greate sugar and oils compared to the non-smoker group. Py showed a significantly positive relationship with serum triglyceride level, and a negative relationship with MAR after adjusting for potential confounding factors. In conclusion, male moderate smokers take too little vitamin C, partly due to low diet quality. Therefore, a well-planed diet must be used to replace the nutrients lost from excluded food groups in smoker.
