Cut-Through versus Cut-Out: No Easy Way to Predict How Single Lag Screw Design Cephalomedullary Nails Used for Intertrochanteric Hip Fractures Will Fail?

  • Purpose: This study aims to compare patients in whom fixation failure occurred via cut-out (CO) or cut-through (CT) in order to determine patient factors and radiographic parameters that may be predictive of each mechanism. Materials and Methods: This retrospective cohort study includes 18 patients with intertrochanteric (IT) hip fractures (AO/OTA classification 31A1.3) who underwent treatment using a single lag screw design intramedullary nail in whom fixation failure occurred within one year. All patients were reviewed for demographics and radiographic parameters including tip-to-apex distance (TAD), posteromedial calcar continuity, neck-shaft angle, lateral wall thickness, and others. Patients were grouped into cohorts based on the mechanism of failure, either lag screw CO or CT, and a comparison was performed. Results: No differences in demographics, injury details, fracture classifications, or radiographic parameters were observed between CO/CT cohorts. Of note, a similar rate of post-reduction TAD>25 mm (P=0.936) was observed between groups. A higher rate of DEXA (dual energy X-ray absorptiometry) confirmed osteoporosis (25.0% vs. 60.0%) was observed in the CT group, but without significance. Conclusion: The mechanism of CT failure during intramedullary nail fixation of an IT fracture did not show an association with clinical data including patient demographics, reduction accuracy, or radiographic parameters. As reported in previous biomechanical studies, the main predictive factor for patients in whom early failure might occur via the CT effect mechanism may be related to bone quality; however, conduct of larger studies will be required in order to determine whether there is a difference in bone quality.

호흡 동조 체적 세기조절 회전 방사선치료의 유용성 평가와 진폭모드를 이용한 환자적용 (Usefulness of Gated RapidArc Radiation Therapy Patient evaluation and applied with the Amplitude mode)

  • 목 적 : 본 연구는 이미 상용화가 시작된 호흡 동조 체적 세기조절 회전 방사선 치료(Gated RapidArc) 이전의 자동화가 되지 않는 장비들에서 호흡 동조 방사선 치료(Gated radiation therapy)와 체적 세기조절 회전 방사선 치료(VMAT)를 동시에 시행할 수 있게 Gated RapidArc의 정확성을 분석하여 유용성을 평가하고, 진폭모드(Amplitude mode)를 이용하여 Gated RapidArc가 자동으로 되지 않는 장비에 환자를 적용하여 보고자 하였다. 대상 및 방법 : 방사선량 분포의 분석은 물 등가물질 고체 팬톰과 GafChromic 필름(EBT2 QD+, USA)을 이용하였으며, Film QA (ver. 2.2, USA) 필름 분석 프로그램을 이용하여 Gamma 인자(3%, 3 mm)를 분석하였다. 또한, 삼차원 선량 분포의 정확도를 확인하기 위해서 Matrixx(IBA Dosimery, Germany) 선량 측정 장비와 Compass(IBA Dosimetry, Germany) 선량 분석 프로그램을 이용하였다. 고체 팬톰을 이용한 호흡 동조 주기 신호는 4D 팬톰(Dynamic Thorax Phantom, CIRS, USA)과 Varian RPM(Real-Time Position Monitor) 호흡 동조 시스템을 이용하여 만들었으며, 자유호흡(free breathing)과 호흡정지(breath holding) 시에 따른 방사선량 분포를 각각에 대하여 분석 평가하였다. 환자에게 적용하기 위하여 2013년 2월부터 2013년 8월까지 간암환자 4명을 대상으로 4DCT의 영상을 얻기 위하여 충분한 호흡주기 연습후에 환자의 호흡주기에 맞게 위상모드(Phase mode)를 이용하여 환자가 고글의 호흡주기 패턴을 눈으로 보고 정확하게 따라할 수 있도록 하면서 4DCT의 영상을 획득하였다. Gated RapidArc 치료를 위하여 진폭모드(Amplitude mode)의 호흡주기를 만들어 3회 호흡을 시행한 후 40%~60%의 구간에서 5~6초 호흡을 참을 수 있도록 연습을 하고, 치료 시 40%~60%의 구간에서 환자가 숨을 참을 때 Beam On 버튼을 눌러주는 방식의 반자동으로 치료를 시행하였다. 결 과 : 비 호흡 및 호흡 동조 체적 세기조절 회전 방사선 치료기법 간의 절대선량은 전산화 치료 계획을 이용한 계산값과 1% 이내의 차이를 보였으며, 치료 기법 간의 차이 또한 1% 이내의 차이를 보였다. Gamma 인자(3%, 3 mm)는 99% 이상의 일치함을 보였으며, 각 장기별 선량 차이는 대체로 95% 이상의 일치함을 보였다. 또한 호흡 동조 체적 세기조절 회전 방사선 치료를 위하여 만든 진폭모드(Amplitude mode)의 호흡주기와 실제 환자의 호흡주기가 잘 일치하는 것을 볼 수 있었다. 결 론 : 비 호흡 동조와 호흡 동조 시 체적 세기조절 회전 방사선 치료간의 절대 선량 및 방사선량의 분포가 매우 잘 일치함을 보였다. 이는 호흡 동조 체적 세기조절 회전 방사선 치료 기법을 이용하여 호흡에 따라 움직이는 흉부나 복부의 종양 치료에 적용이 가능한 것으로 사료된다. 또한 실제 치료환자를 대상으로 고글을 통하여 진폭모드(Amplitude mode)의 호흡주기를 만들어 Gated RapidArc가 자동으로 되지 않는 장비에 치료를 적용한 결과, 5~6초정도 정지된 호흡에서 호흡 동조 체적 세기조절 회전 방사선 치료가 원활히 이루어짐을 알 수 있었다.

방사선 치료용 고에너지 전자선의 조직 내 선량분포 특성에 관한 연구 (Study on Characteristics of Dose Distribution in Tissue of High Energy Electron Beam for Radiation Therapy)

  • The purpose of this study is directly measure and evaluate about absorbed dose change according to nominal energy and electron cone or medical accelerator on isodose curve, percentage depth dose, contaminated X-ray, inhomogeneous tissue, oblique surface and irradiation on intracavitary that electron beam with high energy distributed in tissue, and it settled standard data of hish energy electron beam treatment, and offer to exactly data for new dote distribution modeling study based on experimental resuls and theory. Electron beam with hish energy of $6{\sim}20$ MeV is used that generated from medical linear accelerator (Clinac 2100C/D, Varian) for the experiment, andwater phantom and Farmer chamber md Markus chamber und for absorbe d dose measurement of electron beam, and standard absorbed dose is calculated by standard measurements of International Atomic Energy Agency(IAEA) TRS 277. Dose analyzer (700i dose distribution analyzer, Wellhofer), film (X-OmatV, Kodak), external cone, intracavitary cone, cork, animal compact bone and air were used for don distribution measurement. As the results of absorbed dose ratio increased while irradiation field was increased, it appeared maximum at some irradiation field size and decreased though irradiation field size was more increased, and it decreased greatly while energy of electron beam was increased, and scattered dose on wall of electron cone was the cause. In percentage depth dose curve of electron beam, Effective depth dose(R80) for nominal energy of 6, 9, 12, 16 and 20 MeV are 1.85, 2.93, 4.07, 5.37 and 6.53 cm respectively, which seems to be one third of electron beam energy (MeV). Contaminated X-ray was generated from interaction between electron beam with high energy and material, and it was about $0.3{\sim}2.3\%$ of maximum dose and increased with increasing energy. Change of depth dose ratio of electron beam was compared with theory by Monte Carlo simulation, and calculation and measured value by Pencil beam model reciprocally, and percentage depth dose and measured value by Pencil beam were agreed almost, however, there were a little lack on build up area and error increased in pendulum and multi treatment since there was no contaminated X-ray part. Percentage depth dose calculated by Monte Carlo simulation appeared to be less from all part except maximum dose area from the curve. The change of percentage depth dose by inhomogeneous tissue, maximum range after penetration the 1 cm bone was moved 1 cm toward to surface then polystyrene phantom. In case of 1 cm and 2 cm cork, it was moved 0.5 cm and 1 cm toward to depth, respectively. In case of air, practical range was extended toward depth without energy loss. Irradiation on intracavitary is using straight and beveled type cones of 2.5, 3.0, 3.5 $cm{\phi}$, and maximum and effective $80\%$ dose depth increases while electron beam energy and size of electron cone increase. In case of contaminated X-ray, as the energy increase, straight type cones were more highly appeared then beveled type. The output factor of intracavitary small field electron cone was $15{\sim}86\%$ of standard external electron cone($15{\times}15cm^2$) and straight type was slightly higher then beveled type.

고선량률 강내조사선원을 이용한 근접조사선량계획전산화 개발 (Development of Dose Planning System for Brachytherapy with High Dose Rate Using Ir-192 Source)

  • 목적 : 고선량률의 Ir-192 선원을 이용한 근접조사의 모의촬영 영상을 개인컴퓨터(PC)에 입력하여 해부학적 영상에 선량분포를 구현하고 히스토그램, 선량-용적히스토그램 및 3차원 선량분포를 전산화하였다. 대상 및 방법 : 선량전산화에 이용된 선원은 원격근접조사장치(Buchler 3K, 독일)의 Co-60 대체선원으로 한국원자력 연구소와 공동으로 개발한 Ir-192이다. 선원 모양에 의존하는 선량분포의 비등방성은 선원을 미소 분할하여 구한 선량과 선원 중심에서 측방 기준점의 공중선량을 기준으로 규격화한 값을 이용하였다. 선원 주위의 조직선량은 선원 중심에서 측방으로 실측된 조직감쇠와 산란에 의한 보정계수와 에너지에 따른 공기 저지능에 대한 조직의 저지능 비로 공중-조직선량 변환계수를 적용하고 기준점에 대해 규격화한 선량률표를 검색하여 얻도록 하였다. 선량계획 전산화 과정에 모의촬영 영상입력, 선원입력과 선원의 축면 결정과 해부학적 영상을 이용한 선량분포와 점선량, 히스토그램 및 선량-용적 히스토그램을 구현하였다. 결과 : 저자들이 개발한 근접조사 선량계획시스템에는 선원모의촬영 영상을 스켄하여 비트맵 파일로 저장하고, 좌표원점과 확대율을 정해, 선원위치를 결정하고 선량분포와 선량분석 프로그램을 포함한 선량전산화를 구현하였다. 실험에 이용된 Ir-192 선원의 조직내 선량은 공중선량율과 조직에 의한 감쇠 및 산란에 의한 실험식을 이용하였다. 선원 중심에서 축상의 거리와 축에서 떨어진 거리에 따른 선량률표에서 행렬 검색하여 얻도록 하였다. 근접조사선량계획은 선원좌표 입력과 선원의 축면(principal plane)을 결정하여 선원이 포함된 평면상의 선량을 구현하였으며, 시뮬레이션 영상인 관상면과 시상면에 선량분포를 구현하였다. 선량-히스토그램에 의한 선량분포 분석은 임의의 해부학적 영상면 위에 커서가 놓인 위치의 선량 스켓치로 얻었다. 임상에 필요한 선량분석은 선원의 축에서 면의깊이를 이동하여 선량분포를 구할 수 있게 하였으며, 선량-용적 히스토그램과 3차원 선량분포를 구현하였다. 결론 : 고선량률 Ir-192를 이용하여 근접조사선량계획을 전산화하였으며, 선량분포의 분석에는 해부학적 영상의 선량분포와 선량-히스토그램, 선량-용적히스토그램을 구현하였으며, 선량분포의 면을 임의 선택할 수 있고 3차원 선량분포를 포함한 선량계획시스템을 준비하였다.

모델링 공정 연계 시스템을 통한 효율적 폴리곤 모델링 기법에 대한 탐구 (A Study on Effective Methods of Polygon Modeling through Modeling Process-Related System)

  • 3D 컴퓨터 애니메이션의 모델링 작업과정은 사실적 형태 구현을 위한 최적의 작업조건에 대한 방법들이 진일보되고 있다. 1999년 출시된 모델링 전문 소프트웨어인 ZBrush는 기존과 다른 용이한 제작방식으로 인해, 캐릭터 모델 작업에 있어서 필수요소로 자리 잡고 있다. 이에, 본 논문에서는 보다 최적화된 애니메이션 캐릭터 모델 제작을 위한 방법으로써 두 개의 소프트웨어 (Z-Brush, Maya)의 연계된 제작시스템에 대한 효율성에 대해 고찰하고, 그 시사점을 제시하고자 한다. 이를 위해, 첫째 폴리곤 모델링과 리토폴로지(retopology)에 대한 기술적 특성에 대해 살펴보았으며, 둘째, 이를 바탕으로, 기존의 캐릭터 모델링을 위한 ZBrush와 Maya의 연계적 작업 과정의 문제점을 분석한다. 셋째, 새로운 모델링 작업의 최적화 기법의 효율성을 검증하기 위해, 단편 애니메이션 영화 'Cula & Mina'의 Dumvee 캐릭터의 제작과정을 사례로 하여 그 제작과정을 분석하였다. 본 연구를 통해, 두 개의 소프트웨어를 이용한 모델링 공정 연계 시스템은 보다 효과적으로 하이 폴리곤(high polygon) 모델에 대한 제작의 용이함과 높은 완성도를 구현할 수 있음을 알 수 있었다. 이를 통해, 기존과 다른 효율적 모델링 방식을 고찰함으로써, 모델링 관련 수업이나 관련 산업체의 제작 시스템의 최적화를 위한 참고자료가 되리라 사료된다. 또한, 본 연구는 폴리곤 모델링 제작과정의 개선을 위한 '실용적 대안 제시'를 목적으로 하는 기술제작 사례논문으로 접근하였다.

Fe93.5Si6.5 자성분말/에폭시 복합재 필름의 고주파 특성 (High Frequency Properties of Fe93.5Si6.5 Magnetic Powder/Epoxy Composite Film)

  • $Fe_{93.5}Si_{6.5}$ 분말/에폭시 복합재 필름은 열경화과정을 이용하여 준비되었다. 자성분말/에폭시 복합재의 구조와 전자기적 특성 및 전자파 흡수특성을 분석하기 위하여 주사전자현미경(scanning electron microscpoe, SEM), 시료진동형 자력계(vibrating sample magnetometer, VSM), 네트워크 어날라이져(network analyzer) 등을 이용하였다. 분석결과, 포화자속밀도는 복합재 내의 $Fe_{93.5}Si_{6.5}$ 분말이 차지하는 양에 의존하며, 이는 초기투자율에 영향을 미친다. 결과적으로 1 GHz 이상의 주파수에서는 와전류 손실(eddy current loss)이 주요한 인자이며, 자성분말/에폭시 복합재의 공명주파수(resonance frequency)는 복합재 내의 $Fe_{93.5}Si_{6.5}$ 분말의 양이 증가함에 따라 감소한다. 반사손실(reflection loss)은 자성분말/에폭시 복합재의 투자율(permeability)과 유전율(permittivity)로부터 계산에 의해 구해진다. 50 wt% $Fe_{93.5}Si_{6.5}$ 분말의 양과 5 mm 두께를 가진 자성분말/에폭시 복합재는 3.66 GHz와 4.16 GHz 사이에서 -20 dB 이하의 값을 보인다. 따라서 Fe-Si/에폭시 박형 복합재는 마이크로파 흡수체로서 좋은 후보물질이 될 수 있을 것으로 판단된다.

동적 다엽콜리메이터를 이용한 척수의 부분 차폐 기법 (Spinal Cord Partial Block Technique Using Dynamic MLC)

  • 두경부 및 흥부에서 척수 견딤선량은 방사선 치료 제약 조건중의 하나이다. 이 영역에서 척수 견딤 선량을 임상적으로 허용 가능한 수준으로 유지하면서 표적에 임상적으로 유의한 선량을 전달하는 것은 쉽지 않은 문제이다. 기존의 치료 방법을 적용할 때 따르는 문제를 해결하기 위해 동적 다엽콜리메이터(dynamic multi leaf collimator, dMLC)를 사용한 척수의 부분 차폐 기법(spial cord partial block technique)을 개발하였다. 이 기법은 기존의 3차원 방사선 치료 계획 장치를 사용하여 치료계획을 수립한다. 치료 계획 절차는 표적을 잘 포함 할 수 있는 빔 방향 및 빔 개수를 설정한 후, 척수 등 방사선 민감 장기를 보호하기 위한 부분차폐를 적용하여 최적화된 선량분포를 만들어 낸다. 이 기법의 유용성을 평가하기 위해 동일한 환자에 대해 기존의 치료 계획과 PBT 치료 계획을 수립하여 상호 비교 평가하였다. 평균 선량과 DVH를 치료 계획 평가 인자로 사용하였다. 임상 적용 가능성을 위해 일련의 정도관리를 수행하여 평가하였다. 이 정도관리는 선량 분포 검증에 대한 필름 도시메트리와 단일점 측정으로 구성되어 있다. PBT 치료계획이 기존의 치료계획 보다 좋은 결과를 얻을 수 있었으며, 두경부에서 유용한 치료 기법임을 볼 수 있었다.

CLINAC 1800 선형가속기의 15 MV X-선의 특성 (Characteristics of 15 MV Photon Beam from a Varian Clinac 1800 Dual Energy Linear Accelerator)

  • 국내에서 처음으로 사용되는 CLINAC 1800에서 발생된 15MV X-선의 특성을 구하기 위하여 3 Dimensional water Phantom Dosimetry system)를 이용하여 방사선 치료에 근간이 되는 심부선량 백분율(POD), 최대 조직 비율(TMR), 편평도(beam profile), 대칭도, Wedge인자 등을 측정하였고 선량계산을 위하여 출력 인자들을 구하였다. 1. 선축상 최대치 지점(Dmax)은 SSD 100cm일때 조사면이 $10\times10cm^2$에서 $3.0\pm0.1$ cm이였고 $4\times4cm^2,\;35\times35cm^2$에서 각각 $3.1\pm0.1\;cm,2.2\pm0.1$ cm으로 조사면이 넓어지면서 측정치가 표면에 가까워지는 결과를 보였다. 2. 조직표면 선량(Surface Dose)는 SSD 100cm일때 조사면이 $10\times10cm^2$에서 $15.5\%$이였고 $4\times4cm^2,\;35\times35cm^2$에서 각각 $9.8\%\;,51.2\%$로 조사면이 넓어지면서 표면 선량은 증가하는 결과를 보였다. 3. 심부선량 백분율(PDO)은 SSD 100cm에서 측정하였고 조사면이 $10\times10cm^2$이고 10cm depth에서 $76.8\%$이였고 $80\%,\;50\%$ 선량의 깊이는 각각 $9.1\pm0.1\;cm,19.9\pm0.2\;cm$으로 측정되었다. 4. 최대조직비율(TMR)은 심부선량 백분율(PDD)로부터 계산하였고 측정값과의 차이는 $10\times10cm^2$ 조사면에서 평균 $1\;%$ 이내의 오차를 보였다. 5. 대칭도(symmetry)와 편평도(flatness)는 조사면 $10\times10cm^2$일때 각각 $0.73\%,\;2.72\%$이였다. 6. 출력인자(output factor)는 $10\times10cm^2$ 기준 조사면에서 흡수선량을 1로 하였을때 $4\times4cm^2,\;35\times35cm^2$ 조사면에서는 각각 0.927, 1.087로 측정되었는데 조사면이 증가할수록 흡수량이 증가하는 결과를 보였다. 7. Wedge factor는 $15^{\circ}\;30^{\circ}\;45^{\circ}\;60^{\circ}$를 10cm깊이에서 측정하였는데 0.825, 0.099, 0.560, 0.457로 각각 측정되었고 아크릴 0.4 mm Tray의 투과율은 0.976이였다. 8. 15 MV X-선에 의한 납벽층의 반가층 두께는 13 mm였고 Cerrobend의 반가층은 15.5 mm으로 측정되었다.

High Resolution HC$_3$N Observations toward the Central Region of Sagittarius B2

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500,550,570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500,550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100,300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers.
