Curative Radiotherapy of Supralottic Cancer (성문상부암의 근치적 방사선치료)

  • Purpose : The purpose of this study was to evaluate the efficacy of curative radiotherapy in the management of supralottic cancer. Materials and Methods : Twenty-one Patients with squamous cell carcinoma of the supralottis were treated with radiotherapy at Gyeongsang National University Hospital between 1590 and 1994. Median follow-up period was 36 months and 95% were observed for at least 2 years. Results : Actuarial survival rate at 5 years was 39.3% for 21 patients. The 5-rear actuarial survival rate was 75.0% in Stage I, 42.9% in Stage II, 33.3% in Stage III and 28.6% in Stage IV(p=0.54), The 5-year local control rate was 52.0% for 21 patients. The 5-rear local control rate was 75.0% in Stage I, 57.1% in Stage II, 66.7% in Stage III, and 28.6% in Stage IV(p=0.33). Double primary cancer was developed in 3 patients and those were all esophageal cancers. Conclusion : In early stage(Stage I and II) supralottic cancer, curative radiotherapy would be a treatment of choice and surgery would be better to be reserved for salvage of radiotherapy failure. In advanced stage(Stage III and IV), radiotherapy alone is inadequate for curative therapy and combination with surgery should be done in operable patients. This report emphasizes the importance of esophagoscopy and esophagogram at the follow-up of patients with supralottic cancer.

A Study on the Stage of Change of Exercise Behavior and Related Factors in Patients with Chronic Arthritis (만성 관절염 환자의 운동행위 변화단계에 따른 관련변인에 관한 연구)

  • Purpose: This study was done to identify the process of change, decisional balance and self-efficacy corresponding to stage of exercise behavior based on the Transtheoretical model in patients with chronic arthritis. Method: The participants were 151 patients with chronic arthritis using an out-patient pain clinic and hospital in C city. The collected data were analyzed using ANOVA, Sheffe test and discriminant analysis with SPSS/WIN. Results: The patients were distributed as follows for each stage of exercise behavior 21.8% in the precontemplation stage, 26.5% in the contemplation stage, 25.2% in the preparation stage, 7.3% in the action stage, and 19.2% in the maintenance stage. Significant effects for stage of change were found for nine of the 10 processes of change and Pros, Cons, self-efficacy. Scores on these variables were found to increase across the stage of exercise behaviors from precontemplation to maintenance. Discriminant analysis revealed that self-efficacy and stimulus control, and reinforcement management were the major correlates of the stage of exercise behaviors. Conclusion: The findings of this study give useful information for exercise intervention, by using strategies of exercise for chronic arthritis patients in different stages of exercise behavior.

Spermatogenesis and its fine structure of the seminiferous epithelium in the Jindo dog (진도견(珍島犬) 정세관상피(精細管上皮)의 정자발생(精子發生)과 미세구조(微細構造))

  • To investigate the cycle and relative frequences and the fine structure of seminiferous epithelia in mature Jindo dogs, histologic study was performed. The results obtained were summarized as follows; 1. Type A spermatogonia appeared approximately 1.6 times as many at stage II as compared to stage I while type In spermatogonia appeared small amount in stage III, IV and V. type B spermatogonia were found during the stage VI to VIII, though not detectable during stage I to V. The type B spermatogonia divided at stage VII to produce the preleptotene primary spermatocytes at stage VIII. The number of primary spermatocytes of the leptotene phase markedly increased during stage I to II, and the primary spermatocytes of the pachytene phase were shown the least in number at stage IV. The secondary spermatocytes could be seen only at stage IV. 2. The relative frequencies of each stage from stages I to VIII of the cycle of seminiferous epithelia were 31.6, 11.9, 10.0, 3.2, 8.2, 10.1, 11.7 and 13.2% respectively. 3. On electron microscopic observations, acrosomal vesicle of spermatids appeared larger though the bulk of germ cells were the morphologically same as those of the other animal species. Thread line structures light microscopically observed in the cytoplasm of Sertoli cell were the longitudinal orientation of mitochondria.

Features Related to Smoking and Psychological Characteristics of Coronary Artery Patients Associated with the Stage of Change in Smoking Cessation (금연변화단계에 따른 관상동맥질환자들의 흡연관련 특성과 심리적 특성)

  • Purpose: The purpose of this study was to investigate differences in smoking related characteristics and psychological features of coronary artery patients by the stages of change in smoking cessation behaviors. Method: Subjects for this survey were 97 patients who were smoking when doctors diagnosed them to have coronary artery diseases. Result: Subjects were distributed 21.6% in precontemplation stage, 24.7% in contemplation stage, 17.5% in preparation stage, 19.6% in action stage, and 16.5% in maintenance stage respectively. The numbers of previous attempts to quit smoking of subjects in precontemplation stage(mean=3.00, SD=3.71) and contemplation stage(mean=2.63, SD=2.32) were significantly lower than that of subjects in preparation stage(mean=5.82, SD=6.20). Benefit scores of subjects in maintenance stage were significantly greater than those of subjects in precontemplation stage. Self-efficacy, barriers, seriousness, and nicotine dependency were not significantly different by the stages of change. Number of signs and symptoms related to smoking which subjects were experiencing were not significant by the stages of change too. Conclusion: Future intervention programs for smoking cessation should be focused on the strategies to enhance the realization of health benefits that patients might acquire from smoking cessation.

Surgical Treatment of Thymoma (흉선종의 외과적 치료)

  • Eighteen patients operated on for thymoma from 1985 to 1994 were evaluated, 3 with myasthenia gravis and 15 without. Masaoka's clinical staging revealed stage I disease in 5(27.8%), stage ll in 6 (33.3%), stage 111 in 6(33.3%), and stage IV in 1 (5.6%). Of the 18 patients, 10 had surgical resection as the only treatment and the remaining 8 (stage II : 1 patient, stage III : 6 patients, stage IV: 1 patient) had surgical resection with adjuvant preoperative or postoperative radiotherapy and/or chemotherapy There was no operative mortality. Currently, 15 patients are alive, 3 are dead. The mean follow up time for all patients was 3.4 years. Acturlal survival at 5 years was 82.2 $\pm$ 9% for all patients' 100% for those in stage I and II, and 62.5% for those in stage III. Pleural and mediastinal recurrence developed in one patient in stage II which was considered to have noninvasive disease on the operating field. Two patients underwent reoperation for recurrence of thymoma a intervals of 14 months and 52 months. Clinical stage and resectability had a significant prognostic value(p < 0.05). The presence of myasthenia gravis is no longer considered as an adverse factor in survival.

  • The purpose of this study is to investigate the housing behavior through the code's housing history. The data were collected by open descriptive answering of 28 students in Y.University. The major findings are as follows: The characteristics of code's housing history were changed by stages of family life. The greastes changes are in the stage of elementary school(3rd stage). Before the 3rd stage, living in out of town, detached house, rental house is to superior than town(Dae Gu). Apt.. own house. And housing adaptation is to superior than housing adjustment. After 3rd stage go by contraies.

  • This paper develops an algorithm for throughput of a 3-stage flow line with job inspection stations, limited buffer capacity, and exponential processing times. Each stage consists of a single workstation, and an infinite number of jobs always waits in front of the first workstation. Blocking may occur when a processed job is waiting at one workstation for another workstation to become available. Numerical example results provide insights into the problems related to quality inspection and measure of performance of flow lines.

Classification for Types of Damages Caused by Cold Stress at Different Young Spike Development Stages of Barley and Wheat (맥류의 유수발육기 저온장해유형과 피해시기 분류)

  • Although the young spike of barley (Hordeum vulgare L.) or wheat (Triticum aestivum L.) is known as the most susceptible part to spring cold injury, the risk of cold injury is apt to be ignored in most breeding program due to the importance of early maturity. Based on these aspects, the types and inducing time, temperature conditions for induction and effects of cold injury on growth and yield in this study were investigated under greenhouse and field conditions through three years (1997-1999). In natural condition, low temperature around -2.4∼$-10.2^{\circ}C$ caused the death of plant. Several cold injury types such as partial degeneration of spike, partial discoloration of leaf, spike and awn, discoloration of culm and white spike were observed at low temperature around $-3.1^{\circ}C$. Low temperature around -2.4∼$-8.6^{\circ}C$ and 1.3-$7.6^{\circ}C$ caused degeneration and sterility of spike, respectively. Most materials were prepared to the spikelet foundation stage, spikelet differentiation stage, development stage of flower organ, booting stage and heading stage, which were known having risk for cold injury in field condition. Although most of the controlled stages were sensitive to the induced low temperature, booting stage was the most sensitive stage for cold injury. All of growth stages which were treated-heading stage, booting stage, development stage of flower organ, spikelet differentiation stage, spikelet foundation stage-were responded to low temperature treatment but the symptoms revealed were very specific according to the growth stages. Ears of plant in heading stage were discolored to white. Ears of plant in booting stage were degenerated in all or part of one. Plants in spikelet differentiation stage were sterile in all or part of one. When tried to detect the specific differences between normal and cold injured plants in appearance, spike length, distance between spike and flag leaf and the first internode length could be the critical points for occurrence of spike death caused by cold injury. In barley, the elongation of spike was stopped on 3.2cm after occurrence of spike degeneration, 4.7cm after occurrence of partial degeneration of spike, 5.0cm after occurrence of white spike. In wheat, it was stopped on 1.6cm after occurrence of stem death, 3.3cm after occurrence of spike degeneration, 8.3cm after occurrence of partial degeneration of spike, 8.1cm after occurrence of white spike, 7.5cm after partial discoloration of leaf and 9.3cm after partial discoloration of spike. The obtained results from low temperature treatment induced in growth chamber were similar to the field experiment, Beacuse the death of spikes was more when low temperature was treated two times than one times, the temperature should be upgrade to -3$^{\circ}C$ in order to get the same condition with field test.

Surgical Outcomes Associated with Operable Gastric Cancer in a Tertiary Care Indian Hospital

  • Purpose: Data on operable gastric cancer from India is sparse. The purpose of this study was to investigate the clinical details, histopathological demographics, and 5-year overall survival (OS) and disease free survival (DFS) associated with operable, non-metastatic gastric cancer in a dedicated upper gastrointestinal (GI) surgical unit in India. Materials and Methods: Data for patients diagnosed with operable gastric cancer between January 2006 and December 2014 were retrospectively analyzed. Data were collected from electronic hospital records in addition to mail and telephonic interviews when possible. Results: A total of 427 patients were included. The tumor was located in the pyloro-antral region in 263 patients (61.7%). Subtotal gastrectomy was performed in 291 patients and total gastrectomy in 136 patients. Tumor stage classification revealed 43 patients (10.0%) with stage I, 40 patients (9.4%) with stage IIA, 59 patients (13.9%) with stage IIB, 76 patients (17.8%) with stage IIIA, 96 patients (22.5%) with stage IIIB, and 113 patients (26.4%) with stage IIIC disease. Follow-up data were available for 71.6% of the patients with a mean duration of 32.4 months. Five-year DFS and OS were 39% and 59%, respectively. Conclusions: Despite presenting at an advanced stage, the 5-year DFS and OS of patients with operable gastric cancer treated at a dedicated upper GI unit of a tertiary care center in India was good.

Surgical Treatment of Primary Lung Cancer and its Long-term Results (원발성 폐암의 외과적 치료 및 장기 성적)

  • We have experienced 120 non-small cell primary carcinomas of the lung between June, 1974 and December, 1984, at Seoul National University Hospital. They were 107 males and 13 females. 95% of all were ranged from 40 years to 69 years of age with 56 years of mean age. They were composed of 70 [66.7%] squamous cell ca., 20 [19%] adenoca., 6 [5.7%] undifferentiated large cell ca., 4 [3.8%] undifferentiated small cell ca., and 5 [4.8%] mixed adenosquamous cell ca. 41 [36%] and 35 [30.7%] patients have received pneumonectomies and lobectomies with a 66.7% resectability rate. Of the 36 stage I and 21 stage II patients, 56 were resectable but only 20 [31.7%] of the 63 stage III patients were resectable. This informed us the significance of the stage of the disease at the time of operation. The actuarial survival rate in 70 patients was as follow: 1, 3, 5 year survival rate of the patients in stage I were 80%, 80%, and 60% respectively. Both 1, 3 year survival rate of patients in stage II were 84%. But 1, 2, 3 year survival rate of patients in stage III were 40%, 11%, and 5% respectively. By dividing the patients in stage III into resectable group and nonresectable one, both 1, 2 year survival rate of the former were 37% and those of the latter were 42% and 7%. According to the cell type of the cancer, 1, 3, 5 year survival rate of the squamous cell ca. were 63%, 40%, and 26% respectively. 1, 3 year survival rate of the adenoca. were 43% and 34%. Hospital death were only 2 cases with a 1.7% operative mortality rate. We had acceptable long-term survival rate and have convinced the necessity and hope of the early detection and resection of the lung carcinoma.
