Efficacy and safety of vitamin D3 B.O.N intramuscular injection in Korean adults with vitamin D deficiency

  • Objective: There has been no prospective study that examined intramuscular injection of high-dose vitamin D in Korean adults. The aim of this study was to assess the efficacy and safety of high-dose vitamin $D_3$ after intramuscular injection in Korean adults with vitamin D deficiency. Method: This study was a 24-week, prospective, multicenter, randomized, double-blind, placebo-controlled trial. A total of 84 subjects ${\geq}19$ and <65 years of age were randomly allocated to either the vitamin $D_3$ or placebo group in a 2:1 ratio. After randomization, a single injection of plain vitamin $D_3$ 200,000 IU or placebo was intramuscularly administered. If serum 25-hydroxyvitamin D (25[OH]D) concentrations were <30 ng/mLon week 12 or thereafter, a repeat injection was administered. Results: After a single intramuscular injection of vitamin $D_3$ to adults with vitamin D deficiency, the proportion of subjects with serum 25(OH)D concentrations ${\geq}30ng/mL$ within 12 weeks was 46.4% in the vitamin $D_3$ group and 3.6% in the placebo group (p<0.0001). The proportion of subjects with serum 25(OH)D concentrations ${\geq}30ng/mL$ within 24 weeks was 73.2% in the vitamin $D_3$ group and 3.6% in the placebo group (p<0.0001). Mean change in serum 25(OH)D concentrations at weeks 12 and 24 after vitamin $D_3$ injection was $12.8{\pm}8.1$ and $21.5{\pm}8.1ng/mL$, respectively, in the vitamin $D_3$ group, with no significant changes in the placebo group. Serum parathyroid hormone concentrations showed a significant decrease in the vitamin $D_3$ group but no change in the placebo group. Conclusion: Intramuscular injection of vitamin $D_3$ 200,000 IU was superior to placebo in terms of its impact on serum 25(OH)D concentrations, and is considered to be safe and effective in Korean adults with vitamin D deficiency.

삶의 질 향상을 위한 비타민 D와 비만과의 관련성에 관한 연구 (The Relationship between Vitamin D and Obesity to Improve Quality of Life)

  • 본 연구의 목적은 비타민 D와 소변 microalbumin / creatinine (uACR)과 한국 성인의 비만 연관성을 평가하는 것이다. 연구 대상자는 2012년 국민 건강 영양 조사에 참여한 20세 이상의 한국 성인 (4,948명)이었다. 공변량을 보정한 공분산 검사는 비타민 D 결핍과 관련하여 uACR 수준에 대해 실시되었다[비타민 D 결핍, 25 (OH) D < 10 ng / dL; 비타민 D 부족, 25(OH) D ≥ 10, < 20 ng / dL; 충분한 비타민 D, 25(OH) D ≥ 20 ng / dL]. 주요 연구 결과는 다음과 같다: 첫째, 비만하지 않은 집단(BMI < 25 kg / m2)에서 uACR 수준은 관련 변수를 조정한 후에 비타민 D 상태의 증가와 함께 감소했다(p < 0.001). 둘째, 비만 인구 (BMI ≥25 kg/m2)에서 uACR 수준과 비타민 D 상태 사이의 연관성은 유의하지 않았다(p = 0.659). 결론적으로, 소변 microalbumin / creatinine 치는 비만이 없는 한국 성인에서 비타민 D 상태와 반비례 관계를 가지나 비만이 있는 한국 성인에서는 그렇지 않았다.

Association between vitamin D intake and bone mineral density in Koreans aged ≥ 50 years: analysis of the 2009 Korea National Health and Nutrition Examination Survey using a newly established vitamin D database

  • BACKGROUND/OBJECTIVES: Vitamin D plays an important role in skeletal growth and maintenance and in the prevention of various diseases. We investigated the relationship between vitamin D intake and bone mineral density (BMD) in Korean adults aged ${\geq}50$ years using the 2009 Korea National Health and Nutrition Examination Survey data. SUBJECTS/METHODS: This study was conducted in 1,808 subjects aged ${\geq}50$ years with BMD data in Korea. Dietary vitamin D levels were assessed by the 24-hour recall method. BMD was measured using dual-energy X-ray absorptiometry. We investigated general characteristics and the association between these characteristics, vitamin D status, and BMD. RESULTS: Vitamin D intake was significantly lower in the osteoporosis group among women (P < 0.05). Among all subjects, the higher the serum 25(OH)D concentration, the higher the whole-body total BMD (WBT-BMD), femoral total hip BMD, and femoral neck BMD (P < 0.01). In the serum vitamin D-deficient group of both the total population and women, serum 25(OH)D concentration was associated with WBT-BMD (P < 0.05). Among women with a calcium intake < 537.74 mg/day, BMD of those with a vitamin D intake > $2.51{\mu}g/day$ (average intake of women) was higher than that of women with a vitamin D intake ${\leq}2.51{\mu}g/day$ (P < 0.001). CONCLUSIONS: Korean adults should increase their BMD by increasing serum 25(OH)D concentration. Furthermore, increasing vitamin D intake could improve BMD, especially in Korean women who consume less calcium than the estimated average requirement.

Vitamin D deficiency in infants aged 1 to 6 months

  • Purpose: The aim of this study was to recognize the state of vitamin D among healthy infants aged 1 to 6 months in South Korea, and also to identify the risk factors affecting the level of vitamin D. Methods: A total of 117 infants were enrolled in this study for 12 months, from March 1, 2011 to February 29, 2012. Serum levels of 25-hydroxyvitamin D (25[OH]D), calcium, phosphorus, and alkaline phosphatase were measured and data including birth weight, body weight, sex, feeding pattern, delivery mode, siblings and maternal age and occupation were collected. Data was mainly analyzed with independent t-test model. Results: We determined that the prevalence of vitamin D deficiency (serum 25[OH]D<20 ng/mL [50 mmol/L]) was 48.7% in the population investigated. Particularly in breastfed infants, the prevalence of vitamin D deficiency was strikingly high (90.4%). The mean serum level of 25(OH)D in breastfed infants was lower than that of formula fed infants (9.35 ng/mL vs. 28.79 ng/mL). Also female infants showed lower mean serum level of 25(OH)D than male. Mean serum values of calcium and phosphorus had positive correlation with vitamin D state (P<0.001). Conclusion: Vitamin D deficiency was found to be very common in infants aged 1 to 6 months in South Korea, and breast feeding was the most critical risk factor of vitamin D deficiency. Therefore we suggest to start vitamin D supplementation in South Korea, as soon as possible, to all infants, including breastfed and female infants.

MALDI-MS-Based Quantitative Analysis of Bioactive Forms of Vitamin D in Biological Samples

  • Analyzing vitamin D levels is important for monitoring health conditions because vitamin D deficiency is associated with various diseases such as rickets, osteomalacia, cardiovascular disorders and some cancers. However, vitamin D concentration in the blood is very low with optimal level of 75 nmol/L, making quantitative analysis difficult. The objective of this study was to develop a highly sensitive analysis method for vitamin D using matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-MS). 25-hydroxyvitamin D (25(OH)D), which has been used as an indicator of vitamin D metabolites in human biofluids was chemically derivatized using a secosteroid signal enhancing tag (SecoSET) with powerful dienophile and permanent positive charge. The SecoSET-derivatized 25(OH)D provided good linearity (R2 > 0.99) and sensitivity (limit of quantitation: 11.3 fmol). Chemical derivatization of deuterated 25-hydroxyvitamin D3 (d6-25(OH)D3) with SecoSET enabled absolute quantitative analysis using MALDI-MS. The highly sensitive method could be successfully applied into monitoring of quantitative changes of bioactive vitamin D metabolites after treatment with ketoconazole to inhibit 1α-hydroxylase reaction related to vitamin D metabolism in human breast cancer cells. Taken together, we developed a MALDI-MS-based platform that could quantitatively analyze vitamin D metabolites from cell products, blood and other biofluids. This platform may be applied to monitor various diseases associated with vitamin D deficiency such as rickets, osteomalacia and breast cancer.

Subclinical Vitamin D Insufficiency in Korean School-aged Children

  • Purpose: Recently, vitamin D insufficiency has increased and has been correlated to growth and puberty in children. This study was conducted to find the prevalence of subclinical vitamin D insufficiency and its influence on school-aged children in Korea. Methods: The subjects of this study were 397 children aged 7 to 15 years who had been tested for 25-OH vitamin D3 among the outpatients of the Department of Pediatrics in Eulji General Hospital from March 2007 to February 2011. Data for age, sex, comorbidities, serum 25-OH vitamin D3, height, weight, body mass index (BMI), and sunlight exposure time were collected before and after 3 months of vitamin D administration, retrospectively. Results: Vitamin D insufficiency was present in 343 (86%) of the subjects. In the vitamin D insufficient group, chronological age was $8.96{\pm}1.72$ years, mean height (z-score [z]) was $0.51{\pm}1.26$, mean BMI (z) was $0.81{\pm}2.20$, and bone age was $10.26{\pm}1.75$ years. In the vitamin D sufficient group, chronological age was $9.61{\pm}1.77$ years, mean height (z) was -$0.66{\pm}0.98$, mean BMI (z) was -$0.01{\pm}1.16$, and bone age was $9.44{\pm}2.12$ years. A paired t-test showed that three months after vitamin D administration, the mean 25-OH vitamin D3 level in the insufficient group increased to $24.38{\pm}10.03$ ng/mL and mean BMI (z) decreased to $0.67{\pm}1.06$. Conclusion: In Korean school-aged children, vitamin D insufficiency were relatively higher and may be closely related with higher BMI. Insufficient rise of the level of vitamin D after supplementation suggest the new supplementation guidelines, especially for Korean children.

Effect of vitamin D deficiency in Korean patients with acute respiratory distress syndrome

  • Background/Aims: Vitamin D modulates innate and adaptive immune responses, and vitamin D deficiency is associated with increased mortality in hospitalized patients with pneumonia. We evaluated the prevalence of vitamin D deficiency in Korean patients with acute respiratory distress syndrome (ARDS) and its effect on the clinical outcomes of ARDS. Methods: We retrospectively analyzed the data of 108 patients who had a measured serum level of 25-hydroxy vitamin D3 ($25(OH)D_3$) at the time of diagnosis with ARDS. The clinical outcomes were evaluated based on $25(OH)D_3$ levels of 20 ng/mL and stratified by quartiles of $25(OH)D_3$ levels. Results: The mean age of patients was 59.4 years old; 77 (71.3%) were male. Vitamin D deficiency was found in 103 patients (95.4%). The mean $25(OH)D_3$ level was $8.3{\pm}7.0ng/mL$. Neither in-hospital mortality (40.0% vs. 68.0%) nor 6-month mortality (40.0% vs. 71.8%) significantly differed between groups. There were no significant differences in $25(OH)D_3$ level between survivors ($8.1{\pm}7.6ng/mL$) and non-survivors ($8.5{\pm}6.8ng/mL$, p = 0.765). There were no trends toward a difference in mortality among quartiles of $25(OH)D_3$ levels. However, $25(OH)D_3$ levels were inversely related with length of hospital stay and intensive care unit stay among in-hospital survivors. Conclusions: Vitamin D deficiency was prevalent in Korean patients with ARDS. However, levels of vitamin D were not associated with mortality. A large, prospective study is needed to evaluate the effects of vitamin D deficiency on clinical outcomes of ARDS.

Iron and vitamin D status in breastfed infants and their mothers

  • Purpose: We assessed the relationships between iron and vitamin D statuses in breastfed infants and their mothers and evaluated the determinants of iron and vitamin D deficiencies in breastfed infants. Methods: Seventy breastfed infants aged 4-24 months and their mothers participated in this study from February 2012 to May 2013. Complete blood counts, total iron binding capacity, and levels of C-reactive protein, iron, ferritin, calcium, phosphate, alkaline phosphatase, and 25-hydroxyvitamin D (25(OH)D) in infants and their mothers were measured. Results: A history of maternal prepregnancy anemia was associated with lower ferritin and 25(OH)D levels in both infants and their mothers. The 25(OH)D level of infants correlated with maternal 25(OH) D levels. The independent risk factors for iron deficiency in breastfed infants were the duration of breastfeeding (odds ratio [OR], 6.54; 95% confidence interval [CI], 1.09-39.2; P=0.04) and infant body weight (OR, 2.65; 95% CI, 1.07-6.56; P=0.04). The determinants for vitamin D deficiency were the infant's age (OR, 0.15; 95% CI, 0.02-0.97; P=0.046) and maternal 25(OH)D level (OR, 0.74; 95% CI, 0.59-0.92; P=0.01). Conclusion: A maternal history of prepregnancy anemia requiring iron therapy was associated with lower current ferritin and 25(OH)D levels in both infants and their mothers. Therefore, physicians should monitor not only iron but also vitamin D levels in infants who are breastfed by mothers who had prepregnancy anemia.

방사면역 측정법에 의한 25-OH-VitD와 25-OH-VitD3의 비교 평가 (Comparative Evaluation of 25-OH-VitD and 25-OH-VitD3 by Radioimmunoassay)

  • 비타민D는 체내에서 칼슘 대사를 조절하여 뼈의 건강을 유지하며, 세포의 증식 및 분화의 조절, 면역 기능 등에 관여하는 중요한 역할을 한다. 부족 시 구루병, 골다공증의 위험이 높아지고 심혈관계 질환, 당뇨병, 일부 암 등의 발병 위험이 증가하는 것으로 보고되고 있다. 특히 한국은 비타민D 부족의 인구 비율이 매우 높은 국가 중 하나이다. 따라서 비타민D 부족의 진단 및 치료를 위해 혈중 25-OH-VitD 또는 25-OH-VitD3의 정확한 측정이 요구된다. 이에 본원에서 시행하고 있는 25-OH-VitD와 25-OH-VitD3검사를 비교 평가하여 비타민D에 대한 정확한 진단 및 치료에 기여하고자 한다. 25-OH-VitD와 25-OH-VitD3의 결과를 전향적으로 측정하여 상관성, 재현성, 모집단 분포율을 구하였다. 또한 2017년 4월에서 2019년 6월까지 서울아산병원 핵의학과 혈액검사실에서 측정한 내부정도관리와 대한핵의학기술학회에서 주관하는 2018년 상반기, 하반기 외부정도관리(기관간 숙련도 평가)에 참여한 결과를 비교하였다. 그 결과 97개의 검체를 대상으로 25-OH-VitD는 25-OH-VitD3 × 0.9 + 0.3 (R >0.9)로 강력한 양의 상관관계를 보였다. 반복 측정을 통한 재현성 평가 결과 평균 diff(%) 값은 7.7%, 7.4%로 모두 우수한 성적을 나타내었다. 또한 모집단 분포를 비교한 결과 통계적으로 유의한 차이를 보이지 않았다(p>0.05). 2017년 4월에서 2019년 6월까지 서울아산병원 핵의학 혈액검사실에서 측정한 내부정도관리 결과값은 평균(CV%) 6.2%, 6.8%을 나타내었다. 2018년 상반기 외부정도관리(기관간 숙련도 평가) 수행도 평가 결과 Z 값이 25-OH-VitD는 저, 중, 고농도에 대해 -1.10, -1.00, -0.90이며 25-OH-VitD3는 0.22, 0.47, 1.10이고 하반기 수행도 평가 결과는 각각 -0.80, -0.30, -0.10과 -0.01, 1.50, 1.30으로 모두 Z ≤ 2.0의 적합한 결과를 얻었다. 방사면역 측정법에 의한 25-OH-VitD와 25-OH-VitD3 검사의 비교 평가 결과 상관성, 재현성, 모집단 분포율, 내부정도관리, 외부정도관리(기관간 숙련도 평가) 모두 우수한 성적을 보였다. 따라서 혈액 내 25-OH-VitD 또는 25-OH-VitD3로 단독 측정이 가능한 방사면역 측정법은 비타민D 부족에 대한 스크리닝 검사로 적합한 것으로 사료된다.

HPLC에 의한 동물성 수산식품 중의 Vitamin $D_3$와 그 관련물질 정량에 관한 연구 (Quantification of Vitamin $D_3$ and Its Derivatives in Marine Animal Foods by HPLC)

  • Contents of vitamin $D_3$ and 25-OH-vitamin $D_3$ in marine animal products(20 species) were determined by HPLC. The isomers of vitamin D, $D_2$ and $D_3$, were not clearly separated on a reversed phase, ${\mu}$ Bonda Pak, with 20% methanol-acetonitrile, and on a normal phase, Zorbax SIL. with 0.4% isopropanol-hexane, but 25-OH-vitamin $D_2$ and-$D_3$ were separated on either ${\mu}$ Bonda Pak with 10% methanol-acetonitrile, or on Zorbax SIL with 2.2% isopropanol-hexane, respectively. Although levels of vitamin $D_3$ and 25-OH-vitamin $D_3$ varied remarkably according to species, their average value(fish : $l,l87{sim}36,007$ I.U/sample 100g, mussel : $58{\sim}1,706$ I.U/sample 100g, pickle: $1,208{\sim}79,358$ I.U/sample 100g) was greatly higher than that of meat($80{\sim}100$ I.U/sample 100g) and dairy products($400{\sim}800$ I.U/sample 100g). Fatty tissues of fish and pickled fish intestines contained high level of vitamin $D_3$ and 25-OH-vitamin $D_3$, while the clam and mussel known to have various kinds of sterol including ${\Delta}^7$-sterol showed very low levels of vitamin $D_3$ and its derivative.
