Purpose: The purpose of this study is to investigate correlations in vital sign changes, the severity of pain, signs of complications, and the duration of sandbag management in order to suggest a standardized practice related to sandbag management in children with Nephrotic Syndrome(NS). Method: From October 2000 to May 2001, seventy children with NS who underwent kidney biopsy were interviewed at one hospital in Seoul Korea, and participated in this study. Result: 1) The average sandbag applying time after kidney biopsy was 18.1 hours. 2) Systolic blood pressure and respiration increased until 15 minutes after kidney biopsy, after then, they decreased signifi- cantly (systolic BP, p= .006; respiration, p= .029). However, no significant changes were noted in diastolic blood pressure and pulse. 3) Pain was reported minimal for 1 hour after kidney biopsy. The severity of pain increased until 12 hours after the procedure, then, decreased significantly(p= .0001). 4) Reported complications were hematuria (74.7%) and abnormal sonogram (32.9%). No apparent bleeding on the biopsy region was reported in any children. Conclusion: From these findings, it is possible to change the protocols of the duration of absolute bed rest time and sandbag application management shortly after kidney biopsy. But it is needed to study the fit protocols for kidney biopsy. Several implications in nursing practice are suggested. 1) Replicated studies for more participants are needed. 2) Further research on the effect of sandbag application after kidney biopsy is required. 3) The best duration of sandbag application management after kidney biopsy need to be investigated.
The purpose of this paper was to investigate the bioavailability of oxytetracycline in pathological rats and rabbits pretreated with carbon tetrachloride and mercuric chloride. The results are as follows: The blood level of oxytetracycline administered orally was mostly decreased significantly in rabbits damaged kidney and liver, and in rabbits severely damaged kidney, the blood level of oxytetracycline was not significant at 4 to 6 hours. Urinary clearance of oxytetracycline in rabbits severely damaged kidney was inhibited at 5 to 6 hours but in rabbits damaged liver. Hepatic clearance of oxytetracycline was accelerated in rabbits damaged kidney but in rabbits damaged liver. AUC of oxytetracycline orally administered in rabbits damaged liver and kidney was largely decreased. The absorption of oxytetracycline was decreased in rats damaged liver and kidney as compared with that of normal rats. Especially, absorption of oxytetracycline in rats damaged liver was more decreased than that of rats damaged kidney. The absorption of oxytetracycline was inhibited by combinated administration of carbon tetrachloride and mercuric chloride.
Changes in handling of $Li^+$ by contralateral kidney during acute $Li^+$ loading were investigated immediately after unilateral ureteral obstruction. Carotid artery, jugular vein, renal vein and ureter of experimental animal were catheterized and renal venous flow was shunted to .external jugular vein. In experimental group right ureter was ligated. One to two hours after operation a single shot of LiCl solution (2 mEq/kg) was intravenously injected and then .arterial, renal venous blood and urine samples were taken sequentially for 1 to $1{\frac{1}{2}}$ hours. Urine volume, plasma and urinary concentrations of $Li^+$, $Na^+$ and $K^+$ were measured and urinary excretion of them were calculated. Results obtained were as follows: 1) In experimental group urine volume, urinary excretion of $Na^+$, and $K^+$ by contralateral kidney after unilateral ureteral obstruction were slightly larger than mean value of both kidney in control group. 2) During acute $Li^+$ loading contralateral kidney in experimental group showed limited $K^+$ excretion, but urinary flow and $Na^+$ excretion were comparable to mean value of both kidney in control group. 3) Urinary osmolar concentration in experimental group was much lower than that in control group, and it was maintained at low level even after Li loading. 4) In experimental group plasma$Li^+$ concentration decreased more slowly than in control group after a single shot of LiCl solution. 5) Urinary excretion of $Li^+$ in experimental group was markedly decreased, even lesseer than mean of both kidney in control group. 6) From the above results it was concluded that immediately after unilateral ureteral obstruction contralateral kidney showed normal water and $Na^+$ diuretic response to Li load but urinay $Li^+$ excretion was decreased and reclaimed $Li^+$ to systemic circulation.
소아 요로 감염의 초음파 검사 유용성검토에 대한 연구결과 다음과 같은 결론을 얻었다. 1. 요로 감염으로 진단된 환 아 122명 중 1세 미만이 100명으로 82%였고 남여 성비는 1.7 : 1로 남아가 높았으며 122명 중 신장초음파 검사(Kidney sonography), 배뇨성방광요도조영술(Voiding cystourethrography), 신주사검사($^{99m}TC$ DMSA-scan), 3가지 영상의학적 검사를 시행한 환 아는 77명이였으며, 3가지 영상의학적검사중 진단율은 신장초음파검사(Kidney Sonography) 51%, 신주사 검사($^{99m}TC$ DMSA-scan) 42% 배뇨성방광요도 조영술 (VCUG) 22%의 순으로 나타났다. 2. 신장초음파검사기술(Kidney Sonography)과 배뇨성 방광요도 조영술(VCUG)의 상관관계 비교에서 민감도 (sensitivity) 82% 특이도(specificity)는 58%였으며, 신장초음파검사(Kidney Sonography)와 신주사검사 $^{99m}TC$(DMSA-scan) 상관관계 비교에서는 민감도(sensitivity) 66%, 특이도(specificity)는 67%로 나타났다. 3. 소아 요로 감염 검사에서 환 아의 고통과 불쾌감, 수치심, 정신적인 스트레스유발, 검사 후 조영제에 대한 부작용, 방사선 피폭 등 검사와 관련된 합병증을 고려할 때 신장초음파검사(Kidney Sonography)의 유용성이 가장 높은 것으로 나타났다.
A high salt diet contributes to kidney damage by causing hypoxia and oxidative stress. Recently, an increase in dietary salt has been reported to induce an inflammatory phenotype in immune cells, further contributing to kidney damage. However, studies on the exact mechanism and role of a high salt diet on the inflammatory response in the kidneys are still insufficient. In this study, a cisplatin-induced acute kidney injury model using C57BL/6 mice was used to analyze the effect of salt intake on kidney injury. Results showed that high salt administration aggravated kidney edema in mice induced by treatment with cisplatin. Moreover, the indicators of kidney and liver function impairment were significantly increased in the group cotreated with high salt compared with that treated with cisplatin alone. Furthermore, the exacerbation of kidney damage by high salt administration was also associated with a decrease in the number of cells in the immune regulatory system. Additionally, high salt administration further decreased renal perfusion functions along with increased cisplatin-induced damage to proximal tubules. This was accompanied by increased expression of T cell immunoglobulin, mucin domain 1 (a biomarker of kidney injury), and Bax (a pro-apoptotic factor). Moreover, cisplatin-induced expression of proinflammatory mediators and cytokines, including cyclooxygenase-2 and tumor necrosis factor-α in kidney tissue, was further increased by high salt intake. Therefore, these results indicate that the kidney's inflammatory response by high salt treatment can further promote kidney damage caused by various pathological factors.
Currently commercially available phantom can reproduce and evaluate only a static situation, the study is incomplete research on phantom and system which is can confirmed functional situation in the kidney by time through dynamic phantom and blood flow velocity, various difference according to the amount of radioactive. Therefore, through this study, it has produced the dynamic kidney phantom to reproduce images through the dynamic flow of the kidney, it desires to evaluate the usefulness of nuclear medicine imaging. The production of the kidney phantom was fabricated based on the normal adult kidney, in order to reproduce the dynamic situation based on the fabricated kidney phantom, in this study it was applied the volume pump that can adjust the speed of blood flow, so it can be integrated continuously radioactive isotopes in the kidney by using 99mTc-pertechnate. Used the radioactive isotope was supplied through the two pump. It was confirmed the changes according to the infusion rate, radioactive isotopes and the different injection speeds on the left and right, analysis of the acquired images was done by drawn five times ROI in order to check the reproducibility of each on the front and rear of the kidney and bladder. Depending on the speed of injection, radioisotope was a lot of integrated and emissions up when adjusting the pressure of the pump as 30 stroke, it was the least integrated and emissions up when adjusting as 40 stroke. The integration of the left & right kidney was not reached in the amount of the highest when adjusting as 10 stroke. In the changes according to the amount of the radioactive isotope, 0.6 mCi(22.2 MBq), 0.8 mCi (29.6 MBq)was showed up similar tendency but, in the result of the injection 0.8 mCi, it was showed up counts close to double of 0.6 mCi. In the result of the differently injection speed of the left & right kidney, as a result of different conditions that injection speed was 20 stroke through left kidney phantom, the injection speed was 30 stroke through right kidney phantom, it was enough difference in the resulting image can be easily distinguished with the naked eye. Through this study, the results showed that the dynamic kidney phantom system is able to similarly reproduce renogram in the actual clinical. Especially, the depicted over time for the flow to be excreted through the kidney into the bladder was adequately reproduce, it is expected to be utilized as basic data to check the quality of the dynamic images. In addition, it is considered to help in the field of functional imaging and quality control.
Background: Chronic kidney disease (CKD) is affective a large portion of the world population prompting the need for extensive healthcare resources such as lifelong dialysis or kidney transplantation. The beneficial effect of conventional therapy in controlling the CKD progression remains a challenge due to their relative efficacy, safety, and accessibility. On the other hand, Unani medicine provides a therapeutic regimen that consists of a combination of treatment from rehabilitation to herbal pharmacotherapy. Methods: Two cases of chronic kidney disease were treated with dietotherapy, regimenal therapy ('bukhoor aam') and oral herbal drugs for 2-3 weeks. Endpoints of evaluation were symptoms and signs of the CKD, kidney function test, urine albumin, urine RBC, hemoglobin and liver function test. Result: Notable improvement was observed in the endpoints. Conclusion: Unani treatment was observed preliminarily beneficial in the treatment of chronic kidney disease. Rigorous pharmacological and clinical studies should be performed to warrant their efficacy and safety in CKD individuals.
Purpose: This study was done to explore adaptation experience of living kidney donors after donation. Specific aims were to identify challenges donors face in the process of adaptation following surgery and how they interact with recipients and other people. Methods: Grounded theory methodology was utilized. Participants were 13 living kidney donors at six months or more after donation. Data were collected by in-depth interviews with individual participants. Data were analyzed using constants comparative method with theoretical saturation. Results: A core category emerged as 'keeping the fences of my family in spite of vulnerability'. The adaptation process after donation was manifested in four phases: exploration, balance, maintenance, and acclimatization. Phenomenon was perception of vulnerability. Strategies to manage the vulnerability were assessing changes of body awareness, tailoring regimen to one's own body condition, coping with health problems, keeping restoration of health, and ruminating on the meaning of one's kidney donation. Consequences were reestablishing family well-being, realizing the values of one's kidney donation, and living with uncertainty. Conclusion: Findings of the study indicate that there is a need for health professionals to understand the vulnerability of living kidney donors and help their family system maintain a healthy and productive life. The results of this study can be used to develop phase-specific, patient-centered, and tailored interventions for living kidney donors.
Effect of freeze drying leek against cadmium poisoning on glutathione peroxidase, on glutathione reductase and on glutathione-s-transferase in liver, kidney and testes of the male rats during the administered period. In this experiment, male rats of Sprague-Dawley strain were used. The rats which were fed for 15 weeks were divided into 4 groups basal diet 3% leek added diet basal diet and cadmium in water and 3% leek added diet and cadmium in water. Cadmium was administered ad libitum 100ppm CdCl$_{2}$ in distilled water. The followings are the result of this experiment. 1. Leek enhanced the glutathione peroxidase activities which were reduced by cadmium treatment in liver, kidney and testes but not significance. 2. Leek reduced glutathione reductase activities which were incresed by cadmium in liver, kidney and testes. 3. Leek incresed the activities of glutathfone-s-transferase in liver but not in kidney and but not in testes. 4. Leek incresed glutathione concentration which was decresed by cadmium treatment in liver and kidney but not testes. This experiment showed that leek-addition group had protective effect against cadmium poisoning and alleviated GR and glutathione-s-transferase activities in tissues. Leek incresed activities of glutathione peroxidase in liver, kidney and testes but not significance. Therefore, this experiment concluded that leek defensive power against long term cadmium poisoning.
