Safety Assessment of Intravenous Administration of Trastuzumab in 100ml Saline for the Treatment of HER2-Positive Breast Cancer Patients

  • Background: The infusion rate is considered to affect incidence and severity of infusion reactions (IRs) caused by protein formulations. Trastuzumab (TRS) is approved for 90-minute infusion as the initial dose followed by 30-minute infusion with 250 ml saline. In the study, we evaluated the safety of TRS intravenously administered over 30 minutes with 100 ml saline to reduce burden of patients, safety of infusion with 250 ml saline already being established. Materials and Methods: Women with HER2 positive breast cancer, ${\geq}18$ years and ${\geq}55%$ left ventricular ejection fraction (LVEF), were registered in the study. Patients received 8mg/kg of TRS 250 ml over 90 minutes followed by 6mg/kg of TRS 100ml over 30 minutes in a three-week cycle. Results: A total of 31 patients were recruited, 24 for adjuvant therapy and seven with metastases. The median age was 59 years (range 39 to 82). The total number of TRS doses ranged from 5 to 17 with the median of 15. Mild IR occurred in two patients at the first dose. However, no IR was observed after reducing to 100 ml saline. No decrease of LVEF, increase of serum brain natriuretic peptide or any other adverse events were reported. Conclusions: Intravenous infusion of TRS with 100 ml saline over 30 minutes in breast cancer patients can be considered safe based on results from the study. It can be given on an outpatient basis as with the currently recommended dilution in 250 ml saline.

고려인삼이 흰쥐의 몸 무게에 미치는 영향 (Influence of Panax Ginseng on the Body Weight of Rats)

  • 인삼주정추출물이 흰쥐의 몸 무게 증가에 어떤 영향을 미치는 지를 알기 위하여 각각 32마리의 흰쥐$(35{\sim}40gm)$ 수컷과 암컷을 다시 인삼군과 식염수군으로 나누어 다음과 같이 실험을 하였다. 인삼군에서는 몸 무게 100mg에 대하여 인삼주정추출액(생리적 식염수 1ml속에 4mg의 인삼주정추출물이 포함된 용액)을 0.5ml의 비율로 매일 등뒤 피하에 54일 동안 주사하였으며, 식염수군에는 생리적 식염수를 몸 무게 100gm에 대하여 0.5ml의 비율로 인삼군과 동일한 방법으로 주사하였다. 주사가 시작된 날로부터 3일 간격으로 54일 동안 몸 무게를 비교적 정확한 저울로 달았다. 이를 측정치를 지표로 하여 인삼이 몸 무게 증가에 미치는 영향을 관찰한 바 다음과 같은 결과를 얻었다. 1) 인삼 투여 후 30일 까지는 성별에 관계 없이 인삼이 몸 무게 증가에 별로 영향을 미치지 않았다. 2) 인삼 투여 30일 이후 부터는 성별에 관계없이 인삼이 흰쥐의 몸 무게를 현저하게 증가시킨다. 3) 실험시작 30일 이후의 인삼군 수컷의 몸 무게는 인삼군 암컷의 그것 보다 현저하게 증가하였다. 위의 결과들로 미루어 인삼은 흰쥐의 몸 무게를 증가시키는데 촉진적 영향을 미친다고 사료된다.

인삼이 흰쥐의 물 및 먹이 섭취에 미치는 영향 (Influence of Panax Ginseng upon Water and Food Intake in Rats)

  • To see if the Panax Ginseng has any influence upon the amount of water and food intake, an experiment was carried out using 30 male rats. The animals were divided equally into the ginseng and the saline groups. For 5 days, animals belonging to the ginseng group received every day 0.5 ml/100g body weight of ginseng extract (4 mg of ginseng alcohol extract in 1 ml of saline), while animals belonging to the saline group received the same amount of saline. After the last medication on the 5th day, each animal was accommodated in a cage which was provided with a food well and a water supplier made of glass tube bent $30^{\circ}$ at the tip. The amounts of water and food consumed in the daytime (from 6 a.m. to 6 p.m.) and at night (from 6 p.m. to 6 a.m. next morning) were measured for 2 days. The ginseng group tended to consume water and food slightly more than the saline group did. However, the difference between the 2 group was far from reaching significancy. The influence of Panax Ginseng upon water and food intake was not evident.

우기종저(牛氣腫疽)에 대한 예방약(豫防藥)과 항혈청(抗血淸)의 검정(檢定)을 위한 연구(硏究) (Studies on the Biological Assay of Black leg Vaccine and Antiserum)

  • Throughout the studies the following experimental results were summarized. 1. It was impossible to infect and kill the mice, weighing 10 to 12 gm, by inoculating 0.2ml of virulent Cl. chauvoei, diluted 1 to 10 with physiological saline, via subcutaneous, intramuscular, intraperitoneal or intraveonus, route. 2. The mice which were inoculated in brain with 0.03ml of Cl. chauvoei diluted 1 : 5120 with physiological saline were resulted in all death after infection, but not in case of attenuated strain even in dilution of one to five. 3. Virulent Cl. chauvoei were diluted with each of those of whole blood, erythrocytes and serum of horse, calf, swine, sheep, rabbit, guinea pig, chicken and duck, human plasma and 2% CaCl solution, and inoculated subcutaneously 0.25 to 0.5ml in mice, weighing 12 to 15gm. It was resulted in significant increase in virulence as comparing with the case of physiological saline solution except when horse and pig sera were used. Such a phenomena were not seen in attenuated strain. 4. Virulence of virulent Cl. Chauvoei could be increased significantly in rat, as the procedures used in mice, by suspending in whole blood, erythrocytes, serum, or plasma of various animals, or 2% $CaCl_2$ solution and by inoculating subcutaneously 0.5 to 10ml in rat, weighing 30 to 60 gm, as compared with those of control group which used physiological saline solutionos diluent. 5. Mice resisted 100 and 80 percent against challenge of $10^3$ and $10^4$ M.L.D.. respectively, 24 hours after inoculation of 0.5ml black leg antiserum. 6. Immune response to the black leg living vaccine in mice could be obtained more favorably in the group of respected vaccination rather than those of single inoculation and the most profitable inoculm size of the vacine was 0.5 to 1.0ml. 7. Challenge for the immunized mice could be carried out effectively 3 weeks after first vaccination. 8. Satisfactory results could be obtained by inoculating subcutaneously for the immunization and intracerebrally or subcutaneously for the challenge. 9. Mice which were inoculated with 0.5ml of black leg living vaccine via subtaneucously two times at seven days interval and 21 days after first inoculation and challenged with 5 and 10 M.L.D. of virulent strain, resited 100 and 70 to 80 percent respectively. Same results were obtainable in black leg killed vaccine as the procedures used in living vaccine. 10. There were significantly different resistances against the definite challenge does between the mice groups which were immnuized with the living vaccine diluted five or 10 times and the undiluted. 11. For the biological assay of black leg living vaccine and antiserum, satisfactory results could be obtained using mice.

햄스터 볼주머니를 이용한 구강점막 자극평가 조건설정 (Condition Setting for Oral Mucosal Irritation Evaluation using Hamster Cheek Pouch)

  • Objectives: The purpose of this study is to evaluate the optimum conditions for oral mucosal irritation testing using the buccal pouch of hamsters. Methods: Test materials were applied to the buccal pouch of seven-week old male Syrian hamsters (SLC, Japan) four times at one-hour intervals and macroscopic changes were examined at 24 hours after final treatment. After sacrifice, the buccal pouches were removed and prepared for histopathological evaluation. In order to set the exposure time, we performed exposure tests of 5, 12, 18 and 23 minutes using sodium lauryl sulfate (SLS) 1% and set the treatment volume from the test results at 2, 3, or 4 ml treatment using SLS 1%, Triton X-100 1% and ethanol. After setting the experimental conditions, seven groups of materials [sodium lauryl sulfate (SLS) (1%), Triton X-100 (1%), hydrogen peroxide (3%), ethanol (100%), chlorhexidine (0.2%, 2%), phosphate buffer saline (PBS)] were assessed. Results: Experimental conditions of material exposure time were fixed as 18 minutes from the exposure tests of 5, 12, 18 or 23 min using sodium lauryl sulfate (SLS) 1%. Treated volume was set as 4 ml per each pouch from the test results of 2, 3, or 4 ml treatments using SLS 1%, Triton X-100 1% and ethanol. The results in terms of irritation degree were in the order of sodium lauryl sulfate (SLS) (1%) > Triton X-100 (1%) ${\fallingdotseq}$ hydrogen peroxide (3%) > ethanol (100%) ${\fallingdotseq}$ chlorhexidine (0.2%, 2%) > phosphate buffer saline (PBS). Conclusion: From this study, suitable conditions for hamster mucosal irritation testing were suggested and this method was verified through materials commonly used on oral mucosal membranes.

기관내 내독소 투여로 유도한 백서의 급성 폐손상 모델에서 surfactant가 호중구의 아포토시스에 미치는 영향 (The Effect of Surfactant on Neutrophil Apoptosis in Lipopolysaccharide Induced Acute Lung Injury in Rat)

  • 연구배경 : 급성폐손상에서 표면활성물질의 치료효과는 허탈된 폐포를 재환기시키는 작용 외에 표면활성물질이 지닌 항염증효과도 기여하는 것으로 알려져있다. 표면활성물질의 항염증효과의 기전에 호중구의 아포토시스를 촉진하는 작용이 관련되어 있을 가능성이 있다. 본 연구에서는 사람의 말초혈액 호중구와 LPS로 급성폐손상을 유발한 흰쥐의 폐포내 호중구의 아포토시스에 대하여 표면활성물질이 미치는 영향을 평가하고자 하였다. 방 법 : 생체외 실험에서는 자원자의 혈액에서 호중구를 분리하여 같은 수의 호중구 ($1{\times}10^6$)에 LPS(10, 100, 1000ng/ml), surfactant(10, 100, 1000${\mu}g$/ml), 그리고 LPS(1000ng/ml)와 표면활성물질(10, 100, 1000${\mu}g$/ml)을 혼합하여 투여하였다. 24 시간 배양한 후에 호중구의 아포토시스를 Annexin V 방법을 이용하여 분석하였다. 생체내 실험에서는 백서의 기관 내로 LPS(5mg/kg)를 투여하여 급성폐손상을 유발한 후에, 한 군은 표면활성물질(30mg/kg)을 다른 군은 생리식염수(5ml/kg)를 30 분 후에 기관 내로 투여하였다. LPS 투여 24 시간 후에 기관지폐포세척술을 시행하여 기관지폐포세척액을 얻었으며, 여기에서 호중구를 분리하여 Annexin V 방법으로 아포토시스를 측정하였다. 또한 LPS 투여 전과 23 시간 후에 one chamber body plethy smography를 이용하여 호흡역학(일호흡량, 호흡수, Penh)의 변화를 측정하였다. 결 과 : 생체외 실험에서 LPS 투여는 사람 말초혈액 호중구의 아포토시스를 억제하였다(대조군; $47.4{\pm}5.0%$, LPS 10ng/ml; $30{\pm}10.9%$, LPS 100ng/ml; $27.5{\pm}9.5%$, LPS 1000ng/ml; $24.4{\pm}7.7%$). 낮은 농도의 표면활성물질 투여는 LPS 투여에 의해 억제 되었던 호중구의 아포토시스를 촉진시켰다(LPS 1000ng/ml=Surf 10${\mu}g$/ml 1; $36.6{\pm}11.3%$, LPS 1000ng/ml+Surf 100${\mu}g$/ml 1; $41.3{\pm}11.2%$). 높은 농도의 표면활성물질($1,000{\mu}g/ml$) 투여는 그 자체도 호중구의 아포토시스를 억제할 뿐만 아니라($24.4{\pm}7.7%$) LPS의 항아포토시스 작용을 촉진하였다(LPS 1000ng/ml+Surf $1000{\mu}g/ml$; $19.8{\pm}5.4%$). 생체내 실혐에서 표면활성물질 투여는 생리식염수 투여에 비해서 급성폐손상을 받은 쥐에서 기관지 폐포세척액의 호중구의 아포토시스를 촉진하였다($6.03{\pm}3.36%$ vs $2.95{\pm}0.58%$). 표면활성물질을 투여한 백서는 생리식염수를 투여한 백서에 비해 LPS 자극 후 23 시간에 측정한 기도저항(Penh) 이 낮았다($2.64{\pm}0.69$ vs $4.51{\pm}2.24$, p<0.05). 결 론 : 이상의 결과로 표면활성은 사람의 말초혈액 호중구와 급성폐손상을 받은 백서의 기관지폐포세척액 내의 호중구의 아포토시스를 촉진하며, 이런 효과가 표면활성불질의 항염증효과의 기전 중 하나일 것으로 추측된다.

누에번데기 및 누에애벌레 밀리타리스동충하초 (Cordyceps Militaris) 열수추출물이 Sarcoma-180 세포로 유발한 마우스 복수암에 미치는 영향 (Inhibitory Effect of Cordyceps Militaris Water Extracts on Sarcoma-180 Cell-Induced Ascities Tumor in ICR Mice)

  • This study was undertaken to evaluate the antitumor activities of Cordyceps militaris of silkworm pupa (CMP) and silkworm larva (CML), as compared with the effect of cordycepin, an active compound found in Cordyceps militaris. Antiproliferation effect of the test materials were evaluated in the sarcoma-180 cells using the MTT test. For the in vivo study, ICR mice were inoculated i.p. with 1.0 ${\times}$ 10$^{6}$ sarcoma-180 cells/mouse on Day 0, and were again i.p. injected with one of the following substances from Day 1 to Day 10 : saline (control group), 50 mg/kg (CMP50, CML50) ,100 ma/kg (CMP100, CML100), or 200 mg/kg (CMP200, CML200) of Cordyceps militaris water extracts, or 1 mg/kg (C1), 2 mg/kg (C2), or 4 mg/kg (C4) of cordycepin. Pretreatment of the sarcoma-180 cells with 100 mg/ml, 500 mg/ml, and 1000 mg/ml of CML (60.1$\pm$2.5%, 49.8$\pm$3.7%, and 45.4$\pm$0.1% of the value for untreated control cells, respectively) or CMP (68.3$\pm$2.1%, 55.1$\pm$0.9%, and 51.4$\pm$3.5% of the value for control cells, respectively) for 48 hrs significantly decreased the survival rate (proliferation) of tumor cells (p<0.05). Body weight of the control mice bearing ascites tumor and injected with saline was 1.4 times of the value for normal animals at day 18. Mice bearing ascites tumor and injected with cordycepin (1, 2, or 4 mg/kg) exhibited a significantly lighter body weight compared with the control mice, while animals injected with CMP or CML (50, 100, or 200 mg/kg) showed a significantly lighter body weight compared with the mice injected with cordycepin. Mice injected with CMP50, CMP100, or CMP200 mg/kg (or CML50, CML100, or CML200 mg/kg) showed a 133% (or 90%), 80% (or 62%), and 68% (or 52%) longer mean survival time, and those treated with C1, C2, or C4 exhibited a 54%, 91% and 80% longer survival time compared to the value for control mice injected with saline. These results indicate that the hot-water extracts of Cordyceps militaris of both silkworm pupa and silkworm larva have an anti-proliferation effect of tumor cells as well as the life prolongation effect in mice bearing ascites tumor, which are superior to the activities of cordycepin.

복강경 담낭절제술에서 연령과 성별이 고려될 때 수술 전 Dexamethasone의 투여가 수술 후 통증에 미치는 영향 (The Effect of Preoperative Dexamethasone Administration, according to Age and Gender on Postoperative Pain in Patients who Undergo Laparoscopic Choelecystectomy)

  • Background: Preoperative dexamethasone improves the surgical outcome after laparoscopic cholecystectomy(LC). The purpose of this study was to determine the effect of preoperative dexamethasone on the postoperativepain according to age and gender in patients who undergo LC.Methods: In this double blind prospective study, 400 patients, males or females :< 45 yr and males or femaless 65 yr (n = 50 in each of eight groups) who undergoing LC were randomized to receive dexamethasone 8mg (5 ml) or saline 5 ml intravenously 100 minutes before their operation, Postoperative pain was assessedon a visual analog scale (VAS) at 1, 6, 12, and 24 hour, and the time to administering the first postoperativeanalgesics was recorded.Results: Dexamethasone was administered without consideration for age and gender, and it reduced thepostoperative pain VAS score at 1, 6, and 12 hours, and the opioid analgesic requirement, but there was nosignificant difference between administering saline or dexamethasone in the same gender and age groups.Females U 45 yr who were administered saline had the most pain sensitivity and males S 65 yr who wereadministered dexamethasone had the least pain sensitivity.Conclusions: Preoperative dexamethasone reduces the pain intensity and opioid consumption, but does notreduce the pain intensity, according to age and gender in the patients undergoing LC. As a result, Preoperativedexamethasone should be considered for routine use for patients who are undergoing laparoscopic cho-lecystectomy. (Korean J Pain 2008; 21: 51 56)

인삼이 흰쥐의 골수조직 핵산 함유량에 미치는 영향 (Influence of Ginseng upon Nucleic Acid Content of Bone Marrow in Rats)

  • 인삼이 흰쥐의 골수조직 핵산 함유량에 어떤 영향을 미치는지를 알기 위하여 30마리의 흰쥐($180{\sim}230\;gm$) 수컷을 인삼군과 식염수군으로 나누어 다음과 같은 실험을 하였다. 인삼군에는 몸무게 100gm에 대하여 인삼주정추출액 (생리적 식염수 1ml 속에 4 mg의 인삼주정추출물이 함유되게 만든 용액)을 0.5ml의 비율로 매일 등뒤 피하에 5일 동안 주사하였으며, 식염수군에는 생리적 식염수를 인삼군에서 한 것과 동일한 방법으로 주사하였다. 인삼주정추출액 혹은 식염수 투여가 시작된지 제 5일째 되는 날에는 해당 약물을 투여한 2시간 후에 동물을 도살하여 대퇴골 골수조직을 적출하고, 이 조직의 핵산 함유량을 Shmidt-Thannhauser-Scheider의 화학적 정량법을 이용하여 측정하였다. 이들 측정치를 지표로 하여 인삼이 골수조직 핵산량에 미치는 영향을 관찰한 바 다음과 같은 결과를 얻었다. 1. 인삼군의 골수조직 RNA 및 DNA 함유량은 식연수군의 그것 보다 유의하게 많았다. 2. 인삼군의 골수조직 RNA/DNA 비율은 식염수군의 그것에 비하여 현저하게 컷다. 위의 결과로 미루어 보건대, 인삼은 골수조직 RNA 및 DNA 함유량을 유의하게 증가시킴은 물론 RNA/DNA 비율도 증가시킨다고 추리된다.
