The Patient Specific QA of IMRT and VMAT Through the AAPM Task Group Report 119 (AAPM TG-119 보고서를 통한 세기조절방사선치료(IMRT)와 부피적세기조절회전치료(VMAT)의 치료 전 환자별 정도관리)

  • The aim of this study was to evaluate the patient specific quality assurance (QA) results of intensity modulated radiation therapy (IMRT) and volumetric modulated arc therapy (VMAT) through the AAPM Task Group Report 119. Using the treatment planning system, both IMRT and VMAT treatment plans were established. The absolute dose and relative dose for the target and OAR were measured by using an ion chamber and the bi-planar diode array, respectively. The plan evaluation was used by the Dose volume histogram (DVH) and the dose verification was implemented by compare the measured value with the calculated value. For the evaluation of plan, in case of prostate, both IMRT and VMAT were closed the goal of target and OARs. In case of H&N and Multi-target, IMRT was not reached the goal of target, but VMAT was reached the goal of target and OARs. In case of C-shape(easy), both were reached the goal of target and OARs. In case of C-shape(hard), both were reached the goal of target but not reached the goal of OARs. For the evaluation of absolute dose, in case of IMRT, the mean of relative error (%) between measured and calculated value was $1.24{\pm}2.06%$ and $1.4{\pm}2.9%$ for target and OAR, respectively. The confidence limits were 3.65% and 4.39% for target and OAR, respectively. In case of VMAT the mean of relative error was $2.06{\pm}0.64%$ and $2.21{\pm}0.74%$ for target and OAR, respectively. The confidence limits were 4.09% and 3.04% for target and OAR, respectively. For the evaluation of relative dose, in case of IMRT, the average percentage of passing gamma criteria (3mm/3%) were $98.3{\pm}1.5%$ and the confidence limits were 3.78%. In case of VMAT, the average percentage were $98.2{\pm}1.1%$ and the confidence limits were 3.95%. We performed IMRT and VMAT patient specific QA using TG-119 based procedure, all analyzed results were satisfied with acceptance criteria based on TG-119. So, the IMRT and VMAT of our institution was confirmed the accuracy.

Dosimetric Comparison of Radiation Treatment Techniques for Breast Cancer : 3D-CRT, IMRT and VMAT (유방암 방사선치료 기법에 따른 선량 비교 : 3차원 입체조형치료, 세기 변조 방사선치료, 입체세기조절회전 방사선치료)

  • The purpose of this study is to compare method in the treatment of breast cancer using dose index. And, it is to find the optimized treatment technique to the patient. The phantom filled with tissue-equivalent material were used simulation and treatment as techniques of 3D-CRT, IMRT, VMAT was planned using Eclipse v10. By using HI(homogeneity index), CI(Conformity index), OED(Organ equivalent dose), EAR(Excess Absolute Risk), were assessed for each treatment plans. HI and CI of 3D-CRT, IMRT, VMAT were calculated 16.89, 11.21, 9.55 and 0.59, 0.61, 0.83. The organ average doses of Lt lung, Rt lung, liver, heart, esophagus, cord, Lt breast, trachea and stomach were 0.01 ~ 2.02 Gy, 0.36 ~ 5.01 Gy, 0.25 ~ 2.49 Gy, 0.14 ~ 6.92 Gy, 0.03 ~ 2.02 Gy, 0.01 ~ 1.06 Gy, 0.25 ~ 6.08 Gy, 0.08 ~ 0.59 Gy, 0.01 ~ 1.34 Gy, respectively. The OED, EAR of the IMRT and VMAT show higher than 3D-CRT. As the result of this study, we could confirm being higher dose index(HI, CI) in IMRT and VMAT than 3D-CRT, but doses of around normal organs was higher IMRT, VMAT than 3D-CRT.

  • This research reviewed the HIPAA/HITECH, 21st Century Cures Act, Common Law, and private Guidances from the perspectives in protecting and utilitizing the medical data, while implications were followed. First, the standards for protection and utilization are relatively clearly regulated through single law on personal medical information in the United States. The HIPAA has been introduced in 1996 as fundamental act on protection of medical data. Medical data was divided into personally identifiable information, non-identifying information, and limited dataset under HIPAA. Regulations on de-identification measures for medical information, objects for deletion of limited data sets, and agreement on prohibition of data re-identification were stipulated. Moreover, in the 21st Century Cures Act regulated mutual compatibility for data sharing, prohibition of data blocking, and strengthening of accessibility of data subjects. Common Law introduced comprehensive consent system and clearly stipulates procedures. Second, the regulatory system is relatively simplified and clearly stipulated in the United States. To be specific, the expert consensus and the safe harbor system were introduced as an anonymity measure for identifiable medical information, which clearly defines the process while increasing trust. Third, the protection of the rights of the data subject is specified, the duty of explanation is specified in detail, while the information right of the consumer (opt-out procedure) for identification information is specified. For instance, the HHS rule and FDA regulations recognize the comprehensive consent system for human research, but the consent procedure, method, and requirements are stipulated through the common rule. Fourth, in the case of the United States, a trust-based system is being used throughout the health and medical data legislation. To be specific, Limited Data Sets are allowed to use in condition to the researcher's agreement to prohibit re-identification, and de-identification or consent process is simplified under the system.

Estimation of Secondary Scattered Dose from Intensity-modulated Radiotherapy for Liver Cancer Cases (간암환자에 대한 세기조절방사선치료에서의 2차 산란선량평가)

  • We estimated secondary scattered and leakage doses for intensity-modulated radiotherapy (IMRT), volumetric arc therapy (VMAT) and tomotherapy (TOMO) in patients with liver cancer. Five liver patients were planned by IMRT, VMAT and TOMO. Secondary scatter (and leakage) dose and organ equivalent doses (OEDs) are measured and estimated at various points 20 to 80 cm from the iso-center by using radiophotoluminescence glass dosimeter (RPLGD). The secondary dose per Gy from IMRT, VMAT and TOMO for liver cancer, measured 20 to 80 cm from the iso-center, are 0.01~3.13, 0.03~2.34 and 0.04~1.29 cGy, respectively. The mean values of relative OED of secondary dose of VMAT and TOMO for five patients, which is normalized by IMRT, measured as 75.24% and 50.92% for thyroid, 75.14% and 40.61% for bowel, 72.30% and 47.77% for rectum, 76.21% and 49.93% for prostate. The secondary dose and OED from TOMO is relatively low to those from IMRT and VMAT. OED based estimation suggests that the secondary cancer risk from TOMO is less than or comparable to the risks from conventional IMRT and VMAT.

  • 에너지는 인류문명발달의 원동력이며 현대 산업사회에서는 모든 산업활동과 국민경제에 필수적인 요소이므로 세계 거의 모든 나라들은 에너지의 안정확보를 국가정책의 제 1로 삼고 있다. 세계적으로 볼 때 시대의 변천에 따라 에너지의 주역도 바뀌어 산업혁명 후 제2차 세계 대전까지는 석탄이 주역이었고, 2차대전 후 1970년대 석유위기 전까지는 석유가 단연 주역이었다가 석유위기 후 1980년대는 천연가스와 원자력이 상당부문 석유를 대체하고 있으며, 21세기에는 가스이용기술의 개발에 따라 편리성과 경제성에서 가스의 우월성으로 타에너지를 대체하고, 특히 지구온난화방지 노력과 관련하여 전세계적인 $CO_2$ 감축 목표에 따라 청정연료로서 타에너지 보다 사용이 크게 늘어 2010년까지는 1995년의 $50\%$가 증가하여 21세기 연료의 주역이 될 것으로 전망된다. 우리나라는 에너지의 $98\%$를 외국에서 도입하는 상황에서 석유의존도가 $60\%$를 넘고 있으며, 원자력발전소의 확장에 어려움이 많고 세계기후변화협약을 비준한 국가로서 지구온난화방지를 위한 의무를 지게 될 것이므로, 에너지 공급원의 다원화를 통한 안정확보와 청정에너지 이용의 확대 및 에너지 산업에 대한 규제완화 등으로 경제성에 입각한 시장경쟁에서 가스는 우위를 확보함으로써 1차 에너지중 가스의 비중이 매우 높아질 것으로 전망되고있다. 따라서 가스산업은 우리나라 에너지산업중 가장 크게 발전하고 또 다른 에너지가 여러면에서 제약을 받게 될 때 이러한 제약을 극복하는 대체에너지로서 역할이 크게 전망되며, 특히 발전, 열병합, 냉방, 자동차 연료부문에서의 사용이 크게 늘어날 것으로 예상되어 이러한 가스이용의 확대에 대비하여 가스자원의 개발, 국내 공급설비의 확충, 연구개발등에 대한 투자와 규제완화에 의한 시장경쟁에 대한 대비가 필요하다.계산모델 및 원자력병원에서 보유하고 있는 방사선 치료계획장치인 CAP-PLAN의 선량계산모델의 계산 결과와 비교하여, 흡수선량은 ${\pm}10\%$ 이내에서 거리로는 0.4mm 이내에서 대부분 일치하였다. 최대 오차는 각각 $11.3\%$ 및 0.8mm로 나타났다. 이상의 결과들로부터 새로 개발한 금으로 된 기구와 Ir-192 seed를 이용한 근접 방사선 치료법으로 안구의 악성종양에 대한 치료를 보다 효과적으로 시행할 수 있을 것으로 기대된다. 구성원을 간소 목적을 위하여 사용할 수 있어야 하겠다 마지막으로 이들 내의 위험 집단을 우선적으로 파악. 접근하여야 할 것이다. 위험 집단으로는 경제적 문제로 적절한 의료 이용을 할 수 없는 저소득층, 문화적으로 열등한 위치에 있는 여성층과 초기 이민 적응에 가장 문제를 일으킬 소지가 있는 노년층을 들 수 있겠다. 이 연구는 몇 가지 제한점을 가진다. 첫째. 연구 대상자 선정이 어려워서 자원자를 대상으로 연구가 행해졌다. 둘째. 적은 수의 연구 대상자를 대상으로 연구가 행해졌다. 셋째. 연구기간이 짧았던 까닭에 좀더 상세한 사례 연구가 이루어질 수 없었다 좀 더 신뢰할 수 있는 표본 추출 방법을 통하여 선정된, 많은 연구 대상자를 가지고, 심도 있는 연구가 추후 반복적으로 이루어져야 할 것이다.더욱 더 발전을 거듭하고 있으며, 외식은 여행과 여가 활동의 필수적인 요소로써 그 역할을 일조하고 있다. 이와 같은 여가시간의 증가는 독신자들에게는 좀더 많은 여유시간을 가족을 이루고 있는 가족구성원들에게는 가족과의 유대를 강화하는 휴식과 오락의 소비 트렌드를 창출시켰다. 이와 더불어 외식은 식사를 해결하기 위한 단순한 수단에서 벗어나 동기와 동반자에 따라 달라지는 행동 패턴을 나타내고 있으며, 연령과 목적에 따라 세분화되는 분명한 선호도를 나타낸다. 지난 10여 년간 외국으로부터 수입된 다양한

Comparison of Dose Distribution between the Techniques of Non-small Cell Lung Cancer (비소세포폐암의 방사선 치료기법간의 선량분포의 비교)

  • Comparison of the dose aspect that radiation therapy treatments using IMRT, tomotherapy, mArc (modulated arc therapy). The experimental subject is non-small cell lung cancer patient. The prescription dose is 58.0 Gy to the volume of PTV(planning target volume). and spinal cord, esophagus, and liver organ is the normal organ(OAR, organ at risk). Average PTV value is 57.60 Gy in mArc and 61.04 Gy in tomotherapy and 58.95 Gy in IMRT. The average dose of the Esophagus is 2.84 Gy in m-Arc, 5.14 Gy in tomotherapy, 1.84 Gy in IMRT. The average dose of the Liver is 19.44 Gy in m-Arc, 12.22 Gy in tomotherapy, 21.97 Gy in IMRT. The average dose of the Spinal cord is 5.72 Gy in m-Arc, 7.08 Gy in tomotherapy, 6.15 Gy in IMRT. Results of this study is no significant difference between mArc and tomotherapy and Linac based IMRT in dose study and also, mArc's dose coverage and dose volume histogram is better than IMRT and tomotherapy. but, This study is limited to a disease of cancer. in addition, fewer number of groups. Understanding Biotechnology: An Analysis of High School Students' Concepts (생명공학의 기본 개념에 대한 고등학생의 이해도 조사 및 개념 분석)

  • Biotechnology is the process of using biological system for the production of materials. Genetic engineering, a subset of biotechnology, is the process of altering biological systems by the purposeful manipulation of DNA It is a new field in biology and no topic in biology is more likely to impact our personal lives and is therefore more worthy of our attention and understanding. The purpose of this study was to investigate students' understanding of the concepts of biotechnology, and a test tool which is made up of 20 basic questions was developed for the study. The subject of this study was high school students and the sample size was 486. In order to find out the source of students' misunderstanding, we also analysed high school textbooks and teachers were given the same tool applied to students. Two-way ANOVA was used for the analysis. Major findings of this study are as following; 1. Mean score of students was 41, and there was a significant difference between the scores of boys and girls(p<0.05). Female students scored higher than male students. The variables "region" and "major" had no significant influence. 2. Students' the most misunderstood concepts were "monoclonal antibody" and "gene cloning". Many students thought that a plamid DNA originally has a useful DNA in it, which is apparently wrong. 3. Mean score of teachers was 82, and the variabes of gender and career did not have statistically significant influence on the result(p>0.05). 4. Teachers got the lowest scores on the concepts of "gene therapy", "the accomplishment of biotechnology in agriculture and medicine", and "plasmid DNA". The results of item analysis implied that teachers' misunderstanding might be a part of the sources of students' misunderstaning. 5. Out of 18 basic concepts selected in the study, only 10 concepts were explained well enough in most textbooks. The results of item analysis indicated that textbooks also could be a part of the source of students' misunderstanding.
