The Educational Need of Forensic Medicine for Emergency Medical Technicians in 119 Rescue Service (119구급대 응급구조사에 대한 법의학교육의 필요성)

  • An Emergency Medical Technician (EMT) is a well-trained emergency responder to provide emergency medical services to the critically ill and injured patient. In various situations, EMT may destroy potential physical evidences associated with the crime scene or determination of real cause of death. This study was aimed to evaluate the educational need of forensic medicine in Korean EMT. Questionnaires were administered to 592 EMTs during March 2007. The response rate was 60.3%(357 EMTs). In questionnaires there were 13 questions regarding the general characteristics, 16 questions about roles of EMTs related with forensic circumstances, 9 questions about the education related to forensic medicine. Questionnaires rated on a 4-point Likert scale or 5-point Likert scale. Most of 119 rescue EMTs had experienced with situations related to crime or unexpected sudden death. EMTs had arrived to the scenes earlier than police and complained of some difficulties related with deficit of forensic knowledge. EMTs wanted to receive continuous educations about forensic medicine. In order to reduce dissatisfaction with EMTs roles and to improve crime scene preservations, Emergency Medical Services policies should provide regular educational curriculum by forensic pathologists and promote legal responsibilities for 119 rescue EMTs.

High endemicity of Metagonimus yokogawai infection among residents of Samchok-shi, Kangwon-do

  • A small-scale epidemiological survey was undertaken during 1997-1998 on the residents along the Osib-chon (Stream), Samchok-shi (City), Kangwon-do (Province), to evaluate the status of Metagonimus yokogawai infection. A total of 165 fecal samples was collected and examined by cellophane thick smear and formalin-ether sedimentation techniques. The egg Positive rate of M. yokogawai was 29.7%, showing a remarkable difference between males (46.6%) and females (16.3%). To obtain the adult flukes of M. yokogawai, 11 egg positive persons were treated with praziquantel and purged with magnesium sulfate. A total of 242,119 adult flukes (average 22,010 per person, 367 119,650 in range) was collected from diarrheic stools, all of which were identified as M. yokogawai. The results show that M. yokogawai is still highly endemic in this area.

Kinematic Characteristics of the Thrower's COM and the Shot in Shot Put : The Woman Record Holder of Korea (포환던지기 동작 시 포환과 신체 무게중심의 운동학적 특성 : 한국 여자 기록보유자를 대상으로)

  • The purpose of this study was to analyze the kinematic characteristics of the thrower's center of mass (COM) and the shot that her performance hits record high for the 29th National Athletic Competition. Two S-VHS video cameras were used to visualize. The Direct Linear Transformation technique was employed to paint a clear picture in the three dimensional coordination. Kwon3D was used to analyze the data. The results showed that release velocity and height were 13.73 m/s and 198.6 cm(119% by height ratio), respectively, which is considered as peak performance comparing an extensive review of previous literature on the shot put. Release angle was $34^{\circ}$, which is lower than the previous studies. The path of thrower's center of mass is needed to travel in a release direction during the flight phase. The vertical movement of the thrower's center of mass during the driving should be controlled. At release, the perfact timing is required without vertical and horizontal movements of the thrower's center of mass.

  • 섬유염색산업 중 면섬유의 염색과정에서 발생되는 머서라이징 폐액은 폐수처리장의 pH(수소이온농도)를 올리는 주원인이 되고 있어, 높은 pH에 따른 폐수처리 부과금이나 황산을 이용한 폐수 중화에 사용되는 약품비용이 과다하게 소요되고 있으며, 머서라이징공정에 사용되는 약품인 가성소다 비용이 계속 증가하고 있어, 기업의 환경처리비용 및 약품소모비용에 대한 부담이 매우 큰 실정이다. 본 기술은 머서라이징 폐액에 멤브레인 공정에 적용하여 폐가성소다로부터 가성소다를 회수할 뿐만 아니라, 회수된 가성소다를 다시 머서라이징 공정에 재이용할 수 있도록 하여 약품 절감효과 뿐만 아니라 폐수의 발생량과 오염물질의 농도의 획기적인 저감이 가능한 것이다. 본 연구에서는 서로 다른 기공크기를 갖는 막을 적용하여 투과플럭스, 부피회수율, 부유물질 및 유기물 제거율, 가성소다회수율, 파울링지수 등을 고려하여 폐수 특성에 맞는 가장 적합한 막을 선정하고, 각각의 막에 대하여 온도조건, 압력조건, 화학세정 등의 운전변수를 최적화, 고효율 폐알카리 회수 장치의 경제성 및 기술적 타당성 평가를 통한 가성소다 회수공정을 구성하였다.

Postoperative Non-Pathological Fever Following Posterior Cervical Fusion Surgery : Is Laminoplasty a Better Preventive Method than Laminectomy?

  • Objective : To analyze the incidence and characteristics of delayed postoperative fever in posterior cervical fusion using cervical pedicle screws (CPS). Methods : This study analyzed 119 patients who underwent posterior cervical fusion surgery using CPS. Delayed fever was defined as no fever for the first 3 postoperative days, followed by an ear temperature ≥38℃ on postoperative day 4 and subsequent days. Patient age, sex, diagnosis, laminectomy, surgical level, revision status, body mass index, underlying medical disease, surgical duration, and transfusion status were retrospectively reviewed. Results : Of 119 patients, seven were excluded due to surgical site infection, spondylitis, pneumonia, or surgical level that included the thoracic spine. Of the 112 included patients, 28 (25%) were febrile and 84 (75%) were afebrile. Multivariate logistic regression analysis showed that laminectomy was a statistically significant risk factor for postoperative non-pathological fever (odds ratio, 10.251; p=0.000). In contrast, trauma or tumor surgery and underlying medical disease were not significant risk factors for fever. Conclusion : Patients who develop delayed fever 4 days after posterior cervical fusion surgery using CPS are more likely to have non-pathologic fever than surgical site infection. Laminectomy is a significant risk factor for non-pathologic fever.

The Result of Quality Control in POSCO's Industrial Hygiene Laboratory (사업장 자체측정기관 국내, 외 정도관리 참여 결과)

  • Since being granted permission for self-assessment of the work environment by the Department of Labor in 1992, POSCO's Industrial Hygiene Laboratory has participated in domestic and Foreign Quality Control Programs 8 times and has obtained remarkable results. It has made a lot of effort to be a proficient laboratory with the ability to observe and analyse environmental monitoring data. The summary of results is as follows: 1. POSCO's Industrial Hygiene Laboratory has received a "P(Proficient)" grade from the round 115 to round 119 of AIHA/NIOSH PAT Program for analysis of such metals as lead, cadmium, zinc, and chromium, and also has received a "P(Proficient)" grade from its round 119 of for organic solvent analysis, i,e,. Trichloroethylene(TCE), Carbon Tetr-achloride(CTC), 1,2-Dichloroethane(DCE), 1,1,1-Trichloroethane(MCM), Tetrachloroethylene (PCE), Chloroform(CFM), Benzene(BNZ), toluene(TOL), and O-xylene(OXY). 2. In the Quality Control Evaluation Program performed by the Industrial Health Research Institute,Korea Industrial Safety Corporation, POSCO has passed impressively in the3-metal analysis test for lead, cadmium, cupper, and in the 6 organic solvent analysis test for Benzene(BNZ), toluene(TOL), O-xylene(OXY), Trichloroethylene(TCE) and Methyl isobutyl keton(MIBK), n-Hexane with 2 standard deviations. These analytic techniques should be practically applied to various fields, and reliable results of environmental monitoring should be considered to improve the work environment and to keep workers from occupation related diseases.

  • The records centers service of public institutions helps efficient work activities of internal users, who actually do the work, and becomes the foundation for strengthening the competitiveness of the institution. Based on QFD technique, one of the quality control techniques, this study was intended to propose a plan to improve the quality of the records center service by analyzing and comparing the quality of the records center service of public institutions. To this end, as a follow-up study of Lee, Su-jin(2018), the research target was expanded to local governments to apply the QFD technique and reconstructed the HoQ based on the results of the expanded QFD application. Moreover, this study suggested measures to improve the quality of service in the records center of public institutions by compiling the implications of comparing the records center service HoQ and prioritizing the technology characteristics of the records center service.

  • In recent years, the intensity of typhoons has increased due to climate change. It is presumed that the tailings and waste rock in the mining area harm the environment owing to flood damage. The Gangneung area has been affected by the largest typhoons in Korea, including No. 3,693 (1936), Rusa (2002), Maemi (2003), and Megi (2004). This study was based on a case in which high concentrations of arsenic were detected in the surroundings along a stream after floods caused by typhoons. Although the environmental damage-related law clarifies the responsibility of polluters, careful implementation is required in potential natural disaster areas. The pollutants from abandoned mines can be widely exposed due to typhoons as artificial causes may be mixed. To minimize the impact of natural disasters in these areas, it is necessary to improve and link relevant laws. This study is expected to help cope with mixed pollutants in downstream areas.

The Impact of Living Alone on the Transfer and Treatment Stages of Acute Ischemic Stroke in the Busan Metropolitan Area (부산권역 급성 허혈성 뇌졸중 환자 이송 및 치료단계에서 독거가 미치는 영향)

  • Background: This study aimed to analyze the prehospital process and reperfusion therapy process of acute ischemic stroke in Busan metropolitan area and examine the impact of living arrangement on the early management and functional outcomes of acute ischemic stroke (AIS). Methods: The patients who diagnosed with AIS and received reperfusion therapy at the Busan Regional Cardiovascular Center between September 2020 and May 2023 were selected. We investigated the patients' hospital arrival time (onset to door time) and utilization of 119 emergency ambulance services. Additionally, various time matrices related to reperfusion therapy after hospital were examined, along with the functional outcome at the 90-day after treatment. Results: Among the 753 AIS patients who underwent reperfusion therapy, 166 individuals (22.1%) were living alone. AIS patients living alone experienced significant delays in symptom detection (p<0.05) and hospital arrival compared to AIS patients with cohabitants (370.1 minutes vs. 210.2 minutes, p<0.001). There were no significant differences between the two groups in terms of 119 ambulance utilization and time metrics related with the reperfusion therapy. Independent predictors of prognosis in AIS patients were found to be age above 70, National Institutes of Health Stroke Scale score at admission, tissue plasminogen activator, living alone (odds ratio [OR], 1.785; 95% confidence interval [CI], 1.155-2.760) and interhospital transfer (OR, 1.898; 95% CI, 1.152-3.127). Delay in identification of AIS was shown significant correlation (OR, 2.440; 95% CI, 1.070-5.561) at living alone patients. Conclusion: This study revealed that AIS patients living alone in the Busan metropolitan region, requiring endovascular treatment, face challenges in the pre-hospital phase, which significantly impact their prognosis.

  • The loss of posterior occlusal support due to tooth loss is likely to lead to compensatory protrusion and labial tilt of the anterior teeth, which may be accompanied by a deep bite and a decrease in vertical dimension. The patient may suffer from a decrease in masticatory efficiency, inaccurate pronunciation, facial appearance changes, and temporomandibular joint disorder, so stable occlusal formation with support of posterior occlusion and restoration of vertical dimension is necessary. We report the case of a patient with reduction of vertical dimension, and inaccurate pronunciation due to multiple tooth loss who underwent full mouth rehabilitation with increased vertical dimension accompanied by phonetic analysis and achieved satisfactory functional and aesthetic results.
