Two Cases of Bilateral Diffuse Cystic Lesion (양측성 미만성 소낭포성 병변 2예)

  • Lymphangioleiomyomatosis(LAM) is a rare disease of unknown etiology that occurs mainly in woman in her reproductive age. We recently experienced two cases of bilateral diffuse cystic lesion of the lung on chest X-ray and HRCT. The first case, a 26-year-old female, who had been diagnosed with tuberous sclerosis by the presence of clinical manifestation such as mental retardation, bilateral renal angiomyolipoma, adenoma sebaceum and generalized seizure, was admitted due to a recently developed hemoptysis. Chest PA showed diffuse ground-glass opacity with radiolucent cystic lesions of various sizes on both lung fields. HRCT showed innumerable small cystic lesions with suspicious diffuse ground-glass opacity on both lung fields The second case, a 30-year-old fema1e was admitted due to dyspnea and spewing of blood-tinged sputum for 2 weeks, shortly after delivery. Chest PA showed diffuse reticular and ground-glass opacities on both lung field. HRCT showed multiple well-defined and relatively uniform size air cysts with a uniform wall thickness on entire both lung fields, with small amount of right pleural effusion. By thoracoscopic lung biopsy she was diagnosed with pulmonary lymphangioleiomyomatosis. We report these cases with a brief review of the literatures.

Primary Malignant Fibrous Histiocytoma of the Diaphragm -One Case Report- (횡격막의 원발성 악성 섬유성 조직구종 -1례 보고-)

  • A 25-year-old female patient was adimitted due to discomfort in the right chest and left epigastric area. Chest X-ray and chest CT showed a round 4$\times$4$\times$4cm sized mass located in the right lower lobe and abutted on the right diaphragm and metastatic lesions in the right lower lobe and right middle lobe. In exploratory thoracotomy, we discovered a mass originating from the right diaphragm which directly invaded the right lower lobe and metastatic subpleural lesions in the right lower lobe and right middle lobe. The mass was diagnosed as sarcoma by frozen section biopsy and we removed the mass by on bloc resection of the right diaphragm and right lower lobe. Metastatic subpleural lesions in the right lower lobe and right middle lobe were removed by wedge resection. The mass was malignant fibrous histiocytoma of the diaphragm in permanent pathologic examination and chemotherapy was done as adjuvant measure. We report one extremely rare case of malignant fibrous histiocytoma of the diaphragm with literature.

  • After the outbreak of the SARS-CoV2 virus that causes COVID-19, it spreads around the world with the number of infections and deaths rising rapidly caused a shortage of medical resources. As a way to solve this problem, chest X-ray diagnosis using Artificial Intelligence(AI) received attention as a primary diagnostic method. The purpose of this study is to comprehensively analyze the detection of COVID-19 via AI. To achieve this purpose, 292 studies were collected through a series of Classification methods. Based on these data, performance measurement information including Accuracy, Precision, Area Under Cover(AUC), Sensitivity, Specificity, F1-score, Recall, K-fold, Architecture and Class were analyzed. As a result, the average Accuracy, Precision, AUC, Sensitivity and Specificity were achieved as 95.2%, 94.81%, 94.01%, 93.5%, and 93.92%, respectively. Although the performance measurement information on a year-on-year basis gradually increased, furthermore, we conducted a study on the rate of change according to the number of Class and image data, the ratio of use of Architecture and about the K-fold. Currently, diagnosis of COVID-19 using AI has several problems to be used independently, however, it is expected that it will be sufficient to be used as a doctor's assistant.

  • Cryptococcosis is a subacute or chronic inflammation that's caused by infection from cryptococcus neoformans in the soil or dust that's contaminated by pigeon droppings. Pulmonary cryptococcosis occurs in immunocompromised patients, and particularly in HIV positive patients, but it rarely occurs in immunocompetent individuals. It is most often detected as asymptomatic single or multiple nodules that are found by chest X-ray examination. In these situations, the diagnosis for most cases is made by the histology of the resected lesion. We report here on a case of solitary nodular pulmonary cryptococcosis that occurred in a 32 years old female patient who underwent thyroidectomy for treating her thyroid cancer.

  • Localized fibrous tumor of the pleura is very rare. Most of them are benign, but some are malignant. This clause does not relate with the rest of the sentence. The single best predictor of clinical benignity is whether the tumor can be totally resected. We experienced a case of localized fibrous tumor of the pleura in a 57 year old man with right chest pain and cough. He was informed of a $8{\times}5cm$ mass in his right lower lung field, which was benign 3 years ago. Preoperative chest x-ray showed an increased hazy density at right lower lung field, and CT scan showed a $12{\times}8cm$ huge mass, which was located in right lower thorax. Left thoracotomy was done to excise a $12{\times}8{\times}5cm$(1200gm) sized large mass delete. The patient was discharged without any complications postoperatively.

  • An unusual placentoid bullous lesion of the lung was reported. The patient was a 27-year- old woman with a 1-week history of chest pain and mild dyspnea. Chest radiographic studies showed multiple huge bullae in right upper lobe and variable-sized bullae in middle and lower lobes with collapse. She underwent right pneumonectomy with preoperative impression of bullous lung disease and emphysema. Histopathologically, the most distinctive features were villous structures within bullous airspace, which resembled placetal villi with degeneration.

  • 흉부 X선 CT 화상을 이용한 폐종류(肺腫瘤)의 경계 형상을 정량적으로 평가하기 위하여 푸리에 변환된 폐종류(肺腫瘤) 음영의 윤곽선 내 power spectrum 고주파 성분의 총합이 폐종류 음영의 경계 형상에 관한 유효한 평가 값이 되는지의 여부를 검토하였다. 이 평가 값은 폐종류(肺腫瘤) 음영의 CT 화상 위의 특징을 명확히 반영한다고 판단된다. 양성 15명과 악성 16명인 환자 31명에 대해서 이 평가 값을 계산하여 통계적 처리를 행한 결과 양성과 악성 간에 뚜렷한 차이를 인식할 수 있었다. 이러한 제안된 평가 방법에 의해, power spectrum 고주파 성분의 총합이 폐종류(肺腫瘤) 경계 형상의 평가치가 되어, 정량적인 폐종류(肺腫瘤)의 양성과 악성 감별을 행할 때 유용한 값이 될 가능성을 시사한다고 볼 수 있다.

  • Pneumocystis carinii pneumonia(PCP) remains the leading cause of death in patients with AIDS. Although the most common radiographic presentation of PCP is the development of diffuse, bilateral interstitial or alveolar infiltrates in 48 to 86 per cent of AIDS patients, PCP may also present with either a completely normal or only minimally abnormal chest radiograph in 6 to 23 per cent of patients. We experienced two patients with AIDS presenting high fever and chest pain but normal chest radiograph, who had been proved to have PCP by bronchoalveolar lavage and trans-bronchial lung biopsy.

A Clinical Review of Primary Tracheal Carcinoma (원발성 악성기관종양의 임상적 고찰)

  • Background : Primary malignant tumors of the trachea are extremely rare entities and account for a mere 0.1 per cent of all malignancies of the respiratory tract. Because of vague localizing signs, symptoms and a usually negative routine chest film, the patients with tracheal tumors are often treated for asthma or chronic obstructive pulmonary disease for considerable period of time before correct diagnosis. Method : We have made a review of the 17 cases of primary tracheal tumors in recent 15 years. We reviewed the clinical features including history of smoking and respiratory symptoms, the official readings of initial routine chest film, the cytologic examination of sputum, the time of delay in diagnosis, and the response according to the therapeutic modalities. Results : Eight out of 9 patients with squamous cell carcinoma(SCC) were above 50 years old, five out of 6 patients with adenoid cystic carcinoma(ACC) were below 50 years old. The most common location of primary tracheal tumors was the upper one-third of trachea in 8 cases(47%). The most frequent symptoms were dyspnea in 13/17 cases(76%) and then stridor or wheezing, cough. and sputum in order. The routine chest roentgenographic examinations were not helpful to diagnose tracheal carcinoma and the cytologic examinations of sputums were helpful to diagnose tracheal carcinoma in only one case with adenocarcinoma. The mean times of delay in diagnosis of patients with sec and ACC were 5 months and 24.9 months respectively. We had bronchial asthma in 8 cases(47%) and tracheal tumors in 4 cases(23%) as initial clinical impression. Conclusion : We would like to perform more comprehensive diagnostic tools(high KVP technique, the fibroptic bronchoscopic examination, chest CT scan etc.) in patients who had the suggestive points for the tracheal tumorse(1. unexplained hemoptysis or hoarsness, 2. inspiratory wheezing or stridor, 3. wax and waning of dyspnea according to changes of position, 4. progressive asthmatics unresponsive to antiasthmatic therapy) and radical resection of tumor or external radiation therapy with curative aim as possible.
