Thoracolumbar Spine Injury (흉요추부 손상)

  • Method of management of the spine injury should be determined, based on the status of neurological injury as well as on the presence of traumatic instability. At the thoracic and lumbar spine, patterns of neurological injury are different from the cervical spine due to their neuro-anatomical characteristics. Especially, at the thoracolumbar junction, neurological injury patterns with their respective prognosis vary from the complete cord injury or conus medullaris syndrome to the cauda equina syndrome according to the injury level. The concept of Holdsworth's instability based on the posterior ligament complex theory has evolved into the current 3-column theory of Denis. Flexion-rotation injury and fracture-dislocation are well known to be unstable that surgical fixation is frequently needed for these injuries. However, there have been some controversies for the stability of burst fractures and their treatment, such as indirect or direct decompression and anterior or posterior approach. In this article, current concepts and management of traumatic instabilities at the thoracic and lumbar spine have been reviewed and summarized.

The Usefulness of Bone Scintigraphy in SAPHO Syndrome (SAPHO 증후군에서 골신티그라피의 유용성)

  • Purpose: SAPHO syndrome is well known to various disease entities including synovitis, acne, pustulosis, hyperostosis and polyarthritis. The purpose of this study is to evaluate sicntigraphic findings and to compare with radiologic findings in SAPHO syndrome. Materials and Methods: Five patients (M:F=5.0, Age $22.8{\pm}4.78$ yrs) with SAPHO syndrome were enrolled in our study. All patients underwent whole-body bone scintigraphy with intravenous administration of 740 MBq of Tc-99m MDP. Among them, two patients were additionally perfomed SPECT of the spine to evaluate the location and extort of spinal lesion. Results: All patients were demonstrated abnormal increased uptakes in sternoclavicular joint (SC), sacroiliac joint (SI), and small joints of both hands. Among them, three patients were bilateral involvement (3/5) and two were unilateral (2/5) in SC. involvement of SI showed bilateral in four patients (4/5) and unilateral in one (1/5). SPECT images demonstrate that the lesion sites of the lumbar spine are more likely facet joints than vertebral bodies or pedicles. Conclusion: As SAPHO syndrome is the disease entity involved polyarticular joints with various dermatologic manifestations, the bone scintigraphy may be a very useful method to evaluate the location and extent of joint involvement, and to avoid inadequate surgical management or ineffective antibiotic treatment.

A Thoracolumbar Pure Spinal Epidural Cavernous Hemangioma - A Case Report - (흉요추부에서 발견된 경막외 해면상 혈관종 - 증례 보고 -)

  • Study Design: Case report. Objectives: We report a case of pure epidural cavernous hemangioma located at the thoracolumbar spine in a 53-year-old woman that mimicked a neurogenic tumor on magnetic resonance imaging (MRI). Summary of Literature Review: A pure spinal epidural cavernous hemangioma without bony involvement is a very rare lesion about which limited information is available in the literature. Materials and Methods: A 53-year-old woman visited our clinic for hypoesthesia with a tingling sensation in the left anterolateral thigh that had begun a month ago. No other neurologic symptoms or signs were present upon a neurologic examination. MRI from an outside hospital showed a $2.0{\times}0.5cm$ elongated mass at the T11-12 left neural foramen. The tumor was completely removed in piecemeal fashion. Results: The histopathologic examination revealed a cavernous hemangioma, which was the final diagnosis. The outcome was favorable in that only operation-related mild back pain remained, without any neurologic deficits, after a postoperative follow-up of 2 years and 3 months. No recurrence was observed on MRI at 2 years postoperatively. Conclusion: Pure epidural spinal cavernous hemangioma is very rare, and it is very difficult to differentiate from other epidural lesions. However, we believe that it should be included in the differential diagnosis of spinal epidural tumors due to its favorable prognosis.

Effect of Oriental Medicine Treatment on Inpatient with Thoracolumbar Compression Fracture (흉요추 압박골절로 입원한 환자에 대한 한의학적 치료 효과)

  • Objectives : This Study was to investigate the tendency of inpatient with Thoracolumbar Compression Fracture. Also we investigated effectiveness of Pharmacopuncture and Warming Acupuncture on Thoracolumbar Compression Fracture. Methods : We investigated 48 cases of patients with thoracolumbar compression fracture. We distributed inpatint with age, sex, number of injured vertebra, level of injured vertebra, admission duration, grade of clinical symptom, result of treatment and applied treatments. And we calculated statistically signification with this data. Results : 1. Female is prominent in distribution of sex, the average age bracket is 70's, and the largest group was accidental fall in the distribution of causes. 2. In regard to level of injury, T12 was found to be most prominent, followed in turn by L2, L1, T12. In regard to grade of clinical symptoms, Grade III was most Prominent, followed in turn by Grade IV, Grade II. In regard to grade of result of treatment, Good was most prominent. 3. There was not statistically significantly correlation about admission duration and result of treatment with number of injured vertebra, grade of clinical symptom, applied treatments. Conclusions : Most inpatients with thoracolumbar compression fracture was cured by oriental medicine treatment.

  • Objectives : This study was designed to investigate the correlation between thoracolumbar junction and back-su points, Hwatahyeopcheok points for treatment of low back pain in the thoracolumbar junction syndrome that was suggested by Maigne R. Method : We Investigate the acupuncture points that was correlated with the location of thoracolumbar junction area. And We tried to find out a common point between thoracolumbar junction and back-su points, Hwatahyeopcheok points for treatment of low back pain. Results and Conclusion : 1. It is considered that these points such as $BL_{20}$, $BL_{21}$, $BL_{22}$, and Hwatahyeopcheok points that are located from 11th thoraic spinous process to 2nd lumbar spinous process are correspond to the thoracolumbar junction area. 2. It is suggested that acupuncture treatment on $BL_{20}$, $BL_{21}$, $BL_{22}$, and Hwatahyeopcheok points can release the tenderness of the muscles, recover autonomic nervous function and release smooth muscles and vascular contraction, so it can treat low back pain caused by thoracolumbar junction.

  • Objectives : The Purpose of this study was to investigate the clinical application of traditional Korean herbal therapy to a patient with thoraco-lumbar compression fracture. Methods : The patient who had sudden low back pain, back pain and ambulation disorder due to pain caused by an accidental fall, we prescribed Doinseunggi-tang (Taorenchenqi-tang) for her with acupuncture therapy. And we adopted several scales to estimate to progress of her symptoms. Results : Soon after taking herbal medication with other treatments, the patients pain was controlled and was able to ambulation without pain. Conclusions : In this case, we assume that Doinseunggi-tang(Taorenchenqi-tang) play an important role in improvement of the patient's symptoms. So, the present results suggest that traditional Korean herbal therapy has the positive effect to reduce the pain and shortening the period of treatment.

A Case Report of Patients Diagnosed with ThoracoLumbar Junction Syndrome Treated by Megadose Pharmacopuncture with Korean Medicine Treatment (대용량 약침치료 및 한의학적 치료를 적용한 흉요추 연접부 증후군 증례 보고)

  • The purpose of this research is to find out the effect of Megadose pharmacopuncture with Korean Medicine Treatment on patients who were diagnosed as ThoracoLumbar Junction Syndrome. Megadose pharmacopuncture was proceeded on 4 patients who received treatment at ${\bigcirc}{\bigcirc}$ Korean Medicine Hospital from November, 2016 to Februrary, 2017. In addition, We also kept a record of NRS, ODI, and EQ-5D on the day of admission and discharge to find out the differences between initial and final figures. As a result, Megadose parmacopuncture showed a significant effect on alleviating pain on patients with ThoracoLumbar Junction Syndrome. There was a significant decrease in NRS, ODI scores. Not only did the scores of NRS and ODI decrease significantly,but also the figures of EQ-5D increased significantly as well. To be specific, the mean of NRS and ODI dcreased from 10 to 1.75 and 61.87 to 12.78 while the mean of EQ-5D increased from 0.258 to 0.850. This research suggests Megadose pharmacopuncture with Korean Medicine Treatment could be a remedy for ThoracoLumbar Junction Syndrome.

  • Objectives : The purpose of this study was to report clinical effects of Lumbar flexion-distraction manipulation and Sitting position thoracic and lumbar vertebra adjustment manipulation on patient with Juvenile idiopathic scoliosis Methods : The patients have been treated with the above two Chuna manual manipulations. We measured Cobb's angle, VNRS before and after treatment Results : After treatment, we confirmed these transition : In first case, Cobb's angle of T-spine changed from 27 to 23 : Cobb's angle of L-spine changed from 17 to 10 : VNRS changed from 6 to 2. In second case Cobb's angle of T-spine changed from 30 to 21 : Cobb's angle of L-spine changed from 16 to 16 : VNRS chagned from 6 to 3 Conclusion : The result suggests that these two Chuna manual manipulations were effective treatment on patients with Juvenile idiopathic scoliosis.

Association between Numerical Variations of Vertebrae and Carcass Traits in Jeju Native Black Pigs, Landrace Pigs, and Crossbred F2 Population (제주재래흑돼지와 Landrace, 교배집단 F2에서 척추의 수적 변이와 도체형질의 상관관계)

  • The number of thoracic and lumbar vertebrae is known to be an unfixed trait among mammals. This study focused on the relationship between numerical variations of cervical (CER), thoracic (THO), and lumbar (LUM) vertebrae and the total number of vertebrae (TNV) and carcass traits in Jejunative black pigs (JBPs), Landrace pigs, and their intercrossed $F_2$ population. There were no numerical variations in CER vertebrae. On the other hand, the numbers of THO and LUM vertebrae and the TNV varied in all three populations. Of the traits investigated in the three populations, only the means${\pm}$SE of the LUM vertebrae did not show statistical significance (p>0.05). The carcass weights (CW), meat color (MC), marbling score (MS), backfat thickness (BFT), carcass length (CLE), THO vertebrae, and TNV all showed statistical significance (p<0.05). The JBP had 14-16 THO vertebrae, 5-6 LUM vertebrae, and 27-29 TNV. The Landrace pigs had 15-16 THO vertebrae, 5-7 LUM vertebrae, and 28-29 TNV. The $F_2$ population had 14-17 THO vertebrae, 5-7 LUM vertebrae, and 27-30 TNV. In the F2 population, increased numbers of THO vertebrae and TNV were associated with a significant increase in the CW, CLE, and BFT (p<0.05). In particular, the increase in the TNV was caused by an increase in the number of THO rather than LUM vertebrae. Although the animals with a greater number of THO and TNV had thicker backfat, they had a longer CLE and a heavier CW. Both these traits are economically more important than the level of backfat when determining the productivity level. These results suggest that genetic selection to increase the number of vertebrae, especially in Landrace pigs, JBPs, and their related populations, may be an excellent strategy for improving productivity.

A Survey Study of the Juvenile Idiopathic Scoliosis Using Radiation Indirect Examination (방사선 간접검사를 이용한 청소년의 척추 측만증에 관한 연구)

  • The purpose of this Study was to investigate the prevalence rate of idiopathic scoliosis to the students from the elementary to the university in S city of Chung-Nam using 100 mm Mirror Camera radiation indirect examination units, with on age range of between eight and thirteen (1.526 subjects), fourteen and sixteen (462 subjects), seventeen and eighteen (291 subjects), nineteen and twenty four(508 subjects) and total of 2,787 participants with the 590 male subjects and 2,197 female subjects. The results of this study can be summarized and compared the primary examination with the secondary test of greater than $10^{\circ}$of Cobb's Angle were obtained as follows; 1. Indirect Examination were conducted to find idiopathic scoliosis amomg total 2.787 subjects, 257 subjects (9.2%) who showed positive sign greater than $10^{\circ}$in the Cobb's Angle ; below age of thirteen (132 subjects), between fourteen and sixteen (52 subjects), seventeen and eighteen (35 subjects), nineteen and twenty four (38 subjects). The $x^2-test$ analysis of Indirect Examination showed no statistical significant difference association between the age range and $10^{\circ}$Cobb's Angle of spinal curve(P>0.059). 2. The numbers of idiopathic scoliosis of greater than $10^{\circ}$Cobb's Angle of spinal curve in the primary examination were observed in 147 subjects (57.2%) at the Thoracolumbar region, 81subjects (31.5%) at the Thoracic region, 20 subjects (7.8%) at the Cervicothoracic region, 7 subjects (2.7%) at the Lumbar region, 2 subjects (0.8%) at the Cervical region. So, the large numbers were Thoracolumbar region, 183 participants (71.2%) showed the right side curve of scoliosis and 74 participant (28.8%) showed the left side curve of scoliosis. 3. The main region of the pain in one's own self more than $10^{\circ}$ Cobb's angle of spinal curve were no pain 219 subjects (85.2%), 18 subjects (7.0%) at the Lumbar region, 9 subjects (3.5%) at the Cervical region, 7 subjects (2.7%) at the Thoracic region, 2 subjects (0.7%) at the shoulder girdle region, 1 subjects (0.39%) at the pelvis and whole body region. There was statistical significant difference association between the Cobb's Angle of spinal curve and the main pain region of one's (P<0.006). This study may be significant to an early stage investigate of the prevalence rate of idiopathic scoliosis in the juveniles using 100 mm Mirror Camera radiation indirect examination units. The results of this study help that the students in a stage on growth the basis of data early discovery and therapy of idiopathic scoliosis.
