This study aimed to provide basic data for 3D printing in the medical health field by developing upper wear fixation device (UWFD), an auxiliary device for shortening chest AP examination time on emergency room beds and non-contact with patients. The standard of hooks was modeled according to the bed frame using the Autodesk Fusion 360. It was printed with Form2 (Formlabs, Somerville, MA, USA), as SLA (stereo lithography apparatus) method, and was washed and hardened using Form Wash and Form Cure. The completed UWFD conducted an online survey on 4 items of stability, convenience, availability, preference and general characteristics. The total stability average was 3.93±0.80, the total convenience average was 3.93±0.68, the total availability average was 4.01±0.89, and the total preference average was 3.80±1.08. This study was significant in suggesting improvements in the general X-ray examination process in the emergency room by designing and making aids to easily fixing the patient's top to the frame of the emergency bed while meeting promptness and non-contact with the patient.
This study was conducted to ultimately reduce unnecessary radiation exposure by emphasizing the need and importance of correct positioning by examining the positioning relationship of anatomical structures in the human body and changes in X-ray images according to changes in patient positioning during the left lateral chest X-ray examination. This study investigated and analyzed previously published papers and books on the left lateral chest X-ray examination to find out the importance of positioning in the left lateral chest X-ray examination. To find out the importance of correct positioning in the left lateral chest X-ray, we compared three images of incorrectly positioned right thorax and left thorax rotated forward and the lower median surface of the body leaning against the image receptor. In the left lateral chest examination, a distorted image was obtained in which the shape of the anatomical structure observed in the image was changed according to the presence or absence of rotation of the patient and the inclination of the median visual surface. X-ray images with the most accurate and large amount of information were obtained from X-ray images with the correct positioning performed during left lateral chest X-ray examination. Therefore, It is believed that the left lateral chest X-ray examination will have beneficial effects such as providing accurate medical information, preventing misdiagnosis, reducing social costs, and ultimately reducing radiation exposure.
X-ray Chest P-A is very important and basic diagnosis examination for pregnant women. For a pregnancy period pregnant women should be treated very carefully not to be exposed to any radiation which might cause harmful damage to women and babies as well. Lead apron is one of the effective methods to protect pregnant women from the X-ray radiation. However, it is difficult to obtain the accurate position of pregnant women during X-ray Chest P-A since conventional lead apron method forces pregnant women to hold the apron by themselves only to make pregnant women very uncomfortable and hard to maintain accurate position during radiation. As a consequence, it is common to get low quality images of X-ray Chest P-A due to the overlap of apex of lung and scapular. In order to fix this problem, we made new design lead apron that allowed pregnant women to be more comfortable to maintain accurate position during X-ray Chest P-A position. Finally, with this new design lead apron, it was possible to get the best optimized images of X-ray Chest P-A of pregnant women by minimizing overlapping apex of lung and scapular.
In this study, we applied AEC(Auto Exposure Control), which is used in many chest examinations, to evaluate whether medical devices inserted into the body affect the dose and image quality of chest images. After attaching three HIMD(Human implantable medical devices) to the ion chamber, the Monte Carlo methodology-based program PCXMC(PC Program for X-ray Monte Carlo) 2.0 was applied to measure the effective dose by inputting the DAP(Dose Ares Product) value derived from the Pacemaker and CRT and Chemoport Additionally, to evaluate image quality, we set three regions of interest and one noise region on the chest and measured SNR and CNR. The final study results showed significant differences in DAP and Effective dose. There was a significant difference between Pacemaker and CRT when AEC was applied and not applied. (p<0.05) When applied, the dose increased by 37% for Pacemaekr and 52% for CRT. Chemoport showed a 10% increase in effective dose depending on whether AEC was applied, but there was no significant difference. (p>0.05) In the image quality evaluation, there was no significant difference in image quality between all HIMD insertions and AEC applied or not. (p>0.05) Therefore, when the HIMD was inserted into the chest during a chest x ray and overlapped with the ion chamber sensor, the effective dose increased, and there was no difference in image quality even at a low dose without AEC. Therefore, when performing a chest X-ray examination of a patient with a HIMD inserted, it is considered that performing the examination without applying AEC is a method that can be considered to reduce the patient's radiation exposure.
The purpose of this study was to evaluate the distribution of spatial scatter ray on the chest radiographs of patients on health examination bus. In this paper, we propose a method for minimize unnecessary exposure by measuring the scattered dose after exposure the actual subject and comparing the body mass index (BMI) with the tube current amount mAs. The results of this study showed that the mean BMI of the subjects was $23.31{\pm}3.12$. The mean mAs value was $2.92{\pm}1.19$, which males was higher than females. The mean value of the scatter ray at position 1 in the radiography room was $771.81{\pm}151.15{\mu}Sv/hr$. The mean value of the scatter rays at the position 2 outside the entrance of the radiography room was measured as $53.86{\pm}25.66{\mu}Sv/hr$. As the BMI and mAs was increase the spatial scatter dose was increased at position 1 and position 2 in the photographing room. In order to minimize the exposure dose of scatter ray, radiation workers should shoot the radiation as low as possible within the range that does not impair the quality of the image. It will be necessary to make efforts to not wait for a waiting person near the entrance door of the photographing room.
The purpose of this study is to examine how much the movement at X-ray examinations like breathing or the positioning affects the image during chest or abdomen X-ray examination so as to create an image containing information as much as possible. The study method adopted is doing the X-ray in each of the states including breathing (inspiration & expiration) and movement in the standing chest PA X-ray and simple abdomen X-ray among the kinds of examination selected the most in hospitals and then evaluating them by applying the standards of image evaluation for each region. According to the study result, about the standing chest PA X-ray, the images taken at inspiration contain more information than those taken at expiration or having subtle movement during the examination. About the simple abdomen X-ray, the images taken at expiration contain more information than those taken at inspiration or movement. The above study results imply that regarding general X-ray examination, information we can find from the images may differ significantly according to the region examined, examination purpose, or movement during the examination like breathing.
This study's purpose is improve image quality to keep accurate tube angle in order to recognize distortion degree conditions by patient's position or tube angle and to provide exact clinical informations when taking chest AP projection for patient which have L-tube in stomach. The experimental equipment was ELMO-T6S by SHIMADZU corporation, then we put L-tube which attached 1 mm gap scales ruler on chest phantom surface. The experiment set by 90 kVp, 4 mAs, 120 cm distance. Each phantom position which changed supine, 30degree, 45degree, 60degree on the table exposured direct, ${\pm}5degree$, ${\pm}10degree$, ${\pm}15degree$ to head and feet directions. As a result, L-tube tip's position was changed by patient's position and tube angle. When patient's position is supine, tip's position change was lower than 30degree, 45degree, 60degree. We have to adjust patient's position or tube angle in order to occur image distortion by fault tube angle when confirming correct position L-tube tip through chest x-ray. Also, Radiological technologist try to make accurate evaluation index for satisfied L-tube insertion.
Background: Blunt chest trauma accounts for 90% of all chest traumas in Europe and the United States and this causes 20% of all trauma-related deaths. The major cause of morbidity and mortality after blunt chest trauma is undetected injuries. For this reason, chest computerized tomography has gained popularity for the evaluation of trauma, but it is expensive and it exposes patients to radiation. This study identified the clinical features associated with the diagnosic information obtained on a CT chest scan, as compared with a standard chest X-ray, for patients who sustained blunt trauma to the chest. This study also evaluated the role of a routine computed tomographic (CT) scan for these patients. The patients who had chest computed tomography done after the initial chest x-ray were analyzed separately for the presence of occult injuries. Material and Method: We studied 100 consecutive patients from November 2006 to July 2007: 74 patients after motor vehicle crashes and 26 patients after a fall from a height >2m. Simultaneous with the initial clinical evaluation, an anteroposterior chest radiograph and a helical chest CT scan were obtained for all the patients. The data extracted from the medical record included the vital signs, the interventions and the type and severity of injury (RTS). Result: Among the 100 cases, 79 patients showed at least more than one pathologic sign on their chest radiograph, and 21 patients had a normal chest radiograph. For 17 of the patients who had a normal chest X ray, the CT scan showed multiple injuries, which were pneumothorax, hemothorax, lung contusion, sternal fracture etc. This represents that a CT scan is statistically superior to a chest radiograph to diagnose the pathologic signs. But on the other hand, as for treatment, only 31 patients were diagnosed by CT scan and they were treated with chest tube insertion ect. 42 patients needed ony conservative management without invasive thoracosurgical treatment such as chest tube insertion or open thoracotomy. 27 patients were treated based on the diagnosis made by the chest radiograph and physical examination. Conclusion: Chest computerized tomography was significantly more effective than routine chest X-ray for detecting lung contusion, pneumothorax and mediastinal hematoma, as well as fractured ribs, scapula and, sternum. Although the occult findings increased, the number of patients who needed treatment was small. Therefore, we suggest making selective use of a CT scan to avoid its overuse in ERs.

There are no recommended test conditions for digital photography in Chest PA examinations. However, each company recommends shooting examinations of the high voltage applied to the previous analog examination. The condition that satisfies the value of 200 ~ 800 which is the recommended Exposure Index value recommended by Philips was selected, and the dose was evaluated by Monte Carlo simulation, and the SNR and CNR were compared. As a result, it was possible to reduce the effective dose up to 77% by controlling the tube voltage, tube current, and additional filter, not the conventional high voltage imaging method. Although there were some differences according to the test conditions, the image evaluation results were similar to the images. We will compare the exposure dose according to changes in tube voltage, tube current, and additional filter at the digital chest radiograph and evaluate the image quality of the image to propose optimal conditions.
