Changing Mechanisms Corresponding to The Changing Stages of Smoking Cessation (금연의 변화단계에 따른 변화기전)

  • The average smoking rate for Adults' in our country is 40.6% : It is 74.2% for men and 5.0% for women. Particularly, the smoking rate for men is reported higher than that of men in U.S.A. or Japan. Since the first report on the association between smoking and cancer appeared, 370 thousand smokers have succeeded in smoking cessation and over 90% of them have responded that they depended on a self-help smoking cessation approach. Despite this positive evidence about self- help approaches for smoking cessation, most studies on smoking cessation have focused on evaluation of formal treatment programs that are provided by clinics. Reports on the smoking cessation process used by smokers in our country could not be found. However, it is believed that the situation in our country would be quite similar to that in U.S.A. as far as approaches to successful smoking cessation are concerned. This study was conducted to classify the smoking stage to which they smoker belong and which changing mechanisms could be included at each changing stage (precontemplation stage, comtemplation stage, action stage) with a sample of 155 college students between 20 and 29 years old. And it also identified which variables related both to smoking pattern and to health, which ones were significantly discriminating in the changing stages. From the results of the data analysis it was found that Self-Determination is the most influential variable as one of the changning mechanisms which can discriminate three changing stages. And as the next significant mechanisms were Reinforcement, Dramatic Relief, Cognitive Restructuring, Helping Relationship, and Information Management in that order. Among variables related to the smoking pattern, years of regular smoking, whether smoking is continued or not even when they are sick, the number of attempts to stop smoking, number of cigarettes smoked per day, and whether they have smoked over 100 cigarettes up to now, but not the time of the first cigarette after waking-up, were the significant factors to descriminate changing stages. It was confirmed that among variables related to health that, perceived control for health, confidence of health maintence ability, and self confidence in smoking cessation, were significant variables in determinating changing stages. The most influential variables among them was self-confidence in smoking cessation. Conclusively, it was shown that smoking cessation is the process of attempting to change smoking habits through the various changing processes. Also it can be shown that a few factors smoking habit, self-confidence of smoking cessation, and belief in self control of his /her health, were influential in discriminating the changing stages of the smoking habit.

The education evaluation of basic CPR on middle school students (중학교 재학생의 기본 심폐소생술 교육 효과 평가)

  • This study is focused on middle school students who are composed of a factor in medical emergency system. In the case of cardiac failure, it is to make its basic data and develop its education program of CPR(cardiopulmonary resuscitation) which can increase the patient's survival rate before his hospitalization. The findings of this study is as follows. The subject of study is composed of 117 persons who are 54 boy-students(46.2%) and 63 girl-students(53.8%) in sex and 72 first-grade students(61.5%) and 45 second-grade students (38.5%) in a school year. In the accuracy degree, the practice of artificial respiration is $42.28{\pm}34.42%$ in case of basic CPR(cardiopulmonary resuscitation). The accuracy degree of the thorax pressure is $82.17{\pm}15.40%.$ In the accuracy degree, the practice of artificial respiration is $92.16{\pm}25.68%$ in boy-students and $20.38{\pm}24.16%$ in girl-students, the second-grade students is $58.40{\pm}33.29%$, which is higher than the first-grade students' $32.21{\pm}31.14%$. In the accuracy degree, the thorax pressure is $92.16{\pm}3.91%$ in boy-studetns and $73.61{\pm}16.41%$ in girl-students. In the accuracy degree of the thorax pressure, the second-grade students are $82.60{\pm}16.54%$ and the first-grade students $81.91{\pm}3.91$, which doesn't show any significant difference in school year. The satisfaction degree after theory & practice education is $2.12{\pm}.85$. In the satisfaction degree of its basic CPR, girl-students are $2.14{\pm}.83$ and boy-students are 2.11. In the satisfaction degree of its basic CPR, the second-grade students are $2.40{\pm}.61$ and the first-grade students are $1.95{\pm}.94$. This study is to lead to some suggestions. First, it is necessary to develop the education program and educate its knowledge & technology in proportion to student's characteristics of sex and school year. Second, education authorities should develop a subject of the accident prevention and first-aid treatment in its curriculum and provide the practical education of CPR for adults, adolescents and children. Third, it is necessary to study the education program as well as the education evaluation of CPR further on.

A Clinical Study of Changes in Serum Potassium Ion Concentration Before and After Extracorporeal Circulation with Heart-Lung Machine (개심술시 체외순환에 의한 혈청 POTASSIUM 변동에 관한 연구)

  • Since the open heart surgery was performed, various kinds of problem concerning the extracorporeal circulation[EGG] have been known. The author investigated the changes of serum potassium ion before and after ECC among the 102 patients including 63 adults and 39 children who underwent open heart surgery from April 19S6 to February 1990 in Chung-Ang University Hospital. The mean values of potassium ion before and after ECC were analyzed according to the influencing factors such as priming solution, aortic cross clamping time, the underlying disease, the type of oxygenator and the amount of cardioplegic solution. The results were as follows: l. In the aspect of congenital and acquired heart disease groups, the mean values of serum potassium ion[Mean\ulcornerS.D.] before and after ECC revealed a significant change only in the acquired heart disease group[congenital; 3.87$\pm$0.48mEq /L vs. 4.05$\pm$0.73mEq /L, P>0.05, acquired: 4.40 $\pm$0.98mEq /L vs. 4.11$\pm$0.52mEq /L, P<0.05]. Between the two groups, the changes of the mean values of serum potassium ion before and after ECC were significant[P<0.05]. But all values were within normal limits. 2. In the aspect of the aortic cross clamping time[ACCT], in the groups of less or more than 120 minutes, the mean values of serum potassium ion before and after ECC revealed no significant change[less than 120 min; 3.97+-0.64mEq /L vs. 3.99+0.67mEq /L, P>0.05, more than 120 min; 4.34+0.82mEq /L vs. 4.27+0.62mEq /L, P>0.05], and The changes of mean values of serum potassium ion between the two groups were not significant[P>0.05]. 3. In both membrane and bubble oxygenator groups, the mean values of serum potassium ion before and after ECC did not reveal a significant difference respectively [membrane; 4.74 +1.40mEq /L vs. 4.28+0.3lmEq /L, P>0.05, bubble; 4.02 +0.60mEq /L vs. 4.05 L0.68mEq/L, P>0.05], and no differences between the membrane and bubble oxygenator groups[P >0.05]. 4. In the groups of membrane and bubble oxygenator in the cases of ACCT more than 120 minutes, the mean values of serum potassium ion before and after ECC did not reveal a significant difference respectively[membrane; 4.36$\pm$0.85mEq /L vs. 4.37$\pm$0.26mEq /L, P>0.05, bubble; 4.30 $\pm$0.80mEq/L vs. 4.23$\pm$0.67mEq/L, P>0.05], and no differences between the two groups[P>0.05]. 5. In spite of increased amount of cold potassium cardioplegic solution, the mean values of serum potassium ion before ECC were similar to those of serum potassium ion after ECC[less than 20ml /kg

Preference, Satisfaction, and Repurchase Intention of Consumers for Home Meal Replacements(HMR) by Product Categories (가정식사 대용식(Home meal replacement) 제품 유형별 소비자의 선호도, 만족도, 재구매 의사 분석)

  • The purpose of this study was to identify the characteristics of consumers who purchase home meal replacements (HMR) and to analyze the differences for frequency of use, preference, satisfaction, and repurchase intention based on HMR product categories. The subjects were adults in their twenties or older, who had used HMRs and lived in Seoul and the Gyeonggi Province of Korea. Five-hundred and fifty subjects were chosen by random sampling, and questionnaires were distributed from March 12 to 30, 2005. A total of 451 questionnaires were returned (the rate of return was 82%). The primary results of the study are as follows. First, by analyzing for differences based on the demographical characteristics of the HMR consumers, men had higher 'frequencies of use' and preference, whereas women had higher satisfaction and 'intentions to purchase again.' This implies that HMRs assist women who are usually in charge of home food preparation, based on time and cooking effort; hence a higher satisfaction among the women. Second, by analyzing for differences based on the characteristics of HMR consumers who ate the products, 'frequency of use' (p<.001), preference (p<.01), satisfaction (p<.001), and 'intention to repurchase' (p<.001) for the 'ready to eat' HMR category, by the person who prepared the food, were significantly higher for those subjects in their 20s than for those in other age groups. As for persons influenced by the meal, the teen age group had significantly lower 'frequency of use'(p<.001), preference (p<.05), satisfaction (p<.01) and 'intention to repurchase' (p<.01) than other age groups. 'Frequency of use' (p<.001) and preference (p<.05) were lower if the person influenced by the meal was one's spouse or child than if the person influenced by the meal was oneself. Third, regarding the preference for eating at home or eating out, 'frequency of use' was significantly higher than the other analyzed factors for the 'ready to eat,' 'ready to heat,' and 'ready to end-cook' HMR categories. In short, the 'frequency of use' for HMRs was higher for those who preferred to eat at home versus eating out, simply as a result of being bored with eating out, which is essentially the reason for the origin of HMR.

Estimated Factors and Clinical Characteristics of Children Diagnosed with Idiopathic Precocious Puberty (특발성 진성 성조숙증으로 진단된 소아의 원인 및 임상적 분석)

  • Objectives: The purpose of this study is to estimate genetic and environmental factors, which can affect Idiopathic true Precocious puberty, and to evaluate the clinical and endocrinologic characteristics. Methods: Retrospective and Comparative analysis of 76 children (72 girls and 4 boys) has been diagnosed with idiopathic true precocious puberty, and treated with GnRHa from December 2008 to July 2011. Results: 1. The Average chronological age (CA. yr) of children diagnosed with idiopathic true precocious puberty was $8.40{\pm}0.81$ (girls), $9.93{\pm}0.12$ (boys). 2. The Average height & weight percentile (%ile) of the girls diagnosed with idiopathic true precocious puberty was $67.38{\pm}22.04$, $67.69{\pm}23.20$. 3. The girls' mothers have diagnosed with idiopathic true precocious puberty, and they were shorter than the average. This shows that mother's small height and idiopathic true precocious puberty are closely related to each other. 4. BMI percentile (%ile) of girls diagnosed with idiopathic true precocious puberty was $63.26{\pm}24.86$. 23.6% of children were diagnosed with overweight or obesity. This result shows that obesity and idiopathic true precocious puberty are proportionally related. 5. Birth weights (kg) of the children diagnosed with idiopathic true precocious puberty were $3.16{\pm}0.43$ (girls), $3.15{\pm}0.38$ (boys). 8.3% of children were diagnosed with Intrauterine growth retardation. 6. The Average bone ages (BA. yr) of the children diagnosed with idiopathic true precocious puberty were $10.51{\pm}0.99$ (girls), $12.10{\pm}0.97$ (boys). The Average BA-CA was $2.11{\pm}0.81$ (girls), $2.00{\pm}0.87$ (boys). 7. The Average predicted adults' height (PAH. cm) of the children diagnosed with idiopathic true precocious puberty was $151.61{\pm}4.00$ (girls), $163.50{\pm}2.15$ (boys). The Average MPH-PAH was $6.84{\pm}4.91$ (girls), $6.00{\pm}5.35$ (boys). 8. 23.6% of the children treated with GnRHa were co-treated with Growth Hormone. Conclusions: Estimated factors which cause Idiopathic true precocious puberty are mother's small height, obesity, and Intrauterine growth retardation. However, the studies of Oriental Medicine for Idiopathic true precocious puberty were lacking. Further clinical and experimental researches are needed.

Evaluations on Driver's Sensibility Changes by Sudden Start and Sudden Stop Conditions in Driving Simulator (자동차 시뮬레이터에서의 급출발 및 급제동에 따른 운전자 감성 평가)

  • The purpose of the study was to measure and compare driver's psychophysiological responses in different driving conditions through driving simulator. Twelve male adults(more than 1 year of driving experience) were assigned to four different driving conditions, such as normal speed(70㎞/h), sudden start(0㎞/h→70㎞/h), and sudden stop(70㎞/h→0㎞/h), and their simulator sickness, subjective pleasantness and arousal, EEG, ECG, skin temperature, and GSR were measured. Subjective and physiological evaluations were executed before and after driving in each condition. The results showed that subjective pleasantness and arousal increased in sudden stop and sudden start conditions, relative to stop and normal speed conditions. As the central nervous responses, beta wave increased and alpha wave decreased in sudden stop and sudden start conditions, relative to stop and normal speed conditions. With regard to the autonomic responses, heart rate and GSR increased, while skin temperature decreased in sudden stop and sudden start conditions, which means an activation of sympathetic nervous system. The results suggested that based upon observation of the distinctive psychophysiological changes by driving conditions, it is possible to evaluate the human sensibility in dynamic environment.

Cognition of dental caries prevention by the level of the social economic status in Korea: Based on Gallup survey (사회경제적 수준에 따른 치아우식증 예방과 관련 인식조사: 한국갤럽자료를 활용하여)

  • Objectives: The purpose of the study is to investigate the cognition of dental caries prevention by the level of socioeconomic status based on Gallup survey in Korea. Methods: This study was done by Korean Gallup survey in October, 2010. A trained researcher carried out the computer aided telephone interview(CATI) using a structured questionnaire. This study was based on the latest population statistics of resident registration and whole country's phone data base. This survey included 869 selected Korean adults over 19 years old, and they were asked to answer a CATI. The questionnaire consisted of general characteristics of the subjects and socioeconomic factors including age, gender, education level, monthly income, and residential area. Cognition of dental caries prevention was measured by Likert 4 scale including 'much', 'a little', 'rarely', and 'never'. The attitude toward dental caries prevention consisted of daily tooth brushing frequency, experience of oral health education, regular dental checkup, chewing gums(xylitol), regular scaling, and use of oral care devices. Data were analyzed using SPSS 20.0 for frequency analysis, t-test, chi-square test, and one way ANOVA. Cronbach's alpha was 0.462 in oral health concern and attitude. Results: Mean of the frequencies of daily tooth brushing in men was 2.54 times and 2.78 in women. By the comparison to age group, 35-44 years old group had 2.82 times, 19-37 years old group had 2.72 times, and 45-64 years old group had 2.51 times. The level of education and monthly income was proportional to the tooth brushing frequency. Highly educated and higher monthly income group received regular dental checkup within a year and used the auxiliary oral health care devices. Conclusions: This study suggested the relationship between dental caries prevention and socioeconomic status. It is important to provide the low socioeconomic group with the better oral health promotion services in the future.

A Study on Mercury Exposure Levels among Schoolchildren and Related Factors in High Mercury Exposure Areas in Korea (수은 고노출 지역 초등학생의 수은노출수준과 노출요인 연구)

  • Objectives: Blood mercury levels among adults living in certain areas of the Gyeongsang Provinces have been shown to be very high (Kunwee County $29.6{\mu}g/L$, Yeongcheon-city $26.7{\mu}g/L$). The purpose of this project was to determine mercury exposure levels in schoolchildren and factors related with their mercury levels in high mercury exposure areas identifyed by the 2007 Korea National Environmental Health Survey. Methods: From June to September 2010, 1,097 students from grades 3 to 6 at 19 elementary schools participated in this study, including 294 students from 10 elementary schools in Kunwee County, 529 students from Yeongcheon City, 122 students from two elementary schools in Pohang City, North Gyeongsang Province, and 152 students from two elementary schools in Ulsan Metropolitan City. Biological samples from schoolchildren, including whole blood, urine and hair, were collected to measure total mercury at the time of a health check up. Information about children was collected by questionnaire. Total mercury concentrations in blood were measured using the Direct Mercury Analyzer 80 with the gold-amalgam collection method. Results: The mean mercury levels were $2.70{\mu}g/L$ in 1,091 blood samples, $2.25{\mu}g/g-creat.$ in 820 urine samples and $1.03{\mu}g/g$ in 1,064 hair samples. Blood mercury levels in the schoolchildren was slightly higher than the result of $2.4{\mu}g/L$ from a 2006 survey of elementary school children on exposure and health effects of mercury by the National Institute of Environmental Research. However, 0.3% and 4.5% of participants exceeded the reference level of blood mercury by CHBMII ($15{\mu}g/L$) and the US EPA ($5.8{\mu}g/L$), respectively. The reference level of urine by CHBMII ($20{\mu}g/L$) was exceeded by 0.4% of participants. As factors, residence period in the study areas, residence type, father's education level and income all showed significant associations with mercury level in the biological samples. The number of dental amalgam sides showed an association with urine mercury. Fish intake preference and fish intake frequency were important factors in mercury levels. In particular, intake of shark meat and recent intake of shark meat were associated with higher mercury levels. In this regard, participation in the performance of an ancestral rite showed a relation with higher mercury levels. Conclusion: The intake of shark meat was very important factor to high mercury exposure level. It is recommended to monitor and manage students with high mercury exposures who exceeded CHBM II and EPA guidelines, and include blood mercury testing in the Children's Health check up for this province.

Middle School Students' Perception of Body Image and Allowance for Plastic Surgery (중학생의 신체상 지각수준과 성형수술 허용도)

  • This study set out to investigate the relations between middle school students' perception of body image and their allowance for plastic surgery, to understand their perception of body image and desire for plastic surgery, and provide some data needed to warn against reckless plastic surgery and guide the students effectively. For those purposes, an examination was conducted of the relationships between the individual characteristics and perception levels of body image, the individual characteristics and allowance for plastic surgery, and perception levels of body image and allowance for plastic surgery. The subjects were drawn from sour middle schools located in two regions of Gyeonggi Province. Total 922 boys and girls were surveyed on a questionnaire, which was developed based on the pretest of previous literature, reviewed for appropriateness, and tested for reliability and reasonableness. The body image on the five scale was greater as the perception level was higher. The allowance for plastic surgery was also greater as the scores were more. The findings were as follows: First, the relationships between individual characteristics and perception levels of body image were examined. The third graders showed the highest perception level, being followed by the first and second graders. The girls were more perceptive than the boys, and those who were extrovert were more perceptive than those who were introvert. Those students whose parents earned 2 million won or more a month and who adapted themselves to the environmental changes had a higher perception level. In a word, the girls from the middle class that were well adapted, felt happy, and were extrovert had a higher perception level of body image. Second, the connections between individual characteristics and allowance for plastic surgery were investigated. The third graders were the most admissive of plastic surgery, followed by the second and first graders. That is, the upper graders were more admissive of plastic surgery. In addition, the girls were more admissive than the boys, and those who were extrovert were more than those who were introvert. There were no significant differences according to the monthly income of the parents, grades, adaptability to surroundings, and happiness, which results almost resembled the findings of a study conducted on adults. Third, there were negative correlations found between the perception levels of body image and the allowance for plastic surgery. To elaborate, the higher the perception levels were, the lower the allowance was, and vice versa. As for the items, the subjects showed more allowance for plastic surgery when they scored less in the item of caring about appearance, importance of looking pretty to others, and efforts to improve appearance. When they had a low value of body and easily felt tired, they were highly acceptive of plastic surgery. The allowance for plastic surgery was also great when their perception was much of how healthy they felt, how important they felt about their bodies, how they were satisfied with their current appearances, how they evaluated the appearance of others, how much they were satisfied physically, and how much demanding they were for physical changes. Meanwhile, there were no correlations between the allowance and physical attraction, the degree for one's activities to be hindered, and sickness. In short, the demand for plastic surgery was 41% for the girls and 20.2% for the boys. Just as the study on adults reported, those who had a low or negative perception of body image were more acceptive of plastic surgery. The middle school students were generally positive about their bodies with the lowest perception level at 2.91 and the highest at 3.21. Their individual allowance for plastic surgery was related to their individual body images, which were in turn affected by the mass communication, surrounding environments, and social values. Thus it's necessary for the entire society to try to improve or change the overall perception. Helping measures should be taken so that the students can form right sense of values about their bodies, avoid the obsession with appearance and appearance-based evaluation, and exercise righteous criteria against humans beings and things. In conclusions, the following suggestions were made: they need to develop such questionnaires or tools as can measure the body image of teens and fit the reality. Moreover, body image improvement programs should be more diverse and more applicable to teens. Despite the consistent reports that prove the correlations between body image and plastic surgery, there has been little effort to apply such factors as experience of the life of the disabled, volunteer activities for the disabled and at the hospitals, and others that can induce changes to body image to the body image improvement programs. In the future, comparative research should be carried out on body image and plastic surgery.

The Change of Cortical Activity Induced by Visual Disgust Stimulus (시각혐오자극으로 유발된 대뇌 피질 활성도 변화)

  • Objectives: There are a lot of studies that analyze the interaction between the emotion of disgust and the functional brain images using fMRI and PET. But studies using sLORETA (standardized low resolution brain electromagnetic tomography) almost do not exist. The aim of this research is to explore the relationship of the emotion of disgust and the cortical activation using sLORETA analysis. Methods: Forty five healthy young adults ($27.1{\pm}2.6$ years) participated in the study. While they were watching 4 neutral images and 4 disgusting images associated with mutilation selected from the international affective picture system (IAPS), participants' EEGs were taken for 30 seconds per one picture. Through these obtained EEG data, sLORETA analysis was performed to compare EEGs associated with neutral and negative images. Results: During looking for visual disgusting stimulus, all participants reported unpleasantness, arousal and stress. In sLORETA analysis, the decrease of current density in theta wave was shown at left frontal superior gyrus (BA10) and middle gyrus (BA10, 11). This voxel cluster consists of a total of 11 voxels and the threshold of t value indicating statistically significant decreases in the current density (p<0.05) was -1.984. There were no differences between male and female in the degree of being disgusted by the stimuli. Conclusion: This finding may suggest that the activation of dorsolateral prefrontal cortex might be associated with regulating disgust emotion.
