Production and Assessing Usefulness of the Moving Phantom for Respiration Gated Radiotherapy (호흡동조 방사선치료용 팬텀의 제작 및 유용성 평가)

  • Purpose: The purpose of this study is that through production of phantom for respiration gated radiotherapy, assessing appropriacy of exposure dose for the therapy using RPM (Real-time Position Management). Materials and Methods: We located measurement object on the phantom for respiration gated radiotherapy made of 2 linear actuator, acrylic panel, stanchion, iron plate ets. to drive (up, down, front, back). Using 4D CT scan, we analyzed patient's respiration and reproduced the movement by computer. On the phantom, we located a 2D-Array (PTW) and an White water phantom (4.5 cm) and used DMLC (interval 2 cm) in the field size $10{\times}10\;cm$, then exposed 21EX X-ray 100 MU, in the case of phantom was (1) static (2) moving (3) gated using RPM respectively gantry $0^{\circ}$ and $90^{\circ}$ We measured with a 0.125 CC ionization chamber (PTW) on the phantom (7.5 cm) in the same condition. Results: Ionization chamber: There were within 0.3% of error with gating respiration and approximately 2% of error without gating in the same condition. 2D-Array: Gantry $90^{\circ}$, field size $10{\times}10\;cm$, using DMLC. There were within 3% of error with gating respiration and approximately 16% of error without gating. Conclusion: The phantom for respiration gated radiotherapy makes plans considering patient's movement, quantitative analysis of exposure dose and proper assessment therapy for IMRT patients using RPM possible.

Effects of Single or Mixed Supplementation of Bacterial Phytase and Fungal Phytase on Laying Performance and Nutrient Digestibility (Bacterial 및 Fungal Phytase의 단일 및 혼합 급여가 산란계의 생산성과 영양소 이용에 미치는 영향)

  • This study was conducted to investigate the effect of single or mixed supplementation of bacterial and fungal phytase using 45-wk-old 450 Hy-Line Brown laying hens housed in individual cages for 12-wk period. The birds were reallocated to have similar egg productivity by examining the egg production for one wk before starting the experiment. Two sources of phytase, bacterial (BP) and fungal (FP), were used either in single or mixture to determine the effects of these phytase. Five dietary treatments consisted of control (BP 0, FP 0), T1 (BP 300, FP 0), T2 (BP 300, FP 300), T3 (BP 300, FP 3000), and T4 (BP 0, FP 3000). The DPU was used for phytase activity in this experiment. The nonphytate phosphorus (NPP) content of control was 0.30%, and those of phytase treatments were set to 60% of the Control. Experimental diets were fed ad libitum throughout the experimental period. The lighting schedule of 17L7D was employed. The egg production was not different between control and bacterial phytase treatments, but the T4 showed significantly low productivity compared to control (P<0.05). No difference was found in average egg weight among all treatments. The daily egg mass did not show any statistical differences among all treatments: however, it was significantly low in T4 compared to ther control during the latter half of the experiment (P<0.05). No significant difference was found among treatments in terms of feed intake, feed conversion and egg quality. The digestibilities of dry matter, crude protein, and fat digestibility were similar regardless of the treatments. No significant trends were detected in Ca and P availability. In conclusion, the BP level of 300 DPU contributed to achieve 40% reduction of recommended nonphytate phosphorus addition. The synergistic effect of bacterial and fungal phytase was not confirmed.

Environmental Characteristics and Vegetation of the Natural Habitats of Korean Endemic Plant Eranthis byunsanensis B.Y. Sun (한국 특산식물 변산바람꽃 자생지의 환경 특성과 식생)

  • Environmental characteristics and vegetation of the natural habitats of Eranthis byunsanensis B.Y. Sun were investigated in order to provide the basic data for conservation, restoration, and utilization of this Korean endemic plant. This study was conducted in Anyang, Byeonsan, Geoje, Gyeongju, Jeju, Ulsan and Yeosu. E. byunsanensis was distributed around the altitudes of 84~585 m with a slope degree of $10{\sim}20^{\circ}$, and mostly formed discontinuous populations in north-east part of valleys. Soil analysis showed the mean organic matter of 9.6% and a slightly acidic pH (mean pH of 4.9). The mean gravimetric water content was 16.5%. Correlation coefficients between environmental factors and community characteristics suggested that there was a positive correlation between slope degree and soil water content, between slope degree and soil pH, between soil organic matter and importance value, and between species richness or evenness and species diversity. The vascular plants from 59 quadrats of 7 habitats were identified into 144 taxa. A few species were dominants and similarly distributed in Byeonsan, Jeju, Ulsan and Yeosu. The highest species diversity was found in Geoje (1.43), while Anyang showed the lowest (0.87). Species evenness of Gyeongju and Jeju was bigger than 0.8, but that of Geoje was the lowest (0.59). Dominant species of woody plants in and around the 59 plots were represented by high frequency of Acer pictum subsp. mono, Carpinus cordata, Lindera obtusiloba, and Carpinus laxiflora. The results of this study can provide useful data for conservation and restoration of natural habitats of Korean endemic Eranthis byunsanensis and for the development and growth of this species for ornamental purposes.

Percutaneous Mini-open Reduction for Mason II or III Radial Head and Neck Fracture (요골 두 및 경부 골절의 경피적 도수 정복술)

  • Purpose: We wanted to evaluate the result of percutaneous, mini-open reduction for the treatment of Mason II or III radial head and neck fractures. Materials and Methods: 13 patients (8 male, 5 female) with Mason II or III radial head and neck fractures were treated by 1 cm percutaneous mini-open reduction under fluoroscopy. The average age of our subjects was 29 years. Follow up duration was 18 months. Results: Union was noted in all cases. Mean radial neck angulation was decreased from 33.2 degrees to 7.8 degrees. The mean change in angulation between the immediate post-operative and last follow-up was 0.7 degrees. The mean range of motion at the elbow joint was at last follow up, 133.1 degrees in flexion, 7.3 degrees in extension, 80 degrees in pronation and 84.3 degrees in supination. Postoperatively, mean Mayo Elbow Performance Index, American Shoulder and Elbow Surgeons elbow score, and Disabilities of the Arm, Shoulder and Hand score were 96.2, 97 and 1.2. Temporary posterior interosseous nerve palsy (1 case) and minimal cubitus valgus deformity (1 case) were noted. Conclusion: Selected Mason II or III radial head and neck fractures can be treated satisfactorily with percutaneous mini-open reduction.

Combined Treatment of Residual, Recurrent and Unresectable Gastric Cancer (수술후 잔존 위암, 재발성 위암 및 절제 불가능한 위암의 병용 요법)

  • A series of 25 patients with residual, recurrent, and unresectable gastric cancer received various combination of surgery, radiotherapy (RT), chemotherapy (CT), and hyperthermia (HT). They were placed into 7 categories; 1) CT and HT-14 patients; 2) RT and HT-15 patients; 3) surgery, RT and HT-2 patients; 4) surgery, RT, HT and CT-1 patient; 5) RT, HT and CT -1 patient; 6) RT and CT-1 patient; 7) RT alone-1 patient. Three patients had curative resection. 21 patients received irradiation with tightly contoured portals to spare as much small bowel, kidney and marrow as possible. Hyperthermia was applied regionally once or twice a week for 23 patients using 8 MHz radiofrequency capacitive heating device (Thermotron RF-8). HT was given approximately 30 min after RT 7 patients were treated with CT: 4 patients received HT and concomitant Mitomycin-C; 3 patients received HT and sequential 5-FU+Adriamycin+Mitomycin-C. There was not any treatment related deaths. There was also no evidence of treatment related problems with liver, kidney, stomach, or spinal cord except only one case of transient diabetic ketoacidosis. The tumor response was evaluable in 22 patients. None achieved complete remission.11 ($50\%$) achieved partial remission. The response rate was correlated with total radiation dose and achieved maximum temperature. 9 of 14 ($64\%$) received more than 4000 cGy showed partial remission; especially, all 3 patients received more than 5500 cGy achieved partial response.8 of the 12 patients ($67\%$) who achieved maximal temperature more than $41^{\circ}C$ showed partial response in comparing with $25\%$ (2 of 8 patients, below $41^{\circ}C$). The numbers of HT, however, was not correlated with the response. 3 of the 25 patients ($12\%$) remain alive. The one who was surgically unresectable and underwent irradiation alone is in progression of the disease with distant metastases. The remaining two patients with curative resection are alive with free of disease, 24 and 35 months, respectively. The median survival by response are 11.5 months in responders and 4.6 months in non-responders.

Radical Radiotherapy for Carcinoma of the Prostate (전립선암의 근치적 방사선치료)

  • Purpose : To evaluate effect and tolerance of external beam radiotherapy for carcinoma of the prostate and define the optimal radiotherapeutic regimen. Materials and methods : We retrospectively analyzed the records of 60 patients with prostate cancer who were treated with external beam radiotherapy with curative intent in our institution between September, 1987 and March, 2000. Histologic diagnosis was established by transurethral resection or ultrasonography guided biopsy. The major presenting symptoms were a nodule at routine prostatic examination and frequency and urgency of urination, along with dysuria. The median age was 63 years with range of 51 to 87 years. There were 6 patients in Stage A, 20 in Stage 8, 26 in Stage C, and 8 in Stage Dl. All patients were treated with megavoltage equipment producing 10 MV photons. The 4 field pelvic brick technique was used to a dose of 45 Gy or 50.4 Gy at 1.8 Gy per day in 5 to 6 weeks, after which a small boost field was delivered 2.0 Gy per day to a total dose of 66 to 70 Gy. The follow-up period ranged from 1 to 8 years. Results : Actuarial 5-year and 7-year survival rates for Stage A, B, C, and D1 were $100\%\;and\;84\%$, $83\%\;and\;72\%$, $67\%\;and\;54\%$, and $v$, respectively. The corresponding 5-year and 7-year relapse free survival rates were $84\%\;and\;84\%$, $77\%\;and\;67\%$, $48\%\;and\;40\%$, and $33\%\;and\;25\%$, respectively. Relapse free 5-year survival rates for Stage B were $80\%,\;80\%,\;and\;50\%$ for well, moderately, and poorly differentiated tumors, respectively. These were $64\%,\;44\%,\;and\;33\%$ for Stage C, respectively. The local control rates at 5 years were $84\%,\;85\%,\;78\%,\;and\;60\%$ for Stage A, B, C, and D1, respectively. Mild to moderate complications were observed in $22\%$ of patients. Severe complications requiring surgical procedures were documented in only $3\%$ of patients. Conclusion : This study confirms that external beam irradiation is an effective and safe treatment for prostatic cancer, providing long-term local control and good survival with acceptable complications.

Fabrication of Backscatter Electron Cones for Radiation Therapy (산란전자선을 이용한 강내측방조사기구의 제작과 특성)

  • Purpose : Irradiation cones by using backscatter electrons are made for the treatment of superficial small lesions of skin, oral cavity, and rectum where a significant dose gradient and maximum surface dose is desired. Methods and Materials : Backscatter electrons are produced from the primary electron beams from the linear accelerators. The design consists of a cylindrical cone that has a thick circular plate of high atomic number medium (Pb or Cu) attached to the distal end, and the plate can be adjusted the reflected angle. Primary electrons strike the metal plate perpendicularly and produce backscatter electrons that reflect through the lateral hole for treatment. Using film and a parallel plate ion chamber, backscatter electron dose characteristics are measured. Results : The depth dose characteristic of the backscatter electron is very similar to that of the hard x-ray beam that is commonly used for the intracavitary and superficial lesions. The basckscatter electron energy is nearly constant and effectively about 1.5 MeV from the clinical megavoltage beams. The backscatter electron dose rate of $35\~85\;cGy/min$ could be achieved from modern accelerators without any modification. and the depth in water of $50\%$ depth dose from backscatter electron located at 6mm for $45^{\circ}$ angled lead scatter. The beam flatness is dependent on the slit size and the depth of treatment, but is satisfactory to treat small lesions. Conclusions : The measured data for backscatter electron energy, depth dose flatness dose rate and absolute dose indicates that the backscatter electrons are suitable for clinical use.

High-Resolution of Paleoenvironmental Reconstruction and Sea-Level History in Delaware Bay, the East Coast of U.S.A. (미국동부 델라웨어만의 고정밀도 해수면 역사와 고환경 복원)

  • The closely spaced cores were analyzed to find detailed reconstruction of paleoenvironments and sealable changes along the Delaware Bay coast. Three areas, Kitts Hummock Beach mars, the St. Jones River marsh, and Bowers marsh near the St. Jones River's mouth, were chosen because these areas are compose of their own geomorphic characteristics and sea-level history. since significance of the stratigraphic correlations was to determine sedimentary fancies and paleoenvironments, multidisciplinary methods such as lithological description, grain-size analysis, organic/inorganic content, water content, mineral composition, botanical analysis, micropaleontological analysis, and /SUP 14/C datings were performed. Five major divisions of marsh environments were recognized in the stratigraphic sections: freshwater marsh, initialfreshwater marsh, slightly brackish marsh, brackish marsh, and salt marsh. Most of the lower part in the stratigraphic sections show freshwater marsh. On the top of this, either brackish marsh or tidal flat/tidal stream was recorded. The pro-Holocene sediments consist of sand, mud, and sandy mud, The pre-Holocene configuration played an important role for developing the Holocene Paleoenvironmental changes. The irregular configuration of the pre-Holocene sediments consist of sand, mud, and sandy mud. The irregular configuration of the pre-Holocene surface within short distances permitted the concurrent development of variable environments such as freshwater marsh, brackish marsh or salt marsh at similar elevations. The freshwater marsh in this case was formed in the areas of isolation, so saline-water cannot encroach upon these areas. This complex development of paleoenvironments leads to a difficulty in stratigraphic correlation and interpretation of local relative sea-level changes. The deposition of subsurface sediments was affected by sediment supply, compaction, fluvial activity, biological competition, local tectonics and isostacy, climate and local relative sea-level changes. It was interpreted that the positions in the changes from freshwater environments to brackish environments or ice versa are the turning points of transgressions and regressions. Therefore, multiple transgressions and regressions were identified in the stratigraphic sections of the study area.

Efficacy of fumagillin against Telohanellus kitauei Infection of Israel carp, Cyprinus carpio nudus (향어의 장포자충증에 대한 Fumagillin의 예방 및 치료 효과시험)

  • The potential of fumagillin dicyclohexylamine salt to treat and prevent intestinal giant-cystic disease in Israel carp, Cyprinus carpio nudus, was monitored in field experimental studies. In experiment 1 (therapeutic), most fish were already naturally infected with more advanced stage of Relohqnellu: kitnuet. Fumagillin was administered to ash (mean body weiht of 830 g for a Penod of one month at a dose of 10.62 mg in the first group and 5.3 mg in the second group per fi sh per day. In experiment 2 (fprophylactlcl), most flesh also were already naturally infected with an early developmental stage of the protozoa and fish (average body weight of 484 g) were administered fumagillln for 45 days at a dose of 3.95 mg per fish per day. In both experiments, the cumulative mortalities of fish and the extrusion rates of the polar filaments of the spores were significantly decreased in a dose-independent fashion. In experiment 2 no dead fish were observed. No adverse side effects of the drug were observed among fish from any dosage group. In experiment 2, an oval or dot-like concave lesion of most cysts developed at the 7th day and the vegetative form was never observed at the 17th day postmedication and the cysts were grossly reduced in size as compared with the control group, beginning at the 24th day until the end of the study. In contrast, it was scarcely effective to the cysts in experiment 1. Taking the seasonal development of the protozoa into consideration, the above results revealed that oral administration of fumagillin at 3.95 mg/500 g body weight/day for a month Is the optimal dose for the treatment and prevention of thelohanellosls caused by T kitnuei among C. carpio nudus.

Epstein-Barr Virus Antibodies in Korean Mothers and Their Neonates (산모와 신생아의 Epstein Barr Virus 항체가에 관한 연구)

  • Purposes : This study was performed to evaluate the seropositivities and levels of term pregnant women and their neonates, and the transplacental transfer rate of maternal Epstein-Barr Virus-specific IgG(VCA IgG and EBNA IgG) from term pregnant women to their neonates. Subjects and Methods : During Jan. 1, 1997 to Mar. 31. 1997, we collected the 42 pairs of sera from pregnant women and umbilical cord of their neonates in Korea University Ansan Hospital. The serum levels of VCA IgG and EBNA IgG were measured by the ELISA method. Results : 1) The seropositivities of VCA IgG were 100% in mothers and neonates. There was no statistical difference of mean VCA IgG levels between mothers and neonates. There was significant correlation of VCA IgG levels between maternal sera and neonatal umbilical cord sera(correlation coefficient r=0.5214, P<0.001). 2) The seropositivities of EBNA IgG were 100% in mothers and neonates. There was no significant difference of the mean EBNA IgG levels between mothers and neonates. There was significant correlation of EBNA IgG levels between maternal sera and neonatal umbilical cord sera (correlation coefficient r=0.7244, P<0.001). 3) There was no correlation between VCA IgG and EBNA IgG levels of maternal sera. Conclusion : Seropositivities of EBV CA IgG and EBNA IgG of term-pregnant women and their neonates were 100% and no significant differences of antibody levels were found in two groups. It seems that EBV Antibody levels in Korean mothers and neonates were high enough to protect primary EBV infection during early infancy.
