Atypical Thymic Carcinoid Tumor with Thymic Cyst - 1 case report - (흉선 낭종을 동반한 비정형 흉선유암종 - 1례 보고 -)

  • Thymic carcinoid or neuroendocrine tumor of thymus is a very rare disease and has poor prognosis due to frequent recurrence and distant metastasis. A 43-year-old man was refered to our hospital because of Rt. chest pain and tightness. Chest X-ray revealed $7{\times}8$cm sized mass on Rt. anterior mediastinum. Surgical excision was performed and light microscopic, immunohistochemical and electron microscopic findings were confirmed as atypical thymic carcinoid tumor with thymic cyst. The patient has been followed up without recurrence or distant metastasis postoperatively for 3 months to now. We report a case of atypical thymic carcinoid with thymic cysts.

  • The incidence of pulmonary leiomyosarcoma as primary lung tumor is very rare. Most of the primary leiomyosarcomas originate in the hilar region in relation to the main bronchus or pulmonary vessels and only a few originate more peripherally. This rare tumor can mimic bronchial carcinoma and present with local or systemic symptoms, or it may be discovered as an incidental finding on a routine chest X-ray. We report with review of literature, a case of incidental primary pulmonary leiomyosarcoma which originated peripherally. Huge mass was found on the left lung of a 61-year-old man on the chest X-ray peripherally. He underwent the surgical resection of the left pneumonectomy and the postoperative course was uneventful.

Endobronchial Inflammatory Myofibroblastic Tumor of Right Lower Lobar Bronchus (우하엽 기관지에 발생한 기관지 내 염증성 근섬유아세포종)

  • Inflammatory myofibroblastic tumor was widely known as inflammatory pseudotumor, commonly developed as a solid mass in lung. The endobronchial inflammatory myofibroblastic tumor is a very rare case where only a few cases have been reported. We report a 13-year-old girl who had coughing for 5 months. The simple chest X-ray and computued tomography of the chest revealed a mass which obstructed the right lower lobe bronchus and pneumonic consolidation. The fiberoptic bronchoscopic finding was mostly gelatinous, gray-yellowish mass that obstructed the airway of right lower lobe bronchus nearly, and was considered as a chondroid hamartoma pathologically. Right lower lobectomy of lung was performed. The mass was confirmed as a endobronchial inflammatory myofibroblastic tumor The patient was discharged without complication and with outpatient followup.

Rate-Distortion Based Segmentation of Tumor Region in an Breast Ultrasound Volume Image (유방 초음파 볼륨영상에서의 율왜곡 기반 종양영역 분할)

  • This paper proposes an efficient algorithm for extracting a tumor region from an breast ultrasound volume image by using rate-distortion (R-D) based seeded region growing. In the proposed algorithm the rate and the distortion represent the roughness of the contour and the dissimilarity of pixels in a region, respectively. Staring from an initial seed region set in each cutting plane of a volume, a pair of the seed region and one of adjacent regions whose R-D cost is minimal is searched and then they are merged into a new updated seed region. This procedure is recursively performed until the averaged R-D cost values per the number of contour pixels in the seed region becomes maxim. As a result, the final seed region has good pixel homogeneity and a much smooth contour. Finally, the tumor volume is extracted using the contours of the final seed regions in all the cutting planes. Experimental results show that the averaged error rate of the proposed method is shown to be below 4%.

Clinical Analysis for Thymic Carcinoma (흉선암의 임상적 고찰)

  • 배경: 흉선암은 드문 질환으로 치료경과 및 예후가 침윤성 흉선종에 비해 나쁜 것으로 알려져 있으며 병기의 분류나 병기에 따른 치료방법이 아직 표준화 되어있지 않아 치료에 어려운 점이 있다. 이에 본교실에서 흉선암으로 진단되었던 환자들의 피료방법 및 성적을 분석하여 그 결과를 보고하고자 한다. 대상 및 방법: 계명대학교 동산의료원 흉부외과학교실에서는 1984년 8월에서 흉선암으로 진단되었던 8례의 환자를 대상으로 의무기록을 참고하여 병기에 따른 치방법료 및 예후 등을 후향적으로 분석하였다. 결과: 연령은 23세에서 67세까지로 평균 46세였으며 전흉부통증이 주증상이었다. 조직학적으로는 임파상피양암(lymphoepithelioma-like carcinoma)이 2례, 편평상피암(squamous cell carcinoma)이 2례, 기저세포암(basaloid carcinoma)이 1례, 혼합형(mixed type)이 3례 있었다. 임상적 병기분류는 Masoka의 분류법을 사용하였으며 제I기 2례, 제II기 4례, 제III기 1례, 제IVAr기가 1례 있었다. 4례의 환자에서는 종양의 완전적출이 가능했으며 3례에서는 고식적 수술을 시행하였다. 1례의 환자는 주위조직으로의 침윤과 심낭에 퍼져있어 조직생검만을 시행하였다. 전례에서 보조적 항암치료를 받았고 술후 병기가 제III기 이상이거나 종양의 절제가 불완전했던 5례의 환자에서는 보조적 방사선치료를 병행하였다. 이들 중 5례에서 술후 보조적 항암치료 및 방사선치료를 받고 현재까지 생존해있다. 평균추적기간은 55.3$\pm$64.6 개원이었고, 3례의 환자는 사망하였으며 4례의 환자는 종양의 재발증거 없이 생존해있다. 결론: 종양의 조기진단과 완전종양적출후 적극적인 보조적 항암치료 및 방사선치료가 흉선암을 치료하는데 도움이 될 것으로 사료된다.

  • Intrathoracic goiters consist of two types: the primary and secondary type. Primary intrathoracic goiters are very rare. We report here on a case that was resected with using the thoracoscopic assist technique. The transthoracic or transsternal approach is necessary for primary goiters to remove them completely without complications.

  • The most common primary heart tumor in infants is the rhabdomyoma. It is known that tuberous sclerosis is found in half of the patients with cardiac rhabdomyoma. Because the prognosis of this tumor associated with subaortic stenosis is poor, the surgical intervention is indicated. We report one case of left ventricular rhabdomyoma with tuberous sclerosis.

  • Most tumors of the tracheobronchial tree are malignant, and benign tumors are less than 10%. Especially, the incidence of primary neurogenic tumors of the lung has been estimated to be less than 2 percent of primary lung cancer, and majority of these tumors are originated from Schwann cells. These tumors can be presented either as a solitary benign neoplasm or as a malignant form, which is rare. We present two cases of bronchial Schwan noma managed by means of lobectomy.

  • Mucoepidermoid carcinoma is an uncommon lesion that accounts for approximately 1% of primary malignant bronchial gland tumors and less than 0.2% of all lung neoplasm. This tumor presents with symptoms of bronchial irritation or obstruction. Distant metastasis is uncommon, therefore complete surgical resection is the treatment of choice. The prognosis of tumor correlates with on the histologic grade of tumor. We experienced mucoepidermoid carcinoma in a 15 year-old girl with symptoms of cough and blood tinged sputum. The patient underwent successful removal of tumor by bilobectomy via explorothoracotomy after chest CT and bronchoscopic biopsy.

A Study on Anastomotic Complications after Esophagectomy for Cancer of the Esophagus : A Comparison of Neck and Chest Anastomosis (식도암 수술후 문합부 합병증에 관한 연구 - 경부문합과 흉부문합 간의 비교-)

  • Background: Leakage, stricture formation, and tumor recurrence at the anastomotic site are serious problems after esophagectomy for cancer of the esophagus or cardia. The prevalence of these postoperative complications may be affected by whether an anastomosis is made in the neck or in the chest, therefore a comparison was made between anastomoses made at these two sites. Material and Method: Between 1987 and 1998, 36 patients with cancer of the esophagus underwent transthoracic esophagectomy with cervical(NA, n=20) or thoracic anastomosis(CA, n=16). The tumors were staged postoperatively(stage IIA, n=13; s tage IIB, n=7; stage III, n=16) and were located in the middle thoracic(n=22) or lower thoracic esophagus and cardia(n=14). Result: The overall operative mortality was 8.3%(5% for NA group, 12.5% for CA group). The anastomotic leak rate for the NA group was 15.0% and 12.5% for the CA group. The anastomotic leak rate differed according to the manual(27.3%) or stapled(8.0%) techniques(p < 0.05). The median proximal resection margins in the NA and CA groups were 9.6 cm and 5.8 cm, and the corresponding rates of anastomotic tumor recurrence were 5.3% and 28.6%(p < 0.05). The prevalence of benign stricture formation (defined as moderate/severe dysphagia) was higher in the NA group(36.8%) than in the CA group(21.4%). When an anastomosis was made by the stapled technique, smaller size of the staple increased the prevalence of stricture formation - 41.7% with 25-mm staple and 9.1% with 28-mm staple(p < 0.05). Conclusion: Wider resection margin could decrease the anastomotic tumor recurrence, and the stapled technique could decrease the anastomotic leak. The prevalence of benign stricture was higher in the cervical anastomosis but the anastomotic leak and smaller size(25-mm) of the staple should be considered as risk factors.
