Effect of Pressure Based Customized 3-Dimensional Printing Insole in Pediatric Flexible Flat Foot Patients (소아 편평족 환자에서 Pressure Based Customized 3-Dimensional Printing Insole의 유용성)

  • Purpose: A flatfoot that fails to form a longitudinal foot arch is a common lower limb deformity in children. This study evaluated the structural and functional effects of the insole for pediatric flexible flat foot (PFFF). Materials and Methods: Twenty-nine PFFF patients (20 boys and 9 girls, 58 feet) with bilateral symptomatic flatfoot deformities between February 2017 and May 2019 were included in this study. Sixteen patients (32 cases, study group) were treated with a pressured based 3-dimensional printing insole, and 13 patients (26 cases, control group) were followed up regularly without any treatment. Flatfoot was diagnosed by a lateral talo-first metatarsal angle of more than 4° in convex downward and talocalcaneal angles of more than 30° and a calcaneal pitch of less than 20°. The foot pressures, including the midfoot pressure, total foot pressure, and the ratio of the midfoot pressure to the total foot pressure, were evaluated by pedobarography. The clinical scores were assessed using the visual analogue scale (VAS), American Orthopaedic Foot and Ankle Society (AOFAS), and Pediatrics Outcomes Data Collection Instrument (PODCI) scores. Results: The mean age of the study group was 9.16 years, and the mean age of the control group was 7.73 years. The mean follow-up period was 16 months. The change in the lateral talocalcaneal angle was -4.664°±1.239° in the study group and -0.484°±1.513° in the control group. A significant difference in the amount of change of the lateral talocalcaneal angle was observed between the two groups (p=0.034). The midfoot pressures were similar in the two groups. Conclusion: Pressure based customized 3-dimensional printing insole in PFFF may have some effect on the hindfoot bony alignment, but it does not affect the changes in midfoot pressure.

Surgical Treatment for Longitudinal Epiphyseal Bracket of the Finger (수지의 종적 Bracket 골단에 대한 수술적 치료)

  • Purpose: To describe the clinical and radiology results of a surgical treatment for clinodactyly due to a longitudinal epiphyseal bracket. Materials and Methods: This study analyzed the records of 11 patients (27 case) with clinodactyly due to a longitudinal epiphyseal bracket who underwent an osteotomy or physiolysis between March 1999 and April 2017. The preoperative range of motion of the proximal and distal interphalangeal joints, the subjective satisfaction of the patient, and the degree of angulation to the ulnar side were examined. The patients were classified into two groups: osteotomy and physiolysis. The results were reviewed retrospectively and compared according to the surgical method. Results: The mean age of the patients who underwent osteotomy was 10.3 years. The average preoperative angle was 25.7° and the average postoperative angle was 13.5° the mean correction rate was 47.4%. The mean age of the patients who underwent physiolysis was 6.0 years the mean preoperative angle was 24.5° and the postoperative angle was 10.7°. The average correction rate was 59.4%. No significant difference in the correction angles was observed between the group who underwent the correction osteotomy and the group who underwent the osteotomy. In each group, the postoperative correction was statistically significant. The range of motion at the last follow-up was not significantly different from the preoperative range of motion. Conclusion: In patients with clinodactyly due to longitudinal epiphyseal bracket, osteotomy or physiolysis may be performed selectively according to age, bone age, and radiological progression. Both surgical methods showed good clinical results and deformity correction.

Complications of Scarf Osteotomy for Hallux Valgus (무지 외반증에서 시행한 Scarf 절골술의 합병증)

  • Purpose: The purpose of this study was to evaluate the frequency of troughing and stress fracture, which are the major complications of scarf osteotomy, and to suggest methods to prevent these complications. Materials and Methods: We reviewed 243 cases of 137 patients treated with the scarf osteotomy for hallux valgus from January 2005 to December 2012. The mean follow-up period was 2.8 years. During the scarf osteotomy, a long oblique longitudinal osteotomy was performed in order to decrease the possibility of troughing and stress fracture. Radiographs of lateral view of the foot were obtained and the thicknesses of the first metatarsal base at the sagittal plane were measured and compared. Results: There was no troughing during fragment translation and screw fixation intraoperatively. Radiographs of lateral view of the foot taken preoperatively and at the last follow-up showed that the mean thickness of the first metatarsal was 22.4 mm preoperatively and 21.6 mm at the last follow-up, with a mean difference of 0.8 mm. And no stress fracture was observed. Conclusion: To prevent troughing and stress fracture, a long oblique longitudinal cut, parallel to the first metatarsal plantar surface, was performed, making both ends of the proximal segment truncated cone-shape, and securing the strong bony strut of the proximal segment. No troughing or stress fracture was experienced with scarf osteotomy.

Water Resistance Evaluation of the Oils Coating for Conservation of Wooden Cultural Heritage (목조문화재 보존을 위한 유지류 코팅제의 방수 성능 평가)

  • Surface of the wooden cultural heritage has been protected from moisture by natural oils (such as perilla oil and tung oil), which accelerated degradation. But we can find seldom the research on the processes and performances of oil coatings. In this study, the water resistant performances by wood direction were compared to 3 types of natural oil and 2 types of oil stain, and the effect of additional indoor conditioning and temperature of oil were appraised in longitudinal direction. The natural oils block moisture about 79.2% comparing to the control in longitudinal direction. Especially the tung oil showed the outstanding water resistance about 90.5%. The water resistant performances decreased about 8.8% by additional indoor conditioning for 2 years. Heated oil improved the water resistance about average 5.0%.(Max. 9.6% with linseed oil).

Longitudinal music perception performance of postlingual deaf adults with cochlear implants using acoustic and/or electrical stimulation

  • In this study, we investigated longitudinal music perception of adult cochlear implant (CI) users and how acoustic stimulation with CI affects their music performance. A total of 163 participants' data were analyzed retrospectively. 96 participants were using acoustic stimulation with CI and 67 participants were using electrical stimulation only via CI. The music performance (melody identification, appreciation, and satisfaction) data were collected pre-implantation, 1-year, and 2-year post-implantation. Mixed repeated measures of ANOVA and pairwise analysis adjusted by Tukey were used for the statistics. As result, in both groups, there were significant improvements in melody identification, music appreciation, and music satisfaction at 1-year, and 2-year post-implantation than a pre-implantation, but there was no significant difference between 1 and 2 years in any of the variables. Also, the group of acoustic stimulation with CI showed better perception skill of melody identification than the CI-only group. However, no differences found in music appreciation and satisfaction between the two groups, and possible explanations were discussed. In conclusion, acoustic and/or electrical hearing devices benefit the recipients in music performance over time. Although acoustic stimulation accompanied with electrical stimulation could benefit the recipients in terms of listening skills, those benefits may not extend to the subjective acceptance of music. These results suggest the need for improved sound processing mechanisms and music rehabilitation.

Prognostic Role of Right VentricularPulmonary Artery Coupling Assessed by TAPSE/PASP Ratio in Patients With Acute Heart Failure

  • BACKGROUND: Right ventricular (RV) dysfunction is a significant risk of major adverse cardiac events in patients with acute heart failure (AHF). In this study, we evaluated RV-pulmonary artery (PA) coupling, assessed by tricuspid annular plane systolic excursion (TAPSE)/pulmonary artery systolic pressure (PASP) and assessed its prognostic significance, in AHF patients. METHODS: We measured the TAPSE/PASP ratio and analyzed its correlations with other echocardiographic parameters. Additionally, we assessed its prognostic role in AHF patients. RESULTS: A total of 1147 patients were included in the analysis (575 men, aged 70.81 ± 13.56 years). TAPSE/PASP ratio exhibited significant correlations with left ventricular (LV) ejection fraction(r = 0.243, p < 0.001), left atrial (LA) diameter(r = -0.320, p < 0.001), left atrial global longitudinal strain (LAGLS, r = 0.496, p < 0.001), mitral E/E' ratio(r = -0.337, p < 0.001), and right ventricular fractional area change (RVFAC, r = 0.496, p < 0.001). During the median follow-up duration of 29.0 months, a total of 387 patients (33.7%) died. In the univariate analysis, PASP, TAPSE, and TAPSE/PASP ratio were significant predictors of mortality. After the multivariate analysis, TAPSE/PASP ratio remained a statistically significant parameter for all-cause mortality (hazard ratio [HR], 0.453; p = 0.037) after adjusting for other parameters. In the receiver operating curve analysis, the optimal cut-off level of TAPSE/PASP ratio for predicting mortality was 0.33 (area under the curve = 0.576, p < 0.001), with a sensitivity of 65% and a specificity of 47%. TAPSE/PASP ratio < 0.33 was associated with an increased risk of mortality after adjusting for other variables (HR, 1.306; p = 0.025). CONCLUSIONS: In AHF patients, TAPSE/PASP ratio demonstrated significant associations with RVFAC, LA diameter and LAGLS. Moreover, a decreased TAPSE/PASP ratio < 0.33 was identified as a poor prognostic factor for mortality.

  • This study investigates the last twenty two years of research trends in the burnout of psychiatric ward nurses and suggests guidance for future studies. Five domestic databases were used in the literature search, and fourteen articles that met the inclusion criteria were reviewed. thirteen articles were descriptive, cross-sectional studies and one study was qulitative study. Among the quantitative study, the most used instrument to measure burnout of nurses working in psychiatric wards was the Maslach Burnout Inventory (MBI). The individual factors related to burnout in nurses in psychiatric wards were age, marital status, years of clinical experience psychiatric mental health nurse license status. monthly income, religious status and education related to psychiatry. The most related situational factor that affect burnout in psychiatric ward nurses was experience of violence. In future research, large-scale cross-sectional and longitudinal studies should be conducted, and based on this, intervention studies should be conducted on reducing the burnout of psychiatric ward nurses.

A Case Study on Freshcode for the Food Online Platform Business: A Focus on the Lean Start-Up (푸드 온라인 플랫폼 비즈니스 프레시코드 사례: 린 스타트업 방식을 중심으로)

  • Food delivery service combined with IT technology and HMR (Home Meal Replacement) are rapidly growing due to the COVID-19. Recently, the demand for salads along with HMR has increased among office workers in their 20s and 30s who are interested in health and beauty. Freshcode is a food startup with 6 years of experience that started selling salad products through O2O service. Freshcode applied for a patent for a service that collects orders from nearby areas and delivers them on the same day to a designated delivery address 'FCOSPOT' to save shipping costs. In March 2021, in recognition of the growth potential of the regular delivery service, Freshcode received an investment of 6 billion won in Series A. This study may have practical implications to early-stage startups and scale-up stage startups through a longitudinal case study on the growth of a single company. As for the research method, the lean startup methodology and lean canvas were used in the early stage of startup. In particular, the process of the build-measure and learn feedback-loop, which is the core of lean startup methodology, was applied to each major decision-making step. In the scale-up stage after 5 years, the business model canvas was used to schematize the growth as a food online O2O platform to verify continuous innovation. This case study has three main findings. First, the idea of 'FCOSPOT' was successfully implemented through the Lean Startup methodology. Second, Freshcode demonstrated the scalability of the differentiated business model of shared base delivery O2O. Third, a key factor of success was the digital integrated communication operation strategy that maximizes the experience for the created customers.

Psychosocial Factors Associated with Problematic Drinking among Middle- and Older-Aged Korean Men (중년 및 노년 남성의 문제음주 관련 요인)

  • Objectives : This study was conducted to examine social factors associated with problematic drinking among middle- and older-aged men in South Korea. Methods : The data were collected from the baseline survey of the Korean Longitudinal Study of Aging, and the sample consisted of 3,631 men between 45 and 64 years of age and 1,173 men aged 65 or older. This study employed variables identifying various socioeconomic features, social ties, and health-related factors. Logistic regressions were used for the data analysis. Results : Middle-aged men having frequent social relationship with close persons were significantly more likely to be problematic drinkers than those having less frequent social relationships with close persons. Lower education, unemployment, smoking, and depression were associated with problematic drinking only among middle-aged men. Conclusions : We suggest that health professionals develop specific intervention strategies that could lead to more moderate alcohol habits and better health in middle aged Korean men who are actively engaged in social networks. In addition, health professionals need to focus more on the unemployed, lower educated, depressed, and smoking groups for early detection of problematic drinking among middle-aged Korean men.

A Study on Adolescents' Internalizing and Externalizing Problem Behaviors and Related Variables in Transition with Latent Growth Model (잠재성장모형을 활용한 청소년 전환기 내면화 및 외현화 문제행동과 관련변인에 관한 연구)

  • This study investigated how variables of internalizing and externalizing problematic behaviors change according to gender and as time passes by and how the variables influence adolescent problematic behaviors. The variables selected for the analysis are personal variables, parent variables, peer and school variables, and community variables. longitudinal data collected for 4 years from the Korea Youth Panel Survey(KYPS) were utilized for the analysis. Data were collected initially from 2,707 fifth grade elementary students in 2005 and they were traced until 2008. The final respondents were 2,448 students. The findings are as follows. Frist, the statistical significance was found in changes of problematic behavioral variables in terms of the gender difference. Second, variables, such as self-esteem and self-control are negatively correlated to the problematic behaviors and stress level is strongly positively correlated to the behaviors. Third, the study pressure and peer attachment level are correlated to the initial value of internalizing problematic behaviors. In conclusion, given that more statistical significances were found at initial values than the change rates among variables, early intervention is important in addressing adolescent problematic behaviors.
