Clinical Evaluation and Diagnosis of Children with Chest Pain (흉부 통증을 호소한 소아에서의 임상적 고찰 및 검사)

  • Purpose : Chest pain in the pediatric population is not rare and mostly benign. Causes of chest pain are diverse, and differential diagnosis is not easy. Chest pain in children is less likely to be cardiac in origin. Furthermore, chest pain in the pediatric population is rarely associated with life-threatening disease. This study was designed to evaluate children with chest pain and the usefulness of several diagnostic examinations. Methods : Between March 2001 and August 2002, 33 patients(15 boys and 18 girls, aged four to 15 years) presented with chronic chest pain. The records of these patients were reviewed. Chest radiography and electrocardiogram were performed in all patients. Cardiologic and gastrointestinal evaluations were carried out when considered necessary. Results : Chest pain was most common in the age group of 10 to 12 years old, and the four to six years old group. The most common diagnostic findings of chest pain were idiopathic(15 cases, 45.5 %), heart disease(9 cases, 27.3%), upper gastrointestinal disease(6 cases, 18.2%), respiratory disease (2 cases, 6%) and trauma(1 case, 3%). In children with abnormal results of cardiologic evaluation, these findings are not major etiologic categories of chest pain. Through history taking and physical examinations, six cases were evaluated concerning gastrointestinal disease and all of them showed gastrointestinal diseases(esophagitis, gastroesophageal reflux disease, nodular gastritis and chronic superficial gastritis). Conclusion : Chest pain is usually benign in children but the possibility of cardiovascular or gastrointestinal disease is considered. Careful history taking, physical examination and proper clinical examinations are usually required to find out the rare life-threatening causes of chest pain.

Dose Assessment during Pregnancy in Chest PA Examination (흉부 후전방향 검사 시 임산부의 선량 평가)

  • One of the causes of death for pregnant female is embolism, when a chest PA examination is performed. In addition, due to small doses, the examinations are performed for the purpose of preparing for pre-delivery emergency surgery or basic examination for pregnant female. Evaluating fetal doses through actual measurements is subject to ethical problems, Monte Carlo simulations assesses the organ and fetal doses of pregnant females according to week of pregnancy. The results of the simulations showed that the fetal dose decreased according to weeks of pregnancy and it showed a dose of about 0.1 mGy. The higher the density and thickness of the shielding material, the better the shielding effect. In addition, the dose reduction effect for each shielding material is between 40 and 98%. Afterwards, it is deemed necessary to study the reduction of fetal doses through various shielding characteristics and methods.

Diagnostic Value of Thoracography in Pneumothorax (기흉에서 흉강조영술(Thoracography)의 진단적 가치)

  • Background: It is important to know the location, number, size and shape of bullae before thoracotomy or VATS bullectomy. Chest X-ray and chest CT may be used but with some limitation. The purpose of this study was to compare the diagnostic value of thoracography with that of chest X-ray in preoperative detection of bullae. Meterial and Method: Thoracography was performed by injection of non-ionic water-soluble dye into pleural space in 22 primary spontaneous pneumothoraces, which underwent thoracotomy or VATS bullectomy. Chest X-ray and thoracography were compared through operative finding. Results: Sensitivity and accuracy of thoracography(75% and 72.7%) were higher than those of chest X-ray(30% and 36.4%). However, specificity of thoracography(50%) was lower than that of chest X-ray (100%). There were no complications during or after thoracography. Conclusion: Thoracography is a safer and more useful method for preoperative detection of bullae when compared with chest X-ray.

Chest X-ray Findings and Serum Tumor Necrosis Factor-αLevels in Patients with Kawasaki Disease (가와사끼병 환아에서 흉부 X-선 검사의 변화와 혈중 Tumor Necrosis Factor-α에 대한 연구)

  • Purpose : Kawasaki disease(KD) is a multisystemic inflammatory vasculitis of unknown etiology. Many complications other than cardiovascular involvement have been recognized in KD. However, there have been few reports published concerning involvement of the lungs in this disease. The purpose of this study was to examine the relationship between serum TNF-${\alpha}$, the degree of coronary artery dilatation and chest X-ray(CXR) findings. In addition, we have investigated serum anti-Mycoplasma antibody(AMA) titers in patients with KD who have abnormal CXR findings. Methods : Eighty four patients with KD were included in this study(group I; 41 patients with normal CXR fndings, group II; 43 patients with abnormal CXR findings). Serum levels of TNF-${\alpha}$ and AMA titer were measured. Results : We reviewed the CXR findings and clinical courses of 84 patients with Kawasaki disease and found abnormal CXR findings in 43 patients(51.2 percent). Peribronchial cuffing was the most frequent abnormality(22.4 percent). In the group with abnormal CXR findings(group II), a statistical difference was not noted in age, sex, duration of fever, hemoglobin, WBC, platelet, ESR, and CRP levels and incidence of coronary arterial lesions as compared with the group having normal CXR findings(group I). No difference was noted in serum TNF-${\alpha}$ level between group I and group II. 2 patients(12.5 percent) of 16 KD patients with abnormal CXR findings have positive AMA titer(above 1 : 320). Conclusion : Most of the abnormal CXR findings in KD patients were peribronchial cuffing. The abnormal CXR findings in KD patients did not mean severe inflammations. It is difficult to consider that CXR abnormalities are related to coronary arterial lesions. In addition, further study on the relationship between Mycoplasma infection and Kawasaki disease is needed.

Diagnostic Imaging for Lung Lobe Torsion in a Miniature Pinscher (Miniature Pinscher 견에서 발생한 폐염전의 진단영상 1례)

  • A four-year old, female Miniature Pinscher was presented with intermittent coughing. Thoracic radiography and ultrasonography revealed the consolidation of left cranial lung lobe surrounded by pleural effusion and the extraction of left lung lobe was performed through thoracotomy. This case has the clinical significance because lung lobe torsion was diagnosed by non-cardiac ultrasonographic examination, therefore surgical correction was performed without delay and led good outcome. Moreover, the authors believe the present case was the first report on lung lobe torsion in Miniature Pinscher dog.

Calculation of Effective Dose on Domestic Chest PA X-ray Examinations (국내 흉부 X-선 검사에 따른 유효선량 계산)

  • Research on effective dose analysis of actual conditions of use based on large data is scarce. In this study, the exposure conditions of Chest X-Ray examinations used by 324 medical institutions in Korea were calculated and evaluated using computer simulations. As a result of the experiment, the effective dose in the low energy parameter bands was 0.024 mSv, followed by spleen, adrenal glands, and lung. The effective dose in the high-energy exposure parameter band was 0.123 mSv, followed by height, spleen and adrenal glands. The effective dose was 0.017 mSv when the optimal conditions considered the quality and exposure proposed in Park's study were used. The results of the study will be a reference for chest X-rays and will help reduce patient exposure.

Dose and Image Evaluation according to Changes in Tube Voltage during Chest X-ray Examination according to Automatic Exposure Control (자동노출제어장치 유·무에 따른 흉부 후·전방향 검사 시 관전압 변화에 따른 선량 및 영상평가)

  • This study was conducted to improve the problems of exposure dose and image reading applied to patients due to the incorrect use of AEC during chest radiography. Images were acquired by dividing the case where AEC was used as the test condition and the case where AEC was not used. As a result of the study, the dose was reduced by 1.17% in 110 kVp without AEC than with AEC, 17.2% decrease at 100 kVp, 30.19% decrease at 90 kVp, and 46.45% decrease at 80 kVp. There was a significant difference in the statistical values according to the tube voltage change in the lung, trachea, and heart SNR average values with AEC and without AEC 110 kVp, but the difference in image quality was insignificant in actual images. When AEC was not applied at the same tube voltage, the dose could be reduced by 17.2% while maintaining the image quality similar to that of with AEC at 100 kVp without AEC. Therefore, rather than relying on AE conditions during chest radiographic examination, it is considered that the conditions should be considered for the examination while lowering the dose by selecting an appropriate tube voltage.

폐암의 조기진단

  • 폐암을 집단 검진으로 조기 진단하려는 노력은 현재까지 뚜렷한 성과를 얻지 못하고 있는 실정이다. 미국의 국립암연구소에서 실시한 흉부엑스선 촬영과 객담세포진 검사를 이용한 폐암의 집단 검진 결과는 검진군에서 대조군보다 유의하게 진단율이 높았으며 절제율도 더 높았고 5년 생존율도 향상시킬 수 있었다. 그러나 폐암을 조기 진단하기 위한 집단 검진의 궁극적인 목표인 사망률을 감소시키지는 못하였다. 최근에는 미국 국립암 연구기관에서 실시했던 결과에 대한 해석이 다양하여 아직까지도 폐암 조기 진단에 있어서 흉부액스선촬영과 객담세포진검사의 의의에 관한 결론에 대해서는 논란이 되고 있다. 1993년부터 새로운 집단 검진에 관한 연구가 진행중에 있어서 그 결과가 나올 때까지는 흉부엑스선촬영과 객담세포진검사를 이용한 집단 검진의 의의에 대한 결과는 기다려 보아야 할 것이다. 분자생물학적으로 폐암을 조기 검진하기 위한 검체로는 혈액보다는 객담이 훨씬 적절하고 합리적인 검체이다. 폐암의 발생은 가장 먼저 기관지 상피 세포에서 일어나서 암화 과정의 여러 단계에서 다양한 종양 표지자가 객담에 섞여 나오기 때문에 이 표지자를 객담에서 측정하는 것이 훨씬 합리적인 조기 진단법이 될 수 있다. 동시에 폐암을 집단 검진으로 조기 진단하기 위해서는 종양 표지자를 대량으로 측정하기 위한 자동측정법도 개발되어야 할 것이다. 암을 예방하기 위하여는 여러면에서 방안이 연구되어야 한다. 즉 암발생의 가능성이 높은 대상을 선태하여야 하며 초기에 집단 검진으로 진단을 할 수 있어야 하고, 이러한 검진으로 추적 검사가 가능하여야 하며 마지막으로 결과를 판정할 수 있어야 한다. 이 가운데 현재까지 유일하게 실시할 수 없는 것은 조기 진단으로 사용할 수 있는 뚜렷한 종양 표지자가 없는 것이다. 이와 같이 현재까지는 폐암을 조기 진단하기 위한 특이한 표지자는 없으나 앞으로 폐암 발생 기전의 여러 단계가 체계적으로 밝혀진다면 그 과정에서 중요한 표지자들이 밝혀질 것이다. 그리고 이들을 간단하게 검사할 수 있는 검사법도 밝혀져 폐암 조기 진단의 궁극적 목적인 폐암으로 인한 사망률을 감소시킬 수 있을 것으로 기대된다.
