PLA/PGA 차폐막과 실크 피브로인 차폐막을 이용한 발치와의 골유도재생술의 비교연구 (Clinical study of guided bone regeneration of extracted socket with PLA/PGA membrane and silk fibroin membrane)

  • Purpose: This study was designed to compare the bond regeneratiom effects of treatment using silk fibroin membrane ( Nanogide-S$^{(R)}$ ) resorbable barrier with control group treated by polyactic acid / polylacticglycolic acid membrane(Biomesh$^{(R)}$ ) Methods: 44 severe bone loss on extraction socket from 44 patients were used in this study. In experimental group 22 sites of them were treated by silk fibrin membrane as and the other 22 sites were treated by polyactic acid/ polylacticglycolic acid membrane as a control group. Clinical parameters including recovered bone width, length and radiographic parameter of vertical length were evlauated at base line and 3 months after surgery. Results: 1) Severe bone width, length was significantlly decreased in two group. 2) Bone width, length was significantlly decreased in two group. 3) Decreased bone width, length and radiographic examination differences between group. Conclusions: On the basis of these results, silk fibrin resorbable membrane has similar bone regeneration ability to polyactic acid / polylacticglycolic acid membrane in guided bone regeneration for severe bone loss defect on extraction socket.

  • Digital substraction technique and computer-assisted densitometirc analysis detect minor change in bone density and thus increase the diagnostic accuracy. This advantage as well as high sensitivity and objectivity which precludes human bias have drawn interest in radiologic research area. The objectives of this study are to verify if Radiographic density can be recognized in linear pattern when density profile of standard periapical radiograph with the aluminium stepwedge as the reference, was investigated under varies circumstances which can be encountered in clinical situations, and in addition to that to obtain mutual relationship between the existing standard radiographic system, and future digital image systems, by confirming the corelationship between the standard radiograph and Digora system which is a digital image system currently being used. In order to make quantitative analysis of the bone tissue, digital image system which uses high resolution automatic slide scanner as an input device, and Digora system were compared and analyzed using multifunctional program, Brain3dsp. The following conclusions were obtained. 1. Under common clinical situation that is 70kVp, 0.2 sec., and focal distance 10cm, Al-Equivalent image equation was found to be Y=11.21X+46.62 $r^2=0.9898$ in standard radiographic system, and Y=12.68X+74.59, $r^2=0.9528$ in Digora system, and linear relation was confirmed in both the systems. 2. In standard radiographic system, when all conditions were maintained the same except for the condition of developing solution, Al-Equivalent image equation was Y=10.07X+41.64, $r^2=0.9861$ which shows high corelationship. 3. When all conditions were maintained the same except for the Kilovoltage peak, linear relationship was still maintained under 60kVp, and Al-Equivalent image equation was Y=14.60X+68.86, $r^2=0.9886$ in the standard radiograhic system, and Y=13.90X+80.68, $r^2=0.9238$ in Digora system. 4. When all conditions were maintained the same except for the exposure time which was varied from 0.01 sec. to 0.8 sec., Al-Equivalent image equation was found to be linear in both the standard radiographic system and Digora system. The R-square was distributed from 0.9188 to 0.9900, and in general, standard radiographic system showed higher R-square than Digora system. 5. When all conditions were maintained the same except for the focal distance which was varied from 5cm to 30cm, Al-Equivalent image equation was found to be linear in both the standard radiographic system and Digora system. The R-square was distributed from 0.9463 to 0.9925, and the standard radiographic system had the tendency to show higher R-square in shorter focal distances.

다양한 골 이식재를 이용한 골유도재생술의 골 치유에 관한 비교 연구: 예비연구 (THE COMPARATIVE STUDY OF GUIDED BONE REGENERATION USING VARIOUS OF BONE GRAFT MATERIALS)

  • The purpose of this study is to evaluate the superficial bony healing after guided bony regeneration using a various bone grafts. Four types of bone grafts were performed by one oral and maxillofacial surgeon to restore the defects around endosseous implants. Group I included the allografts using $Regenaform^{(R)}$. Group 2 included the autograft. Group 3 included the combined grafts using with autogenous symphysis bone and xenograft($BioOss^{(R)}$). Group 4 included the xenograft($BioOss^{(R)}$). After some heling period, superficial bone biopsy was performed with the surgical blade(#15) during the second surgery. Histologic and histomorphmetric examination were carried out by one pathologist. There was the most new bone formation in the group 3, next group 2. However, there were no statistically significant differences. All group except for group 4 showed favorable bone formation and remodeling.

상악에 식립된 Frialit-2 임플란트의 성공률에 대한 후향적 연구 (Retrospective analysis of $frialit-2^{(R)}$ implant system placed in maxilla)

  • Objectives Aim of this study was to evaluate the clinical use and the efficacy of Frialit-2 implant system. Experimental Methods Fifty nine patients received placement of Frilalit-2 implants(137 implants) in their maxillary anterior and posterior sites(40 and 97 implants). Intraoral & clinical examination, chart review and radiographs were taken from each patient. Results 1. The total implant survival rate was 92.7% after a mean follow-up period of 19.9 months. 2. The implant survival rate placed in anterior region was 97.5%. 3. The implant survival rate placed in posterior region was 90.7%. 4. The implant survival rate placed in atrophic posterior maxilla with advanced technique (GBR, Sinus elevation) was 87.2%. 5. The implant survival rate placed in type N(D4) bone was 82%, while 95.7% in type III (D3), and 100% in type II(D2) bone. 6. Most of the failed implants(7 of 10) were removed during the maintenance stage after prosthodontic treatment. Conclusion It was concluded that Frialit-2 implant could be used satisfactorily in the esthetic anterior region, but the use in the posterior region, especially with poor bone quality and quantity, further studies are needed.

  • The aim of this study was to achieve healing of Peri-implantitis defects and hard tissue augmentation using a bovine-derived bone mineral on the defect site. Two patients were treated with the surgical approach. With a full muco-periosteal flap elevation, the implant surfaces were exposed and granulation tissue removed around the implant and between the threads. Each surface of the contaminated implant was prepared with the air-abrasive device(PerioFlow$^{(R)}$) for decontamination. Bovine-derived bone mineral(Bio-Oss collagen$^{(R)}$) was then used to fill the defects and muco-periosteal flaps sutured to achieve transmucosal healing. Radiographs and clinical photographs were taken before and after 6 months of healing and an estimate of bone fill was assessed. Within the limits of the present case report, a surgical approach in treatment of peri-implantitis defects using a collagen form of bovine bone mineral was visited. Although limited, the two cases showed the stability and biocompatibility of a bovine-derived bone mineral and effectiveness of air-abrasive device(PerioFlow$^{(R)}$) as a decontamination method.

임플란트 환자의 분포 및 식립부 유형 (The Distribution of Implant Patients and the Type of Implant Site)

  • Nowdays, the awareness of implant treatment has grown rapidly among dentists and patients alike in Korea, as it becomes a widely accepted treatment. The reason is that unlike crown and bridge or denture treatment, implant treatment helps preserve existing bone and improve masticatory functions. So, It is needed understanding about the type, distribution of implant patient. The following results on patient type and implant distribution were compiled from 4433 implant cases of 1596 patients treated at the periodontal dept. of Y University Hospital during 1992 to 2004. 1. There are no dissimilarities between men and women, with patients in their 40, 50s accounting for 52.5% of patients and 57.5% of implant treatments; the largest share of patients and implant treatments. 2. Mn. posterior area accounted for 54.9% of implant treatments followed by Mx. posterior area(27.6%), Mx anterior area(11.9%) and Mn anterior area(5.6%). 3. Partial edentulous patients treated by single crown and bridge-type prosthesis accounted for 97.5% and fully edentulous patient accounted for the remaining 2.5%. 4. The major cause of tooth loss is periodontal disease, followed by dental caries, trauma and congenital missing. Also, older people are more likely to suffer from tooth loss due to periodontal disease rather than dental caries. 5. In the distribution of bone quality for maxillae, type III was most, followed by type II, r type IV and r type I. As for mandible, type II was most, followed by type III, type IV and for type I. 6. In the distribution of bone quantity for maxillae, type C was most, followed by type B, type D, type A, and for type E. As for mandible, type B was 52% most, followed by type C, type D, type A and type E. 7. The majority of implants were those of 1O-14mm in length (85.2%) and regular diameter in width (64%). The results provided us with basic data on patient type, implant distribution, bone condition, etc. We wish that our results coupled with other research data helps assist in the further study for better implant success/survival rates, etc.

Effect of gamma ray irradiation and ethyl methane sulphonate on in vitro mutagenesis of Citrullus colocynthis (L.) Schrad

  • In the present study in vitro mutagenesis was used to study the effect of gamma irradiation and EMS on callus induction, morphogenesis and production of multiple shoots from different explants of Citrullus colocynthis (L.) Schrad. Gamma radiations (5 kR to 20 kR) and certain chemicals have been effected on plant growth developments and changes of biochemical metabolisms in plants. Murashige and Skoog (MS) medium containing with auxins such as NAA, IAA, 2,4-D (0.5 ~ 2.0 mg/l), cytokinines BAP, kn TDZ, (0.5 ~ 2.5 mg/l), L-Glutamic acid (1 ~ 2 mg/l) and Coconut milk (10 ~ 20%). After 5 weeks on induction media, explants and callus (EC) were exposed to 5 kR, 10 kR, 15 kR and 20kR, of gamma radiation and treated with 1, 2, 3, 4 and 5 mM ethyl methane sulphonate (EMS) for 30 min. The highest percentage of callusing was observed (70%) stem irradiated with 5 kR and significantly decrease in fresh and dry weight of callus in the below 4 kR doses and above 20 kR doses, there was a progressive decrease in the fresh weight and dry weights when compared to control callus. Maximum percentage of plantlet regeneration (59%) was induced from callus exposed to 15 kR gamma irradiation on MS media fortified with 2.0 mg/l 2,4-D + 2.0 mg/l BAP + 2.0 mg/l L-glutamic acid. Increase in gamma irradiation dose above 15 kR and 5 mM EMS reduced regeneration capacity of callus. Doses higher than 20 kR and 7 mM EMS was lethal to micropropagated plants of Citurullus colocynthis.

  • The purpose of this study was to evaluate new bone formation and healing process in rat calvarial bone defects using $BioMesh^{(R)}$. membrane and DFDB. Forty eight rats divided equally into 4 groups of 1 control group and 3 experimental groups. Standardized transosseous circular calvarial defects (8 mm in diameter) were made midparietally. In the control group, the defect was only covered with the soft tissue flap. In the experimental group 1, it was filled with DFDB only, in the experimental group 2, it was covered $BioMesh^{(R)}$. membrane only, and in the experimental group 3, it was filled DFDB and covered with membrane. At the postoperative 1, 2, 4, 8 weeks, rats were sacrificed and histologic and histomorphometric analysis were performed. These results were as follows. In histomorphometric analysis, It showed the greatest amount of new bone formation through experimental in the experimental group 3 (P<0.001). The amount of new bone formation at the central portion of the defect was greater in the experimental group 3 than experimental group 2. $BioMesh^{(R)}$. membrane began to resorb at 1 week and resorbed almost completely at 8 weeks after operation. The collapse of membrane into the defect was observed through the experimental periods in the experimental group 2. In the area of collapsed membrane, new bone formation was restricted. These results suggest that maintenance of some space for new bone to grow is required in the use of $BioMesh^{(R)}$. membrane alone in the defect. It is also thought that use of the membrane may promote new bone growth in DFDB graft.

잔소리졸 함유 껌의 치태 및 치은염 억제 효과 (Effect of Curcuma Xanthorrhiza containing gum on reduction of the plaque and the gingivitis)

  • The purpose of the present study was to determine the effect of a chewing gum containing 70% xylitol and 0.23% Curcuma xanthorrhiza extract to remove dental plaque and reduce gingivitis when used as a supplement to daily toothbrushing for 3 weeks. The study group consisted of 75 adults with moderate gingvitis. Participants were divided into 3 groups(contol 1 group - Gum base, contol 2 group - 70% Xylitol, experiment group - 70% xylitol and 0.23% xanthorrhizol) and instructed to chew the study gum for 3 times daily for 3 weeks in addition to regular daily toothbrushing. Chewing xylitol/xanthorrhizol gum significantly reduced plaque index with significant difference by week 3(p<0.01). Gingival index and bleeding on probing were decreased in xy/ Curcuma xanthorrhiza extract group by 35.9% and 31.65% each in the same period. No adverse effects on the oral tissues were observed in any of the participants for the duration of the study. In conclusion, regular use of a chewing gum containing 70% xylitol and 0.23% Curcuma xanthorrhiza extract appears safe and effective for the removal of dental plaque and reduction of gingivitis when used in conjunction with daily toothbrushing.

  • 이 연구의 목적은 성견의 발치와에 식립된 implants에 새로운 골 형성을 유도할 목적으로 사용한 흡수성막의 골유도재생술을 평가하고자 하였다. 성견의 제 3소구치를 발거하고 cylindrial HA-coated implants를 식립하였다. 실험군에는 흡수성막인 $GUIDOR^{(R)}$ 를 덮어 보호를 하고, 대조군은 막으로 보호하지 않았다. 성견은 1, 2, 4개월에 차례로 희생하였으며, 치유과정 및 골형성 정도의 평가를 위해 임상적, 방사선학적, 그리고 조직학적 관찰을 통한 다음과 같은 결론을 얻었다. 1. 치유리간 동안에 실험군의 흡수성막의 안정성과 조직의 만족할 만한 치유능력을 볼 수 있었다. 2. 술후 4개월 정도에서 막이 완전한 흡수를 볼 수 있었다. 3. 미숙골이 정상적인 충판의 성숙골로 바뀌는데 약 4개월의 시간이 필요하였다. 4. 대조군에 비하여 실험군에서 더 많은 골형성을 관찰할 수 있었다. 5. 흡수성막이 implant 매식에 유용하게 이용될 수 있으리라 평가한다.
