A novel mutation of ABCC8 gene in a patient with diazoxide-unresponsive congenital hyperinsulinism

  • Congenital hyperinsulinism (CHI) is a rare condition that can cause irreversible brain damage during the neonatal period owing to the associated hypoglycemia. Hypoglycemia in CHI occurs secondary to the dysregulation of insulin secretion. CHI has been established as a genetic disorder of islet-cell hyperplasia, associated with a mutation of the ABCC8 or KCNJ11 genes, which encode the sulfonylurea receptor 1 and the inward rectifying potassium channel (Kir6.2) subunit of the ATP-sensitive potassium channel, respectively. We report the case of a female newborn infant who presented with repetitive seizures and episodes of apnea after birth, because of hypoglycemia. Investigations revealed hypoglycemia with hyperinsulinemia, but no ketone bodies, and a low level of free fatty acids. High dose glucose infusion, enteral feeding, and medications could not maintain the patient's serum glucose level. Genetic testing revealed a new variation of ABCC8 mutation. Therefore, we report this case of CHI caused by a novel mutation of ABCC8 in a half-Korean newborn infant with diazoxide-unresponsive hyperinsulinemic hypoglycemia.

Changes in lipid component and quantitative measurement of carbonyl compound during Doenjang fermentation (된장 숙성 중 지질의 변화 및 카보닐 화합물의 함량 변화)

  • Conventional Doenjang, improved Doenjang prepared with Asp. oryzae were made to investigate the changes in the lipid content, and the carbonyl compounds during fermentation. Total lipid contents of conventional Doenjang increased slowly during fermentation, and that of the improved Doenjang increased at first, but showed sharp decrease at moment and then increased. Triglyceride contents of all samples decreased remarkedly during fermentation. Conversely, free acid contents increased. From the result of quantitative analysis of fatty acid by gas chromatography, saturated fatty acid ratio of total lipid in conventional Doenjang increased at early stages and then decreased, but unsaturated fatty acid ratio showed the reverse phenomenon. Saturated fatty acid in improved Doenjang increased during the fermentation but unsaturated fatty acid decreased by degrees. The concentration of total and monocarbonyl compounds in the fermented Doenjang were comparably higher than that found in raw Boenjang. Sensory evaluation revealed that off flavor had a negative effect on overall eating quality of Doenjang and total carbonyl content was related to the off flavor.

Phytolacca Radix Poisoning: A Case Report (자리공 뿌리 섭취로 발생한 급성 중독 1례 : 증례보고)

  • Objectives: This study aims to present the case of poisoning associated with Phytolacca radix. Case Presentation: A 61-year-old male with nausea, vomiting, dizziness, and abdominal pain after ingesting about 10 centimeters of Phytolacca radix visited Korean medicine hospital. He began to show symptoms one hour after eating the plants. Methods: The patient was diagnosed with poisoning by Phytolacca radix. The patient received acupuncture, moxibustion, and herbal medicine treatment including Glycyrrhizae radix. laboratory test including complete blood count, biochemistry was also conducted. Results: Treatment, including acupuncture, moxibustion, and herbal medicine, improved his symptoms. His symptoms subsided within two hours. The laboratory test conducted on the next day show a slight increase in his white blood cell, blood urea nitrogen, urine ketone levels. There was no damage to the liver or kidneys. Conclusions: This report demonstrates the need for caution when consuming wild plants, which could lead to serious adverse effects. Patients should be alerted to the indiscriminate ingestion of wild plants. Further research on how to treat poisoning in Korean medicine needs to be considered to confirm these findings.

Volatile Flavor Components in Soybean Sprouts (콩나물의 향기 성분 분석)

  • The identification of volatile constituents in foods is important in creating flavor compounds to improve the flavor of foods. This study was conducted to identify the flavor compounds in soybean sprouts cultivated with 4 different types of soybean seeds. A total of 52 flavor compounds were identified and composed mainly of alcohols (16), aldehydes (17), ketones (10), acids (2), furans (2), and miscellaneous compounds (5). Sprouts cultivated with Dawonkong and Orialtae showed 46 flavor compounds whereas Pungsannamulkong and Nokchaekong was 49 and 50. In total flavor compounds contents, Orialtae was the highest (19.3 mg/kg RC) and followed by Pungsannamulkong (15.83 mg/kg RC), Dawonkong (13.2 mg/kg RC), and Nokchaekong (11.3 mg/kg RC) in that order. Two groups including alcohols and aldehydes were detected high amounts in which their ratio were analyzed 32% and 51% in total flavor contents, respectively. It may be responsible for flavor in soybean sprouts. In case each flavor compound content, 2-hydroxybenzaldehyde was detected the major compound and hexanol, 1-octen-3-ol, and hexanal that the main compounds in lipid oxidation of soybean products were identified the main volatile flavor compounds in soybean sprouts.

Diminution of Pesticide Residues on Crops and Soil by Accelerated Photolysis (광분해(光分解) 촉진(促進)에 의한 작물(作物) 및 토양중(土壤中) 농약잔유물(農藥殘留物)의 경감(輕減)에 관한 연구(硏究))

  • In an effort to reduce artifically the residual pesticides on crop and soil by accelerated photolysis,some 40 among the naturally occurring and synthetic coumpound were screened for photosensitization and/or photocatalysis and six promising chemicals were selected.The fungicides and the four selected photosensitizers and/or photocatalyst (PS) were applied to each crop.The results obtained are summarized as follows. 1. The wavelengths of maximum absortion (${\lambda}$max) and the molar absorptivites (${\varepsilon}$max) of procymidone,vinclozolin,and carbendazim in acentone were all 209 nm and 853,854,and 8740 respectively. 2. Of the 40 naturally-occuring and synthetic compounds screened,six promising ones were selected and designated as PS-1 (aromatic ketone),PS-2(aromatic amine)PS-3(quinone) ,PS-4 (inorganic compound),PS-5(organic acid salt) and PS-6(semiconductor photocatalyst). 3. In the accelerated photolysis of pesticide in soil by applying PS ,procymidone was decoposed rapidly by virtue of PS-2,being 59% of the control 3 days after application. 4. The vinclozolin residue in soil was reduced to 71% and 21% of the control 1 and 15 days,respectively,after PS-2 application. 5. The photolysis of carbendazim in soil was not accelerated by any of the PS tested. 6. The pesticide residues on the crop were prominently reduced by PS application.The procymidone residue on tomato was reduced to 47% of the control 15 days after PS-1 application and that on red pepper reduced to 57% 15 days after PS-2 application. 7. Vincrozolin residus remaining on tomato 1 and 15 days after PS-2 application were 38% and 56% of the control whereas those on the red pepper were 82% and 64%,respectively. 8. PS-2 was the most effective for the accelerated photolysis of carbendazim residues remaining on tomato, whereas on red pepper, the four of PS tested were all effective, but did not make much difference between them. This might be due to the shielding of sunlight by the leaves of red pepper not to exert the photosensitizing effect of PS-2 to the full.

The effects of Sam-chool-gun-bi-tang administration on urine metabolic responses during carbohydrate loading diet to long distance runners (탄수화물 부하식이요법시 삼출건비탕(蔘出健脾湯) 투여가 시합 전·후 장거리 달리기 선수의 뇨중 대사 성분 변화에 미치는 영향)

  • Objective This experimental study was designed to investigate effects of Sam-chool-gun-bi-tang administration during carbohydrate loading diet to long distance runners on changes of the urine metabolic responses. Materials and Methods All subjects were divided randomly with two groups, Sam-chool-gun- bi-tang administration Group (N=5) and control group (N=5) and attended on domestic marathon race. The urine samples were collected by 15ml conical tube at rest, a day before race, immediately after race, a day after race. These samples were used to analyze for the factors of the change on metabolic responses. At First, the factors of the routine urine test were checked; RBC, WBC, glucose, protein, ketone, bilirubin, urobilirubin. Next, the factors on the changes of the electrolyte metabolism and creatinine were checked; $Na^+ $, $Cl^-$, $K^+$, $Ca^{+ +}$, Creatinine. Results 1. Urine RBC, WBC, glucose, protein, ketone, bilirubin, urobilirubin were all denoted negatively (-). 2. Urine pH response in Sam-chool-gun-bi-tang administration group was shown significant difference between two groups at immediately after race. 3. Urine $Na^+$ response in Sam-chool-gun- bi-tang administration group was shown significant difference between two groups at immediately after race. 4. Urine $K^+$ response in Sam-chool-gun-bi-tang administration group was shown significant difference between two groups at immediately after race and a day after race. 5. Urine $Cl^-$ response in Sam-chool-gun- bi-tang administration group was increased at immediately after race, but was not shown significant difference between two groups. 6. Urine $Ca^{+ +}$ response in Sam-chool-gun-bi-tang administration group was decreased gradually, but was not shown significant difference between two groups. 7. Urine Creatinine response in Control group was decreased significantly at immediately after race, but was not shown significant difference between two groups. Conclusion According to the above results, it was shown that Sam-chool-gun- bi-tang administration had the positive effects on changes of the urine matabolic responses during carbohydrate loading diet to the long distance runners.

Exposure Assessment of Volatile Organic Compounds for Workers Handling Rust-preventive Oils (방청유 취급 근로자의 휘발성 유기화합물 노출 평가)

  • Objectives : This study was conducted to evaluate the level of exposure to volatile organic compounds (VOCs) among workers handling rust preventive oils. Methods : A total of 30 bulk samples and 54 personal air samples were collected using diffusive samplers at 22 workplaces handling rust preventive oils in Daegu and Gyongsangbuk-do Province from March to October 2013. We also investigated detailed information on the related work conditions, such as kinds of products, handling methods, local exhaustive ventilation systems, and the status of the wearing personal protective equipment. All bulk samples and air samples were analyzed using gas chromatography mass spectrometry (GC-MS) to identify components to which workers potentially were exposed. Quantitative airborne concentrations of VOCs were confirmed using gas chromatography with flame ionized detectors. Results : In terms of qualitative analyses for the 30 bulk samples, we found carcinogenic, mutagenic and reproductive toxic(CMR) substances such as butane(carcinogenic Group 1A, mutagenic Group 1B), butoxy ethanol(carcinogenic Group 2), cumene (carcinogenic Group 2), ethyl benzene(carcinogenic Group 2), methyl isobutyl ketone(carcinogenic Group 2) and toluene (reproductive toxic, Group 2). As a result of full-shift based personal air samples, eight substances such as n-hexane, n-heptane, octane, nonane, decane, toluene, ethyl benzene and xylene were detected. Among them, n-hexane and n-heptane were detected in all of 54 air samples with $13.13mg/m^3$ and $8.61mg/m^3$ of maximum concentration, respectively. The level of airborne concentration from all of samples were bellow the occupational exposure limit in Korea. Conclusions : Based on the results of this study, workers handling rust preventive oils could be exposed to CMR substances contained in rust preventive oils and n-hexane and n-heptane were found as the most frequent sources of VOC exposure.

A Study on the Adsorption Effect of Korean zeolite "Clinoptilolite" as Cigarette Cavity Filter Additive. (한국산 Zeolite의 필터첨가제로서의 흡착효과에 관하여)

  • The cavity of triple filter was filled with the mixture of clinoptilolite and charcoal(1:1, V/V). The particle size of clinoptilolite was 30$\pm$5 A.S.T.M mesh. The reduction effects of the important gaseous components by this mixture were obtained as follows: 1) In comparison with the normal cellulose acetate niter, the contents of nicotine and T.P.M. were reduced about 35% and 22% respectively. 2) Many aliphatic and cyclic compounds were also substantially reduced in an average of 60%. 3) In contrast with the charcoal, the removal efficiency of clinoptilolite was revealed as higher (15-20%) in case of aliphatic compounds than the one (10-15%) of cyclic compounds. The above results showed us that the removal function of gaseous components was quite complementary each other (charcoal and clinoptilolite).

Analysis and Assessment by Thermal Desorption Method of Mixed Organic Solvents Collected on Activated Carbon(AC) and Activated Carbon Fiber(ACF) (AC 및 ACF에 포집된 혼합 유기용제의 열탈착 방법에 따른 분석 및 평가)

  • This study was conducted to evaluate desorption efficiencies accuracy and precision by $CS_2$ and thermal desorption method for polar and non-polar organic solvents collected on activated carbon(AC), activated carbon fiber(ACF), carbosieve SIII, materials tested were Methyl alcohol, n-Hexane, Benzene, Trichloroethylene, Methyl isobutyl ketone and methyl cellosolve acetate and six different concentration levels of samples were made. The results were as follows ; 1. Accuracy on kind adsorbent and desorption method was low. In case of $CS_2$ desorption solvent, Overall B and Overall CV on AC and ACF were 43% and 6.63%, respectively. In case of thermal desorption method, accuracy of thermal desorption method appeared higher than solvent desorption method by AC 18.0%, 3.54%, ACF 2.6%, 2.57%, Carbosieve SIII 13.7% and 1.97%, respectively. 2. In the concentration level III, accuracy of thermal desorption method on adsorbent was in order as follow ; ACF > Carbosieve SIII > AC in the methyl alcohol and Carbosieve SIII > ACF > AC in the rest of them all subject material and Concentration levels showed good precision at EPA recommend standard (${\leq}{\;}30%$) 3. DEs by type of organic solvent adsorbent and desorption method are as follows ; In the case that desorption solvent is $CS_2$, DE of Methyl alcohol is AC 47.5%, DE of all materials is ACF about 50%. In the case of thermal desorption method, DE of Methyl alcohol is AC 82.0%, ACF 97.4%, Carbosieve SIII 86.3%. DE of the later case is prominently improved more than one of former. In particular, Except that DE of EGMEA is ACF 88.5%, DE of the rest of it is more than 95% which is recommend standard MDHS 72. With the result of this study, in order to measure various organic solvent occurring from the working environment, in the case of thermal desorption method, we can get the accurate exposure assessment, reduce the cost, and use ACF as thermal desorption sorbent which available with easy.

  • Organic acid content and volatile aroma components in Jehotang were investigated. Organic acids were detceted by HPLC and it is composed of formic acid(0.07%). lactic acid(0.22%) acetic acid (0.32%), and citric acid(3.17%), the last of which is one of the noteworthy features of Jehotang contributing greatly to its sour flavor and taste. Among the 39 volatile aroma components, whose peaks were identified by GC/MSD, and whose structures were analyzable, 17 kinds of hydrocarbon(30.81%) one kind of aldehyde(7.18%), 2 kinds of ketone(4.79%), 8 kinds of terpene(25.96%) a variety of acids (16%), 2 kinds of alcohol(5.42%), 2 kinds of phenol(2.76%) and 3 kinds of the others(7.68%) were found. The hydrocarbons, terpenes and acids occupied 70% of the aroma components, contributing to and also composing the particular flavor of Jehotang. Extracted pigments from the Jehotang showed maximum light absorbance in the wave length ranges of 200~400nm, showing a high degree of light adsorption of yellow to red color.
